During the last years two questions have been continuously asked in chronic coronary syndromes:(1)Do revascularization procedures(coronary artery bypass grafting or percutaneous coronary intervention)really improve symptoms of angina?and(2)Do these techniques improve outcomes,i.***.do they prevent new myocardial infarction events and cardiovascular death?Therefore,there was a need for a large definitive trial.This study was the ISCHEMIA trial,a large,multicentric trial sponsored by the National Heart,Lung,and Blood Institute.The main trial compared coronary revascularization and optimal medical treatment(OMT)vs OMT alone in 5179 patients enrolled after a stress test.During a median 3.2-year follow-up,318 primary outcome events occurred;the adjusted hazard ratio for the invasive strategy as compared with the conservative strategy was 0.93(95%confidence interval 0.80-1.08,P=0.34).The ISCHEMIA trial deeply disrupted many of our prior attitudes regarding management strategies for patients with stable coronary artery disease.The findings underscore the benefits of disease-modifying OMT for stable coronary artery disease patients.The main purposes of ischemia assessment before this trial were:Diagnostic purposes,assessment of outcome,and adding to decision-making processes.Obviously,this changed after the trial results.The results of ISCHEMIA might challenge the current diagnostic approach for stable angina patients recommended in the last European Society of Cardiology guidelines on chronic coronary disease that were based on studies published before the ISCHEMIA trial.In this editorial we propose our approach based on the ISCHEMIA study and the pretest probability for a positive test in patients with chronic coronary syndromes.

Endoscopic ultrasonography(EUS) with or without fine needle aspiration has become the main technique for evaluating pancreatobiliary disorders and has proved to have a higher diagnostic yield than positron emission tomography,computed tomography(CT) and transabdominal ultrasound for recognising early pancreatic tumors.As a diagnostic modality for pancreatic cancer,EUS has proved rates higher than 90%,especially for lesions less than 2-3 cm in size in which it reaches a sensitivity rate of 99% vs 55% for CT.Besides,EUS has a very high negative predictive value and thus EUS can reliably exclude pancreatic cancer.The complication rate of EUS is as low as 1.1%-3.0%.New technical developments such as elastography and the use of contrast agents have recently been applied to EUS,improving its diagnostic capability.EUS has been found to be superior to the recent multidetector CT for T stagingwith less risk of overstaying in comparison to both CT and magnetic resonance imaging,so that patients are not being ruled out of a potentially beneficial resection.The accuracy for N staging with EUS is 64%-82%.In unresectable cancers,EUS also plays a therapeutic role by means of treating oncological pain through celiac plexus block,biliary drainage in obstructive jaundice in patients where endoscopic retrograde cholangiopancreatography is not affordable and aiding radiotherapy and chemotherapy.

Amyloidosis is an infiltrative disease caused by extracellular protein deposition that has accumulated a lot of scientific production in recent years.Different types of amyloidosis can affect the heart.Transthyretin amyloidosis and light chain amyloidosis are the two most common types of cardiac amyloidosis.These entities have a poor prognosis,so accurate diagnostic techniques are imperative for determining an early therapeutic approach.Recent advances in cardiac imaging and diagnostic strategies show that these tools are safe and can avoid the use of invasive diagnostic techniques to histological confirmation,such as endomyocardial biopsy.We performed a review on the diagnostic and prognostic implications of different cardiac imaging techniques in cardiac amyloidosis.We mainly focus on reviewing echocardiography,cardiac magnetic resonance,computed tomography and nuclear imaging techniques and the different safety measurements that can be done with each of them.

迷走神经支配人体的内脏器官,其传出纤维激活后可释放乙酰胆碱调节器官功能,而传入纤维能感知器官中机械和化学信号,并将其传入中枢,这种双向交流使得神经中枢能够反馈性调节器官功能。在呼吸系统中,迷走神经调节气道张力、血流供应、黏液分泌及呼吸型式。近来发现,迷走神经与免疫系统产生交互作用,在肺部疾病的发病机制中发挥重要作用,如哮喘、慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)、急性呼吸窘迫综合征(acute respirtory distress sydrome,ARDS)、肺纤维化和肺癌。在疾病状态下,多种细胞释放的细胞因子、化学因子和其他介质激活迷走神经,将免疫信息传到中枢,从而改变病程。激活迷走神经还可以释放神经肽调节免疫细胞的活动,并通过胆碱能抗炎通路控制肺部炎症,因此了解迷走神经在神经免疫反应中的作用对于肺部疾病的临床治疗有着重要意义。

Sustainability Assessment of Alternatives for Heat Generation and Transfer in Saunas

In the search for renewable energy sources, solar energy appears, among others, as a powerful alternative;mainly for those countries located at Earth’s equatorial region. This is the case of a Fresnel device that uses solar radiation to warm-up the air;for instance, within saunas. Authors coined the term sustainable technological innovation to refer to a technological innovation in accordance to the principles of sustainability. The object of this research was comparing the sustainability of a Fresnel Lens Solar Concentration (FLSC) against the sustainability of other twelve alternatives to generate heat. This article assesses the sustainability of fourteen technological options for heat generation and transfer to saunas in the northwest region of Mexico where temperatures often reach above 45 C and where there is a greater potential for exploiting solar energy. Emergy accounting, economic analysis, and global warming potential are the three indicators considered in quantifying sustainability. Results show that infrared ceramic photovoltaic, infrared carbon photovoltaic and infrared metal photovoltaic are the most sustainable systems. The Fresnel device reached the fourth and tenth position, using electricity from photovoltaic and from grid respectively, among all fourteen assessed devices, respectively. Although Fresnel equipment does not release any global warming gases into the atmosphere, or even it demands low amount of non-renewable resources, 1.40 and 4.47 E17 seJF/yr. it requires high economic investment, 20,800 and 79,600 USD/yr, for the implementation and operation phases, which decrease its sustainability performance compared to twelve other evaluated devices.

An important limitation for the success of chemotherapy in the treatment of primary liver cancer (hepatocellular carcinoma,hepatoblastoma and cholangiocarcinoma) is the marked efficacy of mechanisms of chemoresistance (MOC).These have been previously classified into five groups depending on whether they result in:a reduced drug uptake or enhanced drug export (MOC-1);poor intracellular activation of prodrugs or higher inactivation of active drugs (MOC-2);changes in the molecular targets that impairs the action of the drug by increasing the activity of the metabolic route to be inhibited or stimulating alternative routes (MOC-3);ability of tumor cells to repair drug-induced modifications in the target molecule,usually DNA (MOC-4);and the activation or inhibition of intracellular signaling pathways that lead to a change in the balance between proand anti-apoptotic factors favoring tumor cell survival (MOC-5).Nevertheless,novel information appeared over the last few years has recommended to consider two additional groups,MOC-6 and MOC-7,based on changes in tumor microenvironment,mainly hypoxia and acidity,and epithelial-mesenchymal transition,respectively.These contribute to the defensive armamentaria developed or enhanced in liver cancer cells to resist the pharmacological attack,which accounts for a negligible beneficial effect of commonly used antitumor drugs and only a modest response to novel targeted therapies based on tyrosine kinase inhibitors,such as sorafenib.Therefore,further advances are urgently needed to better understand the molecular and cellular bases of the chemoresistant barrier and help scientists in this field to develop new tools able to overcome cancer cell defenses.

Social media has changed the way we learn,educate,and interact with our peers.The dynamic nature of social media and their immediate availability through our portable devices(smartphones,tablets,smartwatches,etc.)is quickly transforming the way we participate in society.The scope of these digital tools is broad as they deal with many different aspects:Teaching and learning,case discussion,congresses coverage,peer to peer interaction,research are examples worth mentioning.The scientific societies considered more innovative,are promoting these tools between their members.These new concepts need to be known by the cardiologists to stay updated,as countless information is moving rapidly through these channels.We summarize the main reasons why learning how to use these tools to be part of the conversation is essential for the cardiologist in training or fully stablished.

Nonlinear Model of Image Noise: An Application on Computed Tomography including Beam Hardening and Image Processing Algorithms

This paper proposes a more inclusive statistical model for predicting image noise in Computed Tomography (CT), associated with scanning factors, considering the effect of beam hardening and image processing filters. It is based on power functions where the levels of the parameters will determine the rate of noise variation with respect to a given scanning factor. It includes the influence of tube potential, tube current, slice thickness, Field of View (FOV), reconstruction methods and post-processing filters. To validate the model, tomographic measurements were made by using a PMMA phantom that simulates paediatric head and adult abdomen, a PET bottle was used to simulate the head of the new-born. The influence of ROI (Region Of Interest) size over nonlinear model parameters was analysed, and high variations of powers of attenuation and FOV were found depending on ROI size. A nonlinear robust regression method was used. The validation was performed graphically by weighted residual analysis. A nonlinear noise model was obtained with an adjusted coefficient of determination for ROI sizes between 10% and 70% of the phantom diameter or FOV. The model confirms the significance of the tube current, slice thickness and beam hardening effect on image. The process of estimation of the parameters of the model by Nonlinear Robust Regression turned out to be optimal.
