Triangle tilt and humeral surgery:Meta-analysis of efficacy and functional outcome

AIM: To systematically review and analyze the overall impact and effectiveness of bony surgical procedures, the triangle tilt and humeral surgery in a comparative manner in permanent obstetric brachial plexus injury(OBPI) ***: We conducted a literature search and identified original full research articles of OBPI patients treated with a secondary bony surgery, particularly addressing the limitation of shoulder abduction and functions. Further, we analyzed and compared the efficacy and the surgical outcomes of 9 humeral surgerypapers with 179 patients, and 4 of our secondary bony procedure, the triangle tilt surgical papers with 86 patients. RESULTS: Seven hundred and thirty-one articles were identified, using the search term "brachial plexus" and obstetric or pediatric(246 articles) or neonatal(219 articles) or congenital(188 articles) or "birth palsy"(121 articles). Further, only a few articles were identified using the bony surgery search, osteotomy "brachial plexus" obstetric(35), "humeral osteotomy" and "brachial plexus"(17), and triangle tilt "brachial plexus"(14). Of all, 12 studies reporting pre- and postoperative or improvement in total Mallet functional score were included in this study. Among these, 9 studies reported the humeral surgery and 4 were triangle tilt surgery. We used modified total Mallet functional score in this analysis. Various studies with humeral surgery showed improvement of 1.4, 2.3, 5.0 and 5.6 total Mallet score, whereas the triangle tilt surgery showed improvement of 5.0, 5.5, 6.0 and ***: The triangle tilt surgery improves on what was achieved by humeral osteotomy in the management of shoulder function in OBPI patients.

Drug-induced autoimmune hepatitis(DIAIH)is a specific phenotype of druginduced liver injury that may lead to the devastating outcome of acute liver failure requiring liver *** implicated in DIAIH include antimicrobials such as nitrofurantoin and minocycline,non-steroidal anti-inflammatory drugs,statins as well as anti-tumor necrosis *** clinical features of druginduced liver injury are indistinguishable from idiopathic autoimmune hepatitis(AIH)as both may have positive AIH-related autoantibodies,elevated immunoglobulin G,as well as similar histopathological *** patients who show no clinical improvement,or there is progressive liver injury despite cessation of the suspected drug,a liver biopsy should be considered,whereby the presence of advance fibrosis on histology favors the diagnosis of idiopathic *** treatment with corticosteroids may be required in patients with non-resolving liver injury.A typical clinical scenario supportive of DIAIH includes a history of drug exposure with spontaneous resolution of liver injury after drug withdrawal and the absence of relapse after rapid steroid *** this article we report two cases of DIAIH secondary to Sorafenib and Atorvastatin along with a review of currently available *** identification and treatment often lead to a favorable outcome in DIAIH.

Disentangling the spin-parity of a resonance via the gold-plated decay mode

Searching for new resonances and finding out their properties is an essential part of any existing or future particle physics experiment. The nature of a new resonance is characterized by its spin, charge conjugation,parity, and its couplings with the existing particles of the Standard Model. If a new resonance is found in the four lepton final state produced via two intermediate Z bosons, the resonance could be a new heavy scalar or a Z boson or even a higher spin particle. In such cases a step by step methodology as enunciated in this paper can be followed to determine the spin, parity and the coupling to two Z bosons of the parent particles, in a fully model-independent way. In our approach we show how three uni-angular distributions and a few experimentally measurable observables can conclusively tell us about the spin, parity as well as the couplings of the new resonance to two Z bosons. We have performed a numerical analysis to validate our approach and showed how the uni-angular observables can be used to disentangle the spin parity as well as the coupling of the resonance.

Rolling element bearing is the most common machine element in rotating *** extended life is among the foremost imperative standards in the optimal design of rolling element bearings,which confide on the fatigue failure,wear,and thermal conditions of *** fill the gap,in the current work,all three objectives of a tapered roller bearing have been innovatively considered respectively,which are the dynamic capacity,elasto-hydrodynamic lubrication(EHL)minimum film⁃thickness,and maximum bearing *** objective function formulations are presented,associated design variables are identified,and constraints are *** solve complex non⁃linear constrained optimization formulations,a best⁃practice design procedure was investigated using the Artificial Bee Colony(ABC)algorithms.A sensitivity analysis of several geometric design variables was conducted to observe the difference in all three *** excellent enhancement was found in the bearing designs that have been optimized as compared with bearing standards and previously published *** present study will definitely add to the present experience based design followed in bearing industries to save time and obtain assessment of bearing performance before *** verify the improvement,an experimental investigation is worthwhile conducting.

Epidemiology of pediatric burns and future prevention strategies—a study of 475 patients from a high-volume burn center in North India

Background: Pediatric burns have a long-term social impact. This is more apparent in a developing country such as India, where their incidence and morbidity are high. The aim of this study was to provide recent prospective epidemiological data on pediatric burns in India and to suggest future preventive strategies. Methods: Children up to 18 years old admitted to the Department of Burns, Plastic&Maxillofacial Surgery, VMMC&Safdarjung Hospital, New Delhi, between January and December 2014 were included in the study. Data regarding age, sex, etiology, total body surface area (TBSA), circumstances of injury, and clinical assessment were collected. The Mann-Whitney test or Kruskal-Wallis test or ANOVA was used to compare involved TBSA among various cohort groups accordingly. Univariate and multivariate linear regression analyses were used to determine the predictors of TBSA. Results: There were a total of 475 patients involved in the study, including seven suicidal burns, all of whom were females with a mean age greater than the cohort average. Age, type of burns, mode of injury, presence or absence of inhalation injury, gender, and time of year (quarter) for admission were found to independently affect the TBSA involved. Electrical burns also formed an important number of presenting burn patients, mainly involving teenagers. Several societal issues have come forth, e.g., child marriage, child labor, and likely psychological problems among female children as suggested by a high incidence of suicidal burns. Conclusions: This study also highlights several issues such as overcrowding, lack of awareness, dangerous cooking practices, and improper use of kerosene oil. There is an emergent need to recognize the problems, formulate strategies, spread awareness, and ban or replace hazardous substances responsible for most burn accidents.

A randomized controlled trial of omega-3 fatty acids in dry eye syndrome

AIM:To evaluate the role of dietary supplementation of omega-3 fatty acids in dry eye ***:A prospective,interventional,placebo controlled,double blind randomized trial was done at two referral eye *** hundred and sixty-four eyes of patients with dry eye were randomized to receive one capsule(500mg)two times a day containing 325mg EPA and 175mg DHA for 3 months(omega-3 group).The omega-3 group was compared to a group of patients(n=254)who received a placebo(placebo group).There were 4 patient visits(at baseline,1 month,2 months and3 months).On each visit,recording of corrected distance visual acuity(CDVA),slit lamp examination and questionnaire based symptom evaluation and scoring was done.A symptomatic score of 0-6 was mild,6.1-12moderate and 12.1-18 severe dry *** to intervention was monitored by routine tear function tests like Schirmer I test,tear film break-up time(TBUT),Rose Bengal staining and most notably,conjunctival impression ***:Sixty-five percent of patients in the omega-3group and 33%of patients in placebo group had significant improvement in symptoms at 3 months(P=0.005).There was a significant change in both Schirmer’s test value and TBUT values in the omega-3group(P<0.001),both ***,there was a larger drift in TBUT values in omega-3 than the placebo group,in comparison to Schirmer’s test *** mean TBUT score was 2.54±2.34 in the omega-3group and 0.13±0.16 in placebo group,*** mean reduction in symptom score in omega-3 group was 2.02±0.96 as compared to 0.48±0.22 in placebogroup(P<0.001).Despite a slight increase mean score,the Schirmer scores did not correlate well with symptomatic ***:Omega-3 fatty acids have a definite role for dry eye *** benefit seems to be more marked in conditions such as blepharitis and meibomian gland *** role of omega fatty acids in tear production and secretion needs further evaluation.

Turbulence,suspension and downstream fining over a sand-gravel mixture bed

Flume experiments were carried out to study the turbulence and its impact on suspension and segregation of grainsizes under unidirectional flow conditions over the sand-gravel mixture *** components of fluid velocity with fluctuations were measured vertically using 3-D Micro-acoustic Doppler velocimeter(ADV).The theoretical models for velocity and sediment suspension have been developed based on the concept of mixing length that includes the damping effect of turbulence due to sediment suspension in the flow over the sand-gravel mixture *** analysis of segregation of grain-sizes along downstream of the bed has been performed using the principle of unsupervised learning or clustering *** data analysis suggests that there is a progressive downstream fining of sediment sizes with selective depositions of gravels,sand-gravels and sand materials along the stream,which may be segmented into three regions such as,the upstream,the transitional and the downstream *** contribution is relevant to understand the direction of ancient rivers,the bed material character in the river form,sorting process and its role in controlling the sediment flux through landscape.
