Hepatoprotection: A Hallmark of <i>Citrullus colocynthis</i>L. against Paracetamol Induced Hepatotoxicity in Swiss Albino Rats

Objective: To demonstrate the in-vivo hepatoprotective effect of the ethanolic extracts of Citrullus colocynthis (Linn.) against paracetamol induced hepatotoxicity in albino rats. Animal Model: Swiss Albino rats of either sex were used, divided into six groups with six in each group. Group 1-Normal control: The animals were maintained under normal control, which were given distilled water only. Group 2-Induction of hepatotoxicity: The animals received paracetamol 500 mg/kg b.w. (p.o) every 72 h for 10 Days. Groups 3 to 5: Animals received ethanolic extract of Citrullus colocynthis L. at 50, 100 & 200 mg/kg bw/day for 7 days (p.o). Group 6: The animals were treated with Silymarin (100 mg/kg p.o) which served as standard. Groups 3 to 6 were intoxicated with paracetamol (500 mg/kg bw) 1 h before the administration of extract or Silymarin for 10 days. Histopathological findings, different hepatic biochemical parameters viz. AST, ALT, ALP, Total bilirubin, Total cholesterol, Triglycerides, & the body weight before & after treatment were evaluated to investigate the hepatoprotective activity. Results: Paracetamol induced a significant rise in AST, ALT, ALP, Total Bilirubin, Total Cholesterol, Triglycerides. Administration of 200 mg/kg bw of ethanolic extract of Citrullus colocynthis L. effectively reduced these pathological damages caused by paracetamol intoxication. In addition to serum parameters treatment of 200 mg/kg bw of ethanolic extract of Citrulus colocynthis L. also promotes the body weight in albino rats as shown in Figure 6 respectively. Histopathological changes of the liver samples were compared with the normal control as shown in Figures 2-5 respectively. Conclusion: From our results we may infer that the mode of action of 90% ethanolic extract of Citrullus colocynthis L. (200 mg/kg bw) in affording the in-vivo hepatoprotective activity against paracetamol may be due to the cell membrane stabilization, hepatic cell regeneration & normalizing the serum param

Single CDBA Based Voltage Mode Bistable Multivibrator and Its Applications

In this paper, current differencing buffered amplifier (CDBA) based bistable multivibrators are introduced. Each presented circuit is constructed using single CDBA as the basic active building block and three resistors. Two applications namely an astable and a monostable multivibrator are also realized to demonstrate the usefulness of the proposed bistable multivibrators. The presented circuits are simulated using PSPICE from Cadence Orcad16.2 to verify their functionality. Simulation results agree well with the theoretical analysis.

Predictors of severity and outcome and roles of intravenous thrombolysis and biomarkers in first ischemic stroke

Aim: Stroke is one of the leading causes of death and disability. The proportion of patients receiving recombinant tissue plasminogen activator is low in our country. Biomarkers to identify patients at risk of severe disease, and guide treatment and prognosis would be valuable. This article aims to identify the factors that can independently prognosticate the acute phase of ischemic stroke. Methods: All patients with the first episode of ischemic stroke admitted to the Neurology Department between 1st December 2017 to 31st March 2018 were included in this pilot study. Stroke severity was evaluated using the National Institute of Health Stroke Scale (NIHSS). Patients being admitted within 4.5 h of onset of symptoms were thrombolysed with injection alteplase. For each patient, 4 serum biomarkers (D-dimer, fibrinogen, C-reactive pro-tein and neuron specific enolase) were evaluated at admission and 24 h later. Discharged patients were assessed on an outpatient basis using the modified Rankin scale. The study primarily aimed to identify the factors predicting the severity and outcome of stroke, and to evaluate the effect of thrombolysis on the outcome. The secondary aim was to evaluate the role of biomarkers to predict the unfavorable outcome and the chance of post thrombolysis hemor-rhage. Results: Out of 30 patients included in the study, 10 had NIHSS 0-4, 12 had NIHSS 5-15 and 8 had NIHSS 16-42. Sixteen patients had unfavorable outcome (mRS score ≥ 2), and 5 patients expired. Old age, history of diabetes, CHADS2 score ≥ 2, and total anterior circulation stroke (TACS) independently affected stroke severity, whereas lowejection fraction < 35%, and TACS, independently predicted unfavorable outcome and mortality. High mean arte-rial blood pressure (MABP) and capillary blood glucose (CBG) at admission were significant predictors of stroke severity, unfavorable outcome, and mortality. Analysis of Trends of Sleep-Related Breathing Disorders in Patients with Systemic Essential Hypertension

Background: Studies have revealed a strong link between sleep-related breathing disorders and essential hypertension. However, the trends of sleep-disordered breathing in hypertensive patients have not been analyzed frequently. Methods: In this prospective, single-center and observational study, random patients with systemic essential hypertension, who were visiting GSVM Medical College, Kanpur, India between December 2012 and July 2014, were selected. After obtaining informed consent, patients were analyzed for symptoms of sleep-disordered breathing and Epworth Sleepiness Scale. Subsequently, all patients underwent overnight polysomnography to estimate the events of apnea and hypopnea. Observed data were compared between stage I and stage II hypertensive patients. Results: A total of 51 patients (age: 45.7 ± 13.43 years), comprising 36 stage I hypertensives and 15 stage II hypertensives, were enrolled in the study. About 47% of these patients displayed no symptoms of sleep-disordered breathing. Compared to stage I hypertensives, higher percentage of stage II hypertensives reported excessive day time sleepiness. Sleep latency time and sleep architecture were comparable between stage I and stage II hypertensives. Stage II hypertensives displayed shorter total sleep time and higher snore time than stage I hyper-tensives. Apnea Hypopnea Index (AHI) values increased with the severity of hypertension. Nocturnal oxygen desaturation was present in 25.0% and 53.3% of stage I and II hypertensives respectively. Mean lowest oxygen saturation was lower and the mean C-reactive protein levels were higher in stage II hypertensives as compared to stage I hypertensives. The mean C-reactive protein levels increased with increasing AHI scores, while the mean lowest oxygen saturation decreased with increasing AHI scores.
