In this study we present the design and realization of a tunable dual band wireless power transfer(TDB-WPT)coupled resonator *** frequency response of the tunable band can be controlled using a surface-mounted *** transmitter(Tx)and the receiver(Rx)circuits are *** top layer contains a feed line with an impedance of 50Ω.Two identical half rings defected ground structures(HR-DGSs)are loaded on the bottom using a varactor *** propose a solution for restricted WPT systems working at a single band application according to the operating *** effects of geometry,orientation,relative distance,and misalignments on the coupling coefficients were *** validate the simulation results,the proposed TDB-WPT system was fabricated and *** system occupied a space of 40 mm×40 *** can deliver power to the receiver with an average coupling efficiency of 98%at the tuned band from 817 to 1018 MHz and an efficiency of 95%at a fixed band of 1.6 GHz at a significant transmission distance of 22 *** results of the measurements accorded well with those of an equivalent model and the simulation.

This paper studies the effects of Natural convection and Thermal radiation on nanofluid over a non-linearly stretching sheet. The governing equation of nonlinear partial differential equations of the flow is transformed to nonlinear ordinary differential equations by using similarity transformation, earlier than being solved numerically through a Rung-Kutta-Fehlberg method with shooting technique. The numerical results have been obtained for The influence of Brownian motion number (Nb), thermophoresis number (Nt), Grashof number (Gr), Lewis number (Le), stretching parameter (n), and thermal radiation parameter (Ra) on the velocity, temperature, and nanoparticles concentration profiles are shown graphically.

本文设计并实现了一个可调谐双频无线电能传输耦合谐振器系统。可调谐波段的频率响应可以使用表面贴装变容二极管进行控制。发射器和接收器电路是对称的。顶层包含一个阻抗为50Ω的馈线。通过使用变容二极管，两个相同的半环缺陷接地结构被加载在底部。针对工作在单频段（基于工作频率）的受限无线电能传输系统，提出一种解决方案。研究了几何形状、方向、相对距离和偏移对耦合系数的影响。为验证仿真结果，制作并测试了所提可调谐双频无线电能传输系统。系统面积为40 mm×40 mm。在有效传输距离为22 mm时，在817～1018 MHz调谐频段，以98%的平均耦合效率向接收器输送电能，在1.6GHz固定频段，效率为95%。测量结果与等效模型的模拟结果吻合较好。

Purpose:Sepsis is a common acute life-threatening condition that emergency physicians routinely *** options within the Emergency Department(ED)are limited due to lack of infrastructure,consequently limiting the use of invasive hemodynamic monitoring or imaging *** mortality rate due to sepsis can be assessed via multiple scoring systems,for example,mortality in emergency department sepsis(MEDS)score and sepsis patient evaluation in the emergency department(SPEED)score,both of which quantify the variation of mortality rates according to clinical findings,laboratory data,or therapeutic *** study aims to improve the management processes of sepsis patients by comparing SPEED score and MEDS score for predicting the 28-day mortality in cases of emergency ***:The study is a cross-sectional,prospective study including 61 sepsis patients in ED in Suez Canal University Hospital,Egypt,from August 2017 to June *** were selected by two steps:(1)suspected septic patients presenting with at least one of the following abnormal clinical findings:(a)body temperature higher than 38℃ or lower than 36℃,(b)heart rate higher than 90 beats/min,(c)hyperventilation evidenced by respiratory rate higher than 20 breaths/min or PaCO2 lower than 32 mmHg,and(d)white blood cell count higher than 12,000/μL or lower than 4000/μL;(2)confirmed septic patients with at least a 2-point increase from the baseline total sequential organ failure assessment(SOFA)score following *** inclusion criteria included adult patients with an age≥18 years regardless of gender and those who had either systemic inflammatory response syndrome or suspected/confirmed *** were shortly follow-up for the 28-day *** patient was subject to SPEED score and MEDS score and then the results were compared to detect which of them was more effective in predicting *** receiver operating characteristic curves were also done for MEDS and SPE

Prophylactic Cervical Cerclage versus Vaginal Progesterone in Triplet Pregnancy—A Randomized Prospective Comparative Study

Objective: To compare the effect of prophylactic cervical cerclage with vaginal progesterone in triplet (with normal cervical measurements) to evaluate its efficacy in improving pregnancy outcome and prolonging gestation. Design: A randomized prospective comparative study. Setting: At Tanta university hospitals and outpatient clinics. Patients: 51 selected cases of triplet pregnancy were recruited and classified into 2 groups randomly, progesterone group (n = 28 cases) and cerclage group (n = 23 cases). Interventions: Transvaginal ultrasound was done for number of fetuses, viability, cervical measurements, vaginal progesterone for progesterone starting at 20 weeks, and applying McDonald cerclage at 14 - 16 weeks for cerclage group. Main Outcome Measures: Time of occurrence of preterm labor, premature rupture of membranes (PROM), stitch removal, gestational age at delivery, mode of delivery, and neonatal complications. Results: Preterm labor occurred in 9, 8 cases and PROM in 4, and 3 cases in progesterone and cerclage groups respectively. The mean gestational age was 33.57 ± 2.97 and 31.74 ± 3.21 weeks in progesterone and cerclage groups;mean birth weight was 2049 ± 591 gm, and 1686 ± 512 gm in progesterone and cerclage groups respectively;Apgar score 7 or more was found in 52 (60.8%), and 39 (56.3%) newborn in progesterone and cerclage group respectively. Perinatal mortality was lower in progesterone group 26.1% (3 IUFD+19 neonatal) than in cerclage group 30.4% (2 IUFD + 19 neonatal). Respiratory distress syndrome (RDS) occurred in 43.9%, 49.76%, while need of mechanical ventilation occurred in 12.2%, 16.41% of progesterone and cerclage groups respectively and neonatal jaundice was found in 49.38%, 53.7% of progesterone and cerclage groups respectively. Conclusion: Vaginal progesterone seems to be more effective than prophylactic cerclage in reducing preterm delivery in triplet pregnancies with normal cervical measurements even in those with prior history of preter

Iron Depletion Improves Glycemic Control in Poorly Controlled Type 2 Diabetic Patients with Iron Overload and Negative Main HFE-Gene Mutations

Iron overload increases the risk of diabetes via mechanisms of abnormal glucose metabolism: insulin deficiency, insulin resistance, and/or hepatic dysfunction. Iron reduction upregulates glucose uptake and improves hepatocytes insulin receptor activity. This study was conducted to examine the effects of iron depletion—via controlled phlebotomy—on the hypoglycemic treatment in poorly controlled type 2 diabetes mellitus (T2DM) patients with non-genetic iron overload. Forty three patients with poorly controlled T2DM and iron overload were divided into 2 groups: iron depletion group and control group. Regular phlebotomy was performed for iron depletion group on monthly basis until serum ferritin reached 20 μg/L or less. Both groups were examined and compared for blood pressure, serum ferritin, lipid profile, HFE-gene, HbA1c, HOMA-IR and number of medicines used for diabetic control. The results had revealed that group differences of HbA1c (-2.64, 95% CI -3.23 to 2.04, p < 0.001) and HOMA-IR (-0.68, 95% CI -0.98 to -0.37, p < 0.001) showed significant decreases in iron depletion group at end of study. Significant decrease in the numbers of hypoglycemic medicines in iron depletion group was shown at end of study (p < 0.001);66.7% of iron depletion group patients were receiving 1 or 2 medicines at end of studyversus none of the control group. Diastolic blood pressure (DBP), triglycerides and LDL-C decreased significantly while HDL-C levels showed significant rise after iron depletion. It can be concluded from the present study that iron depletion therapy is beneficial for improving the efficiency of glycemic control, DBP, and dyslipidemia in poorly controlled type 2 diabetics with iron over load.

Studies on Enterotoxins and Antimicrobial Resistance in Staphylococcus aureus Isolated from Various Sources

Staphylococcus aureus represents a public health challenge all over the world. Therefore, this study aims to analyze the prevalence of five genes (sea, seb, sec, see and seg) encoding the staphylococcal enterotoxins in S. aureus isolated from different sources and to evaluate the association of these toxins in comparison to susceptibility towards 12 antimicrobials;antimicrobial susceptibility was conducted by disc diffusion method. Detection of staphylococcal enterotoxins was performed by PCR and the ability to express these genes was assessed among isolates by RT-PCR. The most common enterotoxin gene was sea gene (66%), followed by seb, sec, see and seg (38%, 23%, 19% and 5%) respectively. Expression of sea, seb and seg genes was variable. However, sec and see genes were not expressed by any of the tested isolates. No statistically significant association exists between (seb, sec and see) and isolation sources, while the sea was significantly associated with clinical isolates. High significant correlation was found between elevated sea expression and multidrug-resistance. Our findings indicate that the pathogenic potential of S. aureus may be greater than previously thought. This emphasizes the utmost need to implement proactive measures and more emphasis will be placed on the application of hygiene practices in hospitals to control S. aureus infection and enterotoxins production.
