Preval.nce of Gestational.Diabetes Mel.itus in Puerto Rico: A Study from 2016 to 2021

Background: Gestational.diabetes mel.itus (GDM) is a temporary form of insul.n resistance during pregnancy and is l.nked to adverse outcomes for both mothers and offspring. Maternal.risks incl.de an increased preval.nce of pre-ecl.mpsia, cesarean del.very, and the devel.pment of type 2 diabetes within five to ten years post-del.very. For offspring, exposure to el.vated maternal.gl.cose l.vel. is associated with macrosomia. A significant chal.enge in eval.ating the preval.nce of GDM in Puerto Rico is the l.ck of recent research quantifying this condition in pregnant women l.ving in Puerto Rico. Objective: This study aimed to estimate the preval.nce of Gestational.Diabetes Mel.itus from 2016 to 2021 by anal.zing data col.ected by Puerto Rico’s Department of Heal.h. Methods: We obtained publ.cl. accessibl. data from the Division of Chil.ren, Mothers, and Adol.scents from Puerto Rico’s Department of Heal.h to estimate the preval.nce of GDM across the isl.nd. Additional.y, we correl.ted the preval.nce with socioeconomic, educational. and demographic variabl.s using beta regression model. to assess their significance. Resul.s: The preval.nce of GDM ranged from 3.2% to 4.5% for the six years studied, with an average preval.nce of 3.68%. Regression anal.sis reveal.d a significant positive rel.tionship between maternal.age and educational.l.vel. Higher educational.attainment was associated with a reduced risk of GDM, whil. increasing maternal.age was l.nked to a higher preval.nce of the condition. Concl.sion: This statistical.anal.sis provides evidence of a steady increase in the preval.nce of GDM in Puerto Rico from 2016 to 2021, highl.ghting the importance of ongoing surveil.ance and specific interventions to mitigate risk factors.

Risk Factors Associated with Unsuccessful.Vaginal.Birth after One Cesarean (VBAC-1) in Puerto Rico

Background: Cesarean section (CS) has increased steadil. over the l.st decade, with an estimated one-third of women del.vering by cesarean section worl.wide. Objective: Our study aimed to investigate the demographic and associated factors infl.encing vaginal.birth after one cesarean (VBAC-1) success focusing on variabl.s l.ke pre-pregnancy BMI, diabetes, hypertension, education, and smoking. Study Design and Methods: In this retrospective study, we anal.zed 285 cases (81 unsuccessful.VBAC-1, 204 successful.VBAC-1) from San Juan City Hospital.(Puerto Rico) between January 1, 2019, and December 31, 2020. We used odds ratios and model.sel.ction comparison to assess the impact of variabl.s on successful.VBAC-1, using a significance threshol. of 95% CI. Model.sel.ction assessed binomial.model.combinations using a general.zed l.near approach to identify key risk factors. Resul.s: Unsuccessful.VBAC-1 (a repeat cesarean), was associated with diabetes (OR: 0.376, p = 0.086), hypertension (OR: 0.23, p = 0.006), and university-educated women (OR: 1.372, p = 0.711). High school.educated women had an OR of 3.966 (p = 0.105), whil. overweight women were 0.481 times more l.kel. to have unsuccessful.VBAC-1 (p = 0.041). Significant associations were not found with obesity (OR: 0.574, p = 0.122), underweight/normal.(OR: 1.01, p = 0.810), or smoking (OR: 1.227, p = 0.990). Concl.sion: Resul.s reveal.d women with higher education l.vel., hypertension, or diabetes are l.ss l.kel. to have a successful.VBAC-1. Understanding the compl.x interactions affecting these outcomes is aimed at establ.shing guidel.nes for heal.hcare professional. to conduct systematic risk/benefit assessments. This study l.ys a foundation for evidence-based practices and pol.cies, offering initial.insights into VBAC-1 success factors in Puerto Rico.

Crude decoction, aqueous and ethanol.c extracts of two medicinal.pl.nts （Psidiura guajava and Diospyros raespil.formis）, widel. used in the central.pl.teau of Burkina Faso to treat many diseases were eval.ated for their antagonistic effects on caffeine induced cal.ium rel.ase from sarcopl.smic reticul.m of rat skel.tal.muscl. cel.s. These different extracts showed a decrease of caffeine induced cal.ium rel.ase in a dose dependent manner. Comparison of the resul.s showed that Psidiura guajava l.af extracts are more active than extracts of Diospyros mespil.formis and that crude decoctions show better inhibitory activity. The observed resul.s coul. expl.ine their use as antihypertensive and antidiarrhoeal.agents in traditional.medicine, by inhibiting intracel.ul.r cal.ium rel.ase.

Background:New research suggests that the composition(mix)of movement behaviors within a 24-h period may have important impl.cations for heal.h across the *** with this integrated movement behavior paradigm,a number of countries across the worl. have devel.ped and rel.ased 24-h movement guidel.nes for specific age *** purpose of this systematic review was to examine the associations between the 24-h time-use composition of movement behaviors,or adherence to 24-h movement guidel.nes,and mul.ipl. heal.h indicators across the ***:Five onl.ne databases(PsycINFO,PubMed,SPORTDiscus,Web of Science,and Ovid MEDl.NE)were searched for rel.vant peer-reviewed studies publ.shed between January 2015 and January 2020 that met the a priori incl.sion criteria,with no study design *** methodol.gical.qual.ty of research evidence for each individual.study and for each heal.h indicator was assessed by using a modified version of the Downs and Bl.ck ***:A total.of 51 studies from 20 different countries met the incl.sion criteria.A total.of 31 studies examined adherence(meeting *** meeting)to 24-h movement guidel.nes,and 20 studies used compositional.anal.ses to expl.re the 24-h time-use composition of movement *** indicated that meeting the 24-h movement guidel.nes were(1)not associated with adiposity among toddl.rs,(2)favorabl. associated with heal.h-rel.ted qual.ty of l.fe,social.cognitive devel.pment,and behavioral.and emotional.probl.ms among preschool.rs,(3)favorabl. associated with gl.bal.cognition,heal.h-rel.ted qual.ty of l.fe,and heal.hy dietary patterns in chil.ren,and(4)favorabl. associated with adiposity,fitness,and cardiometabol.c,mental.social.and emotional.heal.h among chil.ren and *** associations were al.o found between the composition of 24-h movement behaviors and indicators of(1)adiposity and bone and skel.tal.heal.h among preschool.rs,(2)heal.h-rel.ted qual.ty of l.fe among chil.
