Intestinal Staple Line Reinforcement Using MatriStem

Background: Staple line reinforcement material has been demonstrated to raise the burst pressure threshold after linear intestinal stapling. Numerous bioprosthetic materials have been utilized in surgical practice. Porcine urinary bladder matrix (ACell, Inc.) is an extracellular matrix material derived from porcine bladder used to reinforce surgically repaired soft tissue, and facilitate the body’s regenerative capacity. Objective: This study represents the first evaluation of urinary bladder matrix in gastrointestinal staple line reinforcement. Methods: Pathogen-free pigs underwent midline laparotomy under general anesthesia. Small intestinal division was performed with an endoscopic linear stapler. Nineteen intestinal divisions were performed with urinary bladder matrix staple line reinforcement, and twenty divisions were unreinforced. Staple lines were then subjected to burst pressure analysis by intraluminal infusion of dyed Krebs solution at an infusion rate of 20 ml·min-1 under manometric monitoring. Upon visible staple line extravasation, intraluminal pressure was recorded. Results: Intestinal staple lines reinforced with urinary bladder matrix exhibited significantly higher burst pressure threshold (p < 0.05). Reinforced staple lines had an average burst pressure of 99 ± 33 mmHg, compared to 61 ± 37 mmHg for unreinforced staple lines. Conclusion: Staple line reinforcement using urinary bladder matrix acutely improves burst pressures of intestinal staple lines when compared with unreinforced staple lines. Its regenerative properties may confer a long-term advantage to staple line reinforcement. These findings, along with previous findings of constructive remodeling in the presence of urinary bladder matrix in treatment of the gastrointestinal system, suggest that UBM may serve a role in gastrointestinal staple line reinforcement.

Amnion epithelial cells——a novel therapy for ischemic stroke?

Stroke is a leading cause of death and disability and new therapies are desperately needed. Given the complex nature of ischemic brain injury, it has been postulated that cell-based therapies may be useful. However, cell resources, invasive extraction procedures, immunological rejection, tumorigenesis and ethical challenges make it unlikely that many stem cell types could serve as a practical source for therapy. By contrast, these issues do not pertain to human amnion epithelial cells（h AECs）, which are placenta-derived stem cells. We recently assessed the effects of systemically delivered hAECs on stroke outcome using four animal models of stroke. We demonstrated that when injected intravenously after ischemia onset, hAECs migrate preferentially to the spleen and injured brain to limit apoptosis and inflammation, and attenuate early brain infiltration of immune cells, progression of infarction and systemic immunosuppression and to ultimately ameliorate functional deficits. When administration of hAECs is delayed by 1-3 days poststroke, long-term functional recovery can still be enhanced in young and aged mice of either sex. Moreover, our proof-of-principle findings suggest that h AECs are effective at limiting post-stroke infarct development in non-human primates. Overall, the results suggest that hAECs could be a viable clinical stroke therapy.

Whereas a rich literature exists for estimating population genetic divergence, metrics of phenotypic trait divergence are lacking, particularly for comparing multiple traits among three or more populations. Here, we review and analyze via simula- tion Hedges＇ g, a widely used parametric estimate of effect size. Our analyses indicate that g is sensitive to a combination of unequal trait variances and unequal sample sizes among populations and to changes in the scale of measurement. We then go on to derive and explain a new, non-parametric distance measure, ＇Aft＇, which is caiculated based upon a joint cumulative distribution function （CDF） from all populations under study. More precisely, distances are measured in terms of the percentiles in this CDF at which each population＇s median lies. Ap combines many desirable features of other distance metrics into a single metric; namely, compared to other metrics, p is relatively insensitive to unequal variances and sample sizes among the populations sam- pied. Furthermore, a key feature of Ap--and our main motivation for developing it--is that it easily accommodates simultaneous comparisons of any number of traits across any number of populations. To exemplify its utility, we employ Ap to address a ques- tion related to the role of sexual selection in speciation： are sexual signals more divergent than ecological traits in closely related taxa？ Using traits of known function in closely related populations, we show that traits predictive of reproductive performance are indeed, more divergent and more sexually dimorphic than traits related to ecological adaptation [Current Zoology 58 （3）： 426-439 2012].
