BACKGROUNd Low vitamin d levels are associated with a more severe case of knee osteoarthritis(OA).However,there are few published reports concerning an association between vitamin d deficiency and functional status of individuals with OA and no reports about postural balance in this population.AIM To analyze the relationship between vitamin d deficiency and severity,functional status,and balance in elderly patients with OA.METHOdS In this cross-sectional study,105 elderly patients with hip and knee OA were included.The severity was assessed by the Kellgren-Lawrence criteria.The functional status was assessed with the Lequesne index.Postural balance was assessed using a force platform,and center-of-pressure parameters(velocity at anteroposterior and mediolateral axis)were used as the balance outcomes.Serum 25(OH)vitamin d levels were measured using a chemiluminescence method.RESULTS Most of the patients(mean age:70.6±6.5 years)were female(n=78,74.3%).In the group with vitamin d deficiency,43 patients(56.6%)had severe OA,while 33 patients(43.4%)had mild or moderate OA(χ^(2) test,P=0.04).Patients with vitamin d deficiency showed a higher Lequesne index score(Mann-Whitney test,P=0.04),indicating a worse functional impairment when compared to individuals with normal vitamin d levels.Additionally,patients with vitamin d deficiency had worse postural balance according to the Mann-Whitney test(P=0.03).CONCLUSION Vitamin d deficiency is associated with worse severity,functional status,and postural balance in patients with OA.

Cardiac Myeloperoxidase Activity Is Elevated in Hypertensive Pregnant Rats

Myeloperoxidase（MPO） is released from activated neutrophils. The inflammation in preeclampsia was found to be associated with endothelial dysfunction. We hypothesized that cardiac and circulating MPO levels are elevated in hypertensive pregnancy. Systolic and diastolic blood pressure and heart rate were measured on pregnancy days 14, 16, 18 and 20 in normal pregnant and hypertensive pregnant rats. Left and right ventricle weights, the number of viable fetuses, litter size, fetal and placenta weights were recorded on gestational day 21. Circulating and cardiac MPO activities, soluble fms-like tyrosine kinase-1（sFlt-1） and vascular endothelial growth factor（VEGF） and nitric oxide（NO） were detected. The results showed increases in cardiac（left, but not right ventricle） and circulating MPO activities, and concomitantly lower number of viable fetuses, litter size, and fetal and placenta weights, and decreases in NO in hypertensive pregnant rats. Also, the increases in circulating sFlt-1 and VEGF were found in hypertensive pregnant group. In conclusion, maternal and fetal detrimental changes along with increases in circulating sFlt-1 and VEGF in hypertensive pregnancy may be associated with increases in cardiac and circulating MPO activities, confirming the causative role of inflammatory response in preeclampsia.

Case Study of Coastal Fog Events in Senegal Using LIdAR Ceilometer

This study aims to examine the atmospheric conditions characterising fog phenomena on the Senegalese coast focusing on two specific instances that occurred on April 3 and April 30,2023.These events were detected by the LIdAR Ceilometer installed at LPAOSF/ESP/UCAd and confirmed on the METARs of the meteorological stations at dakar and diass airports.The LIdAR’s backscatter signal showed that the fog of April 3 started around midnight with a vertical extension at 100 m altitude and dissipated around 10 a.m.The April 30 event characterized by a good vertical extension from the surface up to 300 m above sea level,was triggered just after 2 a.***.and lasted around 3 hours.The results showed that a decrease in temperature,accompanied by an increase in humidity and light wind,is favorable for the triggering and persistence of fog.Sea Level Pressure(SLP)anomaly fields show two distinct configurations.The April 3 event was characterized by a zonal dipole of SLP anomalies between the Sahara and the northern Senegalese coast,while the April 30 event was characterized by a meridional dipole between the Sahara and the Gulf of Guinea area as far as the equatorial Atlantic.A weakening of the pressure around the study area was observed in both cases,allowing moisture advection to favor the onset of fog.The hovmoller diagrams of relative humidity and wind show that a good vertical extension of humidity associated with a westerly wind in the lower layers plays an important role in the formation and persistence of fog.The presence of dry air associated with a weak easterly wind in the middle layers could explain the low vertical extension of the fog on April 3.A strong wind in the lower layers would be responsible for the premature dissipation of the April 30 fog.

Association of a single nucleotide polymorphism at 6q25.1, rs2046210, with endometrial cancer risk among Chinese women

A recent genome-wide association study identified a new susceptibility locus for breast cancer, rs2046210, which is a single nucleotide polymorphism (SNP) located upstream of the estrogen receptor α (ESR1) gene on chromosome 6q25.1. Given that endometrial cancer shares many risk factors with breast cancer and both are related to estrogen exposure and that rs2046210 is in close proximity to the ESR1 gene, we evaluated the association of SNP rs2046210 with endometrial cancer risk among 953 cases and 947 controls in a population-based, case-control study conducted in Shanghai, China. Logistic regression models were used to derive odds ratios (ORs) and 95% confidence intervals (95% CIs) after adjusting for potential confounders. We found that the A allele of rs2046210, linked to an increased risk of breast cancer, was associated with increased but not statistically significant risk of endometrial cancer (OR = 1.16, 95% CI = 0.96-1.41 for the GA and AA genotypes compared with the GG genotype); the association was stronger among post-menopausal women (OR = 1.28, 95% CI = 1.00-1.65). The association tended to be stronger among women with higher or longer estrogen exposure than among women with relatively lower or shorter exposure to estrogen. Our study suggests that rs2046210 may play a role in the etiology of endometrial cancer. Additional studies are needed to confirm our findings.

Can the Stepping Stone Enhance the Establishment, Competition and distribution of Sown Grassland Species during Recovery on Ex-Arable Lands?

Most diversity restoration projects are not to improve diversity per se, but rather to enhance the presence and abundance of species that are characteristic of reference or target community. The use of Bromus inermis suppresses annual noxious grasses and increases the control of other-forb group these species are also noxious weeds;these may be substituted with another perennial species of the same functional group all through the whole experimental period, as it occurs with other perennial-forb Carduus tenuifolius. A field experiment was conducted on abandoned arable land with sown low and high diversity treatments and natural colonization following typical farming practice for the site. Experimental plots were installed on former agricultural land that had been cropped with (a rotation of) monocultures until the end of 1995. The experiment was organized according to a block design with five replicate blocks. An opposite trend was performed among the colonizer species, because the colonizer grasses were relegated by the high dominance of sown grasses. But at the same time, the sown grasses facilitated the dominance of other colonizer-forbs species;therefore its functional replacement in the community due to sown effect was again tested. However, in natural conditions the other-forbs group was the dominant group, without taking into account the stepping-stone treatment and there was also a functional change of dominance. Our study has demonstrated the restoration effectiveness of species richness at abandoned arable land and may be enhanced by sowing late successional species.

High-intensity focused ultrasound ablation as a bridging therapy for hepatocellular carcinoma patients awaiting liver transplantation

The scarcity of liver grafts in Asia leads to a significant dropout of patients from liver transplant waiting lists, particularly patients with hepatocellular carcinoma and a low model for end-stage liver disease score. In order to reduce dropping out, different bridging therapies are employed. We report the use of high-intensity focused ultrasound ablation as a bridging therapy for a patient with hepatocellular carcinoma of stage two and an extremely low platelet count (20×10 9 /L). The ablation was successful. Blood tests showed that his liver function was similar before and after the treatment. No adhesion was encountered in the liver transplantation performed six months later.

Classical and Non-Classical Celiac disease Comparison: Ten Years of Study

Objective: Celiac disease (Cd) is an immune-mediated systemic disorder triggered by gluten. It has a variable combination of clinical manifestations and changes that have been occurring in recent decades however they are not known in detail. The purpose of the article is to compare Classical and Non-Classical Cd cases in terms of demographic characteristics, duodenal biopsy, extraintestinal manifestations, and associated comorbidities. Materials and Methods: A comparative retrospective cohort study from January 2008 to december 2018. Results: A total of 128 cases were included: 84 Classical (66%) and 44 Non-Classical Cd (34%). The family history of Cd was identified in 14% of cases without differences between groups. The age at diagnosis was distinct for Classical and Non-Classical Cd (4.9 ± 4 and 8.3 ± 4 years old;p 0.001), respectively. Important changes were found within the classical presentation, including mono symptoms and a significantly higher rate of intestinal atrophy;p = 0.04. The main Non-Classical Cd symptom was recurrent abdominal pain. The extraintestinal manifestations (EIM) were identified in 42% and occurred in both groups. The comparison between groups showed differences in rates of migraine and vitamin d deficiency and was higher for Non-Classical Cd (p 0.05). Associated diseases occurred in 10.9%, and type 1 diabetes was significant for the Non-Classical Cd group (p = 0.04). Conclusion: The classical Cd was the most prevalent profile and presented a decrease in the severity of symptoms however remain a higher rate of intestinal atrophy. Recurrent abdominal pain was the main symptom of Non-Classical Cd. Extraintestinal manifestations and associated diseases presented an increasing trend of occurrence among cases of Non-Classical Cd.

Cirrhotic cardiomyopathy

during the course of cirrhosis, there is a progressive deterioration of cardiac function manifested by the disappearance of the hyperdynamic circulation due to afailure in heart function with decreased cardiac output. This is due to a deterioration in inotropic and chronotropic function which takes place in parallel with a diastolic dysfunction and cardiac hypertrophy in the absence of other known cardiac disease. Other findings of this specific cardiomyopathy include impaired contractile responsiveness to stress stimuli and electrophysiological abnormalities with prolonged QT interval. The pathogenic mechanisms of cirrhotic cardiomyopathy include impairment of the b-adrenergic receptor signalling, abnormal cardiomyocyte membrane lipid composition and biophysical properties, ion channel defects and overactivity of humoral cardiodepressant factors. Cirrhotic cardiomyopathy may be difficult to determine due to the lack of a specific diagnosis test. However, an echocardiogram allows the detection of the diastolic dysfunction and the E/e′ ratio may be used in the followup progression of the illness. Cirrhotic cardiomyopathy plays an important role in the pathogenesis of the impairment of effective arterial blood volume and correlates with the degree of liver failure. A clinical consequence of cardiac dysfunction is an inadequate cardiac response in the setting of vascular stress that may result in renal hypoperfusion leading to renal failure. The prognosis is difficult to establish but the severity of diastolic dysfunction may be a marker of mortality risk. Treatment is non-specific and liver transplantation may normalize the cardiac function.

Impact of vaccine supplies and delays on optimal control of the COVId-19 pandemic:mapping interventions for the Philippines

Background::Around the world,controlling the COVId-19 pandemic requires national coordination of multiple intervention strategies.As vaccinations are globally introduced into the repertoire of available interventions,it is important to consider how changes in the local supply of vaccines,including delays in administration,may be addressed through existing policy levers.This study aims to identify the optimal level of interventions for COVId-19 from 2021 to 2022 in the Philippines,which as a developing country is particularly vulnerable to shifting assumptions around vaccine availability.Furthermore,we explore optimal strategies in scenarios featuring delays in vaccine administration,expansions of vaccine supply,and limited combinations of interventions.Methods::Embedding our work within the local policy landscape,we apply optimal control theory to the compartmental model of COVId-19 used by the Philippine government’s pandemic surveillance platform and introduce four controls:(a)precautionary measures like community quarantines,(b)detection of asymptomatic cases,(c)detection of symptomatic cases,and(d)vaccinations.The model is fitted to local data using an L-BFGS minimization procedure.Optimality conditions are identified using Pontryagin’s minimum principle and numerically solved using the forward-backward sweep method.Results::Simulation results indicate that early and effective implementation of both precautionary measures and symptomatic case detection is vital for averting the most infections at an efficient cost,resulting in>99%reduction of infections compared to the no-control scenario.Expanding vaccine administration capacity to 440,000 full immunizations daily will reduce the overall cost of optimal strategy by 25%,while allowing for a faster relaxation of more resource-intensive interventions.Furthermore,delays in vaccine administration require compensatory increases in the remaining policy levers to maintain a minimal number of infections.For example,delaying the vaccines by 180 days(6 months)will resu
