Coronary allograft vasculopathy (CAV) is an accelerated form of coronary artery disease that is responsible for significant mortality after cardiac transplantation. We report a case of CAV with significant thrombus burden which was managed with mechanical thrombectomy. Both aspiration and mechanical thrombectomy can be safely done in cardiac transplant recipients and may be considered in order to minimize exposure to coronary artery bypass procedure. This is especially valuable in emergency circumstances.

Balloon aortic valvuloplasty is often used as a palliative measure or as a bridge to transcatheter aortic valve replacement in the management of aortic stenosis in high risk or inoperable patients. Severe aortic stenosis coexisting with coronary artery dis-ease is not uncommon. In these circumstances, adjuvant percutaneous coronary intervention may be warranted. The safety and efficacy of combined valve and coronary intervention strategies has been recently studied. An increased incidence of complications when both procedures are performed in the same setting may throw new challenges. We report a case of fatal acute stent thrombosis following balloon aortic valvuloplasty and percutaneous coronary intervention.

New Variant Translocation (8;9;21)(q22;p24;q22) in a Patient with Granulocytic Sarcoma Concurrent with Acute Myeloid Leukemia

Granulocytic sarcoma is a form of acute myeloid leukemia which may occur in any anatomical site. Isolated pancreatic granulocytic sarcoma is however, extremely rare. Translocation t(8;21) is the most common cytogenetic abnormality found in leukemia patients with granulocytic sarcoma and is associated with a relatively good prognosis when treated with chemotherapy. Variants of the t(8;21) are uncommon and account for approximately 3% to 4% of acute myeloid leukemia associated with t(8;21) and are rarely described in acute myeloid leukemia cases associated with granulocytic sarcoma. We report here a patient with acute myeloid leukemia and a novel variant t(8;9;21)(q22;p24;q22) with suspected granulocytic sarcoma in pancreas. A dual-color fluorescence in situ hybridization analysis with RUNX1T1 and RUNX1 probes, revealed the presence of an RUNX1/RUNX1T1 fusion signal in this translocation. To the best of our knowledge, a variant of t(8;21) in GS was rarely described and the involvement of the 9q22 region is the first time described here even in isolated AML-M2. We conclude that further accumulation of similar cases is needed and that genetic exploring of variants of t(8;21) may be helpful for a better understanding of molecular pathogenetic mechanism.

Growth Performance and Digestibility in Karadi Lambs Receiving Different Levels of Pomegranate Peels

The objective of current study was to determine the effect of different ratios of pomegranate peel in diets of Karadi lambs (0%, 1%, 2% or 4% as control, T1, T2 and T3, respectively) on body weight, total body gain, average daily gain, daily feed intake and apparent digestibility of nutrients. Sixteen Karadi male lambs weighing 23.29 ± 0.42 kg and 4.5-5 months old randomly divided into equally four treatment groups and individually penned for the period of 63 days. Results indicated that final body weight was significantly (P 0.75) increased (P < 0.05) significantly in lambs fed 1% pomegranate peel (PP);however, the crude fiber (CF) intake decreased in lambs fed 1% PP as compared to other treatments. While, the dry matter digestibility (DMD), crude protein digestibility (CPD), crude fiber digestibility (CFD) and nitrogen free extract digestibility (NFED) were significantly (P < 0.05) higher in lambs fed 1% PP as compared to T2, T3 and control groups. In addition, EED increased significantly in T3 compared to other treatments. In conclusion, it was indicated that addition of pomegranate peel in diet at the rate of 1% or 2% had significant effect on Karadi lambs performance and digestibility.

Ocular metastasis is an uncommon occurrence that usually has a poor prognosis. The most frequent causes of intraocular metastases as reported in the literature are breast and lung cancers. Intraocular metastasis originating from esophageal cancer is very rare. We discuss a case of 58 years old man known case of adenocarcinoma of the esophagogastric junction presenting with ocular metastasis in King Abdulaziz Medical City, Jeddah, Saudi Arabia. Clinical symptoms led to the investigations and diagnosis of the ocular metastasis which without these symptoms it would have been missed. We concluded that a careful and good review of the whole body is essential in the optimal diagnosis of metastatic lesions especially when it comes to a rare metastasis like ocular metastasis from esophageal cancer.

Psychometrics of the SDQ-I for Palestinian Adolescent Students

The Self-description Questionnaire-I(SDQ-I)is a multidimensional instrument that measures eight self-concept facets hypothesized in Shavelson’s hierarchical *** study investigated self-concept in a sample of Palestinian adolescent students using an Arabic version of the ***-hundred sixty adolescents(163 girls and 197 boys)aged 13 to 16 years(M=14.3,SD=.87)*** 72-item SDQ-I was administered in four Palestinian schools to assess the psychometric properties of the *** included the factor structure and the internal consistency reliability of the SDQ-I subscales and mean score responses of Palestinian *** analysis results,which accounted for the majority of the variance,supported an underlying general self-concept factor structure that demonstrated the eight factors that the SDQ-I is designed to *** is consistent with previous studies in similar age groups and the SDQ-I reliabilities were similar to those reported in the *** perceived total self-concept positively(mean=3.71).Three facets of self-concept(parent relations,reading,and general self-concept)indicated high positive *** among the different dimensions were consistent with the hierarchical structure in Shavelson’s ***,the findings provided compelling support for Shavelson’s model,and the structure validity of Western self-concept *** were provided for the discrepancies regarding the Palestinian-Arab culture.

Bilothorax a Rare Incidence in Thoracic Surgery: Case Report and Literature Review

Bilothorax or Cholethorax is the accumulation of bilious effusion in the thorax. It is a rare condition in which bile passes into the pleural space from the abdominal cavity, necessitating urgent treatment. This article aims to present a case of bilothorax treated successfully in our institute and review the literature on this topic to better understand this medical problem and the best ways of dealing with it. We presented in this article a case of bilothorax after an attempt of biliary drainage in a known pancreatic cancer patient and the way we managed it. We concluded about the importance of the early recognition of this medical problem and the necessity of aggressive and early management to prevent further morbidities and mortality.
