Background:Ayurvedic system of medicine is well established for prevention and treatment of renal problems.There are vast number of medicinal plants mentioned in Ayurvedic system of medicine,including Piper attenuatum Buch.-Ham.ex Miq.,which has been known to exhibit diuretic properties.Objective:To investigate possible mechanism and diuretic activity of ethanol extract of P.attenuatum leaves.Methods:Present study reports an in vivo diuretic activity of ethanol extracts of P.attenuatum leaves.Twenty-four Sprague-Dawley male rats were deprived from food and water for 20 h,followed by being divided into four groups to receive normal saline,Furosemide,and two doses of the ethanol extract,respectively.Further,the ethanol extract were subjected for prelimnary phytochemical screening test,and analytical investigation was performed via high-performance thin-layer chromatography(HPTLC),followed by effectiveness exploration of putative bioactive compounds from P.attenuatum against human carbonic anhydrase(hCA)enzyme using molecular docking tool.Results:During phytochemicals screening,several groups of compounds such as amides,terpenoids,proteins,flavonoids,and glycosides have been identified.Further,HPTLC reveals presence of carbohydrates,proteins,amino acids,steroids,alkaloids,flavonoids,tannins,glycosides,and terpenoids.Significant diuretic action was revealed for both 200 mg/kg bw and 400 mg/kg bw of ethanol extract of P.attenuatum leaves.Interestingly,molecular docking analysis demonstrated greater binding affinity of compounds such as Cepharadione A,Nor-cepharadione B,Galbelgin,Crotepoxide,and Pipoxide chlorohydrin out of total 23 studied compounds against hCA isoforms(a key biomarker for diuretic)and score is comparable with standard drug Acetazolamide.Conclusion:It was concluded from the present studies that ethanol extract of P.attenuatum leaves exhibits signifi-cant diuretic activity.The activity may be correlated due to the presence of Cepharadione A,Norcepharadione B,Galbelgin,Crotepoxide,and Pipoxide chlorohyd

Recently,the coronavirus disease 2019 has shown excellent attention in the global community regarding health and the economy.World Health Organization(WHO)and many others advised controlling Corona Virus Disease in 2019.The limited treatment resources,medical resources,and unawareness of immunity is an essential horizon to unfold.Among all resources,wearing a mask is the primary non-pharmaceutical intervention to stop the spreading of the virus caused by Severe Acute Respiratory Syndrome Coronavirus 2(SARS-CoV-2)droplets.All countries made masks mandatory to prevent infection.For such enforcement,automatic and effective face detection systems are crucial.This study presents a face mask identification approach for static photos and real-time movies that distinguishes between images with and without masks.To contribute to society,we worked on mask detection of an individual to adhere to the rule and provide awareness to the public or organization.The paper aims to get detection accuracy using transfer learning from Residual Neural Network 50(ResNet-50)architecture and works on detection localization.The experiment is tested with other popular pre-trained models such as Deep Convolutional Neural Networks(AlexNet),Residual Neural Networks(ResNet),and Visual Geometry Group Networks(VGG-Net)advanced architecture.The proposed system generates an accuracy of 98.4%when modeled using Residual Neural Network 50(ResNet-50).Also,the precision and recall values are proved as better when compared to the existing models.This outstanding work also can be used in video surveillance applications.

In this paper,we proposed an innovation diffusion model with three compartments to investigate the diffusion of an innovation(product)in a particular region.The model exhibits two equilibria,namely,the adopter-free and an interior equilibrium.The existence and local stability of the adopter-free and interior equilibria are explored in terms of the effective Basic Influence Number(BIN)R_(A).It is investigated that the adopter free steady-state is stable if R_(A)<1.By consideringτ(the adoption experience of the adopters)as the bifurcation parameter,we have been able to obtain the critical value ofτresponsible for the periodic solutions due to Hopf bifurcation.The direction and stability analysis of bifurcating periodic solutions has been performed by using the arguments of normal form theory and the center manifold theorem.Exhaustive numerical simulations in the support of analytical results have been presented.

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)increases the risk of cardiovascular diseases independently of other risk factors.However,data on its effect on cardiovascular outcomes in coronavirus disease 2019(COVID-19)hospitalizations with varied obesity levels is scarce.Clinical management and patient care depend on understanding COVID-19 admission results in NAFLD patients with varying obesity levels.AIM To study the in-hospital outcomes in COVID-19 patients with NAFLD by severity of obesity.METHODS COVID-19 hospitalizations with NAFLD were identified using International Classification of Disease-10 CM codes in the 2020 National Inpatient Sample database.Overweight and Obesity Classes Ⅰ,Ⅱ,and Ⅲ(body mass index 30-40)were compared.Major adverse cardiac and cerebrovascular events(MACCE)(all-cause mortality,acute myocardial infarction,cardiac arrest,and stroke)were compared between groups.Multivariable regression analyses adjusted for sociodemographic,hospitalization features,and comorbidities.RESULTS Our analysis comprised 13260 hospitalizations,7.3% of which were overweight,24.3% Class Ⅰ,24.1% Class Ⅱ,and 44.3% Class Ⅲ.Class Ⅲ obesity includes younger patients,blacks,females,diabetics,and hypertensive patients.On multivariable logistic analysis,Class Ⅲ obese patients had higher risks of MACCE,inpatient mortality,and respiratory failure than Class Ⅰ obese patients.Class Ⅱ obesity showed increased risks of MACCE,inpatient mortality,and respiratory failure than Class I,but not significantly.All obesity classes had non-significant risks of MACCE,inpatient mortality,and respiratory failure compared to the overweight group.CONCLUSION Class Ⅲ obese NAFLD COVID-19 patients had a greater risk of adverse outcomes than class Ⅰ.Using the overweight group as the reference,unfavorable outcomes were not significantly different.Morbid obesity had a greater risk of MACCE regardless of the referent group(overweight or Class Ⅰ obese)compared to overweight NAFLD patients admitted with COVID-19.

Background:Globally,80%people use plant-derived products for treating or preventing diseases.One prevalent perception about medicinal plants is that they are safe and devoid of adverse effects.However,approximately 1,50,000 plants contain toxic substances.Objective:The present review focuses on medicinal plant extracts/fractions toxicity assessments made in preclin-ical models by oral route.Methods:Detail studies were searched from databases including PubMed and Google Scholar.A manual reference screening of the selected studies was done to identify relevant articles,with no language restriction being imposed at the time of searching.Results:The studies included were performed in rodents,and the test substances were administered orally.Our search revealed 33 widely used plants or products with significant toxicity,and phytochemicals from these plants have been summarized.Through a systematic review,we identified a plethora of medicinal plant extracts reporting safety and toxicity concerns.Conclusion:In the future,preclinical toxicokinetic studies of herbs and the determination of their no-observed-adverse-effect levels are required for a complete safety assessment.Finally,the interaction of herbs with com-monly used/over-the-counter drugs in terms of the latter’s metabolic profile should be undertaken.

Sexual dysfunctions and their treatment in liver diseases

Sexual dysfunction(SD)is a prevalent but very commonly ignored aspect in the treatment of liver diseases and cirrhosis.The etiology of SD is multifactorial and therefore treatment strategies are complex,especially in females.Phosphodiesterase inhibitors are useful and effective in erectile dysfunction in males but in females,no single drug is available for SD,therefore multimodal treatment is required depending upon the cause.The foremost and fundamental requirement in both genders is to be stress-free and have adequate control of liver diseases.Improved quality of life is helpful in improving SD and vice versa is also true.Therefore,patients suffering from liver diseases should come forward and ask for treatment for SD,and physicians should actively enquire about SD while history taking and evaluating these patients.SD results in deterioration of quality of life,and both are modifiable and treatable aspects of liver diseases,which are never addressed actively,due to social taboos and fears of SD treatment in the presence of liver diseases.The diagnosis of SD does not require costly investigations,as the diagnosis can be established based on validated questionnaires available for both genders,therefore detailed targeted history taking using questionnaires is essential.Data are emerging in this area but is still at an early stage.More studies should be dedicated to SD in liver diseases.

Job satisfaction and organizational commitment among nurses working on temporary versus permanent jobs at a tertiary care teaching hospital,Uttarakhand,India

Objective:The objective of this study was to assess and compare job satisfaction and organizational commitment among nurses working on temporary versus permanent basis at a tertiary care teaching hospital,Uttarakhand,India.Materials and Methods:A total of 250 nurses(125 temporary and 125 permanent)were selected using stratified disproportionate random sampling techniques from a tertiary care teaching hospital.Sociodemographic datasheet,Job Satisfaction Survey scale,and Organizational Commitment scale were used to collect data.Results:The study results revealed that the job satisfaction score was significantly higher in permanent nurses(139.1±15.4)than in temporary nurses(131.3±12.2)(P<0.001).However,temporary nurses(85.2±14.4)were more committed toward organization than permanent nurses(80.0±16.0)(P<0.001).There was a significant association between nurses’job satisfaction and their gross salary(P<0.001)and their organizational commitment with the selected profession by own will(P<0.001).Conclusion:The present study concluded that majority of temporary and permanent nurses reported a moderate level of job satisfaction and organizational commitment.This study suggests enhancing employee satisfaction can improve organizational commitment.As well,this study’s findings can serve as a valuable foundation for designing policies to enhance organizational commitment in the future.

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP)is an essential therapeutic tool for biliary and pancreatic diseases.Frail and elderly patients,especially those aged≥90 years are generally considered a higher-risk population for ERCP-related complications.AIM To investigate outcomes of ERCP in the Non-agenarian population(≥90 years)concerning Frailty.METHODS This is a cohort study using the 2018-2020 National Readmission Database.Patients aged≥90 were identified who underwent ERCP,using the international classification of diseases-10 code with clinical modification.Johns Hopkins’s adjusted clinical groups frailty indicator was used to classify patients as frail and non-frail.The primary outcome was mortality,and the secondary outcomes were morbidity and the 30 d readmission rate related to ERCP.We used univariate and multivariate regression models for analysis.RESULTS A total of 9448 patients were admitted for any indications of ERCP.Frail and non-frail patients were 3445(36.46%)and 6003(63.53%)respectively.Indications for ERCP were Choledocholithiasis(74.84%),Biliary pancreatitis(9.19%),Pancreatico-biliary cancer(7.6%),Biliary stricture(4.84%),and Cholangitis(1.51%).Mortality rates were higher in frail group[adjusted odds ratio(aOR)=1.68,P=0.02].The Intra-procedural complications were insigni-ficant between the two groups which included bleeding(aOR=0.72,P=0.67),accidental punctures/lacerations(aOR=0.77,P=0.5),and mechanical ventilation rates(aOR=1.19,P=0.6).Post-ERCP complication rate was similar for bleeding(aOR=0.72,P=0.41)and post-ERCP pancreatitis(aOR=1.4,P=0.44).Frail patients had a longer length of stay(6.7 d vs 5.5 d)and higher mean total charges of hospitalization($78807 vs$71392)compared to controls(P<0.001).The 30 d all-cause readmission rates between frail and non-frail patients were similar(P=0.96).CONCLUSION There was a significantly higher mortality risk and healthcare burden amongst nonagenarian frail patients undergoing ERCP compared to non-frail.Larger studies are warranted to in

Mitigation of the impacts of electric vehicle charging on energy-star ratings for residential buildings in India

The star-labelling programme for residential buildings was introduced by India in 2020 and applies to all residential buildings with no lower limit on the built-up area or electrical demand.The energy-star label for a residential building is awarded against the notified standard by the regulatory body and electric vehicles(EVs)have not been accommodated as a load for residential buildings.The en-ergy consumption of an existing residential building is taken from a study already carried out and compared with the requirement of the Indian residential star-labelling programme with an EV as a plugged-in load.An annual energy gap of 6060 kWh for the existing residential buildings considered in this study for five-star building energy labels increases to 7784 kWh if the EV load is added to the building load.The residential building will lose two energy stars if it caters to the EV load and,to bridge this energy gap,the replace-ment of existing electrical appliances with five-star-rated energy appliances,employing grid-connected rooftop solar photovoltaics(PV)and retrofit of the building envelope are considered.The techno-economic potential of rooftop solar PV and building envelope retrofitting for existing residential buildings is explored using RETScreen®and eQUEST software,respectively.The study establishes that the installation of rooftop solar PV can accommodate the additional load of EVs and can bridge half and three-quarters of the energy gap to achieve five energy stars for an existing building with and without EVs,respectively.It is the most economical option among the options explored in this study.The target Energy Performance Index is achievable by high-end energy consumers(12000 kWh/year)by additional measures,the replacement of inefficient electrical appliances and building envelope retrofitting in addition to the installation of rooftop solar PV.

Oxygen evolution reaction(OER)is the key step involved both in water splitting devices and rechargeable metal-air batteries,and hence,there is an urgent need for a stable and low-cost material for efficient OER.In the present investigation,Co-Fe-Ga-NiZn(CFGNZ)high entropy alloy(HEA)has been utilized as a low-cost electrocatalyst for OER.Herein,after cyclic voltammetry activation,CFGNZ-nanoparticles(NPs)are covered with oxidized surface and form high entropy(oxy)hydroxides(HEOs),exhibiting a low overpotential of 370 mV to achieve a current density of 10 mA/cm^(2)with a small Tafel slope of 71 mV/dec.CFGNZ alloy has higher electrochemical stability in comparison to state-of-the art RuO2 electrocatalyst as no degradation has been observed up to 10 h of chronoamperometry.Transmission electron microscopy(TEM)studies after 10 h of long-term chronoamperometry test showed no change in the crystal structure,which confirmed the high stability of CFGNZ.The density functional theory(DFT)based calculations show that the closeness of d(p)-band centers to the Fermi level(EF)plays a major role in determining active sites.This work highlights the tremendous potential of CFGNZ HEA for OER,which is the primary reaction involved in water splitting.
