This research advocates for the construction of Climate Change Haven Communities across the Appalachian Region. The proposed development plan can be extended to the northern tier states across the US and also to the northern and mountainous regions of Europe and Asia. We present an analogy to the earlier climate change period of the Last Glacial Maximum/“Ice Age” in which these same northern regions of the planet were covered in ice sheets making them uninhabitable for most humans and many plant and animal species. In some significant ways, the Ice Age scenario can be a reverse-model for our current climate crisis. We also advocate strongly for the prevention of upscale real estate development projects in these same regions of the globe, as these will foreclose the possibility of safely sheltering the millions of persons who will be displaced by climate change over the next 5 to 10 years.

Objective:To review the risk of prostate cancer(PCa)in men with incidentally reported increased intraprostatic uptake at 2-deoxy-2-[18F]fluoro-d-glucose positron emission tomography/computed tomography(18F-FDG PET/CT)ordered at Department of Urology,The Wesley Hospital,Brisbane,QLD,Australia for non-PCa related ***:Retrospective analysis of consecutive men between August 2014 and August 2019 presenting to a single institution for 18F-FDG PET/CT for non-prostate related conditions was *** were classified as benign,indeterminate,or malignant depending of the results of prostate-specific antigen(PSA),PSA velocity,biopsy histopathology,and three-Tesla(3 T)multiparametric MRI(mpMRI)Prostate Imaging Reporting and Data System score,or gallium-68-prostate-specific membrane antigen(68Ga-PSMA)PET/CT ***:Three percent(273/9122)of men demonstrated 18F-FDG avidity within the ***-five percent(231/273)were further investigated,including with PSA tests(227/231,98.3%),3 T mpMRI(68/231,29.4%),68Ga-PSMA PET/CT(33/231,14.3%),and prostate biopsy(57/231,24.7%).Results were considered benign in 130/231(56.3%),indeterminate in 31/231(13.4%),and malignant in 70/231(30.3%).PCa was identified in 51/57(89.5%)of the men who proceeded to biopsy,including 26/27(96.3%)men with Prostate Imaging Reporting and Data System scores 4-5 mpMRI and six men with a positive 68Ga-PSMA PET/*** most common Gleason score on biopsy was greater than or equal to 4+5(14/51,27.5%).68Ga-PSMA PET/CT was concordant with the 18F-FDG findings in 26/33(78.8%).All 13 men with a positive concordant 18F-FDG,3 T mpMRI,and 68Ga-PSMA PET/CT had PCa on *** was no statistically significant difference in the 18F-FDG maximum standardized uptake value between the benign or malignant groups(5.7 vs.6.1;p=0.580).Conclusion:In this study,after an incidental finding of an avid intraprostatic lesion on 18F-FDG PET/CT,70 of the 231 cases(30.3%;0.8%of the entire cohort)had results c

Evaluation of Clinician Training in Autism Screening, Care Management, and Patient Education

Objective: The demand for pediatric developmental evaluations has far exceeded the workforce available to perform them, which creates long significant wait times for services. A year-long clinician training using the Extension for Community Healthcare Outcomes (ECHO®) model with monthly meetings was conducted and evaluated for its impact on primary care clinicians’ self-reported self-efficacy, ability to administer autism screening and counsel families, professional fulfillment, and burnout. Methods: Participants represented six community health centers and a hospital-based practice. Data collection was informed by participant feedback and the Normalization Process Theory via online surveys and focus groups/interviews. Twelve virtual monthly trainings were delivered between November 2020 and October 2021. Results: 30 clinicians participated in data collection. Matched analyses (n = 9) indicated statistically significant increase in self-rated ability to counsel families about autism (Pre-test Mean = 3.00, Post-test Mean = 3.89, p = 0.0313), manage autistic patients’ care (Pre-test Mean = 2.56, Post-test Mean = 4.11, p = 0.0078), empathy toward patients (Pre-test Mean = 2.11, Post-test Mean = 1.22, p = 0.0156) and colleagues (Pre-test Mean = 2.33, Post-test Mean = 1.22, respectively, p = 0.0391). Unmatched analysis revealed increases in participants confident about educating patients about autism (70.59%, post-test n = 12 vs. 3.33%, pre-test n = 1, p = 0.0019). Focus groups found increased confidence in using the term “autism”. Conclusion: Participants reported increases in ability and confidence to care for autistic patients, as well as empathy toward patients and colleagues. Future research should explore long-term outcomes in participants’ knowledge retention, confidence in practice, and improvements to autism evaluations and care.

Introduction: Adolescence is a period of transition to adulthood, including for HIV-infected adolescents (HIA), when sexual problems emerge. Few studies have been carried out on the sexuality of HIA. This study aimed to assess the sexual behavior of HIV-infected adolescents in Cameroon. Methodology: A cross-sectional study was conducted in three hospitals in the cities of Yaoundé and Douala, from November 2019 to June 2020. All HIA aged 13 to 19 years followed in the study who knew their HIV status were included in the study after obtaining their assent and their parent’s consent. Socio-demographic and clinical characteristics were collected, as well as sexual practices and the determinants of their sexual activity. A multivariate analysis was performed to explore the relationship between the different variables studied and the sexual activity of these adolescents. Results: Of 204 HIA enrolled in the study, 64 (31.7%) were sexually active, the mean age at first sexual intercourse was 15.6 ± 2.8 years and the sex ratio was 0.94. Of the adolescents in the study, 75.7% of girls and 61.2% of boys had regular sexual activity, while 21.3% of girls and 35.4% of boys had multiple sexual relationships in the six months preceding the study. Nearly 16 (25.0%) of the sexually active HIA had more than 2 sexual encounters per month, 6 (9.3%) of these encounters had been paid for and 9 (27.2%) of the girls had already had at least one pregnancy. More than 7 out of 10 HIA (79.6%) had used a condom the last time they had sex. Age less than 18 years [OR = 11.1 (95% CI: 3.1 - 39.4), p = 0.001], lack of remuneration [OR = 9.8 (95% CI: 2.0 - 47.4), p = 0.001] and self-care were significantly associated with sexual activity in HIA, while school attendance was a protective factor. Self-funded HIAs were 21 times more likely to be sexually active [OR = 21.6 (95% CI: 2.3 - 179), p = 0.004]. The sorption capacity and affinity of biochar for metals are both important attributes that determine biochar’s suitability as a soil amendment for contaminant mitigation,yet few analyses have considered both characteristics *** present a systematic review of literature published between 2010 and 2018 to test the hypothesis that sorption capacity and affinity are affected by biochar properties,attributes of the metal contaminant,and experimental conditions,in that *** used random forest(RF)and multi-objective optimization to analyze data of 559 individual Langmuir adsorption isotherms extracted from 133 studies covering the sorption capacity(C_(max))and affinity(K_(L))of biochar for 17 different metals,elabo-rated from six different feedstock classes,three different types of feedstock pretreatment,and five types of post-pyrolysis *** sorption values were obtained for Pb(II),Cr(IV),and Cd(II).The feedstock used was the key determinant influencing biochar’s capacity and affinity to sorb metal contaminants(first and fourth most important variable in RF mod-els for C_(max) and K_(L),respectively)with best results obtained for biochar elaborated from nutrient-dense feedstocks(animal biowaste,sludge,and manure).Biochars that had both high sorption capacity and affinity were the result of a longer dura-tion of pyrolysis;they had lower C and higher N and O content,as well as lower C/N and higher O/C and H/C ratios,higher pore volumes and higher *** some form of pretreatment was better than none,whereas chemical modification was the best of the post-treatment methods *** review demonstrates clearly that multiple parameters during the preparation process influence the effectiveness of biochar to immobilize metal *** research that focuses on mechanisms and the underlying factors for the correlations observed should allow the development of biochar formulations that are even more effective at immobilizing metal

Objective A recent publication has suggested that expedited time to theater in gastroschisis results in higher rates of primary closure and decreases the length of stay(LOS).This study primarily aims to assess the impact of time to first management of neonates with gastroschisis on the *** Neonates admitted between August 2013 and August 2020 with gastroschisis were *** were collected retrospectively,and neonates with complex gastroschisis were *** including gestation,birth weight,time of first management,primary/delayed closure and use of patch were evaluated as possible confounding *** outcome measures were time to full feeds,time on parenteral nutrition(PN)and *** and multivariate linear regression analyses were performed.Psix neonates were identified,and 16 were then excluded(eight patients with complex gastroschisis,eight patients with time to first management not documented).The median LOS for those who underwent primary closure was 21 days(interquartile range(IQR)=16-29)and for those who underwent silo placement and delayed closure was 59 days(IQR=44-130).The mean time to first management was 473 min(standard deviation(SD)=146 min),with only 20% of these infants being operated on at less than 6 hours of *** and multivariate analyses demonstrated no relationship between time to first management and LOS(r^(2)=0.00,p=0.82)but did demonstrate a consistent positive association between time to first feed and LOS and delayed closure,resulting in a longer time to full feeds and a longer time on *** The time to first management was not associated with a change in LOS in these *** prospective evaluation of the impact of reducing the time to first feed on the LOS is recommended.

AIM To assess nutritional recovery,particularly regarding feeding jejunostomy tube(FJT)utilization,following upper gastrointestinal resection for malignancy. METHODS A retrospective review was performed of a prospectively-maintained database of adult patients who underwent esophagectomy or gastrectomy(subtotal or total)for cancer with curative intent,from January 2001 to June 2014. Patient demographics,the approach to esophagectomy,the extent of gastrectomy,FJT placement and utilization at discharge,administration of parenteral nutrition(PN),and complications were evaluated.
