Marfan’s syndrome (MFS) is an autosomal dominant disorder characterized by aortic root dilation, mitral valve prolapse and aortic regurgitation. The typical aneurysm location involves the thoracic aorta (60%). Thoracoabdominal aortic dissections are quite infrequent (10%). A type A aortic dissection can have fatal complications, such as aortic rupture and cardiac tamponade, and a prompt diagnosis is crucial. The present case involves a 46-year-old patient with Marfan’s syndrome who was admitted to the emergency department for dyspnea. The patient presented congestive signs and manifestations suggesting acute heart failure. An initial transthoracic echocardiogram (TTE) showed ventricular hypertrophy and diastolic dysfunction. Additionally, TTE showed plurivalvular regurgitation, pulmonary hypertension (42 mmHg), aortic dilatation, and a dissection flap. Coronary computed tomography angiography (CCTA) was performed, confirming the suspicion of ascending aortic dissection. The patient underwent a three-stage intervention for the repair of all the defects, with adequate recovery. This was a rare presentation of aortic dissection in MFS, which was a diagnostic challenge for the heart team.

A series of lithium metasilicate （Li2SiO3） powder materials has been successfully synthesized by the microwave-assisted hydrothermal route using lithium hydroxide and tetraethyl-orthosilicate-derived sol precursors. Ceramic powders were obtained under hydrothermal conditions of autogenous pressure in the presence of a nonionic surfactant. The production of pure and well-crystallized Li2 SiO3 using very short reaction times at low temperatures was shown by X-ray diffraction, scanning electron microscopy, and N2 adsorption-desorption analyses. Synthesized Li2SiO3 particles were nanocrystalline and exhibited different morphologies and specific surface areas depending on the synthesis conditions. Additionally, the capability of selected Li2SiO3 samples to absorb H20 and CO2 was evaluated via thermogravimet- ric analyses by varying the temperature, carrier gas, and water vapor concentration. Li2SiO3 particles exhibited interesting textural and morphological characteristics that make them suitable for use as a CO2 absorbent and which suggest that they also have the potential to be used in other applications.

Phytoremediation is considered a viable and cost effective emerging technology to clean-up trace elements. This approach has not been fully commercialized due the existence of various concerns about it. Those can be summarized as the uncertainty of the system behaviors at different scenarios, such as: contaminant, contaminant concentration and the behaviors of the physiology in the plant. Previous approaches have implemented diverse mathematical algorithms to characterize phytoremediation systems, such as: differential equation solution sets, statistical correlation and system dynamics approach. Phytoremediation Dynamic Model (PDM) employed the classical plant structure to simulate plant-soil-pollutant interaction. This model has proved its capability to mimic phytovolatilization processes of mercury chloride, obtaining more than 95% of correlation between the experimental data, and also provides the capability to know the contaminant flow rate and its concentration in plant tissue. The differential equations system which describes the model includes a comprehensive parameter which encapsulates plant bioavailability dependence in the contaminant-media interaction as a novel approach because this has not been found on the literature previously. PDM has proved the ability to mimic plant response as a function of contaminant concentration and the applicability as an assessment tool for phytoremediation system performance.

Three-dimensional(3D)hydrodynamic numerical simulations of laser driven thin-shell gas-filled microballoons have been carried out using the computer code MULTI-3D[Ramis et al.,*** 21,082710(2014)].The studied configuration corresponds to experiments carried at the ORION laser facility[Hopps et al.,Plasma *** Fusion 57,064002(2015)].The MULTI-3D code solves single-temperature hydrodynamics,electron heat transport,and 3D ray tracing with inverse bremsstrahlung absorption on unstructured Lagrangian *** emphasis has been placed on the genuine 3D effects that are inaccessible to calculations using simplified 1D or 2D *** include the consequences of(i)a finite number of laser beams(10 in the experimental campaign),(ii)intensity irregularities in the beam crosssectional profiles,(iii)laser beam misalignments,and(iv)power imbalance between *** consequences of these imperfections have been quantified by post-processing the numerical results in terms of capsule nonuniformities(synthetic emission and absorption images)and implosion efficiency(convergence ratio and neutron yield).Statistical analysis of these outcomes allows determination of the laser tolerances that guarantee a given level of target performance.

Background: Infections in ICU’s patients are known to often originate from the colonization of wounds by the patient’s endogenous microbiota, and to eventually lead to secondary sepsis. Aim: to compare in vitro the direct and residual effects after different exposure times of 4% chlorhexidine, and of 0.1% and 0.04% polyhexanide (in gel and solution forms), on ATCC-microorganisms, and too, on bacterial strains obtained from ICU patients. Methods: We used wild multi-drug resistant strains recently obtained from the wounds of patients hospitalized at ICU and reference strains from the American Type Culture Collection (ATCC). Chlorhexidine 4% was studied as a reference solution. The direct and residual effects of the 0.1% and 0.04% polyhexanide, in gel and solution forms, were analyzed using cotton germ carriers. To evaluate the direct effect, we exposed the strains to the antiseptic. To assess the residual effect, the germ-carriers were impregnated with antiseptic and were allowed to dry before we contaminated them. We inoculated the germ carriers in a culture medium with an inhibitor of antiseptic effect to count the number of surviving microorganisms. Findings: 0.1% Polyhexanide solution proved a direct and residual efficacy after 24 hours equivalent to 4% chlorhexidine. Is very important to highlight that this great efficacy did not change according to whether they were ATCC or multidrug-resistant strains. Conclusions: 0.1% polyhexanide demonstrated a great direct and residual efficacy (like 4% chlorhexidine), against multi-drug resistant strains isolated from ICU’s patients. Moreover, due to its few cytotoxicity against keratinocytes and fibroblasts can be an optimal antiseptic for burns, wounds or ulcers.

AIM : To evaluate the prediction error in intraocular lens(IOL) power calculation for a rotationally asymmetric refractive multifocal IOL and the impact on this error of the optimization of the keratometric estimation of the corneal power and the prediction of the effective lens position(ELP).METHODS: Retrospective study including a total of 25 eyes of 13 patients(age, 50 to 83y) with previous cataract surgery with implantation of the Lentis Mplus LS-312 IOL(Oculentis Gmb H, Germany). In all cases, an adjusted IOL power(P IOLadj) was calculated based on Gaussian optics using a variable keratometric index value(n kadj) for the estimation of the corneal power(P kadj) and on a new value for ELP(ELP adj) obtained by multiple regression *** P IOLadj was compared with the IOL power implanted(P IOLReal) and the value proposed by three conventional formulas(Haigis, Hoffer Q and Holladay Ⅰ).RESULTS: P IOLReal was not significantly different than P IOLadj and Holladay IOL power(P >0.05). In the Bland and Altman analysis, P IOLadj showed lower mean difference(-0.07 D) and limits of agreement(of 1.47 and-1.61 D)when compared to P IOLReal than the IOL power value obtained with the Holladay formula. Furthermore, ELP adj was significantly lower than ELP calculated with other conventional formulas(P <0.01) and was found to be dependent on axial length, anterior chamber depth and P kadj. CONCLUSION: Refractive outcomes after cataract surgery with implantation of the multifocal IOL Lentis Mplus LS-312 can be optimized by minimizing thekeratometric error and by estimating ELP using a mathematical expression dependent on anatomical factors.

Mesoamerica and the Caribbean are low-latitude regions at risk for the effects of climate change. Global climate models provide large-scale assessment of climate drivers, but, at a horizontal resolution of 100 km, cannot resolve the effects of topography and land use as they impact the local temperature and precipitation that are keys to climate impacts. We developed a robust dynamical downscaling strategy that used the WRF regional climate model to downscale at 4 - 12 km resolution GCM results. Model verification demonstrates the need for such resolution of topography in order to properly simulate temperatures. Precipitation is more difficult to evaluate, being highly variable in time and space. Overall, a 36 km resolution is inadequate;12 km appears reasonable, especially in regions of low topography, but the 4 km resolution provides the best match with observations. This represents a tradeoff between model resolution and the computational effort needed to make simulations. A key goal is to provide climate change specialists in each country with the information they need to evaluate possible future climate change impacts.
