To evaluate the role of platelet-rich plasma in healing of acute diaphyseal fractures of the femur

Purpose： New research is focusing on the use of autologous growth factors to increase the effect of bone fracture healing while decreasing the amount of healing time for the patient. Platelets have been demonstrated to be the natural storage vessel for several growth factors and cytokines that promote blood coagulation, tissue repair, and the process of bone mineralization. The present study aims to evaluate the role of platelet-rich plasma （PRP） in healing of acute femoral shaft fractures radiologically. We hypothesize that it provides artificial hematoma and releases various growth factors. Methods： This prospective randomized study was carried out in 72 patients of traumatic fracture of the femoral shaft operated with interlocking nails （closed or open）. Patients were divided into two groups： study group A （n - 33） treated with intramedullary nailing ＆ PRP injection/gel application in the same setting; and control group B （n - 39） treated with intramedullary nailing without PRP application. Both groups were further divided into two subgroups. Study group included subgroup AI （n = 14） operated with closed intTamedullary nailing and PRP injection at the fracture site under radiological control, and subgroup A2 （n = 19） operated with open intramedullary nailing and PRP gel along with fibrin membrane application at the fracture site; while control group included subgroup B1 （n - 16） operated with closed intramedullary nailing, and subgroup B2 （n -- 23） operated with open intramedullary nailing. Radiological assessment of fracture healing was done by measuring the cortex to callus ratio every month till union at 6 months. Results： Measurements of mean cortex to callus ratio revealed significant difference between the groups A ＆ B at third and fourth months. Measurements of mean cortex to callus ratio did not reveal significant difference between the subgroups at first and sixth months. PRIMARY synovial chondromatosis is a rarecondition in which foci of cartilage develop in thesynovial membrane of joints, bursae and tendonsheaths. It typically involves a single large jointin a young adult male.1 The ectopic foci of cartilage canresult in painful joint effusion, and the generation of loosebodies can cause mechanical symptoms.2,3 The etiology ofprimary synovial chondromatosis remains unknown, butmetaplastic theory is the most popular hypothesis. Thecommonly involved joints are knee, elbow, and hip.^4 Theshoulder is a rare site of synovial chondromatosis2,3,5 andthe extra-articular involvement even rarer, with only fewcases presented in literature. This report presents a rarecase of primary combined intra-articular andextra-articular synovial chondromatosis of shoulder jointtreated with arthroscopy.

Implantless patellar fixation in medial patellofemoral ligament reconstruction

Purpose:The medial patellofemoral ligament(MPFL)acts as primary restraint to lateral patellar dislocation and its rupture has been reported in almost all cases of acute patellar *** surgical techniques have been described for MPFL reconstruction,using many femoral and patellar fixation techniques and different *** article details our technique for MPFL reconstruction using semitendinosus graft which avoids the use of implant at patellar ***:Twenty patients(8 males and 12 females)with complaints regarding acute and chronic lateral patellar instability were evaluated and treated by MPFL reconstruction *** mean age of patients was 21 years(range 17-34 years).MPFL reconstruction was performed using semitendinosus graft passing through two parallel,obliquely directed tunnels created in *** of graft was done with an interference screw only at the femoral *** follow-up period after intervention was 26.4 months(range 23-30 months).Results were evaluated using Kujala ***:All patients gained adequate patellar stability and full arc of *** incidence of patella fracture was *** were no postoperative complications related to the *** was no recurrence of instability in patella at final ***:Passing the graft through the tunnels in patella without use of any implant has given excellent functional outcome and moreover has the advantages of less implant-related complications and cost-effectiveness.

Objective：Metaphyseal-diaphyseal fractures of the distal radius are a major treatment dilemma and orthopaedic surgeons have to pay due consideration to restoration of anatomy of distal radius together with rotation of the radial shaft and maintenance of radial bow and interosseous *** performed this study to evaluate the clinic-radiological outcome of metaphyseal-diaphyseal fractures of the distal radius treated with long volar locking ***：This prospective study involved 27 patients （22 males and 5 females） with metaphyseal-diaphyseal fracture of the distal *** mean age was （30.12±11.48） years （range 19-52 years） and the follow-up was 26.8 months （range 22-34 months）.All patients underwent open reduction and internal fixation with a long volar locking *** to AO/OTA classification,there were 7 type A3,13 type C2 and 7 type C3 *** assessment was done based on the disabilities of the arm,shoulder and hand （DASH） *** evaluation was done by measuring grip strength and range of motion around the wrist and the radiological determinants included radial angle,radial length,volar angle and ulnar *** final assessment was done according to Gartland and Werley scoring ***：Postoperative radiological parameters were well maintained throughout the trial,and there was significant improvement in the functional parameters from 6 weeks to final *** average DASH scores improved from 37.5 at 6 weeks to 4.2 at final *** assessment using Gartland and Werley scoring system revealed 66.67%（n=l8） excellent and 33.33% （n=9） good *** was one case of superficial infection which responded to antibiotics and another carpel tunnel syndrome which was managed ***：Volar locking plate fixation for metaphyseal-diaphyseal fractures of distal radius is associated with excellent to good functional outcome,early rehabilitation and minimal complications.

The increasing data pool in finance sectors forces machine learning(ML)to step into new *** data has significant financial implications and is *** users data from several organizations for various banking services may result in various intrusions and privacy *** a result,this study employs federated learning(FL)using a flower paradigm to preserve each organization’s privacy while collaborating to build a robust shared global ***,diverse data distributions in the collaborative training process might result in inadequate model learning and a lack of *** address this issue,the present paper proposes the imple-mentation of Federated Averaging(FedAvg)and Federated Proximal(FedProx)methods in the flower framework,which take advantage of the data locality while training and guaranteeing global *** improves the privacy of the local *** analysis used the credit card and Canadian Institute for Cybersecurity Intrusion Detection Evaluation(CICIDS)***,recall,and accuracy as performance indicators to show the efficacy of the proposed strategy using FedAvg and *** experimental findings suggest that the proposed approach helps to safely use banking data from diverse sources to enhance customer banking services by obtaining accuracy of 99.55%and 83.72%for FedAvg and 99.57%,and 84.63%for FedProx.

Traffic Engineering and Quality of Service in Hybrid Software Defined Networks

For Future networks, many research projects have proposed different architectures around the globe;Software Defined Network(SDN) architectures, through separating Data and Control Layers, offer a crucial structure for it. With a worldwide view and centralized Control, the SDN network provides flexible and reliable network management that improves network throughput and increases link utilization. In addition, it supports an innovative flow scheduling system to help advance Traffic Engineering(TE). For Medium and large-scale networks migrating directly from a legacy network to an SDN Network seems more complicated & even impossible, as there are High potential challenges, including technical, financial, security, shortage of standards, and quality of service degradation challenges. These challenges cause the birth and pave the ground for Hybrid SDN networks, where SDN devices coexist with traditional network devices. This study explores a Hybrid SDN network’s Traffic Engineering and Quality of Services Issues. Quality of service is described by network characteristics such as latency, jitter, loss, bandwidth,and network link utilization, using industry standards and mechanisms in a Hybrid SDN Network. We have organized the related studies in a way that the Quality of Service may gain the most benefit from the concept of Hybrid SDN networks using different algorithms and mechanisms: Deep Reinforcement Learning(DRL), Heuristic algorithm, K path partition algorithm, Genetic algorithm, SOTE algorithm, ROAR method, and Routing Optimization with different optimization mechanisms that help to ensure high-quality performance in a Hybrid SDN Network.

Performance of Bacillus tropicus on Mechanical,Durable and Crack Remediation Properties in Sustainable Vermiculite Concrete

Sustainable cement-based concrete materials are primarily used for construction,among which vermiculite as lightweight fine aggregate gains more future development ***,a bacterial solution was sprayed over vermiculite and wrapped using calcium sulphoaluminate(CSA)cement to replace with fine aggregate in ***,based on a preliminary test on compressive strength results,10%of Ground Granulated Blast Furnace Slag(GGBS)and a healing solution proportion of 9:1 was selected for preparing self-healing *** fine aggregate was replaced in concrete using vermiculite in 0%,5%,10%and 15%and the findings suggest that bacterial vermiculite replacement should be at most 5%to achieve better results in strength and durable *** strength enhancement observed for compressive strength,strength regain,split tensile strength,flexural strength,and ultrasonic pulse velocity were 29.22%,45.5%,34.02%,28.03%and 41.4%*** crack healing at 7,14 and 28 days of BIVC was 38.23%,58.82%and 79.41%,which is 3–4%lower than internal crack *** analysis by Scanning Electron Microscopy(SEM),X-Ray Diffractometer(XRD),and Energy Dispersive Spectroscopy(EDS)reveals the existence of calcite,and it was formed due to the bio-mineral action of bacteria with available nutrients in sustainable concrete.
