Correlation between DKK-1 Level and Bone Density Status in Children on maintenance Haemodialysis

Background: Renal osteodystrophy (ROD) is a bone disorder resulting from chronic kidney disease (CKD) and related metabolic diseases. Dickkopf-related protein-1 (DKK-1) is critical in regulating bone biology. This study aimed to evaluate the serum DKK-1 level as a bone marker in children with CKD who undergo regular hemodialysis (HD). Subjects and methods: This case-control study involved 40 children with CKD on HD and 40 healthy children as controls. The study measured serum DKK-1 levels and performed a dual-energy X-ray absorptiometry scan (DEXA) in line with routine laboratory investigations. Results: There was a significant increase in the serum level of DKK-1 in the patient group compared to the control group. The DKK-1 levels were 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, respectively, with a p-value of less than 0.001. In the hemodialysis group, 25 patients (62.5%) had low bone mineral density (BmD) with a Z-score of under -2.0. Eighteen of these patients had low BmD in both the neck of the femur and lumbar spines. Additionally, there was a significant increase in serum DKK-1 level in patients with low BmD (2567.35 (2303.8 - 3108.1) pg/ml) compared to patients with normal BmD (2454 (1859 - 2820) pg/ml) (p = 0.041). There was also a significant positive correlation between DKK1 level and phosphorus, alkaline phosphatase, and Parathormone serum levels. In conclusion, the study indicates a clear correlation between DKK-1 and BmD in children undergoing maintenance hemodialysis. DKK1 is a promising biomarker for CKD-mBD.

Squamous cell carcinoma metastatic to cervical lymph nodes from unknown primary origin:the impact of chemoradiotherapy

The management of cervical lymph node metastases of squamous cell carcinoma from an unknown primary site is still a therapeutic *** report here our experience in treating these patients with chemoradiotherapy as a curative *** from 40 patients were *** total,20(50%) patients underwent excisional *** patients underwent radiotherapy,which was delivered to both sides of the neck and pharyngeal mucosa(extensive field),and concurrent chemotherapy consisting of weekly cisplatin at a dose of 40 mg/*** clinical stage of the cervical nodes at presentation was N1 in 25%,N2 in 60%,and N3 in 15%.most patients(75%) developed at least grade 3 *** patients(20%) had grade 3 xerostomia and 18 patients(45%) required esophageal dilation for *** 5-year overall survival(OS) rate of all patients was 67.5%.The 5-year OS rates of patients with N1,N2,and N3 lesions were 100%,67%,and 41%,respectively(P = 0.046).The 5-year progression-free survival rate was 62.5%.In multivariate analysis,only N stage significantly affected OS(P = 0.022).Emergence of the occult primary was very limited(1 patient only).Our results suggest that extensive irradiation of both sides of the neck and pharyngeal mucosa with concurrent chemotherapy results in a lower emergence of primary *** the survival of patients with unknown primary is comparable to that of patients with known primary,an attempt at cure should always be made.

Prevalence of Toxins and Antimicrobial Resistance among <i>E. coli</i>Isolated from meat

Aim: The present study aims to evaluate the occurrence and characterize E. coli in meat and meat products marketed in Egypt based on their antimicrobial-resistance pattern and production of enterotoxins. methods: A total of 250 meat samples, categorized as 80 fresh beef, 85 ground beef and 85 beef burger purchased from supermarkets and butchers’ shops were used for isolation of E. coli. All isolates were screened for antimicrobial susceptibility. Plasmid profile analyses were done. Polymerase chain reactions were performed for detection of enterotoxin-encoding genes (astA, eaeA, stx1 and stx2). Results: Twenty-five samples were isolated and identified as E. coli. 14 isolates were multidrug resistant. Plasmids isolation from all isolates revealed that 76% of these isolates harbored plasmids. astA gene was amplified in 7 isolates (28%). Eight (32%) isolates harbored eaeA gene. However, none of the isolates harbored stx1 or stx2 genes. Analysis of multiple drug resistant isolates revealed a significant relation between multiple drug resistance and both astA and eaeA. Conclusion: The study confirmed the prevalence of enterotoxin genes (astA and eaeA) in E. coli isolated from meat product and the association between the presence of these genes and multiple drug resistant phenomena.

BACKGROUND The current coronavirus disease 2019(COVID-19)pandemic has affected routine endoscopy service across the gastroenterology *** led to the suspension of service provision for elective *** To assess the potential barriers for resuming the endoscopy service in *** A national online survey,four domains,was disseminated over a period of 4 wk in August *** primary outcome of the survey was to determine the impact of the COVID-19 pandemic on the endoscopy service and barriers to the full resumption of a disabled center(s).RESULTS A hundred and thirteen Egyptian endoscopy centers participated in the *** waiting list was increased by≥50% in 44.9% of areas with clusters of COVID-19 cases(n=49)and in 35.5% of areas with sporadic cases(n=62).Thirty nine(34.8%)centers suffered from staff shortage,which was considered a barrier against service resumption by 86.4% of centers in per-protocol *** multivariate analysis,the burden of cases in the unit locality,staff shortage/recovery and the availability of separate designated rooms for COVID-19 cases could markedly affect the resumption of endoscopy practice(P=0.029,ely)and Odd’s ratio(0.15,1.8 and 0.16,respectively).CONCLUSION The COVID-19 pandemic has led to restrictions in endoscopic *** staff shortage/recovery and the availability of COVID-19 designed rooms are the most important barriers against *** working hours and dividing endoscopy staff into teams may help to overcome the current situation.

Prevalence and Correlates of Overweight and Obesity among School Children in Sohag, Egypt

Background: Continually rising prevalence of obesity and overweight in children and adolescents is a major public health concern. This is due to its various and serious health hazards on one side and its preventable nature on the other side. This study aims at identification of prevalence of overweight and obesity and its risk factors among children between 6 - 14 years of age in Sohag, Egypt. method: A cross sectional study was conducted on 711 school children between 6 - 14 years in four months’ period from February to may 2016. Weight and height of the studied children were measured to calculate BmI, which was compared with standard Egyptian growth charts for determining overweight and obesity. Then a questionnaire containing data about socio demographic, parental obesity, early life and dietary risk factors was completed by the children’s parents. Result: Out of 711 studied children, 117 (16.5%) were overweight and 104 (14.6%) were obese. Residing in urban area, having an obese parent or both of them, low level of maternal education, being on formula feeding during early life and lack of fresh vegetables and fruits in diets were significant risk factors for overweight and obesity in the studied children (P value mproperly educated mothers together with sedentary life style manifested in obesity among the parents and lack of sport practice among children constituted the majors risk factors for childhood obesity. This emphasizes the need for increased health awareness of the community about the importance of proper nutrition and physical activity.

Impact of IL28B gene polymorphisms rs8099917 and rs12980275 on response to pegylated interferon-α/ribavirin therapy in chronic hepatitis C genotype 4 patients

Host genetic factors may predict the outcome and treatment response in hepatitis C virus（HCV）*** of these factors is the single nucleotide polymorphisms of the interleukin 28B（IL28B）*** sought to evaluate the outcome of pegylated interferon and ribavirin therapy in association with IL-28B rs8099917 and rsl2980275 in patients infected with HCV genotype 4.A total of 180 patients with chronic hepatitis C were selected from Egyptians who have received combined therapy with pegylated interferon and ribavirin for 6 months and their response was evaluated after follow-up at 0,6,12,24 and 48 weeks from the beginning of the *** samples were collected from responders and *** DNA was extracted from whole blood and genotyping was carried out by polymerase chain reaction and restriction fragment length polymorphism（PCR-RFLP）.Our results showed that TT genotype of rs8099917 was associated with higher sustained viral response（SVR）rates and G allele represented a risk factor for failure of response（OR=3.7,CI=1.8：7.64）while rs12980275 was not significantly associated with SVR in genotype 4 Egyptian *** determination of 1L-28B SNPs may be useful in enhancing correct prediction of SVR achievement in treating this group of genotype 4 patients.

New smart home energy management systems based on inclining block-rate pricing scheme

There are wide applications of block-rate pricing schemes in many ***,there are no significant studies that apply this common tariff for smart home energy management *** this paper,a three-time-frame energy management scheme has been proposed for photovoltaic(PV)-powered grid-connected smart homes based on the well-known mixed-integer linear programming optimization *** paper provides three original and novel smart home energy management algorithms that depend on the most common residential tariff specifically in developing *** different management concepts have been studied for a typical Egyptian *** concepts of shifting load,vehicle-to-home and reducing air conditioning have been tested according to a commonly applied slab *** proposed scheme considers the home battery extending lifetime *** also preserves comfortable lifestyle limits for home users according to Arab housing climatic conditions and ***,the economic feasibility of integrated PV modules for the studied home has been verified according to the Egyptian *** proposed energy management scheme of PV-powered home reduces the electrical power bill significantly in a wide range from 61%to only 19%of the default case bill according to the applied management technique.

Allocation and sizing of reactive power compensators considering PV power and load demand uncertainty using beetle-antenna grey wolf optimization

Distributed photovoltaic(PV)systems play an important role in supplying many recent *** absence of reactive power support for these small-scale PV plants increases total microgrid losses and voltage-instability *** power compensations(RPCs)should be integrated to enhance both microgrid losses and voltage *** planning is a non-linear,complicated *** this paper,a combined RPC allocation and sizing algorithm is *** RPC-integrating buses are selected using a new adaptive approach of loss sensitivity *** the sizing process,the uncertainties in PV power and load demand are modelled using proper probability density *** simulation techniques for handling uncertainties are compared to define the accurate and fast accurate method as follows:monte Carlo simulation(mCS),scenario tree construction and reduction method,and point estimation method(PEm).The load flow equations are solved using the forward-backward sweep *** are optimally sized using the beetle-antenna-based strategy with grey wolf optimization(BGWO)to overcome the local minima problem that appeared in the other pre-proposed *** have been compared using particle swarm optimization and conventional *** proposed model is verified using the IEEE 33 radial bus *** expected power loss has been reduced by 22% and 31% using compensation of 26% and 44%,*** results obtained prove that the BGWO optimal power flow and PEm to handle the uncertainty can significantly reduce the computation time with sufficient *** the study conditions,PEm reduces the computation time to 4 minutes compared with 4 hours for mCS,with only a 3% error compared with mCS as an uncertainty benchmark method.

One-stage versus staged ureteroscopy and percutaneous nephrolithotomy for simultaneous ureteral and renal stones

Objectives:This study is aimed to investigate the outcome of one-stage ureteroscopy(URS)and percutaneous nephrolithot(PCNL)for simultaneous ureteral and renal stones over 10years at a tertiary urology *** and methods:We retrospectively analyzed the data of patients who were operated on for simultaneous ureteral and renal stones from January 2011 to December *** were divided into 2 groups:group A,who underwent one-stage URS and prone PCNL,and group B,who underwent staged *** overall success,complications,operative time,and hospital stays were compared between the 2 ***:Data for 190 patients were reviewed;mean age was 50±13 years old,and 146(77%)were *** one-stage(A)and staged(B)groups included 102 and 88 patients,*** A included older patients,with a high The American Society of Anesthesiologists score,while group B included more patients with multiple or staghorn *** one-stage group recorded shorter operative time(120±12min vs.140±16min,p=0.02)and shorter hospital stays(3 days[2-6]vs.4 days[3-9],p=0.06).Otherwise,both groups had equal outcomes in terms of success rates and ***:PCNL and URS can be performed in one-session for simultaneous ureteral and renal stones,except for multiple renal and staghorn *** results are comparable to those of the staged procedure in terms of success rate and complications,with the advantage of a shorter operative time and hospital stay.

Breast cancer is marked by large increases in the protein fibers around tumor *** fibers increase the mechanical stiffness of the tissue,which has long been used for tumor diagnosis by manual *** research in bioengineering has led to the development of novel biomaterials that model the mechanical and architectural properties of the tumor microenvironment and can be used to understand how these cues regulate the growth and spread of breast ***,we provide an overview of how the mechanical properties of breast tumor tissues differ from those of normal breast tissue and noncancerous *** also describe how biomaterial models make it possible to understand how the stiffness and viscosity of the extracellular environment regulate cell migration and breast cancer *** highlight the need for biomaterial models that allow independent analysis of the individual and different mechanical properties of the tumor microenvironment and that use cells derived from different regions within *** models will guide the development of novel mechano-based therapies against breast cancer metastasis.
