Increased postoperative complications after protective ileostomy closure delay: An institutional study

AIM: To study the morbidity and complications as-sociated to ileostomy reversal in colorectal surgery pa-tients, and if these are related to the time of closure. METHODS: A retrospective analysis of 93 patients, who had undergone elective ileostomy closure between 2009 and 2013 was performed. Demographic, clinical and surgical variables were reviewed for analysis. All complications were recorded, and classified according to the Clavien-Dindo Classification. Statistical univariate and multivariate analysis was performed, setting a P value of 0.05 for significance.RESULTS: The patients had a mean age of 60.3 years, 58% male. The main procedure for ileostomy cre-ation was rectal cancer(56%), and 37% had received preoperative chemo-radiotherapy. The average delay from creation to closure of the ileostomy was 10.3 mo. Postoperative complications occurred in 40% of the pa-tients, with 1% mortality. The most frequent were ileus(13%) and wound infection(13%). Pseudomembra-nous colitis appeared in 4%. Increased postoperative complications were associated with delay in ileostomyclosure(P = 0.041). Male patients had more complica-tions(P = 0.042), mainly wound infections(P = 0.007). Pseudomembranous colitis was also associated with the delay in ileostomy closure(P = 0.003). End-to-end in-testinal anastomosis without resection was significantly associated with postoperative ileus(P = 0.037). CONCLUSION: Although closure of a protective il-eostomy is a fairly common surgical procedure, it has a high rate of complications, and this must be taken into account when the indication is made. The delay in stoma closure can increase the rate of complications in general, and specifically wound infections and colitis.

Lower serum expression of miR-181c-5p is associated with increased plasma levels of amyloid-beta 1-40 and cerebral vulnerability in normal aging

Background:Previous studies have shown that expression levels of miR-181c are downregulated by amyloid-β(Aβ)deposition and chronic cerebral hypoperfusion,both factors largely associated with the development of AD.Moreover,reduced 2-[18F]fluoro-2-deoxy-D-glucose(FDG)-PET brain metabolism and volume loss of regions of the medial temporal lobe have been generally recognized as hallmarks of AD.Based on this evidence,we have here investigated potential associations between serum levels of miR-181c-5p and these AD signatures in asymptomatic elderly subjects.Methods:Ninety-five normal elderly subjects underwent clinical,cognitive,structural MRI,and FDG-PET explorations.Serum expression levels of miR-181c-5p and plasma Aβ concentrations were further analyzed in this cohort.Regression analyses were performed to assess associations between serum miR-181c-5p levels and cognitive functioning,plasma Aβ,structural and metabolic brain changes.Results:Decreased serum expression of miR-181c-5p was associated with increased plasma levels of Aβ1–40,deficits in cortical glucose metabolism,and volume reduction of the entorhinal cortex.No significant associations were found between lower miR-181c-5p levels and cognitive deficits or cortical thinning.Conclusions:These findings suggest that deregulation of serum miR-181c-5p may indicate cerebral vulnerability in late life.

Emerging evidence suggests that dietary fats may influence testicular function. However, most of the published literature on this field has used semen quality parameters as the only proxy for testicular function. We examined the association of fat intake with circulating reproductive hormone levels and testicular volume among healthy young Spanish men. This is a cross-sectional study among 209 healthy male volunteers conducted between October 2010 and November 2011 in Murcia Region of Spain. Participants completed questionnaires on lifestyle, diet, and smoking, and each underwent a physical examination, and provided a blood sample. Linear regression was used to examine the association between each fatty acid type and reproductive hormone levels and testicular volumes. Monounsaturated fatty acids intake was inversely associated with serum blood levels of calculated free testosterone, total testosterone, and inhibin B. A positive association was observed between the intake of polyunsaturated fatty acids, particularly of omega-6 polyunsaturated fatty acids, and luteinizing hormone concentrations. In addition, the intake of trans fatty acids was associated with lower total testosterone and calculated free testosterone concentrations （Ptrend = 0.01 and 0.02, respectively）. The intake of omega-3 polyunsaturated fatty acids was positively related to testicular volume while the intake of omega-6 polyunsaturated fatty acids and trans fatty acids was inversely related to testicular volume. These data suggest that fat intake, and particularly intake of omega 3, omega 6, and trans fatty acids, may influence testicular function.

BACKGROUND Colorectal cancer(CRC)accounts for 9.4%of overall cancer deaths,ranking second after lung cancer.Despite the large number of factors tested to predict their outcome,most patients with similar variables show big differences in survival.Moreover,right-sided CRC(RCRC)and left-sided CRC(LCRC)patients exhibit large differences in outcome after surgical intervention as assessed by preoperative blood leukocyte status.We hypothesised that stronger indexes than circulating(blood)leukocyte ratios to predict RCRC and LCRC patient outcomes will result from combining both circulating and infiltrated(tumour/peritumour fixed tissues)concentrations of leukocytes.AIM To seek variables involving leukocyte balances in peripheral blood and tumour tissues and to predict the outcome of CRC patients.METHODS Sixty-five patients diagnosed with colon adenocarcinoma by the Digestive Surgery Service of the La Paz University Hospital(Madrid,Spain)were enrolled in this study:43 with RCRC and 22 with LCRC.Patients were followed-up from January 2017 to March 2021 to record overall survival(OS)and recurrence-free survival(RFS)after surgical interventions.Leukocyte concentrations in peripheral blood were determined by routine laboratory protocols.Paraffin-fixed samples of tumour and peritumoural tissues were assessed for leukocyte concentrations by immunohistochemical detection of CD4,CD8,and CD14 marker expression.Ratios of leukocyte concentration in blood and tissues were calculated and evaluated for their predictor values for OS and RFS with Spearman correlations and Cox univariate and multivariate proportional hazards regression,followed by the calculation of the receiver-operating characteristic and area under the curve(AUC)and the determination of Youden’s optimal cutoff values for those variables that significantly correlated with either RCRC or LCRC patient outcomes.RCRC patients from the cohort were randomly assigned to modelling and validation sets,and clinician-friendly nomograms were developed to predict OS and RFS from the

Cytotoxic Activity of Thelesperma megapotamicumOrganic Fractions against MCF-7 Human Breast Cancer Cell Line

Thelesperma megapotamicum (Asteraceae) is commonly used in Argentine to treat various diseases (renal, digestive affections, and as anaesthesia). The present study showed the mechanisms involved “in vitro” cytotoxicity of T. megapotamicum Fractions. Five Fractions (F1 - F5) were separated by column chromatography (Silica gel) using hexane:diethyl ether as eluents. Viability was evaluated in Human breast carcinoma cell line (MCF-7) by staining with crystal violet. With respect to F1 Fraction treatment, the cell survival was 49.14% ± 8.87%, while the F2 and F3 ones exhibited a strong reduction of cell viability to only 26.35% ± 1.63% and 23.3%1 ± 0.53% of the control cell at 50 μg/ml, respectively. Apoptotic effect of these Fractions was detected using FITC-labeled Annexin V and propidium iodide binding assays and was confirmed by a higher proportion of apoptotic cells due to F2 and F3 treatments. T. megapotamicum active Fractions could facilitate the tumoral cells death by decreasing the activity of the enzyme Gamma-glutamyltranspeptidase and causing alteration in cell membrane sialoglycoconjugates and others involved anticancer mechanisms including apoptosis.

脂肪含量是制革企业可以控制的变量，根据商业用途的不同对脂肪含量的控制要求也不同。为了保证生产高品质的皮革产品，通常需要使用一定的脱脂措施，尤其是在原料皮的天然脂肪较丰富的情况下。IUC4作为测定皮革脂肪含量的标准方法，但不能快速的给出结果；同时方法采用产生污染的试剂以及包括粉末样品用弱极性溶剂的Soxhlet抽提。配有纤维-光学探测器的NIR漫反射光谱和多元校准的联用将会是直接测定皮革中脂肪含量以及皮革脱脂监测的最好方法。在本研究中，我们开发了可以迅速测定皮革中脂肪含量并且可以控制脱脂工艺的方法，使用该方法无需样品处理。脱脂实验采用的皮革样品是不同种类及血统的小羊，这样为的是扩大测试对象的范围，掌握不同类羊皮的性质以及天然脂肪含量。建立校准矩阵的NIR光谱在脱脂前后直接对皮革样品记录。脂肪含量采用的数值通过参考文献中的标准方法测得，通过偏最小二乘回归确定。尽管皮革复杂的天然性，使用的标准模型提供了较好的客观信息：对天然和脱脂样品进行单一的校准矩阵，其最大的全部相对误差为10％。所提出的方法优于标准方法。

BACKGROUND The outcomes of Hodgkin's lymphoma(HL)in Mexico have not been widely reported.Simplified and affordable treatments have been adopted in middleincome countries.AIM The aim was to evaluate long-used therapies for HL in Mexico in a long-term basis.METHODS In a 34-year time period,88 patients with HL were treated at a single institution in Mexico.Patients were treated with adriamycin bleomycin vinblastine and dacarbazine(ABVD)or mechlorethamine,vincristine,procarbazine,and prednisone(MOPP).Relapsed or refractory patients were given ifosfamide,carboplatin,and etoposide(ICE)followed by autologous or allogeneic stem cell transplants.RESULTS Thirty-seven women and 51 men were included;the median age was 29 years.Patients were followed for a mean of 128 mo.The 310-mo overall survival(OS)was 83%for patients treated with MOPP and 88%for those treated with ABVD.The OS of patients who received autologous stem cell transplantation was 76%(330 mo)vs 93%(402 mo)in those who did not.CONCLUSION HL may be less aggressive in Mexican population than in Caucasians.Combined chemotherapy renders acceptable results,regardless of clinical stage.

In this work, the localization, density, morphology and ultrastructure of secretory structures in aerial organs of Flourensia campestris (FC) and F. oolepis (FO) (Asteraceae) by means of a combination of light, fluorescence, transmission (TEM) and scanning electron microscopy (SEM) were examined. The possible role of secretory structures in the production and secretion of the phytotoxic sesquiterpene (-)-hamanasic acid A ((-)HAA) in both species was also assessed. Capitate glandular trichomes were found in all reproductive organs of FC and FO, and were being reported for the first time. These glandular trichomes, typically associated to edges and veins, were of the same type as those already described for vegetative organs, and were abundant in involucral bracts and corolla of tubulose and ligulate flowers. Their density in reproductive organs of both species was similar (ca. 30/mm2) and lower than that found in leaves (ca. 100/mm2) and stems (ca. 160/mm2 in FC, and up to 650/mm2 in FO). Glandular trichomes in vegetative organs followed a species-specific pattern of distribution. TEM and SEM observations suggest that each species differs in the way in which secretory materials are released to the outside: through cracks or pores in FC, or through a loose cuticle in FO. Similar inspections of the secretory ducts revealed lipophilic vacuoles localized in subepithelial and epithelial cells, in which secretions accumulated before being transferred to the duct. The presence of wall ingrowths in subepithelial cells suggests that granulocrine secretion operates in these species. Secretory ducts varied in density and diameter among the organs in both species, with the combination being maximal in woody stems. (-)HAA was only detected in surface secreted resins of both species, and its concentration (2D-TLC, GC-FID) was intimately associated with the distribution and density of glandular trichomes in each organ (capitula, leaves, and stems with primary or secondary growth). The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced background.This paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS experiment.Two taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet pseudorapidity.Differences in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.
