Background:Pressure ulcers are one of the most common and dangerous sequelae of spinal cord *** ulcers treatment represents one of the most challenging clinical problems faced by ***:This report examines a case of complicated stage-4 pressure ulcers in a young adult who had gunshot injury in his spine above T-11 level treated following a multidisciplinary approach.A multidisciplinary team formulated a multifaceted program that includes meticulous pressure relief program,wound care and flap reconstructive surgery,osteomyelitis treatment,nutrition optimization,post-operative physiotherapy,patient education,and psychological support,that recruited for this patient over the course of his stay at our ***:During the third day postoperatively showed good blood circulation without necrosis or hematoma and site of ulcer completely has ***:Implementation of a multidisciplinary approach was key to optimizing surgical outcomes in spinal cord injury patients who have pressure sores,achieving a low recurrence rate and reducing readmissions.

The sweetpotato whitefly Bemisia tabaci（Hemiptera：Aleyrodidae）is an extremely polyphagous insect pest that causes significant crop losses in Israel and *** is a species complex of which the B and Q biotypes are the most widespread and damaging *** change in biotype composition and resistance to insecticide in Israel was monitored during the years 2008-2010 to identify patterns in population dynamics that can be correlated with resistance *** results show that B biotype populations dominate crops grown in open fields,while Q biotype populations gradually dominate crops grown in protected conditions such as greenhouses and nethouses,where resistance outbreaks usually develop after several insecticide *** in previous years,Q biotype populations were widely detected in many regions in Israel,significant domination of the B biotype across populations collected was observed during the year 2010,indicating the instability of the *** population from one year to *** for the changing dynamics and the shift in the relative abundance of *** biotype,and their resistance status,are discussed.

Probiotics use has increased tremendously over the past ten *** was coupled with a surge of data relating their importance in clinical *** diarrhea,whose frequency has risen recently,was one of the earliest targets with data published more than ten years ***,available trials suffer from severe discrepancies associated with variability and heterogeneity of several *** published randomized controlled trials and subsequent meta-analyses suggest benefit for probiotics in the prevention of antibiotic-associated *** same seems to also apply when the data is examined for Clostridium difficile-associated ***,the largest randomized double-blind placebo-controlled trial to date examining the use of a certain preparation of probiotics in antibiotic-associated diarrhea showed disappointing results,but it was flawed with several *** commonest species of probiotics studied across most trials is Lactobacillus;however,other types have also shown similar *** have enjoyed an impeccable safety *** a few reports of severe infections sometimes leading to septicemia,most of the available trials confirm their harmless behavior and show similaradverse events compared to *** a consensus dictating its use is still lacking,it would be advisable at this point to suggest prophylactic use of probiotics to certain patients at risk for antibiotic-associated diarrhea or to those who suffered previous episodes.

AIM:To elucidate surgical outcomes of pancreaticoduodenectomy(PD)in patients with liver ***:We studied retrospectively all patients who underwent PD in our centre between January 2002and December *** A comprised patients with cirrhotic livers,and Group B comprised patients with non-cirrhotic *** cirrhotic patients had ChildPugh classes A and B(patient’s score less than 8).Preoperative demographic data,intra-operative data and postoperative details were *** primary outcome measure was hospital mortality *** outcomes analysed included duration of the operation,postoperative hospital stay,postoperative morbidity and survival ***:Only 67/442 patients(15.2%)had cirrhotic *** blood loss and blood transfusion were significantly higher in group A(P=0.0001).The mean surgical time in group A was significantly longer than that in group B(P=0.0001).Wound complications(P=0.02),internal haemorrhage(P=0.05),pancreatic fistula(P=0.02)and hospital mortality(P=0.0001)were significantly higher in the cirrhotic *** stay was significantly longer in group A(P=0.03).The median survival was 19 mo in group A and 24 mo in group *** hypertension(PHT)was present in 16/67 cases of cirrhosis(23.9%).The intraoperative blood loss and blood transfusion were significantly higher in patients with PHT(P=0.001).Postoperative morbidity(0.07)and hospital mortality(P=0.007)were higher in cirrhotic patients with ***:Patients with periampullary tumours and well-compensated chronic liver disease should be routinely considered for PD at high volume centres with available expertise to manage liver *** is associated with an increased risk of postoperative morbidity in patients with liver cirrhosis;therefore,it is only recommended in patients with Child A cirrhosis without portal hypertension.

Background: Coronavirus disease 2019 which is officially known as COVID-19 belongs to family viruses. COVID-19 manifestations vary among affected people. These symptoms may become more serious among patients suffering from chronic disease and those who are on treatment which may af-fect their defense mechanism or immune-compromised patients who become more vulnerable to complications of COVID-19, and at high risk for morbidity and mortality with any bacterial or viral illness. Method: A retrospective, non-experimental research design was applied with a quantitative approach among patients with COVID-19 who were admitted to COVID-19 department at AVH with a total of 72 patients. Data were extracted from a patients’ elec-tronic medical record. Results: During COVID-19 outbreak 72 patients were admitted to COVID department at AVH, 54.2% were female and 33% of study participants were from Gaza governorate followed by Jerusalem 27%. Most participants 34.7% had first clinic visit after two days from the onset of COVID-19 symptoms. 45.8% were discharged to home while 13.9% died. All inflammatory markers that include ferritin, C-RP, IL-6 and D-dimer are in-creasing in all patients that were admitted to hospital;IL-6 and D-dimer were significant inflammatory markers in relation to the mortality rate. The study found the risk of mortality with IL-6 mean (218.5), and D-dimer mean (12). Furthermore there was a relation between increased risk of mortality and im-mune comprised. Conclusion: Mortality rate increased among COVID-19 pa-tients when IL-6 was higher than 218.5 and D-dimer higher than 12, and there was a relationship between increased risk of mortality and immune comprised.

The reaction of picoline derivatives 3-6 with hydrazonoylhalide 1a produced imidazo[1,2-a]pyridines 7-10, while the reaction of the same picoline derivatives with hydrazonoylhalide 1b afforded imidazo[1,2-a]pyridine-2-ones 11-13. The reaction of 1b, c with 3-amino-1,2,4-triazole 14 produced the acyclic adducts 18 and 19, respectively. Reaction of 1b, 1c with 5-aminotetrazole 20 produced the acyclic products 23 and 24, respectively. Finally, the reaction of 1b with 4, 6-dimethyl-2-aminopyrimidine 27 afforded compound 29 rather than its isomeric structure 28. The structure of the products was confirmed by the different spectroscopic analytical methods including IR, MS, 1HNMR and 13CNMR.

AIM: To compare the performance of the re-expressed Modification of Diet in Renal Disease equation vs the new Chronic Kidney Disease Epidemiology Collaboration equation in patients with non-valvular atrial ***: We studied 911 consecutive patients with non-valvular atrial fibrillation on vitamin-K antagonist. The performance of the re-expressed Modification of Diet in Renal Disease equation vs the new Chronic Kidney Disease Epidemiology Collaboration equation in patients with non-valvular atrial fibrillation with respect to either a composite endpoint of major bleeding, thromboembolic events and all-cause mortality or each individual component of the composite endpoint was assessed using continuous and categorical ≥ 60, 59-30, and < 30 m L/min per 1.73 m2 estimated glomerular filtration ***: During 10 ± 3 mo, the composite endpoint occurred in 98(10.8%) patients: 30 patients developed major bleeding, 18 had thromboembolic events, and 60 died. The new equation provided lower prevalence of renal dysfunction < 60 m L/min per 1.73 m2(32.9%),compared with the re-expressed equation(34.1%). Estimated glomerular filtration rate from both equations was independent predictor of composite endpoint(HR = 0.98 and 0.97 for the re-expressed and the new equation, respectively; P < 0.0001) and all-cause mortality(HR = 0.98 for both equations, P < 0.01). Strong association with thromboembolic events was observed only when estimated glomerular filtration rate was < 30 m L/min per 1.73 m2: HR is 5.1 for the re-expressed equation, and HR = 5.0 for the new equation. No significant association with major bleeding was observed for both ***: The new equation reduced the prevalence of renal dysfunction. Both equations performed similarly in predicting major adverse outcomes.
