Implementation of safety and security standards for high dose rate brachytherapy sealed radioactive source used in a medical radiation facility

Objective:To provide guidance on basic concepts and elements for radiation safety and security measures for a HDR brachytherapy sealed radioactive source in a medical radiation facility.Methods:Brachytherapy sources are Category 2(High risk-very dangerous)radioactive source and Security Level B applied to cover the range of security measures needed for the associated risk with remote afterloading HDR brachytherapy sources.These security measures were applied based on a risk informed graded approach.A series of in-build and operational safety measures were implemented for equipment and radiation installation.Results:The medical radiation facility equipped with adequate radiation protection,detection and monitoring equipment such as survey meter,digital contamination monitor,gamma zone monitor,digital radiation monitor,personnel dosimeter,pocket dosimeter,long handled forceps,shielding material and emergency source storage container etc.to deal with radiological incidences.The contingency plan with security objectives for incidents and procedures for action was prepared in order to implement response function to fulfill the goal of adequate security of radioactive source.The emergency response plan documented against emergency scenarios during transport,loss of radioactive source,theft,sabotage,natural calamities/disasters such as fire,flood and earthquake.Conculsions:The paper provides various radiation safety and security aspects for the HDR brachytherapy.The HDR brachytherapy radioactive sources are Type A packages.Transport of radioactive material is the responsibility of consignors,consignee and carriers under the legislation,supervision and control of regulatory authority.

Evaluation and analysis of entrance surface air kerma of the thyroid gland during breast cancer screening mammography

Objective:To evaluate and analyze entrance surface air kerma(ESAK)of the thyroid during mammographies performed in a breast cancer screening program.Methods:The ESAK was measured for the left and right lobes of the thyroid gland for 120 female subjects undergoing routine mammographic screenings.All measurements were single-handedly performed on the Hitachi Hologic LORAD M-IVTM screen-film mammography system.Radiation dose measurements were made using optically stimulated luminescent dosimeters(OSLD)nanoDot™taped appropriately to the skin overlying the right and left lobes of the thyroid gland.The standard technique of two craniocaudal(CC)views and two mediolateral oblique(MLO)views with appropriate compression was used during the screening.Results:The average ESAK was(287102)μGy.The value of radiation dose to the thyroid was estimated as 71.17μGy.Compresssed breast thichness(CBT)was found as key parameter influencing radiation dose to the thyroid gland during mammography.ESAK changed approximately two folds for the CBT range of 2–5 cm.Radiation dose to the thyroid gland was increased with higher body mass index(BMI).Conculsions:A proper compression of the breast is crucial for the mammographic examination.In addition to applied exposure factors for examination,breast compression is the principal and influencing factor that affects various other parameters used during mammographic examinations,which in turn impact radiation dose to the thyroid gland.

Medication adherence and quality of life among type-2 diabetes mellitus patients in India

BACKGROUND Diabetes mellitus(DM)is a progressively increasing metabolic disorder and a significant public health burden that demands immediate global attention.However,there is a paucity of data about adherence to antidiabetic drugs among patients with type-2(T2)DM in Uttarakhand,India.Outpatient research reported that more than 50%of patients do not adhere to the correct administration and appropriate medicine dosage.It has been reported that patients with chronic diseases who adhere to treatment may experience improvement in quality of life(QoL)and vice versa.AIM To assess the adherence to antidiabetic medication and QoL among patients with T2DM.METHODS This cross-sectional descriptive study was conducted at a tertiary care hospital in Uttarakhand,India.The Medication Adherence Rating Scale and World Health Organization QoL-BREF scale were used to assess medication adherence and QoL.RESULTS Two hundred seventy-seven patients suffering from T2DM participated in the study.Their mean age was 50.80(±10.6)years,155(56%)had a poor adherence level and 122(44%)had a good adherence level to antidiabetic medications.After adjusting for sociodemographic factors,multiple linear regression analysis found patients who were adherent to antidiabetic medications had significantly higher mean overall perception of QoL and overall perception of health,with beta scores of 0.36 and 0.34,respectively(both P=0.000)points compared with nonadherent patients.CONCLUSION There was an association between medication adherence and QoL in patients with T2DM.Hence,there is a need to plan awareness and counseling programs followed by regular follow-up to motivate patient adherence to recommended treatment and lifestyle regimens.

Facial Expression Recognition with High Response-Based Local Directional Pattern (HR-LDP) Network

Although lots of research has been done in recognizing facial expressions,there is still a need to increase the accuracy of facial expression recognition,particularly under uncontrolled situations.The use of Local Directional Patterns(LDP),which has good characteristics for emotion detection has yielded encouraging results.An innova-tive end-to-end learnable High Response-based Local Directional Pattern(HR-LDP)network for facial emotion recognition is implemented by employing fixed convolutional filters in the proposed work.By combining learnable convolutional layers with fixed-parameter HR-LDP layers made up of eight Kirsch filters and derivable simulated gate functions,this network considerably minimizes the number of network parameters.The cost of the parameters in our fully linked layers is up to 64 times lesser than those in currently used deep learning-based detection algorithms.On seven well-known databases,including JAFFE,CK+,MMI,SFEW,OULU-CASIA and MUG,the recognition rates for seven-class facial expression recognition are 99.36%,99.2%,97.8%,60.4%,91.1%and 90.1%,respectively.The results demonstrate the advantage of the proposed work over cutting-edge techniques.

Surface Modification Aspects for Improving Biomedical Properties in Implants:A Review

The primary focus during surgery is to ensure successful implantation by achieving long-term and stable fixation of implants.Orthopaedic surgery is now more focused on the development of novel biomaterials intended to improve implant performance.To obtain a better understanding of metal implants,this article investigated the causes of material failures in certain cases and analysed a few case studies.The interaction between the implant and bone tissue was a crucial aspect of successful implantation,and this study explored the factors influencing this interaction as well as ways to improve it.Several modern approaches used for modifying implant surfaces were systematically illustrated and briefly analysed.Thermal spray coatings were often favoured because of their wide range of coating materials,but other substantial surface modifications(such as friction stir processing and laser surface texturing)were also used for a selection of applications.Notably,implant surfaces with desirable features,such as biocompatibility,antibacterial properties,corrosion resistance,and wear resistance,were essential for optimising implant functionality.This systematic review's main aim is to provide exhaustive reference information and a broad overview to advance the production and design of orthopaedic implants.

Dynamics of some new solutions to the coupled DSW equations traveling horizontally on the seabed

The system of(1+1)-coupled Drinfel’d-Sokolov-Wilson equations describes the surface gravity waves travelling horizontally on the seabed.The objective of the present research is to construct a new variety of analytical solutions for the system.The invariants are derived with the aid of Killing form by using the optimal algebra classification via Lie symmetry approach.The invariant solutions involve time,space variables,and arbitrary constants.Imposing adequate constraints on arbitrary constants,solutions are represented graphically to make them more applicable in designing sea models.The behavior of solutions shows asymptotic,bell-shaped,bright and dark soliton,bright soliton,parabolic,bright and kink,kink,and periodic nature.The constructed results are novel as the reported results[26,28,29,30,33,38,42,49]can be deduced from the results derived in this study.The remaining solutions derived in this study,are absolutely different from the earlier findings.In this study,the physical character of analytical solutions of the system could aid coastal engineers in creating models of beaches and ports.

Gestational diabetes mellitus(GDM)is a risk to maternal-fetal health due to uncertain diagnostic criteria and treatment options.Luo's study demonstrated the efficacy of customized nutritional therapies in controlling GDM.Tailored strategies led to significant body weight loss,improved glucolipid metabolism,and fewer prenatal and newborn problems.This holistic approach,which emphasizes the notion of’chrononutrition’,takes into account optimal meal timing that is in sync with circadian rhythms,as well as enhanced sleep hygiene.Implementing tailored dietary therapy,managing meal timing,and ensuring appropriate sleep may improve results for women with GDM,opening up a possible avenue for multi-center trials.

Evaluation of slope stability through rock mass classification and kinematic analysis of some major slopes along NH-1A from Ramban to Banihal, North Western Himalayas

The network of Himalayan roadways and highways connects some remote regions of valleys or hill slopes,which is vital for India’s socio-economic growth.Due to natural and artificial factors,frequency of slope instabilities along the networks has been increasing over last few decades.Assessment of stability of natural and artificial slopes due to construction of these connecting road networks is significant in safely executing these roads throughout the year.Several rock mass classification methods are generally used to assess the strength and deformability of rock mass.This study assesses slope stability along the NH-1A of Ramban district of North Western Himalayas.Various structurally and non-structurally controlled rock mass classification systems have been applied to assess the stability conditions of 14 slopes.For evaluating the stability of these slopes,kinematic analysis was performed along with geological strength index(GSI),rock mass rating(RMR),continuous slope mass rating(CoSMR),slope mass rating(SMR),and Q-slope in the present study.The SMR gives three slopes as completely unstable while CoSMR suggests four slopes as completely unstable.The stability of all slopes was also analyzed using a design chart under dynamic and static conditions by slope stability rating(SSR)for the factor of safety(FoS)of 1.2 and 1 respectively.Q-slope with probability of failure(PoF)1%gives two slopes as stable slopes.Stable slope angle has been determined based on the Q-slope safe angle equation and SSR design chart based on the FoS.The value ranges given by different empirical classifications were RMR(37-74),GSI(27.3-58.5),SMR(11-59),and CoSMR(3.39-74.56).Good relationship was found among RMR&SSR and RMR&GSI with correlation coefficient(R 2)value of 0.815 and 0.6866,respectively.Lastly,a comparative stability of all these slopes based on the above classification has been performed to identify the most critical slope along this road.

The connection and interaction between the eye and the brain are crucial to understanding brain disorders(Marchesi et al.,2021).Both the eye and the brain have a limited regenerative capacity as there are few progenitor cells,and nerve cells do not replicate.Hence,neurodegeneration implicates irreversible damage to the central nervous system,as observed in several neurodegenerative diseases(Marchesi et al.,2021).

Therapeutic potential and pharmacological mechanism of visnagin

Visnagin is a furanochromone and one of the most important compound in the Ammi visnaga (L.) Lam (a synonym of Visnaga daucoides Gaertn.) plant,which is used to cure various ailments.Many investigations into the bioactive properties of visnagin have been studied to date.The literature on visnagin demonstrates its biological properties,including anti-inflammatory,anti-diabetic,and beneficial effects in cardiovascular and renal diseases.Moreover,visnagin improves sperm quality parameters,stimulates steroidogenesis,and increases serum gonadotropins and testosterone levels,while decreasing proinflammatory cytokines,oxidative damage,genomic instability,and it modulates apoptosis.Thus,visnagin has emerged as an exciting lead for further research,owing to its potential in various unmet clinical needs.The current review summarized its basic structure,pharmacokinetics,and pharmacological effects,focusing on its mechanisms of action.The review will help to understand the potential of visnagin as an alternative treatment strategy for several diseases and provide insight into research topics that need further exploration for visnagin’s safe clinical use.
