Can we predict the incidence of high-grade Clavien-Dindo complications in patients with forgotten encrusted stents undergoing endourologic management?

Objective:Ureteral stents are customarily inserted to facilitate urinary drainage,but they come with their own glitches of being forgotten and/or encrusted leading to serious consequences.The present study aimed to report the complications in patients with forgotten and encrusted stents according to the Clavien-Dindo system specific to urological procedures and identify the factors leading to high-grade(Clavien-Dindo Grade 4A or above)complications.Methods:The hospital records of patients with forgotten encrusted double-J stents over a period of 8 years were reviewed.The parameters recorded included patient demographics,indwelling time,need for percutaneous nephrostomy,hemodialysis,urine culture,blood culture,total blood counts,serum creatinine,radiologic findings,management techniques,number of surgical interventions,modified Clavien-Dindo complications,follow-up,and mortality,if any.Results:Forty patients were included in the study.The median age was 52(range 6-85)years.Of the total,25(62.5%)patients had a“significant”stent load;31(77.5%)had renal failure or acute kidney injury on presentation;19(47.5%)patients had sepsis at presentation.Among the patients presented with sepsis,11(57.9%)patients demonstrated a positive urine culture;and 7/11(63.6%)patients exhibited pan-resistant organisms.Twelve out of 40(30.0%)patients in our series developed high-grade Clavien-Dindo complications.On univariate analysis,sepsis at presentation(p=0.007),stent load(p=0.031),diabetes(p=0.023),positive urine culture(p=0.007),and stent indwelling time of more than 1 year(p=0.031)were found to be significant.On multivariate logistic regression analysis,sepsis at presentation(p=0.017)and positive urine culture(p=0.016)were significant predictors for high-grade complications.Conclusion:It is prudent to identify specific risk factors,namely sepsis at presentation and positive urine culture to triage and optimize these patients before surgical management.

Background:Hard mineral and acid salt deposits that clump together in concentrated urine are known as kidney stones.A complicated chain of physicochemical events,including hyper saturation,nucleation,growth,aggrega-tion,and retention in the urinary tract,leads to urolithiasis,the third most prevalent urinary tract issue.Numerous traditional medications and therapies are less successful in treating urolithiasis and frequently cause recurrences.Objective:The present study is focused on evaluating the anti-urolithiasis activities of the polyherbal formulation prepared from Spinacia oleracea L.and Coriandrum sativum L.seeds.Methods:To evaluating the anti-urolithiasis activities of the polyherbal formulation prepared from S.oleracea and C.sativum seeds against oxalate crystals by employing an in vitro inhibition,aggregation,and nucleation assay,employing a spectrophotometer followed by microscopic observation,and further monitored by egg semiperme-able membrane model,using cystone as a standard drug.Results:In the present work,the alkaloids,glycosides,flavonoids,glycosides,phenols,saponins,steroids,tannins,terpenoids,and triterpenoids were found in the extracts of S.oleracea and C.sativum.The in vitro assays in the present study clearly indicated that all the mixture extracts readily prevented crystal nucleation,growth and ag-gregation.The aqueous extract of the mixture inhibited significant inhibitory potential against the crystallization process.Conclusion:It can be said that the phyto-constituents enriched homemade preparation possibly will be used as a medication for the management of kidney stones by dissolving the oxalate stone in the kidney.

An improved evaporation rate water cycle algorithm for energy-efficient routing protocol in WSNs

Purpose-The paper aims to introduce an efficient routing algorithm for wireless sensor networks(WSNs).It proposes an improved evaporation rate water cycle(improved ER-WC)algorithm and outlining the systems performance in improving the energy efficiency of WSNs.The proposed technique mainly analyzes the clustering problem of WSNs when huge tasks are performed.Design/methodology/approach-This proposed improved ER-WC algorithm is used for analyzing various factors such as network cluster-head(CH)energy,CH location and CH density in improved ER-WCA.The proposed study will solve the energy efficiency and improve network throughput in WSNs.Findings-This proposed work provides optimal clustering method for Fuzzy C-means(FCM)where efficiency is improved in WSNs.Empirical evaluations are conducted to find network lifespan,network throughput,total network residual energy and network stabilization.Research limitations/implications-The proposed improved ER-WC algorithm has some implications when different energy levels of node are used in WSNs.Practical implications-This research work analyzes the nodes’energy and throughput by selecting correct CHs in intra-cluster communication.It can possibly analyze the factors such as CH location,network CH energy and CH density.Originality/value-This proposed research work proves to be performing better for improving the network throughput and increases energy efficiency for WSNs.

Provisioning Intelligent Water Wave Optimization Approach for Underwater Acoustic Wireless Sensor Networks

In the Acoustics channel,it is incredibly challenging to offer data transfer for time-sourced applications in an energy-efficient manner due to higher error rate and propagation delay.Subsequently,conventional re-transmission over any failure generally initiates significantly larger end-to-end delay,and therefore it is not probable for time-based services.Moreover,standard techniques without any re-transmission consume enormous energy.This investigation proposes a novel multi-hop energy-aware transmission-based intelligent water wave optimization strategy.It ensures reduced end-to-end while attaining potential amongst overall energy efficiency end-to-end packet delay.It merges a naturally inspired meta-heuristic approach with multi-hop routing for data packets to reach the destination.The appropriate design of this Meta heuristic-based energy-aware scheme consumes lesser energy than the conventional one-hop transmission strategy without re-transmission.However,there is no hop-by-hop re-transmission facilitated.The proposed model shows only lesser delay than conventional methods with re-transmission.This work facilitates extensive work to carry out the proposed model performance with the MATLAB simulation environment.The results illustrate that the model is exceptionally energyefficient with lesser packet delays.With 500 nodes,the packet delivery ratio of proposed model is 100%,average delay is reduced by 2%,total energy consumption is 8 J,average packet redundancy is 1.856,and idle energy is 6.9Mwh.The proposed model outperforms existing approaches like OSF,AOR,and DMR respectively.

The Internet of Vehicles(IoV)has evolved as an advancement over the conventional Vehicular Ad-hoc Networks(VANETs)in pursuing a more optimal intelligent transportation system that can provide various intelligent solutions and enable a variety of applications for vehicular traffic.Massive volumes of data are produced and communicated wirelessly among the different relayed entities in these vehicular networks,which might entice adversaries and endanger the system with a wide range of security attacks.To ensure the security of such a sensitive network,we proposed a distributed authentication mechanism for IoV based on blockchain technology as a distributed ledger with an ouroboros algorithm.Using timestamp and challenge-responsemechanisms,the proposed authentication model can withstand several security attacks such asMan-in-Middle(MiM)attacks,Distributed Denial of Service(DDoS)attacks,server spoofing attacks and more.The proposed method also provides a solution for single-point failure,forward secrecy,revocability,etc.We exhibit the security of our proposed model by using formal(mathematical)analysis and informal analysis.We used Random Oracle Model to perform themathematical analysis.In addition,we compared the communication cost,computation cost,and security of the proposed model with the related existing studies.We have verified the security of the model by using AVISPA tool simulation.The security analysis and computation analysis show that the proposed protocol is viable.

Combined Economic and Emission Power Dispatch Control Using Substantial Augmented Transformative Algorithm

The purpose of the Combined Economic Emission Dispatch(CEED)of electric power is to offer the most exceptional schedule for production units,which must run with both low fuel costs and emission levels concurrently,thereby meeting the lack of system equality and inequality constraints.Economic and emissions dispatching has become a primary and significant concern in power system networks.Consequences of using non-renewable fuels as input to exhaust power systems with toxic gas emissions and depleted resources for future generations.The optimal power allocation to generators serves as a solution to this problem.Emission dispatch reduces emissions while ignoring economic considerations.A collective strategy known as Combined Economic and Emission Dispatch is utilized to resolve the above-mentioned problems and investigate the trade-off relationship between fuel cost and emissions.Consequently,this work manages the Substantial Augmented Transformative Algorithm(SATA)to take care of the Combined Economic Emission Dispatch Problem(CEEDP)of warm units while fulfilling imperatives,for example,confines on generator limit,diminish the fuel cost,lessen the emission and decrease the force misfortune.SATA is a stochastic streamlining process that relies upon the development and knowledge of swarms.The goal is to minimize the total fuel cost of fossil-based thermal power generation units that generate and cause environmental pollution.The algorithm searches for solutions in the search space from the smallest to the largest in the case of forwarding search.The simulation of the proposed system is developed using MATLAB Simulink software.Simulation results show the effectiveness and practicability of this method in terms of economic and emission dispatching issues.The performance of the proposed system is compared with existing Artificial Bee Colony-Particle Swarm Optimization(ABC-PSO),Simulated Annealing(SA),and Differential Evolution(DE)methods.The fuel cost and gas emission of the proposed system are 128904$/hr and 138094.465

Wireless sensor networks(WSNs)encompass a massive set of sensor nodes,which are self-conﬁgurable,inexpensive,and compact.The sensor nodes undergo random deployment in the target area and transmit data to base station using inbuilt transceiver.For reducing energy consumption and lengthen lifetime of WSN,multihop routing protocols can be designed.This study develops an improved rat swarm optimization based energy aware multi-hop routing(IRSO-EAMHR)protocol for WSN.An important intention of the IRSO-EAMHR method is for determining optimal routes to base station(BS)in the clustered WSN.Primarily,a weighted clustering process is performed to group the nodes into clusters and select cluster heads(CHs).Next,the IRSO-EAMHR approach derives aﬁtness function containing three input parameters(residual energy,dis-tance,and node degree)for routing process.The IRSO technique was designed by the integration of Levy movement concepts into the traditional RSO algorithm.The experimental result analysis of the IRSO-EAMHR technique is carried out and the outcomes are examined in various aspects.The simulation outcomes demonstrate the promising performance of the IRSO-EAMHR technique over the recent state of art approaches.

Buck Converter Current Measurement Using Differential Amplifier

The accuracy of the measured current is a preeminent parameter for Current Control based Power Converter applications to ensure genuine operation of the designed converter.The current measurement accuracy can be affected by several parameters which includes the type of technology used,components used for the selected technology,aging,usage,operating and environmental conditions.The effect of gain resistors and their manufacturing tolerances on differential ampliﬁer-based buck converter current measurement is investigated in this work.The analysis mainly focused on the output voltage variation and its accuracy with respect to the change in gain resistance tolerances.The gain resistors with 5%,1%,0.5%and 0.1%manufacturing tolerances taken for the worst-case analysis and the calculated performance results are compared and veriﬁed with the simula-tion results.The Operational ampliﬁers(Op-Amp)for high frequency power con-verter applications must operate in a high frequency noise environment and the intended current measuring system must manage common mode noise distur-bances paired with the signal to be measured.Based on the Common Mode Rejec-tion Ratio(CMRR)the common mode voltages and noise signals will effectively getﬁltered out.Lesser CMRR results in lower common mode signal rejection,resulting in poor precision and noise rejection.In differential ampliﬁers,the CMRR predominantly depends on gain resistors.So,the variations in Common Mode Rejection Ratio due to gain resistor tolerances also analyzed and compared with the output voltage variations.Besides the effects of resistor tolerances,this paper also examines the effect of Op-Amp offset voltage on output accuracy spe-ciﬁcally for low magnitude input currents.The obtained results from this analysis clearly shows that the gain resistors with 0.1%tolerance gives maximum accuracy with improved CMRR and accuracy at low magnitude input currents will get well improved by using Op-Amps with Low Offset voltage speciﬁcations.

Evaporative/radiative electrospun membrane for personal cooling

Functional textiles that promote daily comfort and productivity must efficiently release body sweats and transmit radiative heat through sweat evaporation and mid-infrared radiation(MIR)(8–13μm).However,most of the traditional clothing cannot provide simultaneous sweat evaporation and mid-infrared radiation transmission efficiently,leading to a poor design of personal cooling wearables.Herein,an evaporative/radiative integrated functional fibrous electrospun membrane is meticulously designed and controllably fabricated via facile electrospinning technology for personal cooling management.The developed membrane can be applied as a smart wearable with distinct personal thermal management applications.The promising temperature and humidity responsive vapor transmission of the membrane grants 1.2 times of evaporative cooling than that of traditional cotton.Besides,based on its high mid-infrared radiation transmission(53%)property in the range of 8–13μm,the as-spun membrane provides extra cooling of 1.5°C than that of cotton.Moreover,the building energy saving performances demonstrated that 47.1%annual building cooling can be achieved using the developed electrospun membrane.In general,the evaporative/radiative electrospun membrane creates a passive cooling microclimate for the human body,meeting the growing demand of wearable for personal cooling.

The development of intelligent algorithms for controlling autonomous mobile robots in real-time activities has increased dramatically in recent years.However,conventional intelligent algorithms currently fail to accurately predict unexpected obstacles involved in tour paths and thereby suffer from inefficient tour trajectories.The present study addresses these issues by proposing a potential field integrated pruned adaptive resonance theory(PPART)neural network for effectively managing the touring process of autonomous mobile robots in real-time.The proposed system is implemented using the AlphaBot platform,and the performance of the system is evaluated according to the obstacle prediction accuracy,path detection accuracy,time-lapse,tour length,and the overall accuracy of the system.The proposed system provide a very high obstacle prediction accuracy of 99.61%.Accordingly,the proposed tour planning design effectively predicts unexpected obstacles in the environment and thereby increases the overall efficiency of tour navigation.
