Timing of surgery in Crohn’s disease: A key issue in the management

The timing of the decision for operation in Crohn’s disease is based on an evaluation of the several factors such as the failure of medical treatment, complications due to the Crohn’s disease or to the farmacological therapy, development of dysplasia or cancer and growth retardation. A complete evaluation of these factors should result in operation timed to the patient’s best advantage, achieving maximal relief of symptoms with improvement of quality of life. Given the complexity and heterogeneity of the disease and the different options for treatment, is difficult to systematize wh.n the optimal moment for the surgery is arrived. A very important factor in the management of Crohn’s disease is the multidisciplinary approach and the patient preference should be a significant factor in determining the choice of therapy. The surgery should be considered such another option in the sequential treatment of Crohn’s disease. We have analyzed the factors that are involved in the decision taking of the surgical treatment regarding to the experience and the published literature. Wh.n did the medical therapy fail? wh.n is the appropriate moment to operate on the patient? Or which complications of Crohn’s disease need a surgery? These are some of the questions we will try to answer.

hepatitis B virus （hBV） infection is a global public health problem that concerns 350 million people worldwide. Individuals with chronic hepatitis B （CriB） are at increased risk of developing liver cirrhosis, hepatic de-compensation and hepatocellular carcinoma. To maintain undetectable viral load reduces chronic infection complications. There is no treatment that eradicates hBV infection. Current drugs are expensive, are associated with adverse events, and are of limited efficacy. Current guidelines try to standardize the clinical practice. nevertheless, controversy remains about management of asymptomatic patients with CriB who are hepatitis B e antigen （hBeAg）-positive with.normal alanine aminotransferase, and what is the cut-off value of viral load to distinguish hBeAg- negative CriB patients and inactive carriers. We discuss in detail why DnA level alone is not sufficient to begin treatment of CriB.

COX-2 in liver,from regeneration to hepatocarcinogenesis:What we have learned from animal models?

The use of animals lacking genes or expressing genes under the control of cell-specific promoters has signifi cantly increased our knowledge of the genetic and molecular basis of physiopathology,allowing testing of functional hypotheses and validation of biochemical and pharmacologic approaches in order to understand cell ***,with unexpected frequency,gene knockout animals and,more commonly,animal models of transgenesis give experimental support to even opposite conclusions on gene *** we summarize what we learned on the role of cyclooxygenase 2(COX-2) in liver and revise the results obtained in 3 independent models of mice expressing a COX-2 transgene specifi cally in the *** challenge with pro-inflammatory stimuli,the animals behave very differently,some transgenic models having a protective effect but others enh.ncing the *** addition,one transgene exerts differential effects on normal liver physiology depending on the transgenic animal model used.

A numerical analysis to the non-linear fin problem

In this paper a numerical analysis is carried out to obtain the temperature distribution with.n a single fin. It is assumed that the heat transfer coefficient depends on the temperature. The complete highly non-linear problem is solved numerically and the variations of both, dimensionless surface temperature and dimensionless surface temperature gradient as well as heat transfer characteristics with the governing non-dimensional parameters of the problem are graphed and tabulated.
