Objectives: To assess the relationships between the maxillary first molar and the maxillary sinus floor in a group of patients referred to a dental clinic. Methods: Ninety-seven patients were recruited for this study. The distances between the examined roots (mesio-buccal, disto-buccal and palatal) as well as furcations, and the sinus floor, were evaluated using cone beam computed tomography, and grouped as follows: class 0: distance = 0 mm;class 1: 0 mm < distance < 2 mm;class 2: 2 mm ≤ distance < 4 mm;class 3: 4 mm ≤ distance < 6 mm;class 4: 6 mm ≤ distance. The Spearman’s Rank Correlation coefficient was used to test the univariate associations between furca-tion-sinus floor distance and each root class. Results: The prevalence of class 0 was the highest for the palatal root (44.33%), followed in descending order by mesio-buccal (40.21%), and disto-buccal (38.14%) roots. The highest correlation coefficient was recorded when assessing the relationship between furcation-sinus floor distance and palatal root classes (rho = 0.66, p < 0.001, n = 97). Conclusions: Altogether, the results suggest that the palatal root of the maxillary first molar not only had the closest relationship with the sinus floor, but also proved to be the best predictor for the furcation-sinus floor distance. The clinician should be aware of the anatomical and morphological details of this root, especially when taking surgical decisions.

AIM:To find out if by combining 2 ultrasound based elastographic methods:acoustic radiation force impulse(ARFI)elastography and transient elastography(TE),we can improve the prediction of fibrosis in patients with chronic hepatitis ***:Our study included 197 patients with chronic hepatitis *** each patient,we performed,in the same session,liver stiffness(LS)measurements by means of TE and ARFI,respectively,and liver biopsy(LB),assessed according to the Metavir score.10 LS measurements were performed both by TE and ARFI;median values were calculated and expressed in kilopascals(kPa)and meters/second(m/s),*** TE and ARFI measurements with IQR<30%andSR≥60%were considered ***:On LB 13(6.6%)patients had F0,32(16.2%) had F1,52(26.4%)had F2,47(23.9%)had F3,and 53(26.9%)had F4.A direct,strong correlation was found between TE measurements and fibrosis(r=0.741),between ARFI and fibrosis(r=0.730)and also between TE and ARFI(r=0.675).For predicting significant fibrosis(F≥2),for a cutoff of 6.7 kPa,TE had 77.5% sensitivity(Se)and 86.5%specificity(Sp)[area under the receiver operating characteristic curve(AUROC)0.87] and for a cutoff of 1.2 m/s,ARFI had 76.9%Se and 86.7%Sp(AUROC 0.84).For predicting cirrhosis(F=4),for a cutoff of 12.2 kPa,TE had 96.2%Se and 89.6% Sp(AUROC 0.97)and for a cutoff of 1.8 m/s,ARFI had 90.4%Se and 85.6%Sp(AUROC 0.91).When both elastographic methods were taken into consideration,for predicting significant fibrosis(F≥2),(TE≥6.7 kPa and ARFI≥1.2 m/s)we obtained 60.5%Se,93.3% Sp,96.8%positive predictive value(PPV),41.4%negative predictive value(NPV)and 68%accuracy,while for predicting cirrhosis(TE≥12.2 kPa and ARFI≥1.8 m/s) we obtained 84.9%Se,94.4%Sp,84.9%PPV,94.4% NPV and 91.8%***:TE used in combination with ARFI is highly specific for predicting significant fibrosis;therefore when the two methods are concordant,liver biopsy can be avoided.

With ultrafast laser systems reaching presently 10 PW peak power or operating at high repetition rates,research towards ensuring the long-term,trouble-free performance of all laser-exposed optical components is *** work is focused on providing insight into the optical material behavior at fluences below the standardized laser-induced damage threshold(LIDT)value by implementing a simultaneous dual analysis of surface emitted particles using a Langmuir probe(LP)and the target current(TC).HfO_(2) and ZrO_(2) thin films deposited on fused silica substrates by pulsed laser deposition at various O_(2) pressures for defect and stoichiometry control were irradiated by Gaussian,ultrashort laser pulses(800 nm,10 Hz,70 fs)in a wide range of *** TC and LP collected signals were in good agreement with the existing theoretical description of laser–matter interaction at an ultrashort time *** approach for an in situ LIDT monitoring system provides measurable signals for below-threshold irradiation conditions that indicate the endurance limit of the optical surfaces in the single-shot energy scanning *** LIDT value extracted from the LP-TC system is in line with the multipulse statistical analysis done with ISO 21254-2:2011(E).The implementation of the LP and TC as on-shot diagnostic tools for optical components will have a significant impact on the reliability of next-generation ultrafast and high-power laser systems.

Psychiatry Trainees’ Attitudes towards Psychotherapy

Background: Psychotherapy education is one of the essential components of specialist training in psychiatry according to the Royal College of Physicians of Canada. How psychiatry trainees perceive psychotherapy has major implications for how trainees will learn about psychotherapy during training, how they will utilize it in their future practice, and how they will integrate it into their professional identity. Methods: The study involved the distribution of questionnaires to psychiatry residents at an Atlantic Canadian Medical School. The survey consisted of Likert-scaled items which assessed residents’ attitudes towards being a psychotherapist and their future study and practice plans in psychotherapy. Results: Surveys were completed by 18 out of 25 residents (72% response rate). Half of participants indicated that conducting psychotherapy was highly rewarding and the majority of residents planned to incorporate psychotherapy into their future practice. Several mean differences were also noted between those residents who completed their undergraduate medical education in Canada versus at International locations. Conclusion: Despite the recent changes in psychotherapy training requirements and concerns about psychotherapy’s role in psychiatry, many residents perceive psychotherapy skills as being important for competent psychiatry practice and plan to incorporate their psychotherapy training into their future practice. Additional implications for teaching, future research, and practice are discussed.

Introduction: Elderly patients represent a difficult category of surgical candidates for orthopedic surgery because they have multiple associated diseases and a high degree of osteoporosis. Presentation of Case: We present the case of a 103 years old woman with a pertrohanteric hip fracture treated successfully using a 135° dynamic hip screw. Discussion: There are many discussions about this kind of patients regarding postoperative complications, duration of hospitalization, stage of recovery and the patient’s status at discharge. Conclusion: We conclude that surgery may benefit patients who are over 100 year-old.

Hydraulic hybrid propulsion is a solution to improve greenhouse emissions of *** density,ability to allow new engine concepts,like free piston with improved thermodynamic cycles,capability for the internal combustion engine to run at constant,and here after low emission conditions,also the ability to recover different sizes of energy and last but not least,using conventional technology and applicability on all vehicles,even it is bicycle,micro mobility dedicated,conventional or heavy duty are arguments to further improve efficiency and technical solutions.A development solution for hydraulic hybrid propulsion systems is based on the principle that the column of liquid that transfers the energy is involved in an alternate movement and is not flowing continuously,just oscillating about an average *** paper presents the performances of an off road vehicle,Dacia Duster,equipped with an alternant hydraulic transmission *** hydraulic circuit is designed to allow dynamic phenomena,like inertia and accumulating effects to increase the energy concentration for low dissipation *** designing the working principle of the alternant hydraulic generator and motor,flow is optimized using a Lattice Bolzmann CFD software,to identify under source of flow loss under dynamic conditions.1D CFD and multi- physics simulation is used in order to design the hydraulic circuit.A test rig model is used to validate the concept under real running *** attention is given to the leak- pressure wave *** test rig model is adapted to the vehicle and dynamic performances evaluated and compared to the base model.

Aim: This study aims to analyze the particularities of hepatitis C induced hepatocellular carcinoma (HCC), developed during or after treatment with direct-acting antivirals. Methods: We conducted an observational prospective study on 278 patients, who underwent treatment for hepatitis C related liver cirrhosis and respectively for F3 chronic hepatitis C. Liver status was assessed using biological parameters and imagistic evaluation (ultrasonography, computed tomography scan, magnetic resonance imaging). Results: The follow-up time was 14 months. Before therapy, 69.3% of the cirrhotic patients and 26.7% of those with F3 degree of liver fibrosis had high levels of alpha-fetoprotein, with no imagistic evidence of HCC. During treatment, HCC was confirmed in 5 patients, 2 of them presenting portal vein thrombosis (PVT). Antiviral therapy was not interrupted. Two patients developed HCC at the end of treatment, while 4 of them were diagnosed with HCC after three months of ending the treatment. Excepting the ones with PVT, all patients underwent trans-arterial chemoembolization. Conclusion: All patients acquired sustained virological response. The screening for HCC should not be stopped after achievement of sustained virological response. Patients who develop HCC after antiviral treatment often need to be evaluated by magnetic resonance imaging in order to detect the extensionof the disease.
