Ionized calcium level predicts in-hospital mortality of severe sepsis patients:A retrospective cross-sectional study

Objective:To evaluate the effect of serum ionized calcium levels on the prognosis of severe sepsis ***:This retrospective cross-sectional study included sepsis patients who were hospitalized in an intensive care unit between January 2011 and December *** demographic and baseline data of the patients who died and survived were *** cutoff value of ionized calcium for in-hospital mortality was determined by the receiver operating characteristics curve(ROC).In-hospital mortalities and the survival rates were compared between patients with different ionized calcium ***,the risk factor of in-hospital mortality was ***:This study included 145 patients with 113 patients who died in the *** patients who died had significantly lower ionized calcium levels(U=2.25,P=0.034).A cut-off value of 0.93 mmol/L of ionized calcium was determined by the ROC *** patients with ionized calcium>0.93 mmol/L showed a significantly lower morality(χ2=9.90,P=0.002)and higher survival rate than with≤0.93 mmol/L(log rank=6.20,P=0.010).Multivariate Cox regression revealed that ionized calcium≤0.93 mmol/L was a risk factor of in-hospital ***:Ionized calcium level≤0.93 mmol/L was an independent predictor of in-hospital mortality of severe sepsis.

Plasma Concentrations of Some Trace Element and Heavy Metals in Patients with Metastatic Colon Cancer

Purpose: It is well known that trace elements and heavy metals play a significant role in human health and disease. These elements play an important role in different metabolic pathways of cells and tissues. Trace element deficiency or excess is implicated in the development or progression of some cancers. We present a study which investigates serum/ plasma levels of trace-heavy elements in metastatic colon cancer patients and healthy subjects. Methods: Blood samples were collected from 29 healthy subjects and 40 patients with metastatic colon cancer in Oncology Department of Medical School of Yuzuncu Yil University. In both groups, the serum levels of copper (Cu), magnesium (Mg), lead (Pb), chromium (Cr), zinc (Zn) selenium (Se), manganese (Mn), and cadmium (Cd) were determined. Results: Higher levels of Cu, Mg, Pb, Cr, Zn, Mn and Cd were seen in patients with metastatic colon cancer compared to healthy subjects and these findings were statistically significant (p 0.05). Se levels were noted to be lower in patients with colon cancer in comparison to healthy subjects and this finding was also statistically significant (p 0.05). Conclusions: According to our findings there was significant diffence in trace elements and heavy metals levels between healthy subjects and metastatic colon cancer patients. Therefore heavy metal and trace elements may have prognostic significance in complex disorders including colon cancer. However, further comprehensive studies are needed in order to advance our understanding of the relationship between heavy metal and trace elements and their role in cancers.

Hemorrhagic cystitis is a common and in its severe form potentially life threatening complication of hematopoietic stem cell transplantation. Hemorrhagic cystitis is defined as a diffuse inflammatory condition of the urinary bladder due to an infectious or noninfectious etiology resulting in bleeding from the bladder mucosa. Hemorrhagic cystitis is characterized by lower urinary tract symptoms including dysuria, hematuria and hemorrhage. The most common cause is a bacterial infection that usually responds promptly to treatment. But chronic and recurrent hemorrhagic cystitis often arises from anticancer chemotherapy or radiotherapy for the treatment of pelvic malignancies. Infectious etiologies are less common causes of chronic hemorrhagic cystitis except in immunocompromised hosts like bone marrow transplant recipients. Hemorrhagic cystitis is a significant complication of bone marrow transplantation which influences economic and survival outcome. Hemorrhagic cystitis can be divided into two classes according to onset time;early and late onset time. Earlyonset hemorrhagic cystitis is commonly associated used with chemo-radiotherapy protocols in some of the preparatory regimens. More than one factor is accused in the etiology of late onset hemorrhagic cystitis. Here, we present a patient whose hematuria started after 54 days from allogeneic stem cell transplantation.

Gastrointestinal infection is a leading cause of gut diseases attracting global health *** emerging antimicrobial resistance in enteric pathogens drives the search of viable and renewable alternatives to antibiotics for the health of both human beings and ***-forming probiotic Bacillus have received extensively interests for their multiple health benefits,including the restoration of microbiota dysbiosis and the reduction of drug-resistant *** promising benefits are mainly attributed to the activity of structurally diverse Bacillus-derived metabolites,such as antibacterial compounds,short-chain fatty acids,and other small *** metabolites show the capacity to directly target either the individual or community of bacterial pathogens,and to potentiate both host cells and gut *** better understanding of the mechanisms by which probiotic Bacillus and the metabolites modulate the metabolism of hosts and microbiota will advance the screening and development of probiotic *** this review,we discuss the interaction among probiotic Bacillus,microbiota and host,and summarize the Bacillus-derived metabolites that act as key players in such interactions,shedding light on the mechanistic understanding of probiotic Bacillus against enteric bacterial infections.

Diabetic macular edema:Efficacy and safety of antivascular endothelial growth factor therapy

Diabetic retinopathy is one of the prominent causes of vision impairment in the working-age population in industrialized countries and is related to 1%-5% of cases of blindness in the world. Among patients with diabetic retinopathy, diabetic macular edema(DME) is the major reason of vision impairment and represents a significant public health problem. Previous studies demonstrated the role of vascular endothelial growth factor(VEGF) in diabetic retinopathy and DME pathogenesis, and also revealed the efficacy of anti-VEGF agents for the management of these disorders. This review summarizes the outcomes of clinical studies that evaluated the anti-VEGF therapy including pegaptanib, ranibizumab, bevacizumab, and aflibercept for the management of DME. A significant number of clinical trials indicated favorable functional and anatomical results of anti-VEGF therapy for DME. Therefore, these agents should be considered an option in the treatment of DME in routine clinical practice.

The relationship between ischemic stroke and weather conditions in Duzce, Turkey

BACKGROUND: Weather conditions are thought to increase the risk of stroke occurrence. But their mechanism has not yet been clarified. We investigated possible relationships between ischemic stroke and weather conditions including atmospheric pressure, temperature, relative humidity, and wind ***: One hundred and twenty-eight patients with ischemic stroke who had been admitted to our hospital between January 1 and December 31, 2010 were enrolled in this study. We investigated the relationship between daily cases and weather conditions the same day or 1, 2, and 3 days before ***: A negative correlation was found between maximum wind speed and daily cases 3 days before stroke. As the relationship between daily cases and changes of weather conditions in consecutive days was evaluated, a negative correlation was found between daily cases and change of atmospheric pressure in the last 24 ***: The maximum wind speed 3 days before stroke and change of atmospheric pressure in the last 24 hours were found to increase the cases of ischemic stroke. We recommend that individuals at risk of ischemic stroke should pay more attention to preventive measures, especially on days with low maximum wind speed, on subsequent 3 days, and on days with low atmospheric pressure in the last 24 hours.

The incidence of diabetes mellitus(DM) is increasing rapidly. DM is the leading cause of cardiovascular diseases, which can lead to varied cardiovascular complications by aggravated atherosclerosis in large arteries and coronary atherosclerosis, thereby grows the risk for macro and microangiopathy such as myocardial infarction, stroke, limb loss and retinopathy. Moreover diabetes is one of the strongest and independent risk factor for cardiovascular morbidity and mortality, which associated frequently rhythm disorders such as atrial fibrillation(AF) and ventricular arrhythmias(VA). The present article provides a concise overview of the association between DM and rhythm disorders such as AF and VA with underlying pathophysiological mechanisms.
