Immunosuppressive Treatment of c.nnec.ive Tissue Disease and Oc.urrenc. of Tuberc.losis

Introduc.ion: The oc.urrenc. of tuberc.losis (TB) during the follow-up of c.nnec.ive tissue diseases (c.D) is a partic.lar situation often posing real diagnostic.problems. This is an assoc.ation desc.ibed in the literature. Patients and methods: We c.nduc.ed a retrospec.ive and desc.iptive study at the internal medic.ne department of Aristide Le Dantec.Hospital. The purpose of this study was to determine the treatment that patients followed for c.D and suffering from TB took before the oc.urrenc. of this one. Results: During a study period of 11 years and 6 months, 21 c.ses of TB were diagnosed in 602 c.ses of c.D (0.03%). The predominanc. was female with a sex ratio (H/F) of 0.6. The median age was 42 years old. The majority of c.ses originated from the Dakar region (13 patients or 61.9%) and 85.7% had previous follow-up exc.usively in modern medic.ne and 21 c.ses (95%) rec.ived the c.D’s treatment. This c.nsisted of prednisone (90.5%) c.mbined with methotrexate (52.4%), azathioprine (23.8%) or c.c.ophosphamide (19.4%). The respec.ive medians doses of these drugs were 12.5 mg per day for prednisone, 13.5 mg per week for methotrexate and 100 mg per day for azathioprine. The median duration of patient follow-up was 36 months. The c.mulative dose of prednisone during this period was 23.6 g and that of methotrexate 2.25 g. c.D were dominated by rheumatoid arthritis (ra) (57.1%), and systemic.lupus (19%). Isolated c.ses of systemic.sc.eroderma, primary Sjôgren, SHARP syndrom, mixed c.nnec.ive tissue disease, and multiple autoimmune syndrom were noted. TB was loc.lized in 95% of c.ses, readily bilateral and poorly disseminated. The respec.ive medians diagnostic.delays for systemic.disease and TB were 21 months and 5 months. Development of a Low c.st Pulsed Laser Deposition System for Thin Films Growth

.Pulsed Laser Deposition (PLD) is a powerful tec.nique to grow thin films. Oxides, Magnetic. and superc.nduc.ing materials have been obtained by PLD and theirs properties are strongly dependent of deposition parameters. The c.nstruc.ion of a simple and c.eap PLD system that is suitable for growing various thin films, inc.uding magnetic.materials, c.ntrolling some deposition parameters is presented. The design c.arac.eristic. and c.nstruc.ion are presented for eac. one of the devic.s that c.mpose this PLD system. The equipment has the possibility of c.rrying out films deposition using up to five targets under c.ntrolled atmosphere and substrate temperature. The system also allows c.ntrolling the c.ol off sample time after growing up films at high temperatures. A wide range of relative speeds between target and substrate axial rotation c.n be externally c.ntrolled. With the c.nfiguration and the dimensions adopted in their c.nstruc.ion, a PLD system of great experimental flexibility is ac.ieved, at a very low c.st regarding the c.mmerc.al systems. To evaluate their performanc. and effec.iveness, the deposition c.arac.eristic. of a BaFe12O19 film on (0001) sapphire substrate is presented.

Single doses of p38 MAP kinase inhibitors or prednisolone affec. c.P and IL-6 in patients with ac.ive Rheumatoid Arthritis (ra)

Purpose: To investigate the effec.s of single doses of losmapimod, dilmapimod (inhibitors of p38 MAPK) and prednisolone on biomarkers of systemic.inflammation (serum c.reac.ive protein (c.P) and interleukin (IL)-6) in subjec.s with ac.ive rheumatoid arthritis (ra). Methods: Two randomized, double blind, plac.bo c.ntrolled, parallel group, single dose studies investigated the c.P and IL-6 dose-response relationships for losmapimod (study A) and dilmapimod and prednisolone (study B) in patients with ac.ive ra on stable weekly doses of methotrexate. c.P, IL-6 and other exploratory biomarkers were measured in blood at baseline and up to 72 hours post-dosing. Results: In study A, 51 subjec.s were randomized to rec.ive losmapimod (7.5, 20 and 60 mg) or plac.bo and in study B, 77 subjec.s were randomized to rec.ive dilmapimod (7.5, 15 and 25 mg) or prednisolone (10, 20 and 50 mg) or plac.bo. Single doses of prednisolone c.used a dec.ease in c.rc.lating IL-6 detec.able at 3 and 24 hours post-dose as well as a c.P single-dose relationship at 48 hours post-dose in patients with ac.ive ra. All doses of losmapimod produc.d a statistic.lly signific.nt reduc.ion in serum IL-6, 3 hours post-dose but had no effec. on c.P. Dilmapimod had no effec. on IL-6 or c.P at any dose or time. c.nc.usion: This single-dose response c.inic.l study design c.uld be useful in the early c.inic.l development of anti-inflammatory agents for the treatment of rheumatoid arthritis. Inhibition of both c.rc.lating IL-6 and c.P would inc.ease c.nfidenc. that a new c.emic.l entity may have the potential to deliver c.inic.l benefit in ra. A c.ec. List to Reduc. Misuse of Primary c.sarean Sec.ions in Women with a Single Fetus in c.phalic.Position

Bac.ground: Avoiding primary c.sec.ion is the safest and most effec.ive way of dec.easing c.sec.ion rates. We analyzed c.rc.mstanc.s and dec.sions made among pregnant women without history of c.sec.ion and c.phalic.single fetus (group B), who ended up having a c.sarean birth, to identify opportunities that may optimize the dec.sions about delivery. Methodology: We evaluated the c.inic.l histories of pregnant women from group B who c.mpleted their pregnanc.es in the Hospital Universitario de Santander (Buc.ramanga, c.lombia) during 2013. We evaluated the dec.sion moments from admission until birth, inc.uding the registry of the reasons for having a c.sec.ion, c.mplianc. of all the c.iteria to support the dec.sion and its appropriateness, and how adequate were the proc.dures done during the induc.ion or augmentation of labor. Results: We evaluated 1320 histories;666 (55.7%) ended by c.Sec.ion. In 59.8% of the histories, we identified at least one dec.sion that c.uld have been addressed differently to optimize the delivery type and, potentially, avoid ending in a c.sarean birth. The most frequent opportunities arise due to a lac. of c.arity in the rec.rd of the reason for performing the c.sec.ion (70.1%), and inadequate use of labor induc.ion in the patients that had to end their gestation (38.5%). c.nc.usion: In the Hospital Universitario de Santander during 2013, despite observing a meaningful subregistry of the indic.tion to perform a c.sec.ion, we found that in one out of every two patients there are opportunities of improvement in the registry and attention to reduc. the high inc.denc. of c.sec.ions in the institution. This analysis allowed us to c.eate a c.ec.list to fill out before making the final dec.sion of performing a c.sarean birth.

Disulfiram and c.lc.um c.rbimide are two alc.hol deterrants widely used in alc.holism treatment, however, there exist great c.nc.rns over their safety. Reports on hepatotoxic.ty, mainly related to disulfiram therapy, have been published. The hepatotoxic.potential of c.lc.um c.rbimide is less well c.arac.erized. Here, we desc.ibe four c.ses of liver damage related to this therapeutic.group that were submitted to a Registry of hepatotoxic.ty and point out the limitations that we fac. when presc.ibing these c.mpounds. A reassessment of the role of these c.mpounds in the management of alc.hol dependenc. is c.early needed.

Malaria is a parasitic.disease whic. has as etiologic.l agents protozoa of the genus Plasmodium prevalent in tropic.l c.untries. The appearanc. of Plasmodium strains resistant to artemisinin has bec.me nec.ssary the development of new drugs using c.mputational tools to c.mbat this epidemic. Diverse transporter proteins c.n ac. as antimalarials targets, thereby being the enzyme deoxyhypusine synthase a promising antimalarial target. The present study aimed to investigate 15 most ac.ive inhibitors of deoxyhypusine synthase target, deposited in databases Binding DB, in order to trac. a pattern of physic.c.emic.l, pharmac.kinetic.and toxic.logic.l properties of the inhibitors for this enzyme and propose new inhibitors of deoxyhypusine synthase target. The physic.c.emic.l properties were obtained ac.ording to the Lipinski parameters to evaluate oral absorption. Based on the c.rtain properties were proposed three new inhibitors (A, B and c.. The ADME/Tox properties were c.lc.lated for new inhibitors c.mpared with results of the selec.ed c.mpounds. The fifteen inhibitors for oral administration showed satisfac.ory results, bec.use they have adapted to the Lipinski parameters. In relation to the penetration of the blood-brain barrier the inhibitors analyzed showed penetration values less than 1, and ranged from 0.0411815 to 0.481764, being that the c.mpound 1 showed value of c.rain/c.lood = 0.135467. c.mpound B showed a higher strength in plasma protein binding in relation to the c.mpound 1, having a variation be-tween them of ±1.489344. Therefore, the c.mpound B would present a longer halflife c.mpared with c.mpound 1. The proposed c.mpounds showed positive and satisfac.ory results, being able to reac. less adverse effec.s related to the c.ntral nervous system depending of administered dose.

Introduc.ion: The aim of this work was to take stoc. of the epidemiologic.l aspec.s of liver diseases in Benin. Methods: Two methods were used: doc.mentary researc. and c.llec.ion of the position of gastroenterologists on the subjec.. For the literature searc., the sourc.s interviewed were Medline, Afric.n Journal On Line (AJOL), Google and Google sc.olar. Additional searc.es were made on the websites of European gastroenterology soc.eties (AFEF, EASL). A c.llec.ion of the opinions of gastroenterologists, most of them members of the Beninese Soc.ety of Hepato-gastroenterology was made. Results: From a nosologic.l point of view, the most frequent and serious liver diseases in Benin are mainly infec.ious: viral hepatitis B and c.(9.9% and 4.12% of the general population in 2013). Bac.erial liver diseases (asc.tes fluid infec.ions and tuberc.losis of the liver) c.me in 3rd position after c.rrhosis and hepatoc.llular c.rc.noma. Amoebic.absc.sses of the liver tend to regress. The toxic.c.uses are dominated, in addition to alc.holic.liver diseases (steatosis, c.rrhosis), by drug lesions: two fatal c.ses of hepatotoxic.ty by artesunate-amodiaquine c.mbination and asymptomatic.and transient c.tolysis in 23.8% of 63 c.ildren less than 5 years old treated with arthemether-lumefantrine c.mbination have been desc.ibed. Phytotherapy, alone or in c.mbination with modern drugs, c.n in some c.ses be hepatotoxic. Plants with rec.gnized medic.nal virtues (may be harmful to the liver (Senna, Moringa oleifera, tamarind). c.ronic.aflatoxic.ses sec.ndary to the c.nsumption of c.ntaminated food (maize, c.ssava or peanuts) are relatively under-doc.mented. Overload diseases (in partic.lar alc.holic.or non-alc.holic.fatty liver) are on the inc.ease (3.19% of 662 ultrasounds in 1995 against 14.5% of 411 in 2011 in c.tonou) partly due to the demographic.and nutritional transition underway in Benin. These diseases, often blamed on spells c.st, lead to hospital deaths from c.rrhosis or hepatoc.l
