Role of coronary physiology in the contemporary management of coronary artery disease

Coronary artery disease(CAD) remains the leading cause of death worldwide with approximately 1 in 30 patients with stable CAD experiencing death or acute myocardial infarction each year. The presence and extent of resultant myocardial ischaemia has been shown to confer an increased risk of adverse outcomes. Whilst, optimal medical therapy(OMT) forms the cornerstone of the management of patients with stable CAD, a significant number of patients present with ischaemia refractory to OMT. Historically coronary angiography alone has been used to determine coronary lesion severity in both stable and acute settings. It is increasingly clear that this approach fails to accurately identify the haemodynamic significance of lesions; especially those that are visually "intermediate" in severity. Revascularisation based upon angiographic appearances alone may not reduce coronary events above OMT. Technological advances have enabled the measurement of physiological indices including the fractional flow reserve, the index of microcirculatory resistance and the coronary flow reserve. The integration of these parameters into the routine management of patients presenting to the cardiac catheterization laboratory with CAD represents a critical adjunctive tool in the optimal management of these patients by identifying patients that would most benefit from revascularisation and importantly also highlighting patients that would not gain benefit and therefore reducing the likelihood of adverse outcomes associated with coronary revascularisation. Furthermore, these techniques are applicable to a broad range of patients including those with left main stem disease, proximal coronary disease, diabetes mellitus, previous percutaneous coronary intervention and with previous coronary artery bypass grafting. This review will discuss current concepts relevant to coronary physiology assessment, its role in the management of both stable and acute patients and future applications.

The present paper discusses the effects of small plants on the dump mass reinforcement and slope *** roots of smaller plants(such as grasses and shrubs)do not go ***,they stabilize the slope by binding the upper layer of dump *** strength of the dump mass with and without root reinforcement is determined by laboratory shear box *** increased cohesion(apparent cohesion)of upper layer of the dump mass due to plants is determined by fabricated shear *** kinetic behavior of the dump has been investigated using the FLAC *** factor of safety has been calculated in order to determine the possible effect of small plants on the stability of the dump *** is observed that the small plants do not significantly improve the factor of safety(FOS)of ***,it could be useful for early *** grasses quickly bind the upper surface,whereas shrubs too immensely strengthen the stability of the dump in the initial stage.

Electrical Discharge Machining (EDM) is considered as the much traditional, adaptableand unconventional machining process that is proficient in generating complicated structures. Further, it is self-sufficient over the mechanical properties of workpiece materialsonly if it is conductive. The viability of the EDM process regarding the application typesmight get considerably improved with the constant enhancement in the personificationof numerical control and the metal removal efficacy. This survey intends to make a reviewof 65 papers related to the optimization and modeling of EDM process machining. Theanalysis is thus made with regards to the following factors like Al alloy composite in DieShrinking EDM, Optimization and Al metal matrix composites. Particularly, the hybridoptimization models in various papers are analyzed concerning their logic. Further, thealgorithmic and parametric analysis is also made with the contributed papers. Finally,the research gaps and challenges in this EDM processing are discussed, which wouldpave the way for efficient research works in this area.

A study was done to determine the effect of best stacking sequence(position and orientation)on the mechanical properties,friction and wear response of hybrid *** main purpose of the study was to determine the best available stacking sequence for which the physical,mechanical and three body abrasive wear rate is *** for mechanical properties(tensile strength,flexural strength,inter laminar shear strength,impact strength and hardness)was done on glass-carbon hybrid *** composites were then subjected to test for three body abrasive wear on a designed experimental *** different factors(sliding velocity,fabric weight percentage,normal load,sliding distance and abrasive particle size)varied in steps to evaluate the friction and three body abrasive wear response of the *** design of experiment using Taguchi’s orthogonal array and analysis of variance(ANOVA)was applied to find out minimum specific wear *** results revealed the significance in arrangement of two different layers of fabric(glass and carbon)and the difference in properties obtained by *** Taguchi experimental analysis it has been observed that three body abrasive wear rate is minimum at 72 cm/s sliding velocity,30 wt.%fabric content,80 N normal load,90 m sliding distance and 125μm abrasive particle *** scanning electron microscope(SEM)studies revealed the dynamics of three‐body abrasive wear and underlying micro‐mechanisms that result in the analysis of three body abrasive wear.

In past decades,cellular networks have raised the usage of spectrum resources due to the victory of mobile broadband *** devices create massive data than ever before,facing the way cellular networks are installed pre-sently for satisfying the increased trafﬁc *** development of a new exclusive spectrum offered to meet up the trafﬁc requirements is challenging as spectrum resources are limited,hence *** radio technology is pre-sented to increase the pool of existing spectrum resources for mobile users via Femtocells,placed on the top of the available macrocell network for sharing the same ***,the concurrent reuse of spectrum resources from Femto networks poses destructive interference on macro *** resolve this issue,this paper introduces an optimal channel allocation model using the Oppo-sitional Beetle Swarm Optimization Algorithm(OBSOA)to allocate the channel with interference avoidance.A new OBSOA is derived in this paper by the inclu-sion of opposition-based learning(OBL)in *** algorithm allocates the channels used by PUs(PUs)to the secondary users(SUs)in such a way that inter-ference is *** proposed approach is implemented in the MATrix LABoratory(MATLAB)*** performance of this proposed approach is evaluated in terms of several measures and the experimental outcome veriﬁed the superior nature of the OBSOA-based channel allocation *** mod-el has resulted in a maximum signal-to-interference-plus-noise ratio value of 86.42 dB.
