Prognostic factors and time-related changes influence results of colorectal liver metastases surgical treatment:A single-center analysis

AIM:To analyze the prognostic factors involved in survival and cancer recurrence in patients undergoing surgical treatment for colorectal liver metastases(CLM) and to describe the effects of time-related changes on survival and recurrence in these ***:From January 1994 to January 2006,236 patients with CLM underwent surgery with the aim of performing curative resection of neoplastic disease at our institution and 189(80%) of these patients underwent resection of CLM with curative ***,intraoperative and postoperative data,including primary tumor and CLM pathology results,were retrospectively *** were divided into two time periods:a first period from January 1994 to January 2000(n = 93),and a second period from February 2000 to January 2006(n = 143).RESULTS:Global survival at 1,3 and 5 years in patients undergoing hepatic resection was 91%,54% and 47%,*** with preoperative extrahepatic disease,carcinoembryonic antigen(CEA) levels over 20 ng/dL,more than four nodules or extrahepatic invasion at pathological analysis had worse *** recurrence rate at 1 year was 48.3%,being more frequent in patients with preoperative and pathological extrahepatic disease and CEA levels over 20 ng/*** patients in the second time period had more adverse prognostic factors,no differences in overall survival and recurrence were observed between the two ***:Despite advances in surgical technique and better adjuvant treatments and preoperative imaging,careful patient staging and selection is crucial to continue offering a chance of cure to patients with CLM.

Non Convulsive Status Epilepticus: An Elusive Diagnosis

Absence status is the most common form of non-convulsive status epilepticus and is characterized by confusion with varying degrees of memory loss and cognitive impairment. Patients and Method: Three children were sent to neurological consultation due to behavioral alterations and a prolonged confused state;they were hospitalized and treated with sodium diphenylhydantoinate (DPH) IV at a dose of 10 mg/Kg. Results: The duration of symptoms varied from 6 months to 10 days. All three patients presented with global mental alterations, showing slowness in response and action. The electroencephalogram showed a pattern of slow, generalized stem and poly-stem-wavelengths of 3 - 4 Hz, which were registered for one hour. After the DPH bolus, the attack spontaneously ended in the 3 patients and upon examination all three presented with amnesia of the events occurring during the attack. In the follow-up, two of the patients did not experience further episodes and they showed normal scholastic achievement. The third patient however, after suffering a 6-month status epilepticus, failed the school year and finished his elementary education until the age of 15, experiencing similar difficulties with his secondary education. Discussion: Non-convulsive status epilepticus is more difficult to diagnose mainly because the manifestations are predominantly psychiatric and can be confused with other diseases or with an overdose of anti- convulsive drugs. A prolonged state of mental confusion, with no other explanation, should alert the attending physician to take an electroencephalogram in order to confirm the diagnosis. In our patients, DPH immediately controlled paroxysmal activity. We can therefore conclude that the problem is not in the treatment, but rather in making the correct diagnosis.

Efficacy of the vaccination in inflammatory bowel disease

Inflammatory bowel disease (IBD) is associated with conditions that may predispose to infections, such as the lack of an appropriate innate immune response to infectious agents, malnutrition, surgery, and immunosuppressive and biological drugs. Some of these infections may be preventable by vaccination. Therefore, for this particular patient population, the benefits of implementing a well-established immunization protocol in daily clinical practice are potentially even greater than for the general population. In recent years international consensus guidelines have been published, but in spite of theses recommendations, studies have shown that a significant number of patients with IBD remain inadequately immunized. Another important issue regarding immunization in this population is that vaccine efficacy among patients receiving immunosuppressive therapies has been variable. In a healthy population, a humoral immune response to hepatitis B vaccination (HBV) is expected in > 90%, whereas a much lower rate is achieved in the IBD patients. Immunosuppressive, anti-tumor necrosis factor therapy and disease activity have been implicated in the impaired efficacy of the vaccination. The serological response to HBV should be confirmed and patients with an inadequate response should receive a second full series of vaccine. Modified dosing regimens, including doubling the standard antigen dose, might increase the effectiveness. Response to influenza, pneumococcal and tetanus immunization is still not clear, as there are studies that show a normal response to the vaccination while others demonstrate a lack of efficacy. We pose a series of questions on the efficacy of the different vaccinations recommended for IBD patients and attempt to answer them using scientific evidence.

考虑到当前与气候变化、能源和其他服务以及生态系统相关的环境和资源问题,国内和国际层面的机构和政策制定者必须提倡当代大多数资源利用领域以及与生态系统联系的领域显著的改变。本研究旨在评估生态创新对经合组织和金砖国家碳排放的影响。因此,本文将34个经合组织和5个金砖国家作为一个小组进行了抽样,用来详细解释这一现象。该研究提供了一个探索各国异同的机会,这也使区域政策制定者能够吸取经验,在环境保护新技术方面采用最佳措施。本研究的数据来自经合组织数据库和世界银行2006年至2019年的世界发展指标。采用多种计量经济学方法得出本研究的结论。计量经济学方法,如单位根检验、相关矩阵、协整检验、格兰杰因果检验、普通最小二乘回归方法和完全修正的普通最小二乘回归方法。研究结果表明,生态创新可以对所使用的代理类型产生积极和消极的碳排放影响。从调查结果可以看出,能源强度和专利对碳排放有积极影响,能源强度增加一个百分点可以导致碳排放量增加0.677%和0.705%,专利增加一个百分点也可以导致碳排放量分别增加经合组织国家碳排放量的0.073%和0.087%。对于金砖国家而言,能源强度每增加一个百分点,在1%的显著性水平上,碳排放量会增加0.202%。此外,在1%的显著性水平上,专利增加一个百分点可能会分别导致0.126%和0.087%的碳排放增加。另一方面,研发支出似乎有助于减少碳排放,研发支出每增加一个百分点,就可以减少0.032%的碳排放。但在金砖国家样本中,研究发现研发支出对碳排放产生积极影响。这意味着实际收入的增加可以鼓励研发并降低能源强度。此外,为确保低碳经济,应鼓励推行降低能源强度、加强环境法规和改进研发的节能政策。

Environmental Enrichment and Its Effect on the Welfare of Two Asian Elephants in Africam Safari Wildlife Conservation Park (Puebla, Mexico)

Animal welfare is important in zoos to maintain the physical well-being and psychological health of individuals. An animal is considered to have welfare if it has good nutrition and also expresses its innate behavior, including sensations and feelings experienced as a result of physical health. The objective of this study was to evaluate the effect of environmental enrichment on the behaviors of two Asian elephants in a wildlife conservation park. The behavior of two Asian elephants, a 46-year-old male and a 59-year-old female, was observed. Behavior was measured by quantifying the frequency, latency, and duration of behavioral actions. Measurements were done with ad libitum sampling during 5 days of testing, then observations were recorded with focal sampling for 25 days with environmental enrichment and 25 days without enrichment. Data were analyzed with the Chi-square statistical test using the statistical program SPSS 20, observing significant differences (P the application of environmental enrichment. The frequency of maintenance behaviors was higher when environmental enrichment was provided, and the frequency of social conducts (affiliative and agonistic) decreased. In fact, the behaviors most frequently presented by elephants with and without environmental enrichment were definitely maintenance behaviors. The environmental enrichment program helped to reduce the duration of abnormal behaviors and increase the frequency of typical behaviors of the species. It also increased independent movements within the exhibition area and helped to know each of the elephants individually in order to apply each enrichment combination according to their needs.

AIM:To assess:(1)frequency and clinical relevanceof gluten sensitive enteropathy(GSE)detected by serology in a mass screening program;(2)sensitivity of antitransglutaminase(tTGA)and antiendomysium antibodies(EmA);and(3)adherence to gluten-free diet(GFD)and follow-up. METHODS:One thousand,eight hundred and sixtyeight subjects recruited from an occupational health department underwent analysis for tTGA and EmA and, if positive,duodenal biopsy,DQ2/DQ8 genotyping, clinical feature recording,blood tests,and densitometry were ***>98%of individuals had tTGA <2 U/mL,this value was established as the cut-off limit of normality and was considered positive when confirmed twice in the same *** to a GFD and follow up were registered. RESULTS:Twenty-six(1.39%)subjects had positive tTGA and/or EmA,and 21 underwent biopsy:six Marsh Ⅲ(oneⅢa,fourⅢb,oneⅢc),nine MarshⅠand six Marsh 0(frequency of GSE 1:125).The sensitivity of EmA for GSE was 46.6%(11.1%for MarshⅠ,100% for MarshⅢ),while for tTGA,it was 93.3%(88.8% for MarshⅠ,100%for MarshⅢ).All 15 patients with abnormal histology had clinical features related to ***ⅠandⅢsubjects had more abdominal pain than Marsh 0(P=0.029),and a similar trend was observed for distension and *** differences in the percentage of osteopenia were found between MarshⅠandⅢ(P=0.608).Adherence to follow-up was 69.2%.Of 15 GSE patients,66.7%followed a GFD with 80%responding to it. CONCLUSION:GSE in the general population is frequent and clinically relevant,irrespective of histological *** is the marker of *** screening programs are useful in identifying patients who can benefit from GFD and follow-up.

Content-Based Image Unequal Error Protection Strategies for an Open Loop MIMO System

The paper deals with strategies for optimizing the transmission of JPEG2000 coded images over a MIMO quasi-static Rayleigh fading channel, by exploiting different available MIMO STBC (Space Time Block Codes), as well as the image content to be transmitted. The aim is to propose a link adaptation scheme based on the variable radio channel’s conditions, by establishing an UEP (Unequal Error Protection) scheme that leads to the best tradeoff between the robustness to errors and the image visual quality. Comparisons are made with the EEP (Equal Error Protection) case, in terms of the PSNR (Peak Signal to Noise Ratio) fidelity metric, as well as the subjective image quality.

Synthesis and Determination of Antitumor Activity of Jacaranone and Synthetic Analogs

Natural product jacaranone, 1, and three analog derivatives were synthesized and their apoptotic and necrotic activity against four cancer cell lines was tested. One of these derivatives 7, was more active than the natural product, and exhibited an important necrotic effect against three of the cell lines tested (ovarian carcinoma, liver cancer and breast cancer cells). Derivative 6 was more active than the natural product, and showed a significant apoptotic activity against breast cancer and ovarian carcinoma cells. Some derivatives analyzed in this study showed promising anti-tumor results, nevertheless, further studies have to be done in order to determine their in vivo activity, their mechanism as well as their safety and stability.
