Feasibility and accuracy of coronary imaging in elderly patients using the 64-row multi-detector computed tomography: a correlation study with conventional coronary angiography

Background Elderly patients generally have higher occurrence of coronary calcification, increased heart rate and difficulty with prolonged breath-holding. The aim of our study was to investigate the feasibility and accuracy of using 64-row multi-detector computed tomography (MDCT) in the assessment of coronary artery stenoses in elderly patients. Methods One hundred and fifty two patients with suspected or known coronary artery disease were divided into 4 groups according to their age (Group A: 40-49 years,n=34; Group B: 50-59 years, n=57; Group C: 60-69 years, n=48; Group D: 70 years and above; n=13). Coronary CT angiography (CTA) using a 64-row MDCT was performed and the findings were compared with that of conventional coronary angiography (CCA).Using axial images, multi-planar reconstructions (MPR) and maximum intensity projections (MIP), coronary segments of lumen diameter = 1.5mm were analyzed for the presence of significant stenosis (= 50% ). Results Percentages of poor image quality from coronary CTA preventing reliable correlations with CCA were 21%, 14%, 19% and 62% in Groups A to D respectively. Patients in Group D had significantly higher calcium scores compared with the other groups (P＜0.001). In patients where CTA images were of acceptable quality, percentages of accurate correlations with CCA were 89.8%, 93.4%, 86.6% and 78.0% for Groups A to D *** were no significant difference in serum creatinine, heart rate and contrast volume between the 4 groups. Conclusions The 64-row MDCT coronary angiography was less accurate and feasible for patients aged 70 years or above due to heavy coronary calcification and inability to perform a satisfactory breath-hold. However, a high diagnostic accuracy with the MDCT is possible in patients aged less than 70 years.

Objectives To compare left ventricular ejection fraction (LVEF) determined from 64-row multi-detector computed tomography (64-row MDCT) with those determined from two dimensional echocardiography (2D echo) and cardiac magnetic resonance imaging (CMR). Methods Thirty-two patients with coronary artery disease underwent trans-thoracic 2D echo, CMR and contrast-enhanced 64-row MDCT for assessment of LVEF within 48 hours of each other. 64-row MDCT LVEF was derived using the Syngo Circulation software; CMR LVEF was by Area Length Ejection Fraction (ALEF) and Simpson method and 2D echo LVEF by Simpson *** The LVEF was 49.13 ± 15.91% by 2D echo, 50.72 ± 16.55% (ALEF method) and 47.65 ± 16.58%(Simpson method) by CMR and 50.00 ± 15.93% by 64-row MDCT. LVEF measurements by 64-row MDCT correlated well with LVEF measured with CMR using either the ALEF method (Pearson correlation r = 0.94, P ＜0.01) or Simpson method (r = 0.92, P＜0.01). It also correlated well with LVEF measured using 2D echo (r = 0.80, P ＜ 0.01). Conclusion LVEF measurements by 64-row MDCT correlated well with LVEF measured by CMR and 2D echo. The correlation between 64-row MDCT and CMR was better than the correlation between 2D echo with CMR. Standard data set from a 64-row MDCT coronary study can be reliably used to calculate the LVEF.

High-resolution computed tomography in patients with atypical 'cardiac' chest pain: a study investigating patients at 10-year cardiovascular risks defined by the Framingham and PROCAM scores

Background and objective Atypical 'cardiac' chest pain (ACCP) is not usually caused by myocardial ischaemia. Current noninvasive investigations for these symptoms are not yet as accurate as invasive coronary angiography. The latest 64-row multi-detector computed tomography (MDCT) technology is non-invasive, has high specificity and negative predictive values for the detection of significant coronary disease. Our aim was to investigate if this modality can provide more information in the assessment of outpatients with ACCP in addition to established cardiovascular risk scores. Methods Seventy consecutive patients presenting to the outpatient clinic with ACCP underwent 64-row MDCT scan of the coronary arteries. They were categorized into low, medium or high risk groups based upon the Framingham and PROCAM scores. We defined a clinically abnormal MDCT scan as coronary stenosis =50% or calcium score ＞400 Agatston. Results Fifty-three (75.7%) patients did not have clinically abnormal scans. Framingham score classified 43 patients as low-risk while PROCAM classified 59 patients as low-risk. MDCT scans were abnormal for 18.6% and 22.0% of the respective low-risk group of patients. For patients with medium-to-high risk, 33.3% and 36.4% of Framingham and PROCAM patient groups respectively had abnormal MDCT scans. Conclusion MDCT adds valuable information in the assessment of patients with ACCP by identifying a significant proportion of patients categorized as low-risk to have underlying significant coronary stenosis and coronary calcification by established cardiovascular risk scores.

X-ray source population in the polar ring galaxy NGC 660 as observed by Chandra

We present Chandra observations of the nearby polar ring galaxy NGC 660 to study its X-ray source *** on our analysis,we detected a total of 23 X-ray sources in the 0.5-8 keV band,with luminosities ranging from~1037 to~1039 erg ***-two of these sources are located offnuclear and have luminosities below the ultraluminous X-ray source(ULX)threshold value of L0.5-8 keV<1039 erg s-1,suggesting that they are likely to be X-ray binary(XRB)*** remaining source is located at the center of the galaxy,suggesting it is an active galactic nucleus(AGN).However,we estimated that four of the detected sources could be associated with background *** on the source count rates in each of the Chandra observations,we found evidence for variability in nine of the 23 sources,including the ***,further investigation with spectral analysis suggested no significant differences in the AGN luminosities between the *** X-ray luminosity distribution of the galaxy was found to be generally lower than that expected from previous studies on star forming and collisional ring *** ULX was also detected in the galaxy,in contrast with what was expected from the galaxy’s SFR and metallicity(i.e.,SFR=14.43±0.19 M⊙yr-1 and Z=0.94±0.01 Z⊙,respectively).These results suggest a deficit in the X-ray sources *** on source hardness ratio distribution,we found evidence that the fainter sources have a harder source spectrum,indicating higher *** further suggests that there could be more X-ray sources that were not detected in the galaxy due to significant obscuration.

NGC 4117:A New Compton-thick AGN Revealed by Broadband X-Ray Spectral Analysis

In this paper,we present the broadband(0.5–195 keV)X-ray spectral analysis for one of the newly detected AGNs in the Swift-BAT 105 month Hard X-ray Survey Catalog,NGC *** to our ongoing research on low luminosity AGNs from the Swift-BAT 105 month catalog,we examine whether they are indeed low luminosity or heavily obscured *** of the AGNs in our sample is NGC 4117,where we discover it could be potentially a *** to examine NGC 4117 in detail,we combined the high energy Swift-BAT data with low energy data from XMM-Newton and Swift-XRT,and fitted the spectra simultaneously using physically-motivated models.A high absorption occurred at lower energies,i.e.,below 3 *** studies suggested that NGC 4117was heavily obscured with a large column density(~10^(23)cm^(-2)).Our fitting suggests that this AGN is in the Compton-thick regime with a predicted line-of-sight column density(N_(H,los))of 3.82×1024cm^(-2)and a torus column density(N_(H,tor))of 3.07×10^(24)cm^(-2).Both models yield intrinsic luminosity of 1.03×10^(42)erg s^(-1)at 2–10 keV while our bolometric luminosity is 2.05×10^(43)erg s^(-1),indicating that NGC 4117 is not an intrinsically low luminosity ***,it is a standard *** value of Eddington ratio that we obtained is 0.22,suggesting a very high accretion rate for this AGN.

A geomagnetic storm is a global disturbance of Earth?s magnetosphere,occurring as a result of the interaction with magnetic plasma ejected from the *** considerable research,a comprehensive classification of storms for a complete solar cycle has not yet been fully developed,as most previous studies have been limited to specific storm *** study,therefore,attempted to present complete statistics for solar cycle 24,detailing the occurrence of geomagnetic storm events and classifying them by type of intensity(moderate,intense,and severe),type of complete interval(normal or complex),duration of the recovery phase(rapid or long),and the number of steps in the storm?s *** analysis was applied to data from ground-based magnetometers,which measured the Dst index as provided by the World Data Center for Geomagnetism,Kyoto,*** study identified 211 storm events,comprising moderate(177 events),intense(33 events),and severe(1 event)*** 36%of ICMEs and 23%of CIRs are found to be geoeffective,which caused geomagnetic *** to four-step development of geomagnetic storms was exhibited during the main phase for this solar *** showed the geomagnetic storms developed one or more steps in the main phase,which were probably related to the driver that triggered the geomagnetic storms.A case study was additionally conducted to observe the variations of the ionospheric disturbance dynamo(Ddyn)phenomenon that resulted from the geomagnetic storm event of 2015July *** attenuation of the Ddyn in the equatorial region was analyzed using the H component of geomagnetic field data from stations in the Asian sector(Malaysia and India).The variations in the Ddyn signatures were observed at both stations,with the TIR station(India)showing higher intensity than the LKW station(Malaysia).

In this paper,we present the analysis of the stellar system HIP 101227 to determine the actual number of components in the system,and their *** use dynamical modeling and complex spectrophotometric(involving atmospheric modeling)techniques with recent data,to determine the physical properties and orbital solution for the system,respectively,with better accuracy than past *** on our analysis,we found that the system is more consistent with being a quadruple rather than a binary or a triple system as suggested by previous *** total mass of the system determined from our SED analysis is 3.42±0.20 M,which is distributed almost equally between the four *** stars are found to be zero-age main sequence stars;i.e.,at the last stage of pre-main sequence,with age less than 200 Myr and spectral types *** four stars have very similar physical characteristics,suggesting that the fragmentation process is the most likely theory for the formation and evolution of the system.

Banks＇ Earnings, Risks and Returns in China

This study aims to find the effect of financial risks, price risks and market risks on the Earning Response Coefficients （ERC） for China Commercial Banks. The research methodologies use the traditional cumulative abnormal returns and the unexpected earning as the main dependent and independent variables. The evidences show that：（1） There is a strong returns-to-earnings relation for banks; （2） The liquidity risk has information content beyond earnings changes in the returns-to-earnings relation. This probably due to the reason that managers of banks find the level of liquidity that fulfilled the need of investors and at the same time earns good profits for the banks.

Are Islamic incomes relevance to commercial banks in Malaysia？

A significant price-to-earnings relation is evident in many research findings, which includes study on Malaysia Stock Markets, that the strength, consistency and magnitude of the relation are not as large as those reported in institutionally more developed markets. However, the price adjustment is stronger if price reaction is measured over a long periods, which is consistent to speculative trading. Firm-specific variables such as revenue, firm size and debt-equity have no incremental information content beyond earnings implies that the market monitoring process in an emerging market focuses on earnings only during financial announcement. Therefore, this paper revisited the study on the relevant of accounting banks in Malaysia using the earnings response coefficients, earnings by looking at Islamic Income for commercial which are the earnings-to-share return coefficients that are estimated for sub earnings components by including Islamic banking incomes in the analysis. This paper discovered that though there is a positive effect, the earnings from Islamic banking operation are small. Thus, the total earnings as well as the sub-categories of earnings of banks appear to enter the pricing considerations of investors.

Optical Code Generating Device Using 1×N Asymmetric Hollow Waveguide Couplers

An optical code generating device for security access system application is presented. The code generating device constructed using asymmetric hollow optical waveguide coupler design provides a unique series of output light intensities which are successively used as an optical code. The design of the waveguide is made using two major components which are asymmetric Y-junction splitter and a linear taper. Waveguiding is done using a hollow waveguide structure. Construction of higher level 1×N hollow waveguide coupler is done utilizing a basic 1×2 asymmetric waveguide coupler design together with a cascaded design scheme. Non-sequential ray tracing of the asymmetric hollow optical waveguide couplers is performed to predict the optical transmission properties of the waveguide. A representation of the code combination that can be generated from the device is obtained using combinatory number theory.
