Sonography in surveillance for HCC in NAFLD patients

Over the last decade,we have been facing a new aetiology responsible for the development of HCC-the nonalcoholic fatty liver disease(NAFLD).The prevalence of HCC development in this group is higher than that observed in the general population and in non-cirrhotic subjects with other causes of liver *** ultrasound(US)is the first-line tool for HCC surveillance,but,in this population,it has a decreased diagnostic accuracy due to several particular features,including obesity and ***-enhanced ultrasound(CEUS)appeared as a new branch of US due to its ability to depict the vascular architecture of all types of focal lesions(FLs).Nevertheless,CEUS has several limitations besides those inherited from US,which renders this method unreliable as the first-line HCC diagnostic tool and for HCC *** intelligence eliminates operator limitations,which has led to an increased sensitivity and specificity of ***,this approach is still in its early stages and more data are ***,the purpose of the current study is to highlight the strengths and limits of US,along with its alternatives to HCC screening in NAFLD population.

A Comparative Analysis of Binary Logistic Regression and Analytical Hierarchy Process for Landslide Susceptibility Assessment in the Dobrovǎt River Basin,Romania

A correct assessment of the landslide susceptibility component is extremely useful for the diminution of associated potential risks to local economic development, particularly in regard to land use planning and soil conservation. The purpose of the present study was to compare the usefulness of two methods, i.e., binary logistic regression(BLR) and analytical hierarchy process(AHP), for the assessment of landslide susceptibility over a 130-km^2 area in the Moldavian Plateau(eastern Romania) region, where landslides affect large areas and render them unsuitable for agriculture. A large scale inventory mapping of all types of landslides(covering 13.7% of the total area) was performed using orthophoto images, topographical maps, and field surveys. A geographic information system database was created, comprising the nine potential factors considered as most relevant for the landsliding process. Five factors(altitude, slope angle, slope aspect, surface lithology, and land use) were further selected for analysis through the application of a tolerance test and the stepwise filtering procedure of BLR. For each predictor, a corresponding raster layer was built and a dense grid of equally spaced points was generated, with an approximately equal number of points inside and outside the landslide area, in order to extract the values of the predictors from raster layers. Approximately half of the total number of points was used for model computation, while the other half was used for validation. Analytical hierarchy process was employed to derive factor weights, with several pair-wise comparison matrices being tested for this purpose. The class weights, on a scale of 0 to 1, were taken as normalized landslide densities. A comparison of results achieved through these two approaches showed that BLR was better suited for mapping landslide susceptibility, with 82.8% of the landslide area falling into the high and very high susceptibility classes. BACKGROUND Primary appendiceal tumors are histologically diverse and have an insidious onset and few specific clinical *** the majority of cases,these tumors are discovered after appendectomy during pathological exam of the resected *** may include appendectomy(simple or radical)and right hemicolectomy depending on factors such as histological type,tumor size and lymph node/organ *** aim of this case study is to describe a rare case of a giant appendicular mucocele and raise awareness of this condition and its management options and follow-up *** SUMMARY We present the case of a 43-year-old patient who presented to our emergency department with mild right lower quadrant *** the initial check-up and imaging exams,he underwent surgery,where a giant 20 cm×13 cm appendicular tumor was found and *** mucocele was suspected due to the macroscopic appearance and was later confirmed by the pathological *** patient’s postoperative evolution was uneventful,and after discharge,he was included in our follow-up *** In conclusion,mucinous appendiceal neoplasms embody a rare pathology;they are asymptomatic or have few,unspecific clinical signs and in many cases are discovered after appendectomy.

HTTP Adaptive Streaming(HAS)of video content is becoming an undivided part of the Internet and accounts for most of today’s network *** compression technology plays a vital role in efficiently utilizing network channels,but encoding videos into multiple representations with selected encoding parameters is a significant ***,video encoding is a computationally intensive and time-consuming operation that requires high-performance resources provided by on-premise infrastructures or public *** turn,the public clouds,such as Amazon elastic compute cloud(EC2),provide hundreds of computing instances optimized for different purposes and clients’***,there is a need for algorithms and methods for optimized computing instance selection for specific tasks such as video encoding and transcoding ***,the encoding speed directly depends on the selected encoding parameters and the complexity characteristics of video *** this paper,we first benchmarked the video encoding performance of Amazon EC2 spot instances using multiple×264 codec encoding parameters and video sequences of varying ***,we proposed a novel fast approach to optimize Amazon EC2 spot instances and minimize video encoding ***,we evaluated how the optimized selection of EC2 spot instances can affect the encoding *** results show that our approach,on average,can reduce the encoding costs by at least 15.8%and up to 47.8%when compared to a random selection of EC2 spot instances.

AIM:To investigate the diagnostic accuracy of acoustic radiation force impulse (ARFI) imaging as a noninvasive method for the assessment of liver fibrosis in chronic hepatitis C (CHC) ***:We performed a prospective blind com-parison of ARFI elastography,APRI index and FibroMax in a consecutive series of patients who underwent liver biopsy for CHC in University Hospital Bucharest. His-topathological staging of liver fibrosis according to the METAVIR scoring system served as the reference. A to-tal of 74 patients underwent ARFI elastography,APRI index,FibroMax and successful liver biopsy. RESULTS:The noninvasive tests had a good correlation with the liver biopsy results. The most powerful test in predicting fibrosis was ARFI elastography. The diagnostic accuracy of ARFI elastography,expressedas area under receiver operating characteristic curve (AUROC) had a validity of 90.2% (95% CI AUROC = 0.831-0.972,P < 0.001) for the diagnosis of significant f ibrosis (F ≥ 2). ARFI sonoelastography predicted even better F3 or F4 fibrosis (AUROC = 0.993,95% CI = 0.979-1).CONCLUSION:ARFI elastography had very good accuracy for the assessment of liver fibrosis and was superior to other noninvasive methods (APRI Index,FibroMax) for staging liver fibrosis.
