Background: Paraoxonase 1 (PON1) is reported to have an antioxidant and cardioprotective properties. Recently, an association of glutamine (Gln) or (type A)/arginine (Arg) or (type B) polymorphism at position 192 of PON1 gene has been suggested with coronary artery disease (CAD) among patients with diabetes mellitus (DM). However, conflicting results have also been reported. Objectives: To investigate the relationship between PON1 gene (Gln192-Arg) poly-morphism and the presence, extent and severity of CAD in type 2 DM. Methods: The study comprised 180 patients recruited from those undergoing coronary angiography for suspected CAD, who were divided according to the presence or absence of CAD and DM into 4 groups;Group I (n = 40 patients) nondiabetic subjects without CAD, Group II (n = 45 patients) diabetic patients without CAD, Group III (n = 47 patients) non diabetic patients with CAD and Group IV (n = 48 patients) diabetic patients with CAD. PON1 (Gln192-Arg) genotype was assessed using polymerase chain reaction (PCR) followed by AlwI digestion. Results: The frequency of Gln allele (Type A) was significantly higher in group I and group II compared to group III and group IV (62.5%, 60% vs 38.3%, 31.25% respectively, p 100 mg/dL [OR 4.31, CI (1.25 - 12.5), P ele [OR 4.62, CI (1.67 - 13.57), P ele of PON1 192 gene polymorphism is an independent risk factor for CAD and it is associated not only with the presence of CAD but also with its extent and severity and its impact is clearly more pronounced in diabetic patients.

Background: Blood stream infections (BSI) are considered key issues in critical care units. Methicillin resistant Staphylococcus aureus, MRSA-related infections, are considered a major health problem. This is attributed to the emerging new society dangerous strains with continuous antibiotics pressure and fluctuations in resistance patterns. Aim: We aimed to study epidemiology of methicillin resistance S. aureus (MRSA) infections by using conventional phenotypic methods [cefoxitin disk diffusion (CDD) and oxacillin screening agar] and molecular typing of the mec-gene (SCCmec) using multiplex PCR in Suez Canal University Hospital. Methods: 100 non-repetitive staphylococcus aureus were collected and identified morphologically and biochemically by standard laboratory procedures. The strains were considered MRSA if the MIC of oxacillin ≥ 4 μg/ml, and the inhibition zone of cefoxitin was ≤21 mm (CDD). Characterization of SCCmec elements in isolated MRSA strains was done via multiplex-PCR. Results: From total of 100 isolates, eighty were detected as MRSA by using CDD (sensitivity and specificity were 83.6% and 24.4% respectively) and only 65 by using oxacillin screening agar (sensitivity and specificity were 85.5% and 60% respectively). MecA gene was identified in 55 samples;the majority of isolates were SCCmec type IVa (63.7%). Both type I and III of SCCmec couldn’t be detected. Antimicrobial sensitivity rates among SCCmec-V isolates were expectedly higher than those among Type-II isolates. SCCmec type II was characterized by 100% resistant to ciprofloxacin, erythromycin, oxacillin and cefepime as well as greater resistance to clindamycin (70%) with the same pattern between all typing strains (7 strains). Conclusion: SCCmec types IVa and V are generally dominant in our community with no detection of SCCmec types I or III. PCR is the optimum method for MRSA detection.

Grape (Vitis vinifera L.) is assumed as one of the most valuable and popular fruit crops all over the world. In this study, ten of local grape germplasm were characterized considering phenotypic diversity and genetic features under Egyptian conditions. Selected local grapes;Baltim Eswid, Edkawy, Matrouh Eswid, Bez el-Naka, Bez el-Anza, Romy Abiad, Romy Ahmer, Gharibi, Fayoumi and Banaty, were spread geographically among seven national governorates. A total of 58 attributes were characterized grape germplasm for the distinctness of vine parts. For molecular characterization, 9 nuclear SSR loci were analysed. Data revealed a broad sense of similarity at the level of studied morphological attributes, where the highest similarity (0.93) was between Romy Ahmer and Romy Abiad. A total of 24 alleles, ranging from 124 - 253 bp in size, were detected at the nine tested loci with an average of 2.67 alleles per locus. The amplification products in all of the 9 SSRs loci showed polymorphism among the 10 grapevine cultivars. The genetic relatedness among most of the cultivars was in accordance with their identification based on ampelographic properties. Data of phenotypic and molecular analysis revealed high variability of Vitis germplasm in Egypt and contributed substantially to our awareness of valuable local grape genetic resources which are on the verge of extinction.

Corncobs could serve as a substrate for citric acid production using solid state fermentation technique. The culture optimization concerning substrate concentration, culture duration, pH, temperature and substrate hydrolysis was carried out for maximum productivity of citric acid. Under the optimized conditions, 48.4 g of citric acid was produced from 1 kg dry corncobs. Biological evaluation was carried out for citric acid such as melanin synthesis inhibitory, anti-allergy, anti-bacterial, and hyaluronic acid production activities. The results showed that citric acid has potent melanin inhibitory activity, good inhibition for β-hexosaminidase release and potent stimulatory effect for the production of hyaluronic acid. These activities (melanin synthesis inhibitory, anti-allergy and hyaluronic acid productive activities) of citric acid have been reported for the first time.

In this study we report the effect of laser parameters such as laser energy, laser wavelength as well as focusing condition of laser beam on the size and morphology of the gold nanoparticles (GNPs) prepared in deionised water by pulsed laser ablation. The optimum conditions at which gold nanoparticles obtained with controllable average size have been reported as these parameters affected on the size, distribution and absorbance spectrum. Effect of energy was studied. The laser energy was divided into three regions (low, middle and high). A noteworthy change was observed at each region, as the average size changed from 5.9 nm at low energy to 14.4 nm at high energy and the gold nanoparticles reached a critical size of 8 nm at 100 mJ. The Effect of the wavelength on the particle size was examined at 1064 nm, 532 nm. It was found that, the optimum ablation laser wavelength was 1064 nm. Finally, significant results obtained when the effect of focusing conditions studied.

Acute and chronic hyperkalemia is linked to higher hospitalization rates and mortality rates. It has a high prevalence among dialysis and non-dialysis patients in Egypt. The current therapy options are not that ideal. Moreover, we have a critical management paradox in cardiorenal diseases: Should we use the optimum dose of RAASi with its higher incidence of HK, or should we decrease the dose or even stop it with all its harmful hazards? Therefore, in light of the recent updates in different clinical practice guidelines, we, a country-specific (Egypt) task force, gathered to develop a clear, evidence-based, and multi-disciplinary consensus for managing HK. This manuscript illustrates the recommendations of this expert committee. The panel recommends new evidenced K binders like Sodium Zirconium Cyclosilicate (SZC) and patiromer to help manage HK in cardiorenal patients as stated by different international guidelines. In emergency settings, SZC can have a role in managing acute HK;however, it should be used in addition to other drugs like insulin and glucose. Local research studies on the utilization of these novel K binders are highly recommended. The utilization of these novel K binders as prophylaxis should be tested first in a well-designed randomized controlled trial.

Bortezomib-Induced Multiple Chalazia: A Case Report

Bortezomib is a novel agent for the treatment of patients with multiple myeloma. Several cutaneous lesions have been associated with its use and chalazia have been reported in a few cases with a delayed appearance ranging from 1 to 6 months. We describe the case of a 59-year-old man with multiple myeloma IgG kappa, ISS2, who developed a chalazion induced by bortezomib. Following the failure of conservative treatment, a biopsy was performed to exclude extramedullary plasmocytoma. Bortezomib was discontinued due to the persistence of lesions and a therapeutic approach combining systemic antibiotics with incision and curettage was adopted. An active inflammatory lesion developed after 1 month, similar to that observed under bortezomib therapy. Symptoms resolved only 5 months later and the patient remained free of any ocular manifestation. Physicians should be aware that chalazia may be resistant to conventional treatment and lead to local complications, such as cellulitis.

Background: The local population in developing countries is at higher risk of contracting hepatitis B (HBV) due to some of the factors they practice which promote its establishment and distribution within their communities and may constitute a burden for them. Objective: This study aimed to determine the prevalence and risk factors associated with the hepatitis B virus in populations of the Algamosi region, Gezira State, central Sudan. Method: A cross-sectional study was conducted among 492 people, populations of the Algamosi region, Gezira state in central Sudan. A questionnaire was used to collect information on socio-demographic factors, transmission, and prevention of hepatitis B infections. As well, blood samples were taken from each participant, and serum was used for rapid tests for HBsAg. Positive samples were then tested by the elISA method for confirmation. Data were obtained using SPSS version 21 and a P value of less than 0.05 was considered statistically significant. Results: The prevalence of hepatitis B was 16.10% and was higher in men than in women (05.7 - 1.00). People aged 31 to 45 had the highest prevalence of 27 (20,600%) for HBV infection. There was a significant association between HBV and age groups (X2 = 7.816 and P value = 0.05). This study also found that there was a significant association between viral hepatitis and knowledge about transmission and prevention (P value Conclusion: Although most people who live in developing countries are at high risk for transmission of hepatitis B, this indicates the need to implement the screening policy and integrate it with other health services and create awareness such as proper treatment and surveillance for hepatitis infection in all nations.

Hepatitis C (HCV) remains a global health challenge and is transmitted via contact with contaminated blood and body fluids. This study aimed To find the factors influencing hepatitis c viral infections and to identify its prevalence among populations of the Algamosi area, Gezira state central Sudan. This cross-sectional community-based study was conducted. A total of 492 participants were randomly selected from among 4 villages and were interviewed to find out the factors that lead to transmission of infection and tested for markers of hepatitis C infection. elISA confirmed all the positive card tests. The results showed that the individuals within the age groups > 45 and 31 - 45 years are more affected by HCV the percentage was 3.37% and 0.6% respectively, while the age groups of 15 - 30 and p 2 = 14.588 and p value = 0.001) The study concluded that the prevalence of hepatitis C viruses was only detected in three areas. This study explored many possible causes associated with viral hepatitis, tattooing is the common cause of hepatitis C virus. Based on the finding the study recommends, the crucial intervention program to screen most of the Algamosi population who have been not screened for HCV, proper treatment for HCV, and the health education program is strongly recommended to create awareness among the general population.

AIM: To study the preoperative and postoperative role of upper esophagogastroduodenoscopy(EGD) in morbidly obese patients. METHODS: This is a multicenter retrospective study by reviewing the database of patients who underwent bariatric surgery(laparoscopic sleeve gastrectomy, laparoscopic Roux en Y gastric bypass, or laparoscopic minigastric bypass) in the period between 2001 June and 2015 August(Jahra Hospital-Kuwait, Hafr elbatin Hospital and King Saud Medical City-KSA, and Mansoura University Hospital- Egypt). Patients with age 18-65 years, body mass index(BMI) > 40, or > 35 with comorbidities after failure of many dietetic regimen and acceptable levels of surgical risk were included in the study after having an informed signed consent. We retrospectively reviewed the medical charts of all morbidly obese patients. The patients' preoperative data included clinical history including upper digestive symptoms and preoperative full workup including EGD. Only patients whose charts revealed weather they were symptomatic or not were studied. We categorized patients accordingly into two groups; with(group A) or without(group B) upper digestive symptoms. The endoscopic findings were categorized into 4 groups based on predetermined criteria. The medical record of patients who developed stricture, leak or bleeding after bariatric surgery was reviewed. Logestic regression analysis was used to identify preoperative predictors that might be associated with abnormal endoscopic findings. RESULTS: Three thousand, two hundred and nineteen patients in the study period underwent bariatric surgery(75% LSG, 10% LRYDB, and 15% MGB). Mean BMI was 43 ± 13, mean age 37 ± 9 years, 79% were female. Twenty eight percent had presented with upper digestive symptoms(group A). EGD was considered normal in 2414(75%) patients(9% group A vs 66% group B, P = 0.001). The abnormal endoscopic findings were found high in those patients with upper digestive symptoms.
