Explainable Artificial Intelligence(XAI)has an advanced feature to enhance the decision-making feature and improve the rule-based technique by using more advanced Machine Learning(ML)and Deep Learning(DL)based algorithms.In this paper,we chose e-healthcare systems for efficient decision-making and data classification,especially in data security,data handling,diagnostics,laboratories,and decision-making.Federated Machine Learning(FML)is a new and advanced technology that helps to maintain privacy for Personal Health Records(PHR)and handle a large amount of medical data effectively.In this context,XAI,along with FML,increases efficiency and improves the security of e-healthcare systems.The experiments show efficient system performance by implementing a federated averaging algorithm on an open-source Federated Learning(FL)platform.The experimental evaluation demonstrates the accuracy rate by taking epochs size 5,batch size 16,and the number of clients 5,which shows a higher accuracy rate(19,104).We conclude the paper by discussing the existing gaps and future work in an e-healthcare system.

Adult neurogenesis,the process of creating new neurons,involves the coordinated division,migration,and differentiation of neural stem cells.This process is restricted to neurogenic niches located in two distinct areas of the brain:the subgranular zone of the dentate gyrus of the hippocampus and the subventricular zone of the lateral ventricle,where new neurons are generated and then migrate to the olfactory bulb.Neurogenesis has been thought to occur only during the embryonic and early postnatal stages and to decline with age due to a continuous depletion of neural stem cells.Interestingly,recent years have seen tremendous progress in our understanding of adult brain neurogenesis,bridging the knowledge gap between embryonic and adult neurogenesis.Here,we discuss the current status of adult brain neurogenesis in light of what we know about neural stem cells.In this notion,we talk about the importance of intra cellular signaling molecules in mobilizing endogenous neural stem cell prolife ration.Based on the current understanding,we can declare that these molecules play a role in targeting neurogenesis in the mature brain.However,to achieve this goal,we need to avoid the undesired proliferation of neural stem cells by controlling the necessary checkpoints,which can lead to tumorigenesis and prove to be a curse instead of a blessing or hope.

Automated screening of primary cell-based aptamers for targeting and therapy of pancreatic cancer

Although it has been developed for many years, nucleic acid aptamer screening technology still fails to be widely used, a considerable part of it is due to the variability of tumor cell morphology, which leads to the use of immortalized cell lines in the laboratory to screen nucleic acid aptamers for recognition ability of tumor cells in the diseased body.To address this, primary cells that can be stably passaged were isolated and extracted from spontaneous tumors of genetically engineered pancreatic ductal adenocarcinoma model mice in this study.Next, an automated screening instrument for nucleic acid aptamers developed autonomously by our group was used to perform efficient aptamer screening using a limited number of cells, and the obtained nucleic acid aptamers were affinity verified at the cellular level.Finally, to answer the question of the cell growth environment difference on the recognition ability of nucleic acid aptamers, we verified its targeting ability to tumors in vivo on a nude mice xenograft tumor model, and further used a common antitumor drug doxorubicin combined with nucleic acid aptamers to verify the drug loading ability of this aptamer combined with the targeting therapeutic ability.

Emotion processing in Parkinson's disease: a blood oxygenation level-dependent functional magnetic resonance imaging study

Parkinson's disease is a neurodegenerative disorder caused by loss of dopamine neurons in the substantia nigra pars compacta. Tremor, rigidity, and bradykinesia are the major symptoms of the disease. These motor impairments are often accompanied by affective and emotional dysfunctions which have been largely studied over the last decade. The aim of this study was to investigate emotional processing organization in the brain of patients with Parkinson's disease and to explore whether there are differences between recognition of different types of emotions in Parkinson's disease. We examined 18 patients with Parkinson's disease(8 men, 10 women) with no history of neurological or psychiatric comorbidities. All these patients underwent identical brain blood oxygenation level-dependent functional magnetic resonance imaging for emotion evaluation. Blood oxygenation level-dependent functional magnetic resonance imaging results revealed that the occipito-temporal cortices, insula, orbitofrontal cortex, basal ganglia, and parietal cortex which are involved in emotion processing, were activated during the functional control. Additionally, positive emotions activate larger volumes of the same anatomical entities than neutral and negative emotions. Results also revealed that Parkinson's disease associated with emotional disorders are increasingly recognized as disabling as classic motor symptoms. These findings help clinical physicians to recognize the emotional dysfunction of patients with Parkinson's disease.

Return to Dialysis after Kidney Graft Failure

Introduction: The transition period from renal transplantation to dialysis is associated with high morbidity and mortality. The aim of this study is to describe the clinical and paraclinical characteristics, therapeutic management and evolutionary profile of patients returning to dialysis after kidney graft failure. Material and Methods: This was a retrospective, descriptive study conducted in the Nephrology-Dialysis-Renal Transplant Department at university hospital IbnSina between January 1998 and December 2021. We included all renal transplant recipients who had experienced kidney graft dysfunction and returned to dialysis. Patients with a follow-up after return to dialysis of less than 1 year were excluded. Results: Among 166 renal transplant recipients, 20 returned to dialysis after a median renal graft life of 85.5 months [42 - 186], corresponding to a prevalence of 12%. The mean age of our patients was 38.7 ± 11.9 years, with a M/F sex ratio of 2.3. Dialysis was initiated urgently in 10 patients (50%). Hemodialysis was the most commonly used modality (75%). Central venous catheterization was used in 35% of cases, including tunneled catheters. General condition is impaired in all patients, with persistent hypertension in 70% of cases. Mean uremia was 2.35 ± 0.8 g/l, mean creatinine 116 ± 48.3 mg/l, giving a mean GFR of 5.1 ± 2.2 ml/min. Mean albuminemia was 32.9 ± 6 g/l and mean hemoglobinemia 8.6 ± 1.9 g/dl. During the first year of follow-up, none of the patients died. However, 13 patients required hospitalization, with a mean length of stay of 15 days. Eight patients were hospitalized for infections and 5 for renal graft intolerance syndrome. After a mean follow-up of 22 months, 6 patients were detransplanted following graft necrosis. Conclusion: Return to dialysis after RT is fraught with a high rate of complications. The management of these patients must be optimized to improve their vital prognosis and quality of life.

Newcastle disease (ND) virus is a leading threat to commercial and domestic poultry in Pakistan. The virus infects and constitutes irreversible impairment to the nervous system, damages the respiratory system, and marks severe gastrointestinal lesions leading to heavy mortality in short-living birds and substantial losses in layers and breeders. The continuous emergence and evolution of the virus made it inclined to evade the humoral response and indirectly the circumvention of artificial active immunization. Newcastle disease is caused by the orthoavula genus of the paramyxoviridae family and has shown high genetic diversity even in their genotypes while information regarding enzootic trends of the virus is scanty in Pakistan. A total of 40 tracheal samples of NDV were collected from different commercial broiler farms and 11 isolates of NDV were identified. In the current study, we determined the genetic diversity of the Newcastle disease virus based on the partial sequencing of the fusion protein gene available in the NCBI database. Genetic analysis showed that seven isolates belonged to class I genotype VII and four belonged to class II genotype II. Interestingly, two isolates had epidemiological connections with vaccine-like class II genotype II. Our findings, concerning the recent outbreaks of class I genotype VII and class II genotype II of NDV in vaccinated commercial flocks, suggest possible potential partial mutations in the fusion protein gene. Genetic diversity and formation of the new cleavage site in an important neutralizing protein of wild strain are linked with the potency of artificial active immunization and a major cause of vaccine failure.

Plasma electrolytic oxidation(PEO)coatings were carried out in silicate-based electrolyte embedded with various amounts of carbon nanotube(CNT)additives(0,0.5,1,2 and 4 g/L)on AZ80 magnesium alloy substrate.Microstructure,tribological and electrochemical corrosion properties of the coated specimens were investigated.The results demonstrated that the increasing CNT additions into the electrolyte resulted in a gradual increase in the thickness of PEO-coating layer.The CNT addition to the electrolyte by 0.5 g/L resulted in a slight decrease in the roughness of PEO-coating above which it continually increased.Wear resistance of the PEO-coated specimens showed a gradual improvement with increasing CNT-incorporation within the coating.The electrochemical corrosion tests revealed that the best corrosion resistance was found after the CNT addition into the electrolyte by 0.5 g/L due to the better roughness values,more homogenous coating layers and less pore formation.

Predictors of long-term anxiety and depression in discharged COVID-19 patients:A follow-up study

BACKGROUND Patients who were hospitalized for coronavirus disease 2019(COVID-19)faced an extremely stressful experience that challenged their mental health and the longterm effects are not definitely known yet.AIM To identify both the course of mental symptoms(anxiety and depressive symptoms)and the related risk factors of recovered patients at the 20-22 mo follow-up.METHODS One hundred and seventy-two patients were enrolled.The patients were evaluated with a telepsychiatry interview and the Hospital Anxiety and Depression Scale(HADS).Sociodemographic and clinical features were analyzed by regression analysis.RESULTS The mean HADS-Anxiety(HADS-A)score was 9.08±4.90,and the mean HADSDepression(HADS-D)score was 8.55±4.39.The mean HADS-A(P=0.484)and HADS-D(P=0.011)scores were increased compared to scores during hospitalization.Being over 50 years old,having lower financial status,and being vaccinated were associated with symptoms of depression(adjusted R2=0.168)while being over 50 years old,female sex,being vaccinated,and dyspnea were associated with higher anxiety(adjusted R2=0.245).CONCLUSION To prevent the deterioration of mental health,psychiatrists should play an active role in identifying emerging mental problems as soon as possible,more vulnerable groups should be characterized,and psychological support should be sustained after discharge.

Optical and electrical conductivity properties of rare earth elements(Sm, Y, La, Er) co-doped CeO_(2)

In this research,un-doped CeO_(2) and Ce_(0.85)La_(0.10)MO_(0.05)O_(2)(M:Sm,Er,Y) compounds were synthesized by hydrothermal method and the multi-functional properties are reported.Oxygen defects were created with the additives of rare earth ions.The electrical and luminescence behaviors of the synthesized compounds were investigated in accord with the types of additives.The synthesized products were characterized by X-ray diffraction(XRD) analysis,Brunauer-Emmett-Teller(BET) measurement,UV-vis diffuse reflectance spectroscopy(DRS),scanning electron microscopy(SEM),photoluminescence(PL)spectroscopy and electrochemical impedance spectroscopic(EIS).All synthesized compounds are found to be nano-structured and have cubic phase.The total conductivity of all samples was calculated.Hence,the total conductivity of un-doped CeO_(2),Ce_(0.85)La_(0.10)Y_(0.05)O_(2),Ce_(0.85)La_(0.10)Er_(0.05)O_(2) and Ce_(0.85)La_(0.10)Sm_(0.05)O_(2) is found to be 2.07×10^(-10),5.70×10^(-4),1.0×10^(-3) and 0.0747 S/cm,respectively.Also,bandgap energy(E_(g)) of these samples calculated from UV visible absorption spectra is discussed,and the optical results show variation between 3.2 and 2.15 eV.Additionally,the luminescence properties of the compounds were investigated and different emissions occur depending on the additive type.Accordingly,photoluminescent emission spectra of Ce_(0.85)La_(0.10)Y_(0.05)O_(2),Ce_(0.85)La_(0.10)Er_(0.05)O_(2) and Ce_(0.85)La_(0.10)Sm_(0.05)O_(2) phosphors indicate that these phosphors have red,green and orange-red colors,respectively.
