BACKGROUND: Primary biliary cirrhosis (PBC) is a rare cause of chronic liver disease in India. We analyzed the clinical, biochemical, serological and histological features of patients with PBC for over a 10-year period. METHODS: PBC was diagnosed by the presence of raised level of serum alkaline phosphatase (ALP), anti-mitochondrial antibody (AMA) positivity (1:40 dilution), and/or diagnostic liver histology. RESULTS:Fifteen female patients with mean age of 46.5±11 years were studied. Pruritis (80%) followed by jaundice (67%), skin changes (pigmentation, coarsening, xanthelesma and vitiligo) (67%) and fatigue (60%) were common symptoms. The mean duration of the symptoms was 3.5± 5.4 years (3 months to 20 years). Dryness of eyes was observed in only 2 patients. Hepatomegaly was noted in 87% of the patients and ascites at presentation in 40%. Mean levels of bilirubin and albumin at the time of diagnosis were 3.4±3.3 mg/dl and 3.5±0.8 g/dl, respectively. The level of serum ALP ranged from 54 to 2400 IU/L, with a median being 552 IU/L (2×ULN). In all the 15 patients with AMA positive, 8(53%) were also positive for either anti-nuclear or anti-smooth muscle antibodies. Two patients presented with persistently elevated SAP after an acute hepatitic illness. Liver biopsy was available in 13 patients, diagnostic of PBC Ⅱ & Ⅲ(8) and with evidence of cirrhosis (5). Associated autoimmune disorders were observed in 5 patients (33%). The mean time for follow-up was 26±21 months (1 to 87 months). In 4 deaths, 3 were due to liver related causes. CONCLUSION: PBC is a rare cause of chronic liver disease in India. PBC in India, unlike in the West, presents late, often with features of cirrhosis and decompensation.

Readmission rate and early complications in patients undergoing total knee arthroplasty:A retrospective study

BACKGROUND Total knee arthroplasty(TKA)can improve pain,quality of life,and functional *** uncommon,postoperative complications are extremely consequential and thus must be carefully tracked and communicated to patients to assist their decision-making before *** of the risk factors for complications and readmissions after TKA,taking into account common causes,temporal trends,and risk variables that can be changed or left unmodified,will benefit this *** To assess readmission rates,early complications and their causes after TKA at 30 days and 90 days *** This was a prospective and retrospective study of 633 patients who underwent TKA at our hospital between January 1,2017,and February 28,*** the 633 patients,28 were not contactable,leaving 609 who met the inclusion *** inpatient and outpatient hospital records were retrieved,and observations were noted in the data collection *** Following TKA,the 30-day and 90-day readmission rates were determined to be 1.1%(n=7)and 1.8%(n=11),*** unplanned visit rate at 30 days following TKA was 2.6%(n=16)and at 90 days was 4.6%(n=28).At 90 days,the unplanned readmission rate was 1.4%(n=9).Reasons for readmissions included medical(27.2%,n=3)and surgical(72.7%,n=8).Unplanned readmissions and visits within 90 days of follow-up did not substantially differ by age group(P=0.922),body mass index(BMI)(P=0.633),unilateral vs bilateral TKA(P=0.696),or patient comorbidity status(30-day P=0.171 and 90-day P=0.813).Reoperation rates after TKA were 0.66%(n=4)at 30 days and 1.15%(n=8)at 90 *** average length of stay was 6.53 *** In this study,there was a low readmission rate following *** was no significant correlation between readmission rate and patient factors such as age,BMI,and co-morbidity status.

In the present study,an approximation is used to study viscosity as a function of pressure at different *** correlation so obtained is applied to study the viscosity of biodiesels extracted from soybean,Vistive soybean,canola,used canola,coconut and *** computed values of viscosity from the proposed model were found to be in good agreement with experimental data throughout the range of pressure and temperature *** maximum average absolute relative deviation(AARD%)and mean AARD%are found to be 0.52 and 0.20,respectively,over the entire range of pressure(0.1-140 MPa)and temperature(283.15-373.15 K)for all biodiesels except rapeseed biodiesel,for which the values are 1.1 and 0.62,***,this work includes the very first investigation conducted so far on the variation of the pressure-viscosity coefficient(PVC)with pressure at different temperatures for *** variation in PVC with the temperature is more sensitive at elevated pressures as compared to atmospheric pressure whereas the variation in PVC with pressure is more sensitive at elevated temperatures as compared to room temperature.

BACKGROUND Cardiovascular outcome trials have demonstrated cardiovascular safety of glimepiride(a sulfonylureas) against dipeptidyl peptidase-4 inhibitor ***(another newer sulfonylureas) has shown similar glycemic efficacy and 50% decreased risk of hypoglycemia compared to *** Considering the absence of cardiovascular outcome trials for gliclazide, we decided to conduct a systematic review of the literature to assess the cardiovascular(CV) safety by assessing the risk for major adverse CV events and hypoglycemia risk of gliclazide vs linagliptin in patients with type 2 diabetes(T2D).METHODS This systematic review followed the current Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines to analyze all the clinical studies published from 2008 that compared the two drugs in patients with T2D with no risk of CV disease(CVD). We included only evidence designated high quality by the Oxford Center for Evidence-based Medicine-Levels of *** Eight clinical studies were included in the narrative descriptive analysis(gliclazide: 5 and linagliptin: 3). The CV safety of gliclazide in the Action in Diabetes and Vascular Disease: Preterax and Diamicron Modified Release Controlled Evaluation trial and of linagliptin in the Cardiovascular and Renal Microvascular Outcome Study With Linagliptin(CARMELINA) and CARdiovascular Outcome study of LINAgliptin vs glimepiride in patients with T2D(CAROLINA)trials were excluded from the comparative analysis as these trials demonstrated CV and hypoglycemia benefits in patients at high risk of CVD. However, since these are landmark trials,they were discussed in brief to show the CV benefits and low hypoglycemia risk of gliclazide and linagliptin. Hunting of hunted:an ensemble modeling approach to evaluate suitable habitats for caracals in India

Background:Large-scale hunting and various anthropogenic pressures in the recent past have pushed the Asiatic caracal(Caracal caracal schmitzi),an elusive medium-sized and locally threatened felid species towards local extinction in *** widely distributed historically,it has been sparsely reported from several regions of central and northern states in India till twentieth ***,the species distribution became confined only to the states of Rajasthan,Gujarat and Madhya Pradesh,which have had reported sightings in the twenty-first *** order to highlight the potentially suitable habitats for Asiatic caracals in India,we targeted forth-filtering of the spatial model ensemble by creating and utilizing the validated and spatially thinned species presence information(n=69)and related ecological variables(aridity,NDVI,precipitation seasonality,temperature seasonality,terrain ruggedness),filtered with anthropological variable(nightlight).Results:Out of eight spatial prediction models,the two most parsimonious models,Random Forest(AUC 0.91)and MaxEnt(AUC 0.89)were weighted and *** ensemble model indicated several clustered habitats,covering 1207.83 km^(2)areas in Kachchh(Gujarat),Aravalli mountains(Rajasthan),Malwa plateau(Rajasthan and Madhya Pradesh),and Bundelkhand region(Madhya Pradesh)as potentially suitable habitats for *** probabilities of pixels were further regressed with converted vegetation height data within selected highly potential habitats,i.e.,Ranthambore Kuno Landscape(RKL)(suitability~0.44+0.03(vegetation height)^(**),R^(2)=0.27).The regression model inferred a significant positive relation between vegetation height and habitat suitability,hence the lowest ordinal class out of three classes of converted vegetation height was masked out from the RKL,which yielded in an area of 567 km^(2) as potentially highly suitable habitats for caracals,which can be further proposed as survey areas and conservation priority are

Influence of particle size and temperature on the dielectric properties of CoFe_2O_4 nanoparticles

The objective of this study was to establish the dielectric properties of CoFe2O4 nanoparticles with particle sizes that varied from 28.6 to 5.8 nm. CoFe2O4 nanoparticles were synthesized using a chemical coprecipitation method. The particle sizes were calculated accord-ing to the Scherrer formula using X-ray diffraction （XRD） peaks, and the particle size distribution curves were constructed by using field-emission scanning electron microscopy （FESEM） images. The dielectric permittivity and loss tangents of the samples were determined in the frequency range of 1 kHz to 1 MHz and in the temperature range of 300 to 10 K. Both the dielectric permittivity and the loss tangent were found to decrease with increasing frequency and decreasing temperature. For the smallest CoFe2O4 nanoparticle size, the dielectric per-mittivity and loss tangent exhibited their highest and lowest values, respectively. This behavior is very useful for materials used in devices that operate in the microwave or radio frequency ranges.

Molecular dynamics study on growth of carbon dioxide and methane hydrate from a seed crystal

In the current work,molecular dynamics simulation is employed to understand the intrinsic growth of carbon dioxide and methane hydrate starting from a seed crystal of methane and carbon dioxide *** comparison was carried out because it has relevance to the recovery of methane gas from natural gas hydrate reservoirs by simultaneously sequestering a greenhouse gas like *** seed crystal of carbon dioxide and methane hydrate was allowed to grow from a super-saturated mixture of carbon dioxide or methane molecules in water *** different concentrations(1:6 and 1:8.5)of CO2/CH4 molecules per water molecule were chosen based on gas–water composition in hydrate *** molecular level growth as a function of time was investigated by all atomistic molecular dynamics simulation under suitable temperature and pressure range which was well above the hydrate stability zone to ensure significantly faster growth *** concentration of CO2 molecules in water played a significant role in growth kinetics,and it was observed that maximizing the CO2 concentration in the aqueous phase may not result in faster growth of CO2 *** the contrary,methane hydrate growth was independent of methane molecule concentration in the aqueous *** have validated our results by performing experimental work on carbon dioxide hydrate where it was seen that under conditions appropriate for liquid CO2,the growth for carbon dioxide hydrate was very slow in the beginning.
