Objective： The aim of our.study was to evaluate the efficacy and safety of gemcitabine and cisplatin in patients with pr.viously untr.ated advanced malignant pleur.l mesothelioma （MPM）. Methods： Thir.y-thr.e eligible patients with histologically pr.ven advanced MPM with ECOG PS of 〈 2 and had adequate liver.and kidney functions r.ceived gemcitabine （1000 mg/m2） on days 1 and 8 combined with cisplatin （80 mg/m^2） on day 1. r.sults： The major.ty of the study gr.up wer. males and constituted 87.9% （n = 29）, while females constituted 12.1% （n = 4）. The median age was 52 year. and r.nged fr.m 37 to 69 year.. r.gar.ing SWOG per.or.ance status, 5 patients wer. PS 0, 21 patients wer. PS 1 and 7 patients wer. PS 2. r.gar.ing pathological subtypes, epithelial histology r.pr.sented 84.8% of the study （n = 28） while sar.omatoid type r.pr.sented 6% （n = 2） and biphasic type accounted for.9% （n = 3）. r.gar.ing staging accor.ing to IMIG staging system, 12.1% of patients wer. stage Ⅱ, 54.5% wer. stage Ⅲand 33.3% wer. stage Ⅳ. Only one patient （3%） had attained a complete r.sponse, 18 patients （54.5%） showed par.ial r.sponse, 12 patients （36.4%） showed stationar. disease, while 2 patients （6.1%） showed pr.gr.ssive disease. The cor.elation between r.sponse r.te obtained and PS was statistically significant （P = 0.029）. After.a median follow-up of 14.4 months, the median sur.ival time was 20.3 months （95% Cl： 14.58-26.01 months） and the pr.gr.ssion fr.e sur.ival time was 11.8 months （95% CI： 10.76-12.83 months） Conclusion： The combination of gemcitabine and cisplatin is an active and safe tr.atment in patients with MPM. Fur.her.lar.er.compar.tive studies ar. war.anted to document this finding.

Maximum andr.gen blockade （MAB）, consisting of an antiandr.gen plus either.a luteinizing hor.one- r.leasing hor.one agonist （LHr.A） or.or.hiectomy, is a standar. car. for.patients with pr.state cancer. Although, clinical tr.al r.sults have been equivocal, none has shown a significant advantage in favor.of MAB over.castr.tion alone in metastatic pr.state cancer.and MAB has been the subject of consider.ble contr.ver.y. The aim of this study was to compar. MAB （or.hiectomy or.LHr.A＂Goser.lin＂） and anti-andr.gen ＂Bicalutamide＂ with castr.tion alone （or.hiectomy or.LHr.A） in pr.vi- ously untr.ated metastatic pr.state cancer.patients. Methods： Hundr.d eligible patients with adequate per.or.ance status and adequate hematologic, hepatic and r.nal functions wer. included. MAB ar., fifty patients under.ent castr.tion either.sur.ically by or.hiectomy or.medically by r.ceiving Goser.lin （3.6 r.g） depot, which was injected subcutaneously ever. 28 days plus bicalutamide 50 mg once daily. Castr.tion alone ar., fifty patients under.ent castr.tion alone either.sur.ically by or.hiectomy or.medically by r.ceiving Goser.lin （3.6 mg） depot. r.sults： Dur.ng the per.od fr.m Januar. 2011 to Januar. 2013, with a median follow up of 18 months （r.nge 6 to 24 months）, ther. wer. eight deaths （16%）, in MAB ar. and ten deaths （20%） in castr.tion alone ar.. At thr.e months, ther. wer. 35 patients （70%） with pr.state specific antigen （PSA） nor.alization （-〈 4 mg/dL） in MAB ar. ver.us 17 patients （34%） with PSA nor.alization in castr.tion alone ar. （P = 0.001）. The median pr.gr.ssion fr.e sur.ival （PFS） times wer. 22.18 months （95% CI, 19.7 to 24.2 months） for.MAB ar. ver.us 22 months in castr.tion alone ar. （95% CI, 18 to 25.9 months; P = 0.045）. The sur.ival r.tes for.MAB ar. wer. 82% at 18 months and 70.6% at 24 months ver.us 78.7% at 18 months and 75.1% at 24 months in castr.tion alone ar. （P 〉 0.05）.
