Etiology and management of urethral calculi:A systematic review of contemporary series

Objective:To conduct a systematic literature review on urethral calculi in a contemporary cohort describing etiology,investigation,and management ***:A systematic search of MEDLINE and Cochrane Central Register of Controlled Trials(CENTRAL)databases was ***,including case reports and case series on urethral calculi published between January 2000 and December 2019,were ***-text manuscripts were reviewed for clinical parameters including symptomatology,etiology,medical history,investigations,treatment,and *** were collated and analyzed with univariate ***:Seventy-four publications met inclusion criteria,reporting on 95 *** symptoms(41.1%),pain(40.0%),and acute urinary retention(32.6%)were common presenting *** calculi were most often initially investigated using plain X-ray(63.2%),with almost all radio-opaque(98.3%).Urethral calculi were frequently associated with coexistent bladder or upper urinary tract calculi(16.8%)and underlying urethral pathology(53.7%)including diverticulum(33.7%)or stricture(13.7%).Urethral calculi were most commonly managed with external urethrolithotomy(31.6%),retrograde manipulation(22.1%),and endoscopic in situ lithotripsy(17.9%).Conclusion:This unique systematic review of urethral calculi provided a summary of clinical features and treatment trends with a suggested treatment *** in contemporary urological practice should be according to calculus size,shape,anatomical location,and presence of urethral pathology.

Objective:To review the risk of prostate cancer(PCa)in men with incidentally reported increased intraprostatic uptake at 2-deoxy-2-[18F]fluoro-d-glucose positron emission tomography/computed tomography(18F-FDG PET/CT)ordered at Department of Urology,The Wesley Hospital,Brisbane,QLD,Australia for non-PCa related ***:Retrospective analysis of consecutive men between August 2014 and August 2019 presenting to a single institution for 18F-FDG PET/CT for non-prostate related conditions was *** were classified as benign,indeterminate,or malignant depending of the results of prostate-specific antigen(PSA),PSA velocity,biopsy histopathology,and three-Tesla(3 T)multiparametric MRI(mpMRI)Prostate Imaging Reporting and Data System score,or gallium-68-prostate-specific membrane antigen(68Ga-PSMA)PET/CT ***:Three percent(273/9122)of men demonstrated 18F-FDG avidity within the ***-five percent(231/273)were further investigated,including with PSA tests(227/231,98.3%),3 T mpMRI(68/231,29.4%),68Ga-PSMA PET/CT(33/231,14.3%),and prostate biopsy(57/231,24.7%).Results were considered benign in 130/231(56.3%),indeterminate in 31/231(13.4%),and malignant in 70/231(30.3%).PCa was identified in 51/57(89.5%)of the men who proceeded to biopsy,including 26/27(96.3%)men with Prostate Imaging Reporting and Data System scores 4-5 mpMRI and six men with a positive 68Ga-PSMA PET/*** most common Gleason score on biopsy was greater than or equal to 4+5(14/51,27.5%).68Ga-PSMA PET/CT was concordant with the 18F-FDG findings in 26/33(78.8%).All 13 men with a positive concordant 18F-FDG,3 T mpMRI,and 68Ga-PSMA PET/CT had PCa on *** was no statistically significant difference in the 18F-FDG maximum standardized uptake value between the benign or malignant groups(5.7 vs.6.1;p=0.580).Conclusion:In this study,after an incidental finding of an avid intraprostatic lesion on 18F-FDG PET/CT,70 of the 231 cases(30.3%;0.8%of the entire cohort)had results c

Background:In essential hypertension,renal noradrenaline(NA)spillover is commonly increased and in experimental hypertension renal sympathectomy abolishes the blood pressure *** renal sympathetic nerve ablation with catheter-based radiofrequency energy(Symplicity,Ardian Inc.)was tested in patients with resistant hypertension(SBP above 160 mmHg on 3 or more anti-hypertensive drugs,including a diuretic).Methods: Office blood pressure(BP)and safety data were acquired prior to,and at 1,3,6 and 12 months post-procedure. Whole body and renal noradrenaline spillover were measured with isotope dilution in a subset of 10 patients at baseline and 30 days ***:45 pts with a baseline mean office blood pressure of 177/ 101 mmHg(SD 20/15),(mean 4.7 antihypertensive medications)were studied;office blood pressures after procedure were reduced by 14/10,21/10,22/11,24/11,and 27/17 mmHg at 1,3,6,9,and 12 months,respectively. Thirty days after denervation,mean reduction in total renal noradrenaline spillover was 47%,P=0.023 (95%CI 28%～65%),and total body NA spillover was reduced 28%,P =0.043(95%CI 4%～52%).One intra-procedural renal artery dissection occurred before radiofrequency energy delivery without further *** other renal vascular complications were identified with short term repeat angiographic studies or with CTA or MRA 6 months after the ablations Conclusions:Renal sympathetic nerve ablation is associated with reduction of renal NA spillover indicative of disabling of renal efferent sympathetic *** renal sympathetic nerves from the kidney to the brain influence central sympathetic output,and the observed reduction of total body NA spillover suggests renal sympathetic afferent traffic is *** afferent nerve disabling is a significant cause of the reduction of BP, the effect is expected to be lifelong,since these nerves have little capacity for *** disabling of renal sympathetic nerves is associated with

Evaluation of Pre-Cecal, Ileal Digestibility and Glycemic Response in Fistulated Horses Fed with Different Starch Sources

The objective of this study was to evaluate the coefficients of pre-cecal, ileal digestibility, and glycemic response in cannulated horses fed with different starch sources. Four male horses were used in a 4 × 4 Balanced Latin Square cannulated in the cecum, with 36 months of age and average weight of 350 kg. The horses were fed with hay and the following sources of starch in concentrates: 1) corn (C);2) oats (O);3) sorghum (S);and 4) corn/oats (CO). The experiment consisted of four periods of eighteen days each, being the first twelve days of adaptation and six days to collect samples. There was no difference in cecal and ileal digestibility of starch, CP, ADF, NDF and OM among diets evaluated. However, horses fed with oats diet had higher values of pre-cecal apparent digestibility of DM in relation at the horses fed to sorghum and corn/oats diets. The values of CF digestibility were higher for equines fed with oats and sorghum diet compared to corn/oats diet. The horses fed with corn/oats diet had lower plasma glucose concentrations 30 minutes before the supply and higher insulin concentrations 210 minutes after ingestion of the foods. The digestibility of nutrients in equine diets, in pre-cecal and pre-ileal evaluation, presents significant differences, depending on the starch source provided, oat, corn or sorghum. The plasma glucose concentrations are directly correlated with the digestibility of starch.

Background: Heightened central sympathetic nervous outflow is common in essent ial hypertension, contributing to hypertension development and possibly also to complications. Acute sympathetic nervous activation is a proven trigger for adve rse cardiovascular events. Accordingly, antihypertensive drugs inhibiting sympat hetic outflow represent a theoretically attractive therapeutic option. Objectiv e s: To study the sympatholytic and blood pressure-lowering activity of the imida zoline binding agent rilmenidine at rest and during reflex sympathetic activatio n. Design and methods: We used a randomized, double-blind, 6-week cross-over study, with a 1-week placebo runin period, two 2-week active treatment interva ls (rilmenidine 1 mg twice daily or placebo) and intervening 1-week placebo was hout. In 15 hypertensive patients, noradrenaline and adrenaline plasma kinetics and intraarterial blood pressure measurements were performed at rest, after ment al stress (difficult mental arithmetic) and during head-up tilting, at the end of the 2-week dosing periods. Results: The noradrenaline spillover rate, indica tive of whole body sympathetic activity, was reduced 35%by rilmenidine at rest (Pel, a fall in supine resting pressure, but no reduction in blood pressure rise during mental stress and a lack of fall in blood pressure with tilting. On placebo, adrenaline secret ion was 0.88 ±0.15 nmol/min (mean ±SE) at rest, increased by 0.42 ±0.23 nmol/ min with mental stress (P=0.019) and was unchanged with tilting. Rilmenidine lef t adrenaline secretion untouched under all conditions. Conclusions: The present study confirms a sympatholytic effect of rilmenidine during supine rest but pres ervation of sympathetic responses during mental stress and tilting, with the lat

Objectives : To study the sympatholytic and blood pressure lowering activity of the imidazo line binding agent rilmenidine at rest and during reflex sympathetic activation. Design and methods: the HERA study (Hyperium Effect on the sympathetic Reflex a ctivation and Adrenaline) is a randomised, double blind, 6-week cross over tr ial, with a 1-week placebo run in period, two 2-week active treatment interva ls (rilmenidine 1 mg bid, placebo) and intervening one week placebo wash out. I n 15 hypertensive patients, noradrenaline and adrenaline plasma kinetics and int ra arterial blood pressure measurements were performed at rest, after mental st ress (difficult mental arithmetic) and during head up tilting, at the end of th e 2-week dosing periods. Results: the noradrenaline spillover rate, indicative of whole body sympathetic activity,was reduced 35%by rilmenidine at rest (pel, a fall in supine resting pressure, but no reduction in BP rise during mental stress and a lack of fall i n BP with tilting.
