Endoscopic ultrasound guided fine needle aspiration and useful ancillary methods

The role of endoscopic ultrasound(EUS) in evaluating pancreatic pathology has been well documented from the beginning of its clinical use. High spatial resolution and the close proximity to the evaluated organs within the mediastinum and abdominal cavity allow detection of small focal lesions and precise tissue acquisition from suspected lesions within the reach of this method. Fine needle aspiration(FNA) is considered of additional value to EUS and is performed to obtain tissue diagnosis. Tissue acquisition from suspected lesions for cytological or histological analysis allows, not only the differentiation between malignant and non-malignant lesions, but, in most cases, also the accurate distinction between the various types of malignant lesions. It is well documented that the best results are achieved only if an adequate sample is obtained for further analysis, if the material is processed in an appropriate way, and if adequate ancillary methods are performed. This is a multi-step process and could be quite a challenge in some cases. In this article, we discuss the technical aspects of tissue acquisition by EUS-guided-FNA(EUS-FNA), as well as the role of an on-site cytopathologist, various means of specimen processing, and the selection of the appropriate ancillary method for providing an accurate tissue diagnosis and maximizing the yield of this method. The main goal of this review is to alert endosonographers, not only to the different possibilities of tissue acquisition, namely EUS-FNA, but also to bring to their attention the importance of proper sample processing in the evaluation of various lesions in the gastrointestinal tract and other accessible organs. All aspects of tissue acquisition(needles, suction, use of stylet, complications, etc.) have been well discussed lately. Adequate tissue samples enable comprehensive diagnoses, which answer the main clinical questions, thus enabling targeted therapy.

Improving energy efficiency of apple production by reduced application of pesticides

A large number of apple orchards are treated over 20 times during the vegetation period with high application rates(over 1000 L/hm^(2))or medium application rates(500-1000 L/hm^(2))of pesticides which require significant energy *** research was carried out in the Serbian region of Vojvodina with the aim to show the possibilities to reduce energy usage in apple production by reducing pesticide application rates(200-500 L/hm^(2))and smaller controlled number of treatments with pesticides while maintaining the biological efficiency of apple chemical *** results showed that the cumulative life cycle energy demand of apple production in Vojvodina,assuming a typical 22 annual treatments and relatively high pesticide application rate(1150 L/hm^(2)),was 48 GJ/hm^(2)and energy output was 94 GJ/hm^(2).Reduced number of treatments and lower pesticide application rates have a favorable impact on energy inputs associated with diesel fuel,machinery,chemicals,water and electricity consumption and usage,whereas other energy inputs remain *** energy input for 12 treatments with pesticide application rates of 381 L/hm^(2)was 36 GJ/hm^(2),which is a 25%reduction in comparison to 22 treatments with a pesticide application rate of 1150 L/hm^(2).Reduced number of treatments and pesticide application rate increased the energy use efficiency from 1.96 to 2.61,energy productivity from 0.82 kg/MJ to 1.09 kg/MJ,and net energy from 46 GJ/hm^(2)to 58 GJ/hm^(2).Results also suggest that applying the correct IPM approach can easily lead to a strong reduction in the number of treatments and a major energy saving.

Splenectomy with chemotherapy vs surgery alone as initial treatment for splenic marginal zone lymphoma

AIM:To evaluate the clinical characteristics of splenic marginal-zone lymphoma (SMZL) following antigen expression and the influence of therapeutic approaches on clinical outcome and overall survival (OS).METHODS: A total of 30 patients with typical histological and immunohistochemical SMZL patterns were examined. Splenectomy plus chemotherapy was applied in 20 patients, while splenectomy as a single treatment-option was performed in 10 patients. Prognostic factor and overall survival rate were ***: Complete remission (CR) was achieved in 20 (66.7%), partial remission (PR) in seven (23.3%), and lethal outcome due to disease progression occurred in three (10.0%) patients. Median survival of patients with a splenectomy was 93.0 mo and for patients with splenectomy plus chemotherapy it was 107.5 mo (Log rank=0.056, P>0.05). Time from onset of first symptoms to the beginning of the treatment (mean 9.4 mo) was influenced by spleen dimensions, as measured by computerized tomography and ultra-sound (t=2.558, P=0.018). Strong positivity (+++) of CD20 antigen expression in splenic tissue had a positive influence on OS (Log rank=5.244, P0.05) effects on the OS. The expression of other antigens (immunohistochemistry) also had no effect on survival-rate, as measured by a χ2 test (P>0.05).CONCLUSION: Initial splenectomy combined with chemotherapy has been shown to be beneficial due to its advanced remission rate/duration; however, a larger controlled clinical study is required to conf irm our findings.
