Ten-year experience with penile prosthetic surgery for the treatment of erectile dysfunction:outcomes of a tertiary referral center and predictors of early prosthetic infection

We aimed to evaluate ten-year outcomes of penile prosthesis(PP)implantation for the treatment of erectile dysfunction and to assess predictors of early prosthetic infection(EPI).We identified 549 men who underwent 576 PP placements between 2008 and *** and multivariate analyses were used to identify potential predictors of *** EPI predictive nomogram was ***-five(6.1%)cases of EPI were recorded with an explant rate of 3.1%.In terms of satisfaction,82.0%of the patients defined themselves as“satisfied,”while partner’s satisfaction was 88.3%.Diabetes(P=0.012),longer operative time(P=0.032),and reinterventions(P=0.048)were associated with EPI risk,while postoperative ciprofloxacin was inversely associated with EPI(P=0.014).Rifampin/gentamicin-coated 3-piece inflatable PP(r/g-c 3IPP)showed a higher EPI risk(P=0.019).Multivariate analyses showed a two-fold higher risk of EPI in diabetic patients,redo surgeries,or when a r/g-c 3IPP was used(all Pnger operative time,and secondary surgeries were the risk factors for *** ciprofloxacin was associated with a reduced risk of EPI,while r/g-c 3IPP had higher EPI rates without an increased risk of PP *** further validation,the proposed nomogram could be a useful tool for the preoperative counseling of PP implantation.

BACKGROUnD Bow hunter’s syndrome(BHS)is a rare but surgically treatable cause of vertebrobasilar insufficiency due to dynamic rotational occlusion of the vertebral ***,patients present with posterior circulation transient ischaemic symptoms such as presyncope,syncope,vertigo,diplopia,and horizontal nystagmus,but irreversible deficits,including medullary and cerebellar infarctions,have also been *** SUMMARY A 70-year-old patient presented an acute onset of vertigo and gait instability triggered by right head *** medical history included previous episodes of unilateral left neck and occipital pain followed by light-headedness,sweating,and blurred vision when turning his head,and these episodes were associated with severe degenerative changes in the atlanto-dens and left atlanto-axial facet joints and right rotation of the C2 cervical *** magnetic resonance imaging revealed the presence of acute bilateral cerebellar ischaemic lesions,while static vascular imaging did not reveal any vertebral artery *** ultrasonography and angiography were performed and confirmed the presence of a dynamic occlusion of the vertebral artery V3-V4 segment when the head was rotated to the right secondary to left C1-C2 bone spur *** decompression led to complete resolution of paroxysmal symptoms without neurological *** BHS should be considered in cases of repeated posterior circulation transient ischaemic attack or ischaemic stroke,particularly when associated with high cervical spine abnormalities.

The use of dendrochronology to study and date geomorphic processes in volcanic environments is still incipient, even more so on the volcanic slopes covered by temperate forests in central Mexico. Mass movements, such as debris flows, often impact forest stands where they cause damage to individual trees, thereby generating growth disturbances(GD) in the tree-ring records. The identification and dating of GD enables reconstruction of the age of trees colonizing bare surfaces after major events, but also allows the assessment of the frequency or spatial distribution of past geomorphic process activity. Here we used increment cores from 65 Pinus leiophylla, Abies religiosa, and Alnus jorullensis trees growing in the Axal gorge, on the southern slopes of La Malinche volcano, to unravel past debris-flow activity both temporally and spatially. Based on the combination of GD records, a weighted tree response index(Wit), field evidence and hydrometeorological records, we reconstructed 23 debris flows since ***, almost two-thirds of the reconstructed years with debris-flow activity in Axal gorge match with events recorded in Axaltzintle gorge located on the nE slopes of La Malinche. These findings suggest a regional triggering mechanism, most likely related to the occurrence of hurricanes. This research could be useful for disaster risk management of the La Malinche national Park.

Lameness prediction in broiler chicken using a machine learning technique

Broiler flock welfare is usually assessed through mortality,physiology,behavior,and walk-ing *** possibility of assessing broiler chicken lameness using the bird walking abil-ity was investigated using the machine learning approach for the first *** on broiler walking speed and acceleration,genetic strain,and sex were recorded and input in a *** were classified according to the 6-point gait score(GS0 is a sound bird,and GS5 is a severely lame bird).Decision trees were built initially using all *** confusion matrix of each developed model was *** pruning technique was used,removing from the dataset the variables that did not infer in the classification *** reorganized the dataset and re-arranged the data by grouping the intermediate target class of gait score using the Borda Count ***-processing data,we obtained a new set of decision *** the 3-point gait score(GS0 is a sound bird,and GS2 is a lame bird),we obtained a new model with better accuracy(78%);however,the model had a lower accuracy for classifying lame broilers(GS2,5%).The final decision tree was selected for classifying broilers,either sound or lame,according to their walking *** developed model presented good accuracy(91%),and it ordered properly sound(86%)and lame birds(92%).The novel model might be used to assess broiler lameness on-farm by registering the bird displacement *** developments using the model might allow flock lameness detection automatically.

Magnetic resonance imaging:A new tool for diagnosis of acute ischemic colitis?

AIM:To define the evolution of ischemic lesions with 7T magnetic resonance imaging(7T-MRI)in an animal model of acute colonic ***:Adult Sprague-Dawley rats were divided into two ***Ⅰ underwent inferior mesenteric artery(IMA)ligation followed by macroscopic observations and histological *** groupⅡ,7T-MRI was performed before and after IMA ligation and followed by histological ***:Morphological alterations started to develop 1 h after IMA ligation,when pale areas became evident in the splenic flexure mesentery and progressively worsened up to 8 h thereafter,when the mesentery was less pale,and the splenic flexure loop appeared very *** 7T-MRI results reflected these alterations,showing a hyperintense signal in both the intraperitoneal space and the colonic loop wall 1 h after IMA ligation;the latter progressively increased to demonstrate a reduction in the colonic loop lumen at 6 *** hours after IMA ligation,MRI showed a persistent colonic mural hyperintensity associated with a reduction in peritoneal free *** 7T-MRI findings were correlated with histological alterations,varying from an attenuated epithelium with glandular apex lesions at 1 h to coagulative necrosis and loss of the surface epithelium detected 8 h after IMA ***:MRI may be used as a substitute for invasive procedures in diagnosing and grading acute ischemic colitis,allowing for the early identification of pathological findings.
