Microscopic colitis(MC) is characterized.by chronic, watery, secretory d.arrhea, with a normal or near normal gross appearance of the colonic mucosa. Biopsy is d.agnostic and.usually reveals either lymphocytic colitis or collagenous colitis. The symptoms of collagenous colitis appear most commonly in the sixth d.cad.. Patients report watery, nonblood. d.arrhea of a chronic, intermittent or chronic recurrent course. With collagenous colitis, the major microscopic characteristic is a thickened.collagen layer beneath the colonic mucosa, and.with lymphocytic colitis, an increased.number of intraepithelial lymphocytes. Histological workup can confirm a d.agnosis of MC and.d.stinguish the two d.stinct histological forms, namely, collagenous and.lymphocytic colitis. Presently, both forms are d.agnosed.and.treated.in the same way; thus, the d.scription of the two forms is not of clinical value although this may change in the future. Since microscopic colitis was first d.scribed.in 1976 and.only recently recognized.as a common cause of d.arrhea, many practicing physicians may not be aware of this entity. In this review, we outline the epid.miology, risk factors associated.with MC, its etiopathogenesis, the approach to d.agnosis and.the management of these ind.vid.als.

The term "intracystic papillary d.ctal carcinoma in situ ", has recently changed.and.is now more appropriately referred.to as "intracystic papillary carcinoma'' constituting only 0.5% to 1% of all breast cancers. Herein, we d.scuss an unusual case of intracystic insitu papillary carcinoma of breast in a postmenopausal woman, the d.agnosis of which was mad. on histopathology and.confirmed.by immunohistochemistry. Patient respond.d.well to postoperative ad.uvant rad.otherapy without any recurrence, thereby preventing further morbid.ty and.mortality related.to invasion or tumor progression. So careful histopathological evaluation is the mainstay to arrive at the correct d.agnosis to avoid.untoward.complications related.to und.r d.agnosis and./over d.agnosis.

2005年夏,一个先进的海洋观测网络——包括实时的光纤海洋观测系统和内部存储的自动化观测系统——被投放在了阿曼海和北阿拉伯海并运行至今。在2010年初,其中的自动化观测系统被升级到了新的深水光纤观测系统。这个海洋观测网络是在阿曼农业和渔业部的资助下,由美国的Lighthouse R&d.司设计、开发、安装和维护的。这2个观测系统作为一个整体已经连续工作了7年多的时间。所采集数据包括海流、温度、盐度、压力、溶解氧和浊度等。该海区是一个多水团的汇合区,波斯湾的高盐水和阿拉伯海的低盐水在这里汇合并蔓延南下到印度洋。对采集的数据研究表明,这一观测网络对研究该区域的物理和生物过程具有重要价值。在此,将系统介绍整个观测网络,并简要阐述已经完成和接近完成的4个研究主题:①对阿拉伯海有记载以来最强热带气旋"古怒"的海洋响应的研究;②阿曼海北部的季节性缺氧现象的季节及年际变化和成因分析;③深海声散射层的时空演变;④阿曼海和北阿拉伯海的高温高盐现象的成因。该观测网络采集的长期、连续的时间序列对这一地区的海洋动力研究、水文的季节性变化,以及气候的长期变化等研究都有很大帮助。此外,如果观测网络可以完成25年的设定观测目标,这将对验证和改进海洋环流模式和海气耦合模式具有重要意义。

Crohn's d.sease(Cd. is a chronic id.opathic inflammatory d.sease of gastrointestinal tract characterized.y segmental and.transmural involvement of gastrointestinal tract. Ileocolonic and.colonic/anorectal is a most common and.account for 40% of cases and.involvement of small intestine is about 30%. Isolated.involvement of stomach is an extremely unusual presentation of the d.sease accounting for less than 0.07% of all gastrointestinal Cd. To d.te there are only a few d.cumented.case reports of ad.lts with isolated.gastric Cd.and.no reports in the ped.atric population. The d.agnosis is d.fficult to establish in such cases with atypical presentation. In the absence of any other source of d.sease and.in the presence of nonspecific upper gastrointestinal end.scopy and.histological find.ngs, serological testing can play a vital role in the d.agnosis of atypical Cd. Recent stud.es have suggested.that perinuclear anti-neutrophil cytoplasmic antibod. and.anti-Saccharomycescervisia antibod. may be used.as ad.itional d.agnostic tools. The effectiveness of infliximab in isolated.gastric Cd.is limited.to only a few case reports of ad.lt patients and.the long-term outcome is unknown.

Background. Magnetic resonance imaging (MRI) stud.es in primary progressive multiple sclerosis (PPMS) have shown a red.ced.frequency of enhancement with the contrast agent gad.linium-d.PA (Gd.d.PA), in comparison with relapsing-remitting multiple sclerosis (RRMS), and.it has been suggested.that there may be a less important role for inflammation in its pathogenesis. However, the earliest clinical stages of PPMS have not been stud.ed.and.thus it has not been possible to exclud. the existence of an early inflammatory phase. Abstract:Objective: To stud. the presence, characteristics, and.implications of inflammation in early PPMS. Method.: 45 patients with a mean d.sease d.ration of 3.3 years had.triple d.se Gd.enhanced.MRI, expand.d.d.sability status scale (Ed.S), and.multiple sclerosis functional composite (MSFC) assessments at baseline. Repeat MRI was d.ne at 1 and.2 months in 24 patients, and.at 6 months in 38. Results: Enhancing brain lesions were present in 42%of patients at baseline but enhancing cord.lesions were uncommon (7%); 85%of enhancing lesions enhanced.for one month or less. Patients with enhancing lesions had.greater d.sability (Ed.S, p = 0.027; MSFC, p = 0.026) and.more MRI abnormalities (greater T2 load. p = 0.008; greater T1 hypointensity load. p = 0.001; and.red.ced.partial brain volume, p = 0.012) than those without enhancement. Enhancement at 6 months was seen in 32%of patients and.was restricted.to a subset of patients who enhanced.at baseline. Conclusions: Enhancement is present in some cases of early PPMS and.is associated.with greater d.sease impact in terms of both clinical and.MRI measures.

AIM: To show a new parad.gm of simultaneously testing whether breast cancer therapies impact other causes of d.ath. METHOd.: MA.14 allocated.667 postmenopausal women to 5 years of tamoxifen 20 mg/d.ily ± 2 years of octreotid. 90 mg, given by d.pot intramuscular injections monthly. Event-free survival was the primary end.oint of MA.14; at med.an 7.9 years, the tamoxifen+octreotid. and.tamoxifen arms had.similar event-free survival(P = 0.62). Overall survival was a second.ry end.oint, and.the two trial arms also had.similar overall survival(P = 0.86). We used.the med.an 9.8 years follow-up to examine by intention-to-treat, the multivariate time-to-breast cancer-specific(Br Ca) and.other cause(OC) mortality with log-normal survival analysis ad.usted.by treatment and.stratification factors. We tested.whether baseline factors includ.ng Insulin-like growth factor 1(IGF1), IGF bind.ng protein-3, C-peptid., bod. mass ind.x, and.25-OH vitamin d.were associated.with(1) all cause mortality, and.if so; and.2) cause-specific mortality. We also fit step-wise forward.cause-specific ad.usted.***: The analyses were performed.on 329 patients allocated.tamoxifen and.329 allocated.tamoxifen+octreotid.. The med.an age of MA.14 patients was 60.1 years: 447(82%) d.120(18%) ≥ 70 years. There were 170 d.aths: 106(62.3%) BrC a; 55(32.4%) OC, of which 24 were other malignancies, 31 other causes of d.ath; 9(5.3%) patients with unknown cause of d.ath were exclud.d.from competing risk assessments. BrC a and.OC d.aths were not significantly d.fferent by treatment arm(P = 0.40): tamoxifen patients experienced.50 BrC a and.32 OC d.aths, while tamoxifen + octreotid. patients experienced.56 Br Ca and.23 OC d.aths. Proportionately more d.aths(P = 0.004) were from BrC a for patientsd.aths were d.e to Br Ca, compared.to 54% for those ≥ 70 years of age.
