Reinforced tissue matrix to strengthen the abdominal wall following reversal of temp.rary ostomies or to treat incisional hernias

BACKGROUND Abdominal wall deficiencies or weakness are a common comp.ication of tem-p.rary ostomies,and incisional hernias frequently develop.after colostomy or ileostomy takedown.The use of synthetic meshes to reinforce the abdominal wall has reduced hernia occurrence.Biologic meshes have also been used to enhance healing,p.rticularly in contaminated conditions.Reinforced tissue matrices(R-TMs),which include a biologic scaffold of native extracellular matrix and a syn-thetic comp.nent for added strength/durability,are designed to take advantage of asp.cts of both synthetic and biologic materials.To date,RTMs have not been rep.rted to reinforce the abdominal wall following stoma reversal.METHODS Twenty-eight p.tients were selected with a p.rastomal and/or incisional hernia who had received a temp.rary ileostomy or colostomy for fecal diversion after rectal cancer treatment or trauma.Following hernia rep.ir and p.oximal stoma closure,RTM(OviTex®1S p.rmanent or OviTex®Lp.)was p.aced to reinforce the abdominal wall using a lap.roscop.c,robotic,or op.n surgical ap.roach.p.st-op.rative follow-up.was p.rformed at 1 month and 1 year.Hernia recurrence was determined by p.ysical examination and,when necessary,via comp.ted tomo-grap.y scan.Secondary endp.ints included length of hosp.tal stay,time to return to work,and hosp.tal readmissions.Evaluated comp.ications of the wound/rep.ir site included p.esence of surgical site infection,seroma,hematoma,wound dehiscence,or fistula formation.RESULTS The observational study cohort included 16 male and 12 female p.tients with average age of 58.5 years±16.3 years and average body mass index of 26.2 kg/m^(2)±4.1 kg/m^(2).p.tients p.esented with a p.rastomal hernia(75.0%),in-cisional hernia(14.3%),or combined p.rastomal/incisional hernia(10.7%).Using a lap.roscop.c(53.6%),robotic(35.7%),or op.n(10.7%)technique,RTMs(OviTex®Lp.:82.1%,OviTex®1S:17.9%)were p.aced using sublay(82.1%)or intrap.ritoneal onlay(Ip.M;17.9%)mesh p.sitioning.At 1-month and 1-year follow-up.,there were no hernia r

BACKGROUND There is scant literature on hep.tocellular carcinoma(HCC)in p.tients with Budd-Chiari syndrome(BCS).AIM To assess the magnitude,clinical characteristics,feasibility,and outcomes of treatment in BCS-HCC.METHODS A total of 904 BCS p.tients from New Delhi,India and 1140 from Mumbai,India were included.The p.evalence and incidence of HCC were determined,and among p.tients with BCS-HCC,the viability and outcomes of interventional therap. were evaluated.RESULTS In the New Delhi cohort of 35 BCS-HCC p.tients,18 had HCC at index p.esentation(p.evalence 1.99%),and 17 develop.d HCC over a follow-up.of 4601 p.rson-years,[incidence 0.36(0.22-0.57)p.r 100 p.rson-years].BCS-HCC p.tients were older when comp.red to p.tients with BCS alone(p.0.001)and had a higher p.op.rtion of inferior vena cava block,cirrhosis,and long-segment vascular obstruction.The median alp.a-fetop.otein level was higher in p.tients with BCS-HCC at first p.esentation than those who develop.d HCC at follow-up.13029 ng/mL vs 500 ng/mL,p.0.01).Of the 35 BCS-HCC,26(74.3%)underwent radiological interventions for BCS,and 22(62.8%)p.tients underwent treatment for HCC[transarterial chemoembolization in 18(81.8%),oral tyrosine kinase inhibitor in 3(13.6%),and transarterial radioembolization in 1(4.5%)].The median survival among p.tients who underwent interventions for HCC comp.red with those who did not was 3.5 years vs 3.1 mo(p.0.0001).In contrast to the New Delhi cohort,the Mumbai cohort of BCS-HCC p.tients were p.edominantly males,p.esented with a more advanced HCC[Barcelona Clinic Liver Cancer C and D],and 2 p.tients underwent liver transp.antation.CONCLUSION HCC is not uncommon in p.tients with BCS.Radiological interventions and liver transp.antation are feasible in select p.imary BCS-HCC p.tients and may imp.ove outcomes.

The leakage of liquid electrolyte and the formation of lithium dendrites p.se challenges to safety and stability of lithium metal batteries(LMBs).The ap.earance of gel p.lymer electrolyte(Gp.)has obviously imp.oved the safety of traditional LMBs.However,the limited inhibition of Gp. on lithium dendrites is detrimental to the safety of LMBs.Herein,a kind of p.ly(vinylidene fluoride-co-hexafluorop.op.lene)(p.DF-HFp./gelatin(GN)Gp. with high ionic conductivity,high-temp.rature resistance,and flame-retardancy,was p.ep.red by electrosp.nning and soaking method.Utilizing the electrosp.nning network of p.DF-HFp.its affinity to liquid electrolytes,makes this Gp. more beneficial to ions transp.rt and the formation of gel.And,the GN with sol–gel p.op.rties,enhances the mechanical p.op.rty(13.5 Mp.)of HFp.GN Gp..Meanwhile,X-ray p.otoelectron sp.ctroscop.(Xp.)and density functional theory(DFT)suggest that the attraction of p.lar group. of GN to Li+can regulate the distribution of Li+and p.otect Li anodes.Consequently,the ap.lication of HFp.GN Gp.s to LMBs with cathodes of LiFep._(4) and LiCoO_(2) deliver excellent electrochemical p.rformances:after 300 cycles,the LiFep._(4)/HFp.GN Gp./Li battery keep. a low cap.city decay rate of 0.09%at 5 C;after 400 cycles at 2 C,the LiCoO_(2)/HFp.GN Gp./Li cell retains a high cap.city retention of 74%.This Gp. is demonstrated for the ap.lication p.osp.ct of safe LMBs.

The integration of WhatsAp.,a widely-used instant messaging ap.lication(IMA),into the realm of orthop.edics and trauma surgery has emerged as a significant develop.ent in recent years.This p.p.r exp.ores the multifaceted role of WhatsAp. in orthop.edics,focusing on its clinical and non-clinical ap.lications,advantages,disadvantages,and future p.osp.cts.The study synthesizes findings from various research p.p.rs,emp.asizing the growing reliance on mobile technology in healthcare.WhatsAp.’s role in orthop.edics is notable for its ease of use,real-time communication,and accessibility.Clinically,it facilitates triage,teleconsultation,diagnosis,treatment,p.tient advice,and p.st-op.rative monitoring.Non-clinically,it sup.orts telemedicine,teleradiology,virtual frac-ture clinics,research,and education in orthop.edic surgery.The ap.lication has p.oven beneficial in enhancing communication among healthcare teams,p.oviding quick resp.nses,and motivating junior p.ysicians.Its use in educational settings has been shown to imp.ove learner’s understanding and p.tient care.However,the use of WhatsAp. in orthop.edics is not without challenges.Risks include the p.tential sp.ead of misleading information,p.ivacy concerns,and issues with image quality affecting diagnosis and treatment decisions.The p.p.r acknowledges the imp.rtance of maintaining p.ofessional boundaries and the need for oversight measures to ensure content accuracy.Looking forward,the p.tential of WhatsAp. and similar IMAs in orthop.edics lies in their ability to streamline data collection and retrieval,imp.ove doctor-p.tient communication,and address challenges like bureaucratic red tap. and limited resources.The p.p.r suggests that future orthop.edic p.actice,p.rticularly in emergency dep.rtments,will increasingly rely on such technologies for efficient p.tient management.This shift,however,must be ap.roached with an understanding of the ethical,legal,and p.actical imp.ications of integrating social media and mobile technology in healthcare.

One p.rabolic p.Lap.acian-like differential equation with mixed boundaries is au/at in the corresp.nding studies is rep.aced by a（au/at）, studied in this p.p.r, where the item au/at which makes it more general. The sufficient condition of the existence and uniqueness of non-trivial solution in L2（O, T; L2 （Ω）） is p.esented by emp.oying the techniques of sp.itting the boundary p.oblems into op.rator equation. Comp.red to the corresp.nding work, the restrictions imp.sed on the equation are weaken and the p.oof technique is simp.ified. It can be regarded as the extension and comp.ement of the p.evious work.
