目的探讨各节段腰椎间盘退变与矢状面平衡各参数之间的关系。方法回顾性分析2009年7月至2012年6月腰椎退行性疾病患者随访资料共126例，其中摄有脊柱全长正侧位片者38例，仅摄有腰椎正侧位者88例，所有病例均摄有腰椎MrI片。采用ImageJ软件测量以下脊柱-骨盆矢状面参数：（1）脊柱参数：矢状面躯干偏移（SVA）、胸椎后凸角（TK）、胸腰后凸角（TLK）、腰椎前凸角（LL）；（2）骨盆参数：骨盆投射角（PI）、骨盆倾斜角（PT）、骶骨倾斜角（SS）；（3）脊柱-骨盆整体参数：T1脊柱骨盆倾斜角（T1-SPI）、T9脊柱骨盆倾斜角（T9-SPI）、脊柱-骶骨角（SSA）、脊柱-骨盆角（SPA）、C7倾斜角（C7T）。根据椎间盘Pfirrmann分级系统，将各节段椎间盘退变等级分级记录。以矢状面平衡各参数的测量数值与常模的大小关系及椎间盘退变等级为依据进行分组，比较各节段椎间盘退变等级与矢状面平衡参数问的关系。结果PI对L5／S1椎间盘退变程度具有显著影响。L5／S1退变程度对矢状面平衡影响显著。大PI组57例中，L5／S1椎间盘轻一中度和重度退变组LL为（36．9±14．9）和（29．7±11．3），差异有统计学意义（P=0．045）；PT为（18．8±8．3）和（25．2±10．2），差异有统计学意义（P=0．012）；SS为（39．5±8．8）和（34．6±8．9），差异有统计学意义（P=0．042）；SVA为（23．0±26．6）和（62．2±33．8），差异有统计学意义（P：0．002）；C7T为（88．9±3．1）和（84．0±4．6），差异有统计学意义（P=0．003）；SPA为（157．9±11．2）和（147．8±14．0），差异有统计学意义（P=0．043）。L1／2椎间盘轻-中度和重度退变组TLK为（4．6±7，8）和（14．9±14．8），差异有统计学意义（P=0．026）。L1／2，L2／3，L3／4退变程度对SPA有显著影响。L1／2两组SPA为（158．5±12．0）和（147．8±12．7），差异有统计学意义（P=0．031）；L2／3两组SPA为（159．9±10．8）和（148．8±13．2），差异有统计学意义（P=0．024）；L3／4两组SPA为（158．5±10．9）和（148．6±13．9），差异有统计学意义（P=0．047）。结论骨盆解剖学参数的改变PI对腰椎间盘退变程度有重要影响；L5／S1椎间盘退变程度对骨盆姿势性参数（町、ss）有显著影响，L5／S1的退变是引起骨盆后旋机制参与代偿过程的重要因素；LS／S1椎间盘退变对脊柱-骨盆整体参数（SVA、SPA、C7T）有重要影响，L5／S1的退变是引起躯干失衡的重要因素。

AIM: To evaluate the efficacy of thoracic epidural analgesia for extracorporeal shock wave lithotripsy (ESWL). METHODS: ESWL is an effective, non-invasive technique for the treatment of difficult pancreatic and large bile duct calculi. The procedure is often painful and requires large doses of analgesics. Many different anesthetic techniques have been used. Patients with either large bile duct calculi or pancreatic duct calculi which could not be extracted by routine endoscopic methods were selected. Thoracic epidural anesthesia (TEA) was routinely used in all the subjects unless contraindicated. Bupivacaine 0.25% with or without clonidine was used to block the segments D6 to D12. The dose was calculated depending on the age, height and weight of the patient. It was usually 1-2 mL per segment ***: Ninety eight percent of the 1509 patients underwent ESWL under TEA. The subjects selected were within American Society of Anesthesiologists grade Ⅰ to Ⅲ. ESWL using EA permitted successful elimination of bile duct or pancreatic calculi with minimal morbidity. The procedure time was shorter in patients with TEA than in those who underwent ESWL under total intravenous anesthesia. CONCLUSION: Almost all patients undergoing ESWL with EA had effective blocks with a single catheter insertion and local anesthetic injection.

目的:经肝动脉的肝脏肿瘤介入治疗时,有时会引起胆囊的暴露损伤。准确定位胆囊动脉可以避免胆囊损伤。该研究目的是比较基于3D血管造影的血管追踪系统与2D血管造影比较对胆囊动脉的识别。方法:一种可以从三维肝固有动脉造影图像快速识别定位胆囊动脉的软件应用于25个不同的患者,并且与二维血管造影人工定位胆囊动脉比较。结果:应用血管追踪系统,25个患者中共发现28个胆囊动脉。在这些逆行追踪发现的28支胆囊动脉中,27支可以采用软件正确定位胆囊动脉的起源,1支胆囊动脉由于伪影被错误定位。相比之下,二维血管造影,25个患者中,只有12个可见并准确定位（P＜0.001）。血管追踪系统顺行追踪发现56%胆囊动脉有胆囊外分布支,最常见的胆囊外分支供给肝脏S5段肝实质。结论:三维血管追踪系统在发现和定位胆囊动脉中较二维血管造影有明显有优势,并且在是不需要超选择造影的前提下即可证实其分布的重要工具。胆囊动脉供给肝实质或肝内肿瘤并不少见,在肝癌的介入治疗时应引起注意。

Sordariomycetes is one of the largest classes of Ascomycota that comprises a highly diverse range of fungi characterized mainly by perithecial ascomata and inoperculate unitunicate *** class includes many important plant pathogens,as well as endophytes,saprobes,epiphytes,coprophilous and fungicolous,lichenized or lichenicolous *** occur in terrestrial,freshwater and marine habitats *** paper reviews the 107 families of the class Sordariomycetes and provides a modified backbone tree based on phylogenetic analysis of four combined loci,with a maximum five representative taxa from each family,where *** paper brings together for the first time,since Barrs’1990 Prodromus,descriptions,notes on the history,and plates or illustrations of type or representative taxa of each family,a list of accepted genera,including asexual genera and a key to these taxa of *** of taxa is supported where possible by molecular *** outline is based on literature to the end of 2015 and the Sordariomycetes now comprises six subclasses,32 orders,105 families and 1331 *** family Obryzaceae and Pleurotremataceae are excluded from the class.

We report a case of a 42-year-old male with symptomatic choledochocele and incidental pancreas divisum diagnosed with magnetic resonance cholangiopancreatography (MrCP). Small choledochocele is rare congenital malformation associated with non-specific symptoms and a delay in diagnosis. The coexistence of choledochocele and pancreas divisum is extremely rare with only two case reports published in literature. In both cases MrCP failed to diagnose any biliary or pancreatic abnormality. This case suggests that the patients with recurrent abdominal pain and pancreas divisum should not be presumed to be suffering from pancreatitis. Careful evaluated for additional anomalies in the biliary tree should be sought for refractory symptoms. MrCP is a useful one-stop-shop for diagnosing pancreatic and biliary ductal anomalies.

目的:研究柚皮苷对束缚性应激引起的小鼠生化指标改变、行为变化及线粒体功能紊乱的影响。方法:将小鼠按照体质量随机分组,分别给予不同药物治疗14 d。在第15天让所有小鼠接受束缚性应激刺激6 h,然后在接受各种不同的行为测试后处死。取小鼠的大脑匀浆进行各种生化指标测量和线粒体功能分析。结果:6 h的急性束缚性应激刺激能显著改变小鼠的行为(焦虑和记忆),引起生化指标变化和线粒体功能紊乱。用柚皮苷预处理(50或100 mg/kg口服)14 d则能显著减轻急性束缚性应激刺激引起的小鼠行为和生化指标变化以及线粒体功能紊乱(P<0.05);而使用一氧化氮的前体左旋精氨酸预处理(50 mg/kg腹腔注射)可以拮抗柚皮苷的保护效应(P<0.05)。此外,采用左旋精氨酸甲酯预处理(5 mg/kg腹腔注射)则导致柚皮苷的保护效应增强。结论:柚皮苷对束缚性应激刺激引起的小鼠行为和生化指标变化以及线粒体功能紊乱的保护作用与氮能神经通路有关。
