Survival Effect of Tube Feeding in Patients with Advanced Dementia and Comorbidities

Patients suffering from advanced dementia who have difficulties in eating and swallowing are often tube fed (TF). They suffer, on most occasions, from additional diseases. The effect of tube feeding in advanced dementia patients is controversial and is largely discussed in the medical literature. The purpose of the present study, is to report the effect of tube feeding on the survival time in patients with advanced dementia who suffered from additional diseases. One hundred and forty nine patients with Functional Assessment Stage of Alzheimer Disease (FAST) stage 5>, hospitalized in skilled geriatric nursing department between July 2008 and December 2011 were included in the study. Oral and tub feeding was decided following the evaluation by speech therapists at admission. 99 (66.5%) of the patients were tube fed and 50 (33.5%) received oral feeding at admission. The mean age was 80 ± 11. Sixty four percent of the patients were admitted from general hospitals. Comorbidities were significantly higher in the tube fed group, median survival time was 164 days. The median survival time of the OF group was 78 days (χ2 = 0.94;p = 0.33). Prevalence of tube feeding increased significantly from 53.85% to 90.9% in patients with 3 risk factors such as pressure ulcers, low Hgb and low BMI. The survival time of the tube fed patients was longer compared to patients who were orally fed. Tube feeding in patients with dementia continues to be controversial, and additional researchable data are helpful in the decision to insert a tube.

＜正＞Service Convergence is a prospective goal to reach considerable economic benefits in a Multi-Service Network （MSN） environment. Elements such as Multi-Service Provisioning Platforms and Multi-Technology Network Management systems together with suitable MSN modeling are the key underlying components supporting that goal. The paper relates to the framework of MSN modeling and specifically addresses the issue of assigning resources in an MSN environment to provide differential survivability. In this paper we specify aspects of MSN survivability, explain the Cycle-oriented approach for Path Protection/Restoration and the survivable traffic types to be supported. We detail two network models for optimal resource allocation with special emphasis on large-network considerations and present some typical results that reflect the strengths of the network models developed.

Nurses＇ unintentional medication errors during treatment are relatively frequent and yet inevitable. Errors provoke emotions which influence the nurses＇ professional careers. Little is known about the relationship between nurses＇ supervisors constructive listening （CL） and the emotional reactions of nurses who committed an error and its relation to patients＇ safety. Our purpose was to explore the relationship between nurses＇ perceptions regarding their supervisors＇ CL and their emotional experiences after committing an error related to patient care. Dependent variables included of guilt, empathy towards the patient, general and professional self-assessment, shame, and Negative and Positive Affect （NA/PA）. In this descriptive study, we used a snowball sampling method. Participants were asked to sign an informed-consent form and complete the questionnaire before or after work. No compensation （material or otherwise） was offered to participants. The study was approved by the ethics committee of the academic institution involved. A total of 162 nurses participated： 103 （63.6%） held a registered and 40 （25%） held a managerial role. Seniority had high variability, ranging from 3 months to 45 years （M=1 3.54, SD=0.78）. The majority of errors reported （67.7%） concerned the administration of medications. We used Structural Equation Modeling to measure relationships between the main variables （X2（9）=14.52, p=.105, CFI=.911, RMSEA=.062 （90% CI=.00-. 11）. The main findings were： a high rating of perceived supervisor＇s CL led to high state-guilt （β=. 15, p=.04）. Next, higher state-guilt led to high PA （β=.18, p=.02） and to high NA （β=.45, p〈.001）. High PA led to reporting the error （β=.17, p=.03）, whereas high NA led to a high degree of empathy towards the patient （β=.17, p=.03）. Our findings show the importance of CL, which led to reporting error and to empathy towards patients, mediated by increased state-guilt and by increased positive and negative effect. Skin cancer, including both melanoma and non-melanoma, is the most common type of malignancy, which causes substantial morbidities and mortalities. Although the significant increase in the understanding of skin cancer formation and the development of novel personalized drug regimens have occurred, new treatment options are always of need. The use of natural compounds to alleviate the symptoms or even to prevent and treat cancer has long been proposed. Specifically, the use of marine-based organisms as a source for cancer cure and remedy is being evaluated extensively. The objective of the current study was to assess the ability of the green microalgae Dunaliella salina, the Dead-Sea-derived Haloferax volcanii, and its combinations to treat skin cancer in vitro. The results demonstrate the Dunaliella and Haloferax can reduce sarcoma and basal cell carcinoma cellular growth. Importantly, their combination acts synergistically in a caspase-3 independent manner. Moreover, a synergistic action was found when evaluated sarcoma cell invasion rate, which was completely blocked at pharmacological relevant amounts of the compounds. Collectively, the results demonstrate that the combination of Haloferax volcanii and Dunaliella salina can be used as a new treatment for skin cancer. The specific mechanism of action and further in vivo validation studies are of need.

Biomechanical aspects of catheter-related thrombophlebitis

Short peripheral catheters (SPCs) are the most common intravenous devices used in medical practice. Short peripheral catheter thrombophlebitis (SPCT) is the most frequent complication associated with SPCs, causing discomfort and usually leading to removal of the catheter and insertion of a new one at a different site. The aim of this research was to explore whether biomechanical factors, in addition to biochemical factors, also play a role in the formation of thrombophlebitis. Hence, two of the biomechanical aspects of SPCT were investigated: the physical pressure load exerted by the SPC on the endothelial monolayer, and disturbances in the flow patterns due to the SPC. Endothelial activation was studied by subjecting human umbilical vein endothelial cells (HUVEC) to a weight load of SPC pieces and measuring the release profile of von-Willebrand Factor (vWF) over time, using ELISA. vWF release was chosen as the measure for endothelial activation since it was the major component of the Weibel-Palade Bodies (WPBs), which underwent exocytosis by endothelial cells during activation. Flow patterns were analyzed on a 3D computational fluid dynamics (CFD) model of a brachiocephalic vein with SPC. vWF release profiles were significantly higher in the HUVECs subjected to the load, indicating HUVEC activation. CFD simulations demonstrated a decrease in flow velocities along the catheter body, between the catheter and the vein, due to an enlarged boundary layer. Results indicate that the contact region between the SPC body and the vein wall can be partially responsible for SPCT development, and inflammatory and coagulatory processes initiated by stimulated endothelial cells may be amplified due to disturbed blood flow.
