Outcomes of Ahmed valve surgery for refractory glaucoma in Dhahran, Saudi Arabia

AIM: To evaluate the outcomes of Ahmed glaucoma valve(AGV) implantation surgery for refractory ***: This one-armed historical cohort study was conducted in 2011. Refractory glaucoma was defined as eyes with an intraocular pressure(IOP) greater than21 mm Hg with maximally tolerated glaucoma medications, failed surgeries, or both. For all eyes with refractory glaucoma that underwent AGV implantation,data were collected on IOP, the best corrected visual acuity(BCVA) and glaucoma medications preoperatively and 4, 6, 12, 24 and 56 wk postoperatively. Logarithm values of IOP were calculated and compared. RESULTS: The study group was comprised of 30patients(30 eyes, 16 males and 14 females) with refractory glaucoma. Mean preoperative IOP was 39.3 ±13.8 mm Hg. Postoperative mean IOP was 15.7±7.1 mm Hg,19.6 ±12.8 mm Hg and 13.9 ±14.2 mm Hg at 12, 24 and56 wk respectively. BCVA was ≥6/60 in 11 eyes preoperatively, and five eyes had BCVA ≥6/60 at 56 wk postoperatively. Preoperatively, more than four medications were used to treat glaucoma in 21 eyes. At12 wk postoperatively, no medications were required to control IOP in 20 eyes. At 56 wk postoperatively, at least one medication was required to control IOP in 10 *** the entire follow up period, four eyes were treated with yttrium aluminium garnet(YAG) laser and 14 eyes required a second surgery. The AGV was removed in four ***: AGV implantation reduced IOP and the number of medications required to control refractory glaucoma. However, there was a higher risk of decreased vision. Long-term follow up and prompt intervention are recommended.

This cohort study included 36 eyes of 21 patients with high myopia treated with angle supported phakic intraocular lens（p IOL）.Endothelial cell density（ECD） at baseline,6 mo and 3 y were 3017±296,2775±265 and 2558±299 cells/mm^2 *** loss at 6 mo was 7.2% and annual ECD loss was 3% over 36 *** distance visual acuity at 36 mo was 0.4 log MAR or better in 32（88.9%） *** pressure did not change（P=0.9）.No eyes developed cataract,retinal detachment or pupillary *** supported p IOL gives good visual *** cell loss should be monitored.

Validity of autorefractor based screening method for irregular astigmatism compared to the corneal topography-a cross sectional study

AIM：To present a method of screening for irregular astigmatism with an autorefractor and its determinants compared to corneal ***：This cross-sectional validity study was conducted in 2013 at an eye hospital in Spain.A tabletop autorefractor（test 1）was used to measure the refractive status of the anterior surface of the cornea at two corneal meridians of each *** corneal topography（test 2）and Bogan’s classification was used to group eyes into those with regular or no astigmatism（GRI）and irregular astigmatism（GRII）.Test 1 provided a single absolute value for the greatest cylinder difference（Vr）.The receiver operating characteristic（ROC）were plotted for the Vr values measured by test 1 for GRI and GRII *** the basis a Vr value of 1.25 D as cut off,sensitivity,specificity were also ***：The study sample was comprised of 260 eyes（135 patients）.The prevalence of irregular astigmatism was 42%[95%confidence interval（CI）：36,48].Based on test 2,there were 151 eyes in GRI and 109 eyes in *** median Vr was 0.75 D（25%quartile,0.5 D）for GRI and1.75 D（25%quartile,1.25 D）for *** area under curve was 0.171 for GRI and 0.83 for *** sensitivity of test I was 78.1%and the specificity was 76.1%.CONCLUSION：A conventional autorefractor can be effective as a first level screening method to detect irregular corneal astigmatism in places where corneal topography facilities are not available.

目的：研究≥3.00D近视儿童视网膜变性的的发病率,类型和决定因素。方法：选取研究对象为2010/2011年期间在印度马哈拉施特拉邦一所眼科医院就诊的患者。询问并记录高度近视儿童家属成员的近视史,并统计人口信息。由验光师评估每位患者的视力和屈光状态。由眼科医生对患儿进行眼科检查,评估中央和周边视网膜变化。计算出中央及周边视网膜变性的百分比。评估视网膜改变与家族近视史性别、年龄、宗教以及近视程度的相关性。应用单因素和多因素分析。结果：共纳入≥3.00D近视儿童499位（847眼）。检查发现150眼（17.7%,95%可信区间15.4~20.4）玻璃体视网膜退化性改变。黄斑变性、后巩膜葡萄肿及格子样变性比例分别为2.0%、4.6%和18%。眼轴大于24 mm是大于5.00D近视发生玻璃体视网膜退化性改变的显著危险因素（OR=3.88）。视觉辅助的依从性为58.4%。499位患儿中8.8%视力低于6/60,他们可借助于低视力保健。结论：近1/5近视患者存在玻璃体视网膜变性。高度近视玻璃体视网膜变性较少有严重后果。建议应定期对近视眼进行详细的视网膜检查。

AIM:To determine whether the levator palpebrae superioris(LPS)/superior rectus(SR)muscle complex,can influence the position of the upper lid and fornix in acquired anophthalmic ***:This comparative non-randomized and non-interventional study included retrospective data of 21 patients with unilateral acquired anophthalmic sockets repaired with spheric ***-resolution computed tomography(CT)measurements of the LPM/SR muscle complex and clinical topographic position of the upper lid,superior and inferior fornix depth in primary gaze position were *** data were presented as frequency and percentage proportions and quantitative variables comparing the socket measurements with the normal contralateral orbit was statistically analyzed using non-parametric tests considering P0.05,all comparisons).The superior fornix depth was similar in the anophthalmic socket and the contralateral normal orbit(P=0.192)as well the inferior fornix depth(P=0.351).CONCLUSION:Acquired anophthalmic sockets repaired with spheric implants have shorter LPS,thicker SR,and more lagophthalmos than normal *** relationship of the LPS and SR with other orbital structures,associated with passive or active forces acting in the final position of the lids and external ocular prosthesis should be further investigated.

Management of urethral strictures and stenosis caused by the endo-urological treatment of benign prostatic hyperplasiad-a single-center experience

Objective:Urethral stricture disease after endo-urological treatment of benign prostatic hyperplasia(BPH)is a sparsely described *** describe management of five categories of these strictures in this retrospective observational case ***:One hundred and twenty-one patients presenting with symptoms of bladder outflow obstruction after endo-urological intervention for BPH from February 2016 to March 2019 were *** them,76 were eligible for this study and underwent reconstructive *** and postoperative assessments were done with symptom scores,uroflowmetry,ultrasound for post-void residue,and *** intervention during follow-up was classed as a *** recurrence and 95%confidence interval for recurrence percentage were ***:The following five categories of patients were identified:Bulbo-membranous(33[43.4%]),navicular fossa(21[27.6%]),penile/peno-bulbar(8[10.5%]),bladder neck stenosis(6[7.9%]),and multiple locations(8[10.5%]).The average age was 69 years(range:60-84 years).Overall average symptom score,flow rate,and post-void residue changed from 21 to 7,6 mL/s to 19 mL/s,and 210 mL to 20 mL,*** average follow-up was 34 months(range:12-58 months).Overall recurrence and complication rates were 10.5%and 9.2%,*** recurrence in each category was seen in 3,1,2,1,and 1 patient,*** 95% confidence interval for recurrence percentage was ***:Urethral stricture disease is a major long-term complication of endo-urological treatment of *** bulbo-membranous strictures need continence preserving *** fossa strictures require minimally invasive and cosmetic ***-bulbar strictures require judicious use of grafts and *** neck stenosis in this cohort could be treated with endoscopic *** locations need treatment based on their sites in single-stage as far as possible.
