BACKGROUND Congenital knee dislocation(CKD)is a rare condition,which accounts for 1%of congenital hip *** can present as an isolated condition or coexist with other genetic *** options include serial casting,percutaneous quadriceps recession,and V-Y quadricepsplasty(VYQ).The pathogenesis and hereditary patterns of CKD are not fully understood,with most cases being *** is usually managed immediately after ***,in this report,the patient was neglected for 2 *** SUmmARY A 2-year-old girl with bilateral CKD after birth presented to our hospital after failed serial casting;the patient had seizures and limited access to healthcare because of her family’s low socioeconomic *** birth was noted for a breech presentation accompanied by *** delivery took a long time,requiring immediate medical *** an infant,she had chronic diseases,including a small patent ductus arteriole,multicystic dysplastic kidney disease,and *** was found to have a bilateral knee dislocation of approximately-90°on hyperextension.A multidisciplinary team was involved,and medical care was *** underwent VYQ plus semitendinosus and sartorius *** four postoperative follow-ups,her knees were regaining mobility,and she could walk for 2-3 steps without *** This report highlights the importance of early intervention and recommends extensive studies of the management in similar cases.

蛋白膜具有良好的机械性能,是一种环境友好的可食性包装材料,但其抑菌、抗氧化性能等包装活性较差,限制其在某些易腐食品包装领域的广泛应用。本研究以大豆分离蛋白(soy protein isolate,SPI)为成膜基质,加入不同添加量的蛋壳膜(eggshell membrane,ESm)酶解肽制备复合膜,以机械性能、阻隔性能、光学性能、接触角、自由基清除率及抑菌圈直径为指标,考察复合膜的物理和功能特性,并结合傅里叶变换红外光谱、X射线衍射分析和扫描电子显微镜等手段对其进行结构表征,探究ESm酶解肽添加量对复合膜性能与结构的影响及作用机理。结果表明,与纯SPI膜相比,添加4 g/100 g ESm酶解肽复合膜的厚度、拉伸强度、断裂伸长率和接触角分别增加了115.38%、100.78%、57.87%和110.42%,水蒸气透过率和氧气透过率分别降低了63.54%和84.07%,且抗氧化与抑菌性随ESm酶解肽添加量增加而大幅提升,通过结构表征分析结果可知,复合膜表面结构因分子间生成氢键而形成致密的网络结构,且SPI与ESm酶解肽相容性良好。结果可为易腐食品的活性包装、蛋品副产物综合利用方面提供理论基础和技术支持。

Safety Evaluation of Stacked Genetically modified Corn Event (mON89034 ×mON88017) Using Zebrafish as an Animal model

This study assessed the effect of feeding stacked genetically modified (Gm) corn event (mON89034 × mON88017) on embryonic development, adult tissue histology and assessed the possibility of transgenic DNA transfer, using zebrafish as an animal model. Adult zebrafish were separated into male and female groups. After 3 weeks of feeding with experimental diets, males and females were paired once a week and fertilized embryos were collected, then the male and female fish were re-turned to their sex-specific groups. This fertilization and embryo collection process were repeated 3 times. Embryonic morphological measurements were obtained on developing embryos up to 96 hours after fertilization, and then the embryos were fixed in 4% phosphate-buffered paraformal-dehyde for morphologic assessment. At the end of the feeding study, various adult zebrafish tissues were histologically examined for abnormalities. Both zebrafish tissues and embryos were analyzed for presence of plant specific genes and transgenic sequences. No changes or abnormalities were observed in embryonic morphology nor in any of the tissues examined histologically. In addition, no plant DNA reference genes or transgenic DNA were found in any of the analyzed samples. These results demonstrate that the safety and nutrition of mON89034 × mON88017 corn are similar to non-Gm corn.

multiple Primary malignancies: metastatic Renal with Early Breast and Endometrial Cancers: A Case Report

Double primary malignancies could be divided into two categories, depending on the interval between tumor diagnoses. A secondary malignancy could be defined as a new cancer that has occurred as a result of previous treatment with radiation or chemotherapy. Second primary malignancy can occur at any age but it’s commonly at old age. A 46 premenopausal female patient presented to our outpatient clinic complaining from a mass in her right breast, routine metastatic work-up for distant metastasis declared multiple hepatic metastases, RT renal mass, and bone metastases. Palliative radiotherapy to tender and weight bearing sites followed by 4 cycles of systemic chemotherapy FEC regimen were received. Tru-cut needle biopsy from renal mass detected renal cell carcinoma of clear cell type, the patient started sunitinib and tamoxifen with bisphosphonate (Zoledronic acid), assessment of the response revealed reduction of the size and number of HFLs, and the size of renal mass, so the patient was decided to do cytoreductive nephrectomy and then continued on TAm and sunitinib. Collectively, due to the rising incidence of multiple primary malignancies, further studies should be done not only for better clinical evaluation and treatments but also for accurate determination of possible causes, pathogenesis, effective managements and screening programs.

Natural Attenuation Capacity Indicators for Groundwater Remediation to the Northeastern Cairo

The natural attenuation capacity of groundwater is considered to be an important factor for remediation purpose, it is also important for surveying suitability in the early phases of siting hazardous facilities. The present work has been devoted to explore the natural attenuation capacity of the groundwater system northeast Cairo. Some indicators (hydrogeological, hydrochemical and isotopic) have been insighted to explore the system retardability against contaminant dispersion. Groundwater flow and contaminant transport have been pointed out using GOD index which varied from 0.04 to 0.28 reflecting high protectability. The isotopic contents (18O, 3H, and 14C) showed a general comply with the GOD index and helped to delineate the sectors of high contaminate attenuation ability. Kelly index inferred significantly high adsorption potential on the aquifer clay content. The calcite and dolomite tend to precipitate on the aquifer contact providing a chance for specific contaminants to coprecipitate. The Eh-pH relation of the studied groundwater samples indicates high oxidation conditions and openness of the system. The attenuation capacity has been determined mathematically, it assumes the dispersive, advective, sorptive, and degradative processes acting on a solute in a one-dimensional flow system. The value of attenuation capacity increases by decreasing groundwater velocity and completely vanishes at a certain high velocity;determined for the relevant radioactive contaminants.

Capecitabine and Oxaliplatin versus 5-Fluorouracil in the Neoadjuvant Chemoradiation for Patients with Locally Advanced Rectal Carcinoma: A Comparative Study

Aim of work: This study aimed to evaluate the resectability rate, rate of conservative surgery, toxicity, local control, and disease free survival for oxaliplatin and capecitabine based chemoradiation compared to standard 5-FU based chemoradiation in locally advanced cancer rectum. Patients and methods: 65 patients were eligible;33 patients received oxaliplatin and capecitabine based chemoradiation (arm I) and 32 patients received 5-FU based chemoradiation (arm II). Results: The overall response rate in arms I and II were 78.7% and 87.5% respectively. Conservative surgery was done in 81.81% and 53.13% of patients with arms I and II, pathologic complete response (pCR) rate was significantly better in arm I than arm II (30.3% vs. 21.9%, P ms I, II respectively;the median disease free survival (DFS) were 30 months and 15 months in arms I and II respectively. Grade III anemia, grade IV diarrhea and severe proctitis were developed in a significantly large number of patients with arm I;in addition deep venous thrombosis (DVT) was developed in 15.15% of patients with arm I but none in arm II. Conclusion: The addition of oxaliplatin to the preoperative chemo radiation increased the response rate mainly pCR rate which was considered a target goal in the neoadjuvant treatment, but it was not recommended because of higher toxicity and no significant effect on DFS in different response groups of arm I when compared to arm II, but longer follow up may be needed to evaluate the overall survival.

Accelerated Hyper Fractionated Radiotherapy in Localized Ewing’s Sarcoma with or without Surgery: What’s New? A Phase II Study

Background and aim of work: We aimed from this study to determine the response and local relapse free survival (RFS) of pediatric patients with localized Ewing’s sarcoma treated with accelerated hyper-fractionated RT. Patient and methods: This study was a nonrandomized uncontrolled phase II study and was conducted at clinical oncology department and south Egypt cancer institute;it involved 28 patients with histologically confirmed Ewing’s sarcoma;all of them were subjected to PET/CT whenever possible or mRI with contrast of the primary site, mSCT chest, bone scan, and LDH to ensure absence of metastasis followed by the protocol of accelerated hyper fractionated RT. Results: The overall response rate (ORR) was 92.9% by mRI with significant effect of the type of response on local RFS (P 0.002). The median local RFS of 28 patients with localized Ewing’s sarcoma was 30 ± 8.599 months with 95% CI = 13.147 - 46.853;the 3-year local control was 35%. Conclusion: Accelerated hyper fractionated RT didn’t achieve better results than standard fractionation RT, but it is recommended to be done on a large sample size, and multiple centers, and continued follow up is also recommended to evaluate 5-year LRFS, 5-year OS.

The Prognostic Significance of Pre and Post Treatment Neutrophil to Lymphocyte Ratio in Breast Cancer Patients

Background: The neutrophil/lymphocyte ratio (NLR) has been reported to reflect systemic inflammation and independent prognostic significance in different types of cancer. In present study, we analyzed the association between NLR and clinicopathologic features and verified the significance of NLR as a prognostic factor for patients with breast cancer. Patients and methods: A total of 388 patients with stage I - III breast cancer were retrospectively recruited into this study. Associations with clinicopathologic factors and NLR were assessed;disease-free survival and overall survival were estimated. Results: There was no significant association between NLR and clinicopathologic factors. Patients with low pre/post-treatment NLR had longer OS. Conclusion: A high pre/postoperative NLR may be considered an important factor for predicting poor prognosis in non-metastatic breast cancer patients.

Natural Radioactivity Assessment and Radiological Hazards in Soils from Qarun Lake and Wadi El rayan in Faiyum, Egypt

The activity concentrations of naturally occurring 238U, 232Th and 40K in surface soils along Qarun Lake and Wadi El rayan located in Faiyum, Egypt were determined. The measurements were carried out through gamma-ray spectrometry using a coaxial HPGe detector. The results were compared with those reported in the literature. The radiological hazard radium-equivalent activity index, external and internal indices, radioactivity level index, absorbed dose rate, annual effective dose rate and total absorbed dose rate associated with radioactivity in all samples were evaluated and compared with recommended values. Correlation studies between pairs of radionuclides were performed and discussed.

XRF Analysis of Heavy metals for Surface Soil of Qarun Lake and Wadi El rayan in Faiyum, Egypt

The environmental pollution with some heavy metals for twenty four surface soil samples collected from Qarun Lake and Wadi El rayan region in Faiyum, Egypt utilizing X-ray fluorescence (XRF) spectroscopy was measured. The concentrations of 13 elements Cr, Ni, Cu, Zn, Zr, Rb, Y, Ba, Pb, Sr, Ga, V and Nb were determined. The elemental concentrations were compared with the normal values and other studies in different locations from the world. The correlation between elements appears that pollution inside the investigated lake and Wadi result from different sources of contamination present inside them. The results establish a database reference of radioactivity background levels around these regions.
