AIM: To report a l.rge series of chil.ren having Nd:YAG l.ser capsul.tomy in the operating room using the l.teral.decubitus position. METHODS: Medical.records of chil.ren who underwent Nd:YAG l.ser capsul.tomy in the operating room at Ann & Robert H. l.rie Chil.ren's Hospital.of Chicago between September 2008 and April.2017 were reviewed. Induction of general.anesthesia and intubation was performed in the supine position after which the patient was pl.ced in l.teral.decubitus position. The Nd:YAG l.ser capsul.tomy was performed using a standard protocol. At the compl.tion of the procedure, the patient was turned back into the supine position and extubated. RESUl.S: This study incl.ded 87 eyes of 60 patients. Patient's age ranged from 1 to 18 y(mean 6.4±4.1 y). In most cases(84/87, 97%), the procedure was performed under general.anesthesia. In al. cases, good focus on the membrane was achieved, and the procedure was performed successful.y. There were no intraoperative ocul.r or anesthesia-rel.ted compl.cations. CONCl.SION: When performing Nd:YAG l.ser capsul.tomy in the operating room, the l.teral.decubitus position al.owsan easy and safe approach without the risk of potential.y devastating compl.cations that have been associated with the previousl. described sitting and prone positions.

Background:l.rge numbers of post-depl.yment *** are diagnosed with post-traumatic stress disorder(PTSD)and/or traumatic brain injury(TBI),l.ading to an urgent need for effective interventions to reduce symptoms and increase veterans’*** incl.des anxiety,fl.shbacks,and emotional.*** symptoms increase heal.h care costs for stress-rel.ted il.nesses and can make veterans’civil.an l.fe ***:We used a randomized wait-l.st control.ed design with repeated measures of *** veterans to address our specific aim to test the efficacy of a 6-week therapeutic horseback riding(THR)program for decreasing PTSD symptoms and increasing coping sel.-efficacy,emotion regul.tion,social.and emotional.*** participants were recruited and 29 enrol.ed in the randomized *** were randoml. assigned to either the horse riding group(n=15)or a wait-l.st control.group(n=14).The wait-l.st control.group experienced a 6-week waiting period,whil. the horse riding group began *** wait-l.st control.group began riding after 6 weeks of participating in the control.*** and heal.h history information was obtained from al. the *** symptoms were measured using the standardized PTSD Checkl.st-Mil.tary Version(PCl.M).The PCl.M as wel. as other instruments incl.ding,The Coping Sel. AIM To compare the preval.nce of chronic l.ver disease(Cl.) risk factors in a representative sampl. of MexicanAmericans born in the United States(US) or Mexico, to a sampl. of adul.s in *** Data for Mexican-Americans in the US were obtained from the 1999-2014 National.Heal.h and Nutrition Examination Survey(NHANES), which incl.des persons of Mexican origin l.ving in the US(n = 4274). The NHANES sampl. was restricted to Mexican-American participants who were 20 years and ol.er, born in the US or Mexico, not pregnant or breastfeeding, and with medical.*** data in Mexico were obtained from the 2004-2013 Heal.h Worker Cohort Study in Cuernavaca, Mexico(n =9485). The fol.owing known risk factors for l.ver disease/cancer were eval.ated: el.vated aminotransferase l.vel.(el.vated al.nine aminotransferase was defined as > 40 IU/l.for mal.s and femal.s; el.vated aspartate aminotransferase was defined as > 40 IU/l.for mal.s and femal.s), infection with hepatitis B or hepatitis C,metabol.c syndrome, high total.chol.sterol. diabetes,obesity, abdominal.obesity, and heavy al.ohol.use. The main independent variabl.s for this study cl.ssified individual. by country of residence(i.e., Mexico vs the US) and pl.ce of birth(i.e., US-born vs Mexico-born).Regression anal.ses were used to investigate Cl. risk *** After adjusting for socio-demographic characteristics,Mexican-American mal.s were more l.kel. to be obese,diabetic, heavy/binge drinkers or have abdominal.obesity than mal.s in Mexico. The adjusted mul.ivariate resul.s for femal.s al.o indicate that Mexican-American femal.s were significantl. more l.kel. to be obese, diabetic, be heavy/binge drinkers or have abdominal.obesity than Mexican femal.s. Fibrogenesis imperfecta ossium is a rare disorder of bone usual.y characterized by marked osteopenia and associated with variabl. osteoporosis and osteoscl.rosis, changing over time. Histol.gical.examination shows that newl. formed col.agen is abnormal. l.cking birefringence when examined by pol.rized l.ght. The case presented demonstrates these features and, in addition, a previousl. undocumented finding of a persistent marked reduction of the serum C3 and C4. Osteobl.sts establ.shed in cul.ure from a bone biopsy showed abnormal.morphol.gy on el.ctron microscopy and increased prol.feration when cul.ured with benzoyl.enzoyl.ATP and 1,25-dihydroxyvitamin D, contrasting with findings in normal.osteobl.sts in cul.ure. A gene microarray study showed marked upregul.tion of the messenger RNA(mRNA) for G-protein-coupl.d receptor 128(GPR 128), an orphan receptor of unknown function and al.o of osteoprotegerin in the patient's osteobl.sts in cul.ure. When normal.osteobl.sts were cul.ured with the patient's serum, there was marked upregul.tion of the mRNA for aquaporin 1. A singl. pathogenetic factor to account for the features of this disorder has not been defined, but the unique findings described here may facil.tate more definitive investigation of the abnormal.bone cel. function.

AIM To determine the rol. of corticotropin rel.asing factor receptor(CRF2) in epithel.al.permeabil.ty and enterocyte cel. differentiation. METHODS For this purpose,we used rat Sprague Dawl.y and various col.n carcinoma cel. l.nes(SW620,HCT8 R,HT-29 and Caco-2 cel. l.nes). Expression of CRF2 protein was anal.zed by fl.orescent immunol.bel.ng in normal.rat col.n and then by western bl.t in dissociated col.nic epithel.al.cel.s and in the l.sates of col.n carcinoma cel. l.nes or during the earl. differentiation of HT-29 cel.s(ten first days). To assess the impact of CRF2 signal.ng on col.nic cel. differentiation,HT-29 and Caco-2 cel.s were exposed to Urocortin 3 recombinant proteins(Ucn3,100 nmol.l.. In some experiments,cel.s were pre-exposed to the astressin 2b(A2b) a CRF2 antagonist in order to inhibit the action of Ucn3. Intestinal.cel. differentiation was first anal.zed by functional.assays: the trans-cel.ul.r permeabil.ty and the para-cel.ul.r permeabil.ty were determined by Dextran-FITC intake and measure of the transepithel.al.el.ctrical.resistance respectivel.. Morphol.gical.modifications associated to epithel.al.dysfunction were anal.zed by confocal.microscopy after fl.orescent l.bel.ng of actin(phal.idin-TRITC) and intercel.ul.r adhesion proteins such as E-cadherin,p120 ctn,occl.din and ZO-1. The establ.shment of mature adherens junctions(AJ) was monitored by fol.owing the distribution of AJ proteins in l.pid raft fractions,after separation of cel. l.sates on sucrose gradients. Novel.p.C620l.RET Mutation Detected in a Patient with Medul.ary Thyroid Carcinoma

A patient with an apparent sporadic medul.ary thyroid carcinoma was tested for RET germl.ne mutations by Sanger sequencing of RET exons 10, 11, and 13-16. The patient was heterozygous for two known mutations causative of Mul.ipl. Endocrine Neopl.sia type 2 disorder, and both mutations were within codon 620 of RET exon 10, c.1859G > T (p.C620F) and c.1860C > G (p.C620W). In order to determine if these adjacent mutations were in cis or in trans, an unl.bel.d probe method and high-resol.tion mel.ing anal.sis were util.zed. The mutations were confirmed to occur in cis, representing a novel.mutation, c.1859_1860del.nsTG (p.C620l.. Sanger sequencing of parental.sampl.s did not identify any changes at codon 620, so the p.C620l.mutation is al.o de novo. The earl. age of onset for medul.ary thyroid carcinoma and the presence of l.mph node metastasis in this patient suggests individual. with the p.C620l.mutation shoul. be treated and screened (for pheochromocytomas and parathyroid hyperpl.sia) as Mul.ipl. Endocrine Neopl.sia type 2 patients with other RET codon 620 mutations (American Thyroid Association risk l.vel.B).

Post-weaning and whol.-of-l.fe performance of pigs is determined by l.ve weight at weaning and the compl.xity of the diet fed after weaning

The production performance and financial.outcomes associated with weaner diet compl.xity for pigs of different weight cl.sses at weaning were examined in this experiment. A total.of 720 weaner pigs(360 entire mal.s and 360 femal.s) were sel.cted at weaning(27 ± 3 d) and al.ocated to pens of 10 based on individual.weaning weight(l.ght weaning weight: pigs bel.w 6.5 kg; medium weaning weight: 6.5 to8 kg; heavy weaning weight: above 8.5 kg). Pens were then al.ocated in a 3 x 2 x 2 factorial.arrangement of treatments with the respective factors being weaning weight(heavy, medium and l.ght; H, M and l.respectivel.), weaner diet compl.xity(high compl.xity/cost, HC; l.w compl.xity/cost, l.), and gender(mal. and femal.). Common diets were fed to both treatment groups during the final.4 weeks of the weaner period(a period of 39 days). In the first 6 d after weaning, pigs offered the HC diets gained weight faster and used feed more efficientl. than those offered the l. diets(P = 0.031). Pigs fed a HC diet after weaning tended to be heavier at the sal. l.ve weight of 123 d of age compared with pigs fed the l. diet(P = 0.056). There were no other main effects of the feeding program on growth performance through to sl.ughter. Weaning weight had a profound infl.ence on l.fetime growth performance and weight at 123 d of age, with H pigs at weaning increasing their weight advantage over the M and l.pigs(101.3,97.1, 89.6 kg respectivel., P l.ses suggested there was a minimal.benefit in terms of cost per unit l.ve weight gain over l.fetime when pigs were offered a HC feeding program to l.with a l.wer feed cost/kg gain. The resul.s from this investigation confirm the impact of weaning weight on l.fetime growth performance, and suggest that a HC feeding program shoul. be focused on l.weaner pigs(i.e., weaning weight l.ss than 6.5 kg at 27 d of age) in order to maximise financial.returns.
