Hypoxic hepatitis, also known as ischemic hepatitis, is characterized by acute hepatocellular injury due to inadequate oxygen delivery to the liver. Celiac trunk stenosis can lead to hepatic ischemia and subsequent liver damage. We present the case of an 81-year-old patient with a history of hypertension, ischemic heart disease, hypothyroidism, and biliary lithiasis, who developed hypoxic hepatitis secondary to Dunbar syndrome and a stenosis of the superior mesenteric artery. The patient improved symptoms and liver function tests with conservative management, including intravenous fluids and supportive care. Long-term management involved continued antiplatelet therapy and statins, with consideration of further interventions for celiac trunk stenosis. This case underscores the importance of recognizing Dunbar syndrome as well as superior mesentery trunk stenosis as a potential cause of hypoxic hepatitis. It highlights the need for multidisciplinary management in such cases.

Association of anxiety- depressive disorders with irritable bowel syndrome among patients attending a rural family practice center: a comparative cross- sectional study

Background Comorbidity of irritable bowel syndrome(IBS)and psychiatric disorders is common,and the prevalence of at least one psychiatric disorder has been reported as high as 80%among patients with IBS.Aims To explore the association of anxiety-depressive disorders with IBS and its different subtypes,and to evaluate the associations of lifestyle habits,dietary habits and sleeping quality with IBS.Methods A comparative cross-sectional study was conducted at the AL-Mahsama Family Practice Center,Ismailia,Egypt.It was carried out between October 2019 and October 2020.Participants were categorised into 175 patients with IBS,diagnosed using the Rome IV criteria,and 175 patients without IBS.A semistructured questionnaire was used to collect data on sociodemographic characteristics,lifestyle habits,dietary habits and sleep quality from both groups.The Hamilton Anxiety Rating Scale was used to assess anxiety symptoms,whereas the Beck Depression Inventory Second Edition was used to assess depression symptoms.Results There was a high statistically significant difference between both groups with regard to age,education,occupation and socioeconomic status(SES),being a smoker,being physically inactive,having sleep disturbance and having irregular meals;being either obese or overweight was more reported in the IBS group.There was a high statistically significant difference in the rate of anxiety and depression between patients with and without IBS.Mild,moderate and severe anxiety were reported in 37.1%,42.9%and 20.0%of patients with IBS while most(80.0%)of the patients without IBS reported mild anxiety.Regarding depression,mild,moderate and severe depression were reported in 60.0%,14.3%and 25.7%of the patients with IBS while most(82.9%)of the non-IBS participants reported mild depression.Conclusions The study shows a significant association between anxiety-depressive disorders and IBS,but no significant associations between anxiety-depressive disorders and IBS subtypes.

Background: MicroRNAs (miRs) are noncoding gene regulators that may have a role as diagnostic or prognostic biomarkers in systemic lupus erythematosus (SLE). Aim: To measure the blood levels of miR-146a, miR-126 and miR-30a in Sudanese SLE patients and to investigate their potential role in disease pathogenesis and utility as biomarkers for SLE. Material and Methods: A total of 48 SLE patients and 20 matched healthy individuals were enrolled in this study. SLE disease activity index (SLEDAI) was assessed. The blood levels of miR-146a, miR-126 and miR-30a were determined by Real-time polymerase chain reaction (PCR) in all participants. Γ-INF and IL-2 were analyzed by ELISA, and CD markers were used in flow cytometry. Results: The mean age of the patients was 31.5 ± 8.5 years with disease duration > 5 years. In SLE patients, the mean blood level fold changes of miR-146a (0.33 ± 0.277;P 0.305) compared to controls. Down regulation of miR-146a increase expression of γ-INF (P 0.05) as no differences between the SLE patients and healthy control. Conclusion: Circulating miR-146a and miR-126 could be a potential noninvasive biomarker in SLE. This study provides an overview of the current state of research on the role of miRNAs in the immune pathogenesis and regulation of SLE. Further studies are needed in miRNAs profiling expressions of SLE diseases.

BACKGROUND Various liver and gastrointestinal involvements occur in patients with coronavirus disease 2019(COVID-19)at variable prevalence.Most studies report mild liver function disturbances correlated with COVID-19 severity,though liver failure is unusual.AIM To study liver and gastrointestinal dysfunctions in Egyptian patients with COVID-19 and their relation to disease outcomes METHODS This multicentre cohort study was conducted on 547 Egyptian patients from April 15,2020 to July 29,2020.Consecutive polymerase chain reaction-confirmed COVID-19 cases were included from four quarantine hospitals affiliated to the Egyptian ministry of health.Demographic information,laboratory characteristics,treatments,fibrosis-4(FIB-4)index,COVID-19 severity,and outcomes were recorded and compared according to the degree of liver enzyme elevation and the presence of gastrointestinal symptoms.Follow-ups were conducted until discharge or death.Regression analyses were performed to determine the independent factors affecting mortality.RESULTS This study included 547 patients,of whom 53(9.68%)died during hospitalization and 1 was discharged upon his request.Patients’mean age was 45.04±17.61 years,and 21.98%had severe or critical COVID-19.Alanine aminotransferase(ALT)and aspartate aminotransferase(AST)were available for 430 and 428 patients,respectively.In total,26%and 32%of patients had elevated ALT and AST,respectively.Significant liver injury with ALT or AST elevation exceeding 3-fold was recorded in 21(4.91%)and 16(3.73%)patients,respectively.Male gender,smoking,hypertension,chronic hepatitis C,and lung involvement were associated with elevated AST or ALT.AST was elevated in 50%of patients over 60-years-old.FIB-4 was significantly higher in patients admitted to the intensive care unit(ICU),those with more severe COVID-19,and non-survivors.The independent variables affecting outcome were supplementary vitamin C intake(1 g daily capsules)[odds ratio(OR):0.05,95%confidence interval(CI):0.008–0.337];lung consolidation(OR:4.540,95%CI

Phosphorus （P） is a major constraint on agricultural productivity low availability in alkaline soils. Although soils are rich in total P, yet it is unavailable to plants and is considered as a limiting factor of plant growth. Application of super phosphate fertilizers is essentially required to maximize plant growth. Generally, P use efficiency of applied fertilizer is low because of the formation of insoluble complexes with soil colloids. P is sequestered mostly through the mechanisms of precipitation and adsorption on Ca, Fe, and Al etc. Although a large amount of the total P in soils is present as organic form;however, the sole form of P assimilated by microorganisms and plants is mineral orthophosphate ions. Soil microorganisms are the major source of such enzymes. Besides the conventional methods of mineral phosphate fertilization, microbial P-solubilization may help to improve the availability of phosphates in P-deficient soils. Use of phosphorus solubilizing bacteria （PSB） as inoculants directly to soil increased P uptake by releasing organic acids, these microbes could enhance P acquisition by plant indirectly through increased plant growth. This study was conducted to isolate phosphorus solubilizing bacteria PSB from the Chinese soil rhizosphere and numbers of treatments were tested on tomato and wheat in addition to use superphosphate fertilizer （SPF） to manage soil deficient in available P. The study was objective to evaluate the response of PSB activity on growth of tomato and wheat in invitro and pots experiment at Yangzhou University, Jiangsu province, China, during the seasons of tomato and wheat respectively. The objectives of this research were 1. Isolation and characterization of the efficient phosphate solubilizing bacteria from Chinese soil rhizosphere 2. Estimation and Assessment of Plasma D-Dimer Levels in HIV Patients

Background: The elevation of plasma D-dimer levels is fully described and associated with the increase of the mortality of patients with HIV. The present study was aimed to estimate and assess the plasma D-dimer levels in HIV patient. Material and Methods: A case control study done in May 2015. A total of 100 subjects were enrolled in this study;50 were patients professionally diagnosed by HIV;25 (50%) were males and 25 (50%) were females;their mean age is 35 years. Further 50 normal healthy individuals as normal control group: their gender and age were matched with patient groups. The platelets poor plasma (PPP) was immediately prepared from citrated blood, then the plasma D-dimer level was measured using (MISPA-i2 Reagent) Switzerland. Data were analyzed using statistical packing for social sciences program (SPSS) 20. Results: The D-dimer levels was significantly higher in patient with HIV compared with the normal healthy control group (Mean and STD 502.2 ± 287.8 vs 251.8 ± 152.7 P. value 0.01). Conclusion: The D-dimer level was significantly higher in patient with HIV compared with those in normal healthy control group.

This study was conducted to assess the occurrence and nature of extended-spectrum beta lactamase (ESBL) producing Escherichia coli and Klebsiella pneumoniae isolates from patients who presented with urinary tract infection at Federal Teaching Hospital Gombe. Isolates collected were recovered on MacConkey agar at 35°C and were identified as members of Enterobacteriaceae, and further screened for antimicrobial susceptibility and resistance by disc diffusion method. Isolates resistant to oxyimino-cephalosporins were confirmed as ESBL producers using Double Disks Synergy Test (DDST). The study shows 66% resistance to ceftriaxone (30 μg) in K. pneumoniae, which was the highest value recorded and a 51% resistance to cefpodoxime (10 μg) in E. coli. The sensitivity of E. coli and K. pneumoniae isolates to cefpodoxime (10 μg) were 49% and 33.9% respectively. ESBLs were detected among 40% (40/100) of E. coli and 54.13% (59/109) of K. pneumoniae isolates. Molecular characterization of ESBL encoding genes among E. coli isolates using multiplex-PCR showed 10% prevalence of SHV gene and 5% prevalence for CTX-M gene while TEM gene was not detected. In K. pneumoniae isolates, 5% prevalence was recorded for each of the three genes screened. The study revealed a co-occurrence of SHV and CTX-M in 75% of the E. coli and 70% of the K. pneumoniae isolates;the occurrence of all the three genes was seen in 10% and 5% of K. pneumoniae and E. coli respectively. Multiplex-PCR method provided an efficient and rapid detection of ESBL related genes, hence could be used in epidemiological studies among ESBL isolates. Monitoring dissemination and transmissions of ESBL producers are highly recommended for optimum patient care and preventing the spread of multidrug resistant (MDR) pathogens.

Amyloid goiter (AG) is a rare but well-established disease entity that may occur in a number of conditions. In the following article, we will report two cases of AG. Both patients were young males: 28 & 24 years old, presented with rapidly enlarging thyroid gland manifested with pressure effects (dyspnea and hoarseness of voice). Provisional clinical diagnosis was malignant thyroid neoplasm. One of the patients was markedly hypothyroid while the other was euthyroid. Histopathological evaluation revealed extracellular deposition of deep pink homogenous material that was confirmed as amyloid with congo red. Patient in case 1 was primary localized AG while patient in case 2 was systemic amyloidosis secondary to interstitial pulmonary fibrosis (IPF) that was first manifested by AG. The main aims of the article were to describe histopathological features of amyloidosis of the thyroid gland and to raise awareness of AG to be included in the differential diagnosis in patients presented with rapidly enlarging goiter with mass effects.
