We couple a simple model of quarantine and testing strategies for international travellers with a model for transmission of SARS-CoV-2 in a partly vaccinated population.We use this model to estimate the risk of an infectious traveller causing a community outbreak under various border control strategies and different levels of vaccine coverage in the population.Results are calculated from n¼100,000 independent realisations of the stochastic model.We find that strategies that rely on home isolation are significantly higher risk than the current mandatory 14-day stay in government-managed isolation.nevertheless,combinations of testing and home isolation can still reduce the risk of a community outbreak to around one outbreak per 100 infected travellers.We also find that,under some circumstances,using daily lateral flow tests or a combination of lateral flow tests and polymerase chain reaction(PCR)tests can reduce risk to a comparable or lower level than using PCR tests alone.Combined with controls on the number of travellers from countries with high prevalence of COVID-19,our results allow different options for managing the risk of COVID-19 at the border to be compared.This can be used to inform strategies for relaxing border controls in a phased way,while limiting the risk of community outbreaks as vaccine coverage increases.

new Zealand delayed the introduction of the Omicron variant of SARS-CoV-2 into the community by the continued use of strict border controls through to January 2022.This allowed time for vaccination rates to increase and the roll out of third doses of the vaccine(boosters)to begin.It also meant more data on the characteristics of Omicron became available prior to the first cases of community transmission.Here we present a mathematical model of an Omicron epidemic,incorporating the effects of the booster roll out and waning of vaccine-induced immunity,and based on estimates of vaccine effectiveness and disease severity from international data.The model considers differing levels of immunity against infection,severe illness and death,and ignores waning of infection-induced immunity.This model was used to provide an assessment of the potential impact of an Omicron wave in the new Zealand population,which helped inform government preparedness and response.At the time the modelling was carried out,the date of introduction of Omicron into the new Zealand community was unknown.We therefore simulated outbreaks with different start dates,as well as investigating different levels of booster uptake.We found that an outbreak starting on 1 February or 1 March led to a lower health burden than an outbreak starting on 1 January because of increased booster coverage,particularly in older age groups.We also found that outbreaks starting later in the year led to worse health outcomes than an outbreak starting on 1 March.This is because waning immunity in older groups started to outweigh the increased protection from higher booster coverage in younger groups.For an outbreak starting on 1 February and with high booster uptake,the number of occupied hospital beds in the model peaked between 800 and 3,300 depending on assumed transmission rates.We conclude that combining an accelerated booster programme with public health measures to flatten the curve are key to avoid overwhelming the healthcare system.

Children from two-parent families have better outcomes,on average,than children from single-parent families.Yet the mechanisms associated with family structure and family process that produce divergent outcomes are less well understood.Based on data from the 2011-2015 national Health Interview Survey(n=26,783),I leverage the case of military families with deployment and examine the impacts of parenting quality,economic capital,and social capital on children’s psychological well-being.The regression results show that single parenthood produced by divorce,separation,and birth out of wedlock leads to worse child outcomes than single parenthood produced by military deployment,and family process partially explains the variation in children’s well-being beyond family structure.Married families,military or civilian,deployed or not,enjoy advantages that translate into positive child outcomes.Marriage,therefore,emerges as the primary axis of inequality,and maintaining a healthy marriage better promotes children’s well-being.

Background: neonatal mortality is a major public health problem. Its reduction is one of the targets of the objectives for sustainable development. Objectives: To determine the frequency of late neonatal mortality, to determine the causes of death and to identify the factors associated with late neonatal mortality at Brazzaville University Hospital. Patients and methods: This was a descriptive and analytical study conducted in the neonatal department of the Brazzaville University Hospital from 1 January to 30 June 2018. It concerned all newborns admitted to the service and died between 7th and 28th day of life. The variables studied were epidemiological, clinical and evolutionary. For the study of the factors associated with late mortality, we compared the deceased newborns with those hospitalized in the service whose stay was at least seven days. The materiality threshold was 5%. Results: During the study period, 697 newborns were admitted to the service, 286 (41%) died, 79 (27.6%) on the seventh day or beyond. The median age at admission was one hour. These were male (44.3%) and female (55.7%) newborns. The causes of death were prematurity (50.7%), neonatal infection (36.7%), perinatal asphyxia (6.3%) and others (6.3%). The mean age at death was 13.5 ± 6.4 days (range: 7 and 28 days). Prematurity OR = 3.62 (95% CI: 1.2 - 10.6) is the factor associated with late mortality. Conclusion: Reducing late neonatal mortality requires the implementation of measures to improve the human and material capacities of the service.

Management of recreational access on public forest lands is a complex issue of growing global importance. The provision of public recreation opportunities is part of the suite of ecological goods and services that must be considered by many forest managers. Effective access management is predicated on understanding the attitudes and perceptions of recreation users in order to predict and influence visitor behaviour and gauge the acceptance of new management strategies. Potential access management strategies vary given the nature of recreation activities and include: restricting the amount, type, and spatial distribution of use, visitor education, temporal restrictions and enhancing site durability. In this research we examined the views of recreation users on public lands in southwestern Alberta, Canada through implementation of an online survey (n = 945) with a focus on access management options. The results indicate a strong belief that the quality of the recreation experience is declining and that increased management and enforcement are required. More detailed analysis indicates that demographic and user-type variables strongly influence ideas about appropriate management. Forest managers need to engage with, understand, and respond to a wide variety of recreation user needs and preferences.

Objective: To determine morbidity, identify the causes of morbi-mortality and the factors associated with adolescent mortality in Brazzaville. Patients and Methods: A retrospective, descriptive and analytical study was carried out at the Brazzaville Hospital and University Center between January 2015 and December 2016. Adolescents hospitalized in clinical services whose records were workable were included. The Epidemiological, clinical and evolutionary variables were analyzed. The statistical test used was the khi-two, the significance level set at 0.05. Results: In total, Out of 67,488 hospitalized patients, 2062 were adolescents: 3% including 891 (43.2%) boys and 1171 (56.8%) girls mean age 14.8 ± 2.92 years, seen in average 9.38 days after the onset of symptoms. Childbirth 415 (20.1%), malaria 309 cases (14.9%), major sickle cell crisis 223 (10.8%) and traumatic injuries 166 cases (8.05%) are the main causes of morbidity. Material and Method: A quasi-experimental intervention/non-intervention study was carried out between March and October 2020 in the pediatric departments of the Brazzaville University Hospital. Children aged three months to 15 years hospitalized were the target population. Two groups were formed: the n>“intervention” group, that of children hospitalized between March and October 2020 and the “control” group that of those hospitalized between January and August 2015. The study variables were epidemiological, clinical, biological and therapeutic. Chi-square and T-Student tests were used. The impact of the intervention was assessed by the absolute risk difference. Results: Of 1392 children hospitalized, 199 (14.6%) had severe malaria with an average age of 6.94 years. In order to contribute to reduction of morbidity and mortality due to low acute respiratory infections in pediatrics, the objective of this study was to determine the epidemiological profile and to identify the factors favoring and those associated with the risk of deceased. We reviewed the cases of children aged between 1 and 59 months hospitalized between January and December 2015 in the pediatric intensive care unit of the CHU of Brazzaville and retained those whose diagnosis of discharge was an ALRI. The study variables were: epidemiological, clinical, paraclinic, diagnostic, therapeutic and evolutionary. For 2012 hospitalized children, 400 (19.8%) were hospitalized for an ALRI. There were 235 boys (58.8%) and 165 girls (41.2%) of average age. Infants aged 1 to 11 months were the most affected n = 260 (65%). They were insufficiently immunized with haemophilus influenza B and pneumococcus n = 378 (94.5%);Fully immunized n = 20 (5%), the average delay for consultation after the first symptom was 5.03 ± 3.86 days;Denutrition was rated n = 180 (45%), positive retroviral serology n = 19 cases (4.8%). The main nosological varieties were: bronchiolitis n = 223 (55.7%), pneumonia n = 145 (36.2%). One death was rated n = 65 (16.3%). The mortality determinants were: prematurity, vaccination status and HIV/AIDS infection. The frequency and severity of ALRIs necessitates a strengthened program of integrated management of childhood illnesses, and public health actions targeting the factors that promote and contribute to the risk of death.

Objectives: To describe the epidemiological, clinical and therapeutic aspects of asthma in children at the Pediatric Intensive Care Unit of University Hospital of Brazzaville. Patients and methods: We reviewed the records of children hospitalized in the Pediatric Intensive Care Unit of University Hospital of Brazzaville from January 1 to December 31, 2015, and retained those from two months to 17 years hospitalized for asthma. The study variables were: age, sex, influence of seasons, history of atopy;the factors triggering the crisis, the type of follow-up, the degree of severity of asthma exacerbation, the type of treatment instituted and the course of the disease. In total, out of 2012 hospitalized children, 65 children met the inclusion criteria. Results: The hospital prevalence of asthma was 3.5%. Among 65 children of study, 42 (59.2%) were male and 29 (40.8%) female. The mean age was 3.9 ± 2.9 years (range from 2 months to 14 years). Children aged 30 months to 5 years were the most represented (40%). They were known as asthmatics, n = 18 (27.7%). Family atopy was found, n = 11 (16.9%). Acute Otorhinolaryngologic infections were in all cases the factors triggering the crisis. The peak frequency of hospitalizations for asthma occurred during the short rainy season. The crisis was moderate n = 46 (70%) and severe n = 19 (29%). Beta 2 mimetics were administered in all cases, by subcutaneous in 18 cases (27.7%) and inhaled in 47 cases (72.3%). The outcome was favorable in all cases with a mean hospital stay of 1.46 ± 0.92 days. Conclusion: Childhood asthma remains a public health challenge and severe asthma is the paradigm of uncontrolled and costly asthma. This first work suggests that a large-scale study be carried out for a better knowledge of it.
