目的评估腹腔镜袖带胃切除术（LSG）治疗病态性肥胖2型糖尿病的疗效。方法2007年1月至7月，对来自全美的30例体质量指数（BMD大于或等于35、且合并2型糖尿病的病态性肥胖志愿患者行LSG，术后所有病例随访6个月。分析患者手术前后血糖控制情况、BMI变化和超重体重下降百分比（EWL％）及血糖指标变化情况和Ghrelin激素。结果本组30例患者在接受LSG后6个月，糖尿病治愈率达63％（19／30），缓解率达37％（11／30），术后6个月总有效率为100％。结论LSG对病态性肥胖2型糖尿病有着良好的短期治疗效果。

目的评价腹腔镜Roux-en-Y胃分流术（LRYGB）对病态性肥胖合并2型糖尿病患者的治疗效果，探讨该术式治疗糖尿病的可能机制。方法前瞻性分析2007年1月至7月美国克利夫兰医院30例体质量指数（BMI）≥35．00合并2型糖尿病的病态性肥胖患者的临床资料，采用t检验分析患者接受LRYGB后的疗效。结果与术前比较，术后第6个月的BMI、空腹血糖、糖化血红蛋白明显降低（t=27．399，23．025，15．593，P〈0．05）。第2个月治愈率为70％（21／30），第6个月治愈率为90％（27／30），3例糖尿病症状得到改善。进一步分析术后第2个月和第6个月体质量下降程度，改善患者分别为27．89％±5．51％、45．73％±2．82％，治愈患者分别为35．65％±1．97％、58．00％±1．05％，糖尿病治愈患者比改善患者下降幅度大（t=5．755，7．081，P〈0．05）。结论LRYGB对病态性肥胖合并2型糖尿病患者有着良好治疗效果，体质量下降幅度越大，糖尿病治愈可能性就越大。

BACKGROUND The influence of Helicobacter-pylori(***)infection and the characteristics of gastric cancer(GC)on tumor-infiltrating lymphocyte(TIL)levels has not been extensively *** of infiltrating-immune-cell subtypes as well as survival is necessary to obtain comprehensive *** To determine the rates of deficient mismatch-repair(dMMR),HER2-status and *** infection and their association with TIL levels in *** Samples from 503 resected GC tumors were included and TIL levels were evaluated following the international-TILs-working-group recommendations with assessment of the intratumoral(IT),stromal(ST)and invasive-border(IB)*** density of CD3,CD8 and CD163 immune cells,and dMMR and HER2-status were determined by immunohistochemistry(IHC).*** infection was evaluated by routine histology and quantitative PCR(qPCR)in a subset of *** dMMR was found in 34.4%,HER2+in 5%and ***-positive in 55.7%of *** IT-TIL was associated with grade-3(P=0.038),while ST-TIL with grade-1(P<0.001),intestinal-histology(P<0.001)and no-recurrence(P=0.003).dMMR was associated with high TIL levels in the ST(P=0.019)and IB(P=0.01)compartments,and STCD3(P=0.049)and ST-CD8(P=0.05)***2-was associated with high IT-CD8(P=0.009).*** was associated with high IT-TIL levels(P=0.009)when assessed by routine-histology,and with high TIL levels in the 3 compartments(P=0.002-0.047)and CD8 density in the IT and ST compartments(P=0.001)when assessed by qPCR.A longer overall survival was associated with low IT-CD163(P=0.003)and CD8/CD3(P=0.001 in IT and P=0.002 in ST)and high IT-CD3(P=0.021),ST-CD3(P=0.003)and CD3/CD163(P=0.002).CONCLUSION TIL levels were related to dMMR and *** CD8/CD3 and high CD163/CD3 were associated with lower recurrence and longer survival.

Premigratory fat metabolism in hummingbirds： A Rumsfeldian approach

Hummingbird migration is a remarkable feat, given the small body sizes of migratory species, their high metabolic rates during flight and the long distances traveled using fat to fuel the effort. Equally remarkable is the ability of premigratory hummingbirds in the wild to accumulate fat, synthesized from sugar, at rates as high as 10% of body mass per day. This paper summarizes, using Rumsfeldian terminology, ＂known knowns＂ concerning the energetics of hummingbird migration and premi- gratory fattening. Energy metabolism during hover-feeding on floral nectar is fueled directly by dietary sugar through the path- way recently named the ＂sugar oxidation cascade＂. However, flight without feeding for more than a few minutes requires shifting to fat as a fuel. It is proposed that behavior and metabolic fuel choice are coadapted to maximize the rate of fat deposition during premigratory fattening. The hummingbird liver appears to possess extraordinarily high capacities for fatty acid synthesis. The analysis of ＂known knowns＂ leads to identification of ＂known unknowns＂, e.g., the fates of dietary glucose and fructose, the regulation of fat metabolism and metabolic interactions between liver and adipose tissue. The history of science behooves recog- nition of ＂tmknown unknowns＂ that, when discovered serendipitously, might shed new light on fundamental mechanisms as well as human pathological conditions .

Dear editor,The incidence of emergency department(ED)visits from gastroparesis and cannabinoid hyperemesis syndrome has increased by two folds over the past decade.[1-3]Roldan et al[4]reported that patients with these conditions often have prolonged ED stays and are likely to be hospitalized because of the danger of dehydration and challenges inherent in relieving *** analgesia is a particularly troublesome treatment modality;these conditions are often chronic with frequent recurrences,and repeated opiate therapy can cause opioid dependency and narcotic bowel syndrome.[5]Haloperidol(HP)and droperidol,members of the butyrophenone class,have potent antidopaminergic activity in the chemoreceptor trigger zone.[6,7]Intramuscular administration of HP is being used increasingly in the ED to treat nausea,vomiting secondary to gastroparesis,[6]intractable vomiting,[8]and cyclical vomiting.
