This paper aims to examine the influence of thermal radiation on magnetohydrodynamics(MHD)convective two-phase flows through the asymmetric divergent channel under first-order velocity and thermal slip *** transformations are used to nondimensionalize the governed partial differential equations and then solved them numerically.A grid-independent test is done to check the consistency of the numerical method and then validated with existing published results in a limiting *** graphical illustrations are provided to explain the impacts of various emerging parameters on flow *** enhancement in particle velocity is observed with an increment in the velocity slip *** the temperature slip parameter improves a decrement in the fluid as well as particle temperature is *** increment in radiation parameter augments fluid *** type of flow has multifarious range applications like fluidized beds,gas cooling systems,polishing heart valves and internal cavities,etc.

Myositis ossificans is defined as formation of bone at the site of injured muscle. It is one of the rare complications of supracondylar fracture of humerus in children. Myositis mass usually develops on the anterior aspect in the brachialis muscle and produces restriction of range of motion, but complete ankylosis is rare. To the best of our knowledge, this is the first case to be reported in the literature as a consequence of myositis ossificans traumatica. In this case, a six-year-old child presented to the casualtydepartment with pain in the right elbow after a fail on outstretched hand during play. After surgical excision through the anterior approach, the child had no symptoms referable to the elbow and a residual flexion deformity of 15 degrees with further painless flexion up to l00 degrees at last followup of one year after surgery.

Comparison of outcomes for patients with primary sclerosing cholangitis associated with ulcerative colitis and Crohn’s disease

Background:The comparative outcomes of ulcerative colitis(UC)and Crohn’s disease(CD)in patients with primary sclerosing cholangitis(PSC)are unclear;the aim of our study was to make an objective ***:A total of 273 patients with PSC and inflammatory bowel disease(223 with UC and 50 with CD)were *** and demographic variables were ***:The PSC risk score was similar for both *** median follow-up period in patients with PSC-UC was 12 years(range 0–38)and that for PSC-CD was 14 years(range 1–36).The median number of disease flares per year was higher in PSC-UC patients than in the PSC-CD group[1vs.0(ranges 0–20 and 0–9,respectively);P<0.001].More patients with UC developed colon neoplasia than CD(35.9%vs.18%;P=0.009).On proportional hazards analysis for the risk of colectomy,UC patients had a 12%higher risk for colectomy[hazard ratio(HR)=0.88;95%confidence interval(CI)0.51–1.51;P=0.64].Liver transplantation for PSC was associated with decreased risk(HR=0.57;95%CI 0.37–0.89;P=0.013),while colon neoplasia increased the risk(HR=3.83;95%CI 2.63–5.58;P<0.001)for *** proportional hazards analysis for the risk of colon neoplasia,UC patients had 56%higher risk of developing colon neoplasia than CD(HR=0.44;95%CI 0.16–1.25;P=0.12).Conclusions:PSC patients with CD appear to be associated with a lower risk of colon neoplasia and colectomy than PSC patients with UC.

Background: Mental retardation is an important condition in children. People in the community need to be aware of the presence, need to act and prevent this problem. Mental retardation means mental growth of the child is not at par with physical growth. Mental retardation is calculated as intelligence quationent. (IQ) = Mental age/chronological age × 100. There are causes and can be categorized as prenatal, natal and postnatal factors. Mental retardation is classified as Mild Mental retardation IQ of 51 - 70;Moderate Mental retardation IQ of 36 - 50;Severe Mental retardation IQ of 21 - 35;Profound Mental retardation IQ of less than 20. IQ of 71 - 89 is designated as borderline mental retardation. Objectives: Analyzing the procured data creates awareness in the society regarding mental retardation and its burden to the society. Results: The analysis is done using the data provided from the office of Deputy Director, Department of Public Instruction, Sarvashikshana Abhiyana (SSA) Shimoga District [1]. 555 children were having mental retardation ranging from mild to profound severity. The total number of children with various disabilities was 1185, mental retardation alone was nearing 50% of the total disabilities. Conclusion: 50% of total disability was found to be mental retardation which needs to proliferate.

Introduction: Incidence of spondylolisthesis in general population is 5% - 7%. No matter what the etiology is, patients usually have significant functional disability. Few studies have investigated the long term effect of posterolateral fusion on functional outcome. Objectives: To study the efficacy of posterolateral fusion in spondylolisthesis especially in terms of functional outcome. Methodology: From July 2010 to June 2012, a total of 86 patients, operated with postero-lateral fusion were followed up and evaluated based on VAS for low back pain, ODI and neurological deficits. Results: Follow up was 83% of original study population (86). Average follow up was 13 months. The mean difference between pre-operative and post-operative VAS at final follow up was 3.5 cms (SD = 2.94);ODI was 28% at 4 months and 36% at 8 months. Claudication pain relieved in all;functional outcome was good in 67%, fair in 27.5% and failed in 5.5%;75% had fusion at an average of 5.5 months. Conclusion: Posteriolateral fusion is still a safe, promising and appealing technique.

Microscopic colitis: Is it a spectrum of inflammatory bowel disease?

Lymphocytic and collagenous colitis are forms of microscopic colitis which typically presents in elderly patients as chronic watery diarrhea. The association between microscopic colitis and inflammatory bowel disease is weak and unclear. Lymphocytic colitis progressing to ulcerative colitis has been previously reported; however there is limited data on ulcerative colitis evolving into microscopic (lymphocytic or collagenous) colitis. We report a series of six patients with documented ulcerative colitis who subsequently were diagnosed with collagenous colitis or lymphocytic colitis suggesting microscopic colitis could be a part of the spectrum of inflammatory bowel disease. The median duration of ulcerative colitis prior to being diagnosed with microscopic colitis was 15 years. We noted complete histological and/or symptomatic remission in three out of six cases while the other three patients reverted back into ulcerative colitis suggesting lymphocytic or collagenous colitis could present as a continuum of ulcerative colitis. The exact molecular mechanism of this histological transformation or the prognostic implications is still unclear. Till then it might be prudent to follow up these patients to assess for the relapse of inflammatory bowel disease as well as for dysplasia surveillance.

Factors predicting adverse short-term outcomes in patients with acute cholangitis undergoing ERCP: A single center experience

AIM: To identify potential factors that can predict adverse short-term outcomes in patients with acute cholangitis undergoing endoscopic retrograde cholangiopancreatography(ERCP). METHODS: Retrospective analysis of consecutive patients admitted to our center for acute cholangitis and underwent ERCP from 2001 to 2012. Involvement of two or more organ systems was termed as organ failure(OF). Cardiovascular failure was defined based on a systolic blood pressure of 1.9 mg/dL. Variables associated with short term adverse clinical outcomes defined as persistent OF and/or 30-d mortality was determined. RESULTS: A total of 172 patients(median age 62 years, 56.4% female) were included. The median door to ERCP time was 17 h. Bile duct stones were the most common etiology(n = 67, 39.2%). In multivariate analysis, factors that were independently associated with persistent OF and/or 30-d mortality included American Society of Anesthesiology(ASA) physical classification score > 3(OR = 7.70; 95%CI: 2.73-24.40), presence of systemic inflammatory response syndrome(OR = 3.67; 95%CI: 1.34-10.3) and door to ERCP time greater than 72 h(OR = 3.36; 95%CI: 1.12-10.20). Door to ERCP time greater than 72 h was also associated with 70% increase in the mean length of stay(P < 0.001). Every one point increase in the ASA physical classification and every 1 mg/dL increase in the preERCP bilirubin level was associated with a 34% and 2% increase in the mean length of hospital stay, respectively. Transfer status did not impact clinical outcomes. CONCLUSION: Higher ASA physical classification and delays in ERCP are associated with adverse clinical outcomes and prolonged length of hospital stay in patients with acute cholangitis undergo
