Background:Liver biopsy for the diagnosis of non-alcoholic steatohepatitis(NASH)is limited by its inherent invasiveness and possible sampling *** studies have shown that cytokeratin-18(CK-18)concentrations may be useful in diagnosing NASH,but results across studies have been *** aimed to identify the utility of CK-18 M30 concentrations as an alternative to liver biopsy for non-invasive identification of ***:Individual data were collected from 14 registry centers on patients with biopsy-proven non-alcoholic fatty liver disease(NAfLD),and in all patients,circulating CK-18 M30 levels were *** with a NAfLD activity score(NAS)≥5 with a score of≥1 for each of steatosis,ballooning,and lobular inflammation were diagnosed as having definite NASH;individuals with a NAS≤2 and no fibrosis were diagnosed as having non-alcoholic fatty liver(NAfL).Results:A total of 2571 participants were screened,and 1008(153 with NAfL and 855 with NASH)were finally *** CK-18 M30 levels were higher in patients with NASH than in those with NAfL(mean difference 177 U/L;standardized mean difference[SMD]:0.87[0.69–1.04]).There was an interaction between CK-18 M30 levels and serum alanine aminotransferase,body mass index(BMI),and hypertension(Pfor NASH was 0.750(95%confidence intervals:0.714–0.787),and CK-18 M30 at Youden’s index maximum was 275.7 U/*** sensitivity(55%[52%–59%])and positive predictive value(59%)were not ***:This large multicenter registry study shows that CK-18 M30 measurement in isolation is of limited value for non-invasively diagnosing NASH.

Raman probing of carbon electrode and electrolyte under dynamic conditions is performed here using different aqueous electrolytes to elucidate the fundamental events occurring in electrochemical supercapacitor during charge–discharge *** areal capacitance ranges from 1.54 to 2.31μf cm^(-2)μm and it is determined using different *** findings indicate that the Helmholtz capacitance governs the overall charge-storage process instead of the space charge(quantum)capacitance commonly verified for HOPG electrodes in the range of~3 to 7μf cm^(-2).Molecular dynamics simulations are employed to elucidate the origin of the reversible Raman spectral changes during the charge–discharge processes.A correlation is verified between the reversible Raman shift and the surface excesses of the different ionic species.A theoretical framework is presented to relate the effect of the applied potential on the Raman shift and its correlation with the surface ionic *** is proposed that the Raman shift is governed by the interaction of solvated cations with graphite promoted by polarization *** is the first time that a comparative study on different aqueous electrolyte p H and cation ion size has been performed tracking the Raman spectra change under dynamic polarization conditions and contrasting with comprehensive electrochemistry and dynamic molecular simulations *** study shines lights onto the charge-storage mechanism with evidence of Kohn anomaly reduction in the carbon electrode during the reversible adsorption/desorption and insertion/extraction of ionic species.

Background and Aims:Intra-abdominal visceral fat accumulation and patatin-like phospholipase domain containing 3(PNPLA3)rs738409 G/C gene polymorphism confer a greater susceptibility to nonalcoholic fatty liver disease(NAfLD).We examined whether the relationship between visceral fat accumulation and liver disease severity may be influenced by PNPLA3 rs738409 ***:The variant of PNPLA3 rs738409 was genotyped within 523 Han individuals with biopsy-confirmed *** fat area(VfA)was measured by bioelectrical *** liver fibrosis(Sf),defined as stage f≥2 on histology,was the outcome measure of ***:The distribution of PNPLA3 genotypes was CC:27.5%,CG:48.2%,and GG:24.3%.Higher VfA was associated with greater risk of having Sf(adjusted-odds ratio[OR]:1.03;95%confidence interval[CI]:1.02–1.04,pf potential *** subjects with the same VfA level,the risk of Sf was greater among carriers of the rs738409 G genotype than among those who did *** analysis showed that PNPLA3 rs738409 significantly influenced the association between VfA and *** remained significantly associated with Sf only among the rs738409 G-allele carriers(adjusted-OR:1.05;95%CI:1.03–1.08 for the GG group;and adjusted-OR:1.03;95%CI:1.01–1.04 for the GC group).There was a significant interaction between VfA and PNPLA3 rs738409 genotype(Pinteraction=0.004).Conclusions:PNPLA3 rs738409 G allele has a moderate effect on the association between VfA and risk of Sf in adult individuals with biopsy-proven *** of the PNPLA3 rs738409 G allele and VfA interact to increase risk of Sf。

Esophagogastric junction outflow obstruction successfully treated with laparoscopic Heller myotomy and Dor fundoplication: first case report in the literature

BACKGROUND Esophagogastric junction outflow obstruction(EGJOO) is a rare syndrome,characterized by an elevation of the integrated relaxation pressure of the lower esophageal sphincter, not accompanied by alterations in esophageal motility that may lead to the criteria for achalasia. We were unable to find any prior report of the combination of Heller myotomy with anterior partial fundoplication(Dor) as the treatment for EGJOO. We herein report a case of EGJOO treated with laparoscopic Heller myotomy combined with Dor *** SUMMARY A 26-year-old man presented with a 3-year history of solid dysphagia and a 30-kg weight loss. He was treated with oral nifedipine, isosorbide, and omeprazole,without resolution of symptoms. An upper gastrointestinal series(barium swallow) revealed a "bird's beak" sign. Esophagogastroduodenoscopy was positive for Los Angeles grade A peptic esophagitis. High-resolution esophageal manometry was compatible with EGJOO. Esophageal pH monitoring showed pathological acid reflux both in orthostatic and decubitus position. An 8-cm laparoscopic Heller myotomy combined with an anterior 220° Dor fundoplication was performed. Solid diet was introduced on postoperative day 2, and the patient was discharged home the same day. At 17-mo follow-up, he reported no symptoms. Barium swallow was compatible with complete radiologic *** esophageal manometry and upper endoscopy showed normal findings 9 mo after the *** Surgical treatment with Heller myotomy and Dor fundoplication is a potential treatment option for EGJOO refractory to medical treatment.

This paper presents assessments of the fragmentation of the temperate and tropical forests in Mexico at the national level for two dates 1993 and *** study was based on land use and vegetation cover data sets scale 1：250,*** broad forest types（Temperate forests and Tropical forests） and five more specific forest types（Broadleaf forests,and Coniferous forests;Tropical Dry Deciduous forests,Tropical Sub-evergreen forests,and Tropical Evergreen forests） were defined to conduct the *** 3.3 was used to estimate nine metrics of the spatial pattern of the forests for each forest type and date *** results indicate that the land cover transitions that have occurred between 1993 and 2002 have resulted in more isolated forest patches with simpler shapes in both the Temperate and Tropical *** remaining Tropical forest patches have become smaller and more *** contrast,the remaining Temperate forest patches are fewer and on average *** the more specific forest types defined in this study,the Broadleaf forests have the highest indicators of *** these forests are usually embedded or adjacent to Coniferous *** more concern for conservation purposes are the high values of fragmentation metrics found for the Tropical Evergreen forests and Tropical Dry Deciduous forests,because these forest types are usually surrounded by non-forest land covers or anthropogenic land uses.

The use of orthobiologics as a novel therapy for the treatment of numerous musculoskeletal disorders has increased considerably over the past ***,there are multiple alternatives available as suitable treatments;however,the use of autologous blood-derived products such as platelet-rich plasma(PRP),bone marrow aspirate(BMA)and BMA concentrate(BMAC),specifically,is *** many investigations attempted to demonstrate the effectiveness of these therapies,even with positive results,the literature lacks standardized protocols and overall accuracy in study designs,which leads to variance and difficulty in reproducibility of *** efficacy of PRP for the treatment of cartilage,bone and muscle tissues is well *** BMAC has generated optimistic results for the same purposes,its applicability in clinical trials is still relatively recent when compared to *** products demonstrate the potential to set forth reparative processes,each in their own distinct *** combination of these biological products has been previously proposed,yet little is known about their *** indicates that growth factor,cytokine,and chemokine profiles seen in both PRP and BMAC vary but are likely to work synergistically to enhance musculoskeletal *** products seem to work well without PRP;however,the addition of PRP to BMAC has been shown to act as a rich and natural source of culture medium for stem cells located either peripherally or in the bone marrow ***,additional variables associated with the use of BMAC and PRP in orthopedics must be further evaluated in order to consolidate the efficacy of this therapeutic strategy.

In this paper,we analyse an analytical solution of the Einstein–Maxwell field equations that considers matter with anisotropic pressures in a static and spherically symmetric *** report the manner in which we obtained the solution,which is by means of the Karmarkar *** the model,we assume a state equation that describes the interaction of matter from quarks P=(c^(2)ρ-4B_(g))/3 and we consider the presence of electric charge,which can generate that the radial and tangential pressures are not *** a graphic manner,we analyse the physical properties of the model,taking as the observational data those of mass 1M⊙and radius7.69 km which were reported for the star Her *** charge values are found between 5.57×10^(18)C≤Q≤1.31×10^(20)C and the interval of the Bag constant Bg∈[118.7,122.13]MeV/fm^(3).Also,we show the stability of the configuration by means of the static stability criteria of Harrison–Zeldovich–Novikov((?M)/?ρc>0),as well as in regards to infinitesimal radial adiabatic perturbation,since the adiabatic indexγ>3.3 which guarantees the stability of the solution.
