Pulmonary arteriovenous malformations （AVM） lung is defined by an abnormal communication between pulmonary artery（ies） and vein（s） responsible for a right-left shunt. Congenital forms are most common and usually associated with Rendu-Osler disease （ROD）. Inversely, 15-45% of patients with ROD present lung AVM Nowadays, embolization is preferred to surgical resection in the majority of cases. Except for certain cases where surgery is indicated. This paper shows the role of surgery in AVMs.

Introduction: Cranioencephalic trauma is a frequent reason for admission to emergency departments and is a source of mild to severe neuropsychological defects that will persist over time. Their management remains difficult. Objectives: To evaluate the sequelae presented by patients suffering from cranioencephalic trauma. Methods: This was a retrospective, descriptive, cross-sectional study conducted at the National Hospital of Zinder. It will include all patients admitted and hospitalised in the emergency, intensive care and neurosurgery departments of Zinder National Hospital for head injury over a period of 28 months from 1 January 2016 to 30 April 2018. Brain scan, X-ray were the imaging tests used. Results: Out of a total of 974 admissions, 367 were retained, i.e. 37.6%, with a male predominance (82.6%). The average age was 26.5 years. MVAs were represented in 89.7% of cases. Moderate CTE accounted for 64% of cases. Altered consciousness was reported in 295 patients (80.38% of cases). Brain scans were used in 76.7% of cases and skull X-rays in 4.2% of cases. Medical management was performed in all patients. Treatment was operative in 78 patients (21.25%). Recovery without immediate sequelae was found in 187 patients (50.9%). Persistent headache represented 47.7% of the late sequelae observed in the patients, epileptic seizures represented 16.8% of the late sequelae, neurological deficit represented 14.7%. Conclusion: Cranioencephalic trauma represents a major public health issue. Although their in-hospital management remains a challenge, post-hospital management related to the appearance of sequelae remains another.

The sandstone relics of probable Cretaceous age found around Tohoun show an extensional tectonics imprint associated with the Atlantic opening. This imprint consists of normal fault networks well expressed on two sites of outcrop and corresponds to three fracturing episodes materialized by families of conjugated planes striking E-W, NW-SE, and NE-SW. Striated plane data analysis shows three extensional axes successively N-S, NE-SW and NNW-SSE. The reconstructed paleostress tensors can be attributed to pre- to syn-rift phases responsible for the development of primary basin structures in the Gulf of Guinea. This preliminary analysis, concerning only fractures in the basal sequence relics, remains to be extended to the entire Togolese coastal basin to a better definition of the Atlantic dynamic.

Mediastinal mature teratoma is the most common form of germ cell tumors, often located in the anterior mediastinum. A posterior mediastinum location has rarely been described. We report a case of a posterior mediastinal mature teratoma in a 37-year-old patient and a literature review, through which we describe the diagnostic and therapeutic difficulties inherent in this particular location of these tumors.

Thanks to detailed field investigations, microstructural and geochemical analysis and relationship with enclosing rocks, microfabrics, magmatic typology and metamorphic evolution of the Kantè sepentinites have been specified for the first time. The Kantè serpentinites in northern Togo constitute a mega-lens of ultrabasic rocks tectonically intercalated in the sericite chlorite schists of the Atacora structural unit. The brecciated, schitotose or massive rock facies are strongly marked by an S1 schistocity plane superimposed by a flat C shear plane linked to a west vergence thrusting movement. The parageneses that compose the metamagmatites are essentially serpentinous, containing plagioclase, opaque minerals (magnetite, chromite, spinel) and pyroxene porphyroblasts. These microfabrics represent relics of a probable gabbroic protolith. In fact, the geochemical characteristics of the Kantè serpentinites suggest that their magmatic typology is that of komatiites or tholeiitic basalts with oceanic arc affinities. They would have been emplaced in an active margin environment. The retromorphic evolution of the protolith corresponds to the phase of involvement in a major tangential contact during the panafrican tectogenesis.

Introduction: The pleural fistulas are defined by a fistula communicating the pleural cavity to the chest wall. They often occur in the immunocompromised individuals with pyogenic infections, much more rarely in immunocompetent patients with tuberculosis. Materials and Methods: Between February 2010 and August 2012, five patients were followed up and operated for tuberculous pleuro-cutaneous fistula in the thoracic surgery department of University Hospital Hassan II of Fez. Results: These were 2 female and 3 male patients, with the average age of 39 years, and the age ranging from 18 years to 60 years. One patient was operated on for tuberculous empyema;there are 20 years and another 15 years ago drained for tuberculous empyema. All patients had a fistula necessitated operating on an average of three years, with extremes ranging from 15 days to 6 years. The radiological assessment had objectified chronic pleural pocket in one case, a costal osteitis in 2 cases, multiple cystic lesions in one case and apseudotumor appearance and endo exothoracique in another. Patients had benefited from draining the abscess with fistula resection associated with a rib resection in 2 cases and thoracostomy with lung prosthesis in one case. Conclusion: Tuberculous pleuro-cutaneous fistula is a rare pathological entity, occurring most often in an array of multifocal tuberculosis, or as a complication of surgery for tuberculous empyema.

Petrology, Age and Geodynamic Implication of the Panafrican Granitoids Associated with the Glito-Kpatala Shear Zone (South-East Togo)

The granitic plutons associated with the Glito-Kpatala shear zone are composed of biotite and amphibole granodiorites, biotite granites, two-mica granites and aplitic granites, which are very poorly represented. The chemical and mineralogical compositions of these facies indicate that they are I type and belong to high-K calc-alkaline series, with a chemical metaluminous character displayed by the granodiorites relative to the biotite and two-mica facies whose chemical compositions vary between metaluminous and peraluminous caracter. The Th/Ta (14.04 - 43.82 ppm, mean = 26.05), Th/U (2.58 to 15.05 ppm, mean = 5.85 ppm), Zr/Hf (25.27 to 37.21, mean = 30.67 ppm) and Rb/Sr (0.16 to 4.32;mean = 1.67 ppm) ratios of these granitoids reveal a strong crustal involvement in their magmatogenesis. Variations in CaO/Na2O (0.47 - 1.44 ppm), Rb/Sr (0.14 - 0.27 ppm), Rb/Ba (0.07 - 0.14 ppm) and Sr/Y (38.21 - 174.42 ppm) ratios indicate that biotite and amphibole granodiorites with their excessive Ni (135.37 - 139.51 ppm) and Cr (395.73 - 447.74 ppm) were derived from a mafic to intermediate lower continental crust where garnet and/or amphibole were stable residual assemblage minerals. The moderate Sr/Y ratios (1.81 - 9.47 ppm) and low transition elements Ni (1 - 6.44 ppm) and Cr (7.89 - 13.47 ppm) contents in both the two-mica and biotite granites are consistent with their emplacement at relatively shallow depths in the upper to mean continental crust, at pressures below 10 Kbar. In the two-mica granites, moderate CaO/Na2O (0.20 - 0.57 ppm, mean = 0.38 ppm) and Rb/Ba (0.39 - 1.37, mean = 0.84 ppm) ratios and quite varied Rb/Sr (1.53 - 4.23 ppm, mean = 2.85 ppm) ratios indicate a predominant derivation from psammitic and pelitic metasediments rather than metagreywackes. These low ratios (0.25 ≤ CaO/Na2O ≤ 0.32, mean = 0.28 ppm;0.31 ≤ Rb/Ba ≤ 0.44, mean = 0.39 ppm;1.11 ≤ Rb/Sr ≤ 1.78, mean = 0.39 ppm) in biotite granites are more consistent with melt

Surgical Management of Pyothorax: A Series of 172 Cases

Introduction: The pyothorax or thoracic empyema is defined by the presence between the two pleural layers of a purulent liquid, or a non-purulent liquid, but with bacteriological or biochemical characteristics testifying to a microbial invasion. The use of early medical treatment prevents the passage to pleural encystment, whose management remains surgical. Material and Methods: Our study was performed retrospectively, in the department of thoracic surgery of CHU Hassan II of Fez, between 2010 and 2016, involving 172 patients operated for pyothorax. Results: Among the 172 patients included in our study, there were 110 men and 67 women. The average age was 32.11 years old. The clinical picture was dominated by pleural syndrome in 56% of cases, signs of tuberculous impregnation in 44.1% of cases. Preoperative preparation with thoracic drainage, bi-antibiotic therapy and respiratory physiotherapy was performed in 90% of cases. The attack was right in 55.8% of cases and left in 44.1%. A chest CT scan performed in all patients showed pachypleuritis in all cases. The tuberculous pyothorax accounted for 54% of cases, those by intra pleural rupture of a pulmonary hydatid cyst in 10.4%, para-pneumonic origin in 5.2% and post-traumatic in 4.06%. The origin was undetermined in 26.16%. A conservative posterolateral thoracotomy was performed in all our patients. The pulmonary release was done through the extrapleural plane in 94% of cases, and pleuropulmonary decortication performed in all cases. Atypical resection was associated in 4.3% of cases and peri-cytectomy in 13.9% of cases. Operative follow-up was simple in 79.6% of cases. The main postoperative complications were a prolonged aerial leak in 16.7%, atelectasis in 4.5%, a wall infection in 6.5%, a hemothorax refrained in 3.2% of cases. Broncholithiasis is the presence of calcified material in the bronchial lumen. Bronchial fibroscopy and thoracic CT are the main paraclinical elements for diagnosis. However, most broncholithiasis is fortuitous discovery intraoperatively. The evolution may be marked by complications in the underlying lung, or fistulization in the mediastinal structures especially the esophagus. Early on, management can be simple monitoring or endoscopic extraction of broncholithiasis. Surgery remains the only option in case of complications. The authors report 2 cases of broncholithiasis, one of which was diagnosed radiologically and the other of incidental discovery during the surgical exploration of a lesion of the right lower lobe.
