The Association between Preoperative Plasma C-Reactive Protein (CRP) Level and Postoperative Adult Heart Surgery Outcome

Introduction: Careful history and physical examinations are the best ways for preoperative evaluation. Currently, we are recommended to rely on doing them rather than unnecessary and costly laboratory tests for confirmation or diagnosis of disease. The aim of study is the survey of CRP level association to decide further evaluation and expert consultation, newly diagnosed problems and possible effect on postoperative mortality and morbidity. Methods and Materials: In a descriptive retrospective study, hospital documentation of 620 patients older than 18 years undergone heart surgery in Tabriz Shahid Madani hospital was evaluated. Addition to plasma CRP level, patient’s demographic information, type of surgery, preoperative significant tests, delay time in surgery start time after anesthesiology visit, cause of requested specialty consultation and treatment recommendation, postoperative complications and mortality rate were recorded and analyzed. Patients were classified according to preoperative plasma CRP level to five groups as negative, +1, +2, +3 and not measured (i.e. they considered as normal (0 - 5 mg/l), mildly (5 - 40 mg/l), moderately (40 - 200 mg/L) or severely increased (>200 mg/l) groups). Results: Of 620 patients, 402 were male and 218 were female. There was not statistically significant correlation among demographic variables (gender, age, weight, and height), heart disease diagnosis and the type of surgery in five groups. In 79 individuals, they were done specialty consultations that most common of them were neurology consultation because of impaired renal laboratory tests. Only 2 cases were due to high CRP level. In any of cases, this preoperative consultation didn’t result from new disease cases. CRP plasma level hadn’t association with preoperative red blood cell sedimentation level. A systematic review of the efficacy and safety of herbal medicines used in the treatment of obesity

This review focuses on the efficacy and safety of effective herbal medicines in the management of obesity in humans and animals. PubMed, Scopus, Google Scholar, Web of Science, and IranMedex databases were searched up to December 30, 2008. The search terms were ＂obesity＂ and （＇herbal medicine＂ or ＂plant＂, ＂plant medicinal＂ or ＂medicine traditional＂） without narrowing or limiting search elements. All of the human and animal studies on the effects of herbs with the key outcome of change in anthropometric measures such as body weight and waist-hip circumference, body fat, amount of food intake, and appetite were included. In vitro studies, reviews, and letters to editors were excluded. Of the publications identified in the initial database, 915 results were identified and reviewed, and a total of 77 studies were included （19 human and 58 animal studies）. Studies with Cissus quadrangularis （CQ）, Sambucus nigra, Asparagus officinalis, Garcinia atroviridis, ephedra and caffeine, Slimax （extract of several plants including Zingiber officinale and Bofutsushosan） showed a significant decrease in body weight. In 41 animal studies, significant weight loss or inhibition of weight gain was found. No significant adverse effects or mortality were observed except in studies with supplements containing ephedra, caffeine and Bofutsushosan. In conclusion, compounds containing ephedra, CQr ginseng, bitter melon, and zingiber were found to be effective in the management of obesity. Attention to these natural compounds would open a new approach for novel therapeutic and more effective agents.

Iran supports a great share of exotic and/or endemic plant genera and species. The genus Fritillaria(Liliaceae) is a precious part of this botanical richness with 19 species, of which 10 are endemic to the country. However, signs are mounting that the country is truly at a crossroads when it comes to preservation of this national wealth. In this regard, an effective conservation strategy should thoroughly consider the classification of Fritillaria, as conservation practices are compromised by knowledge gaps in systematics and taxonomy. As published studies on Fritillaria in Iran have been sporadic and limited in scope, the aim of this review is to provide information necessary to help bridge these information *** objective is to facilitate increased understanding of the geographic, taxonomic, cytogenetic and phylogenetic status of Iranian Fritilloria, which is vital to meeting the goal of sustainable conservation of the genus in Iran and neighboring areas.

Dry reforming reaction over nickel catalysts supported on nanocrystalline calcium aluminates with different CaO/Al_2O_3 ratios

Nanocrystalline calcium aluminates with different CaO/Al2O3 ratios were prepared by a facile co-precipitation method using Poly（ethylene glycol）-block-poly（propylene glycol）-block-poly（ethylene glycol）（PEG-PPG-PEG, MW： 5800） as a surfactant. They were employed as catalyst support for nickel catalysts in methane reforming with carbon dioxide. The prepared samples were characterized by X-ray diffraction （XRD）, N2 adsorption （BET）, temperature-programmed reduction and oxidation （TPR-TPO）, and scanning electron microscopy （SEM） techniques. Catalysts showed a relatively high catalytic activity and stability. TPR analysis revealed that the catalysts with higher CaO content are more difficult to be reduced. TPO analysis showed that the 5 wt%Ni/CA and 5 wt%Ni/C12A7 catalysts with higher CaO amount were effective against coke deposition.

Microstructural evolution and intermetallic formation in Al-8wt% Si-0.8wt% Fe alloy due to grain refiner and modifier additions

An alloy of A1-8wt% Si-0.8wt% Fe was cast in a metallic die, and its microstrucmral changes due to Ti-B refiner and Sr modifier additions were studied. Apart from usual refinement and modification of the microstructure, some mutual influences of the additives took place, and no mutual poisoning effects by these additives, in combined form, were observed. It was noticed that the dimensions of the iron-rich intermetallics were influenced by the additives causing them to become larger. The needle-shaped intermetallics that were obtained from refiner addition became thicker and longer when adding the modifier. It was also found that a-Al and eutectic silicon phases preferentially nucleate on different types of intermetallic compounds. The more iron content of the intermetallic compounds and the more changes in their dimensions occurred. Formation of the shrinkage porosities was also observed.

Kinetic study on photocatalytic degradation of Acid Orange 52 in a baffled reactor using TiO_2 nanoparticles

In this study, a baffled photocatalytic reactor was used for the treatment of colored wastewater containing the azo dye of Acid Orange 52(AO52). A study on the active species of the photocatalytic process using TiO_2 nanoparticles indicated that hydroxyl radical and superoxide have the greatest contribution to the dye degradation process *** that a level of biological oxygen demand/chemical oxygen demand(BOD5/COD) equal to 0.4 was achieved after about 5 hr from the beginning of the experiment, the reactor seems to be capable of purifying the wastewater containing AO52 dye after this time in order to discharge into a biological treatment system to continue the treatment *** results of the liquid chromatography-mass spectrometry(LC-MS) test showed that during the first 4 hr of the experiment, with the breakdown of the azo bond, the contaminant was decomposed into the benzene annular compounds with less toxicity indicating a reduction in the toxicity of wastewater after removing the dye agent. The study on the kinetics of these reactions followed the pseudo-first-order kinetic model in all conditions and corresponded well to Langmuir–Hinshelwood model. According to the kinetic model for the simultaneous occurrence of possible pathways, the kinetic constant of production and degradation of intermediate products in optimal conditions was estimated to be between 0.0029 and 0.0391 min^(-1).

Numerical simulation of magnetic drug targeting with Eulerian-Lagrangian model and effect of viscosity modification due to diabetics

The targeted drug delivery and targeted drug therapy transport a drug directly to the center of the disease under various conditions and thereby treat it deliber- ately without effects on the body. This paper studies the magnetic drug targeting （MDT） technique by particle tracking in the presence of magnetic field in bifurcation vessels of a healthy person and a diabetes patient. The Lagrangian particle tracking is performed to estimate particle behavior under effects of imposed magnetic field gradients along the bifurcation. The results show that the magnetic field increases the volume fraction of particle in the target region, and the efficiency of MDT on a patient with the diabetes disease is better than a healthy person. Also, for the higher magnetic numbers, the flow in the upper branch is strongly affected by the magnetic field.

Hybrid classifier for fault location in active distribution networks

This paper presents a fast hybrid fault location method for active distribution networks with distributed generation(DG)and *** method uses the voltage and current data from the measurement points at the main substation,and the connection points of DG and *** data is used in a single feedforward artificial neural network(ANN)to estimate the distances to fault from all the measuring points.A k-nearest neighbors(KNN)classifier then interprets the ANN outputs and estimates a single fault *** results validate the accuracy of the fault location method under different fault conditions including fault types,fault points,and fault *** performance is also validated for non-synchronized measurements and measurement errors.

Porosity prediction from seismic inversion of a similarity attribute based on a pseudo-forward equation(PFE):a case study from the North Sea Basin,Netherlands

The objective of this work is to implement a pseudo-forward equation which is called PFE to transform data （similarity attribute） to model parameters （porosity） in a gas reservoir in the F3 block of North Sea. This equation which is an experimental model has unknown constants in its structure; hence, a least square solution is applied to find the best constants. The results derived from solved equa- tions show that the errors on measured data are mapped into the errors of estimated constants; hence, Tikhonov regularization is used to improve the estimated parameters. The results are compared with a conventional method such as cross plotting between acoustic impedance and porosity values to validate the PFE model. When the testing dataset in sand units was used, the correlation coefficient between two variables （actual and predicted values） was obtained as 0.720 and 0.476 for PFE model and cross-plotting analysis, respectively. Therefore, the testing dataset validates rela- tively well the PFE optimized by Tikhonov regularization in sand units of a gas reservoir. The obtained results indi- cate that PFE could provide initial information about sandstone reservoirs. It could estimate reservoir porosity distribution approximately and it highlights bright spots and fault structures such as gas chimneys and salt edges.

Modeling of two-phase particulate flows in a confined jet with a focus on two-way coupling

A computational fluid dynamics model is used for the simulation of laminar flow ofwater-Al2O3 nanofluid in a confined slot impinging jet. The （steady-state and two-dimensional） Eulerian-Lagrangian model is used considering fluid-particle and particle-wall interactions （i.e., two-way coupling）. A collocated grid and the SIMPLE algorithm are used for the coupling of pressure and velocity fields. The deposition model is used to investigate the effect of particle deposition on the impingement surface. Results indicate that the particle trajectory becomes stable farther from the jet with a rising Reynolds number and jet- impingement surface distance ratio. The heat transfer coefficient of the mixture model is higher than that of the Eulerian-Lagrangican model.
