Background: Uterine fibroid is the most common benign gynecological tumor in women of childbearing age and is common in our context. It can be discovered incidentally or by metrorrhagia. Objective: To study the epidemiological, clinical, therapeutic and prognostic aspects of uterine fibroids operated in the gynaecology and obstetrics department of the CHU-T. Patients and Method: This was a descriptive cross-sectional study including all patients operated on for uterine fibromyoma in the gynaecology-obstetrics department. The collection mode was retrospective, over a 5-year period from January 1, 2017 to December 31, 2021 in the mother-child department of the CHU-T. Data entry and analysis were carried out on a microcomputer using Epi info 7.2.5 software. Results: We collected 109 cases of uterine fibroids which represented 42.5% of the surgical activities of the gynaecology department. The average age of the patients was 38.9 years ± 7.8. Married women represented 77.1%. Salaried women accounted for 65.1%. The main reasons for consultation were uterine haemorrhage (53.1%), pelvic pain (40.4%) and hypofertility (31.2%). Ultrasound was performed in all patients to help map the fibroid nuclei. The main indication for surgery was haemorrhagic myoma (43.1%). The surgical treatment was conservative (myomectomy) in 58.7% of cases and radical (hysterectomy) in 27.4% of cases. The most frequent postoperative complication was vulvar haemorrhage and the average hospital stay was 4 days ± 1.4. Anatomical pathological examination of the surgical excision specimen carried out on 30 operated patients concluded that uterine leiomyoma was diagnosed in 100% of cases. Conclusion: Patient education for early detection, universal health insurance and cost subsidies could improve the management of this condition.

Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP has not been well ***: This study was undertaken to compare the pain-relieving effects of EA and conventional treatment in patients with ***, setting, participants and interventions: This study was conducted using a randomized, controlled,three-arm, parallel-group and multi-center design. Patients diagnosed with AP were randomly and equally assigned to EA1, EA2 or control groups. All participants received conventional standard-of-care therapy for AP. Local EA alone was administered in EA1, and local plus distal EA was given in *** EA included two abdominal acupoints, while distal EA included twelve peripheral acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or *** outcome measures: The primary outcome measure was the change in the visual analogue scale(VAS;0–100) pain score between baseline and day ***: Eighty-nine participants were randomized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score change(median [interquartile range]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the change in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) than in the control group(median 2.0 days vs 3.0 days), with a hazard ratio of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events ***: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.

The Effect of Speech Fragmentation and Audio Encodings on Automatic Parkinson’s Disease Recognition

Parkinson’s disease is a neurological disease which is incurable according to current clinical knowledge. Therefore, early detection and provision of appropriate treatment are of primary importance. Speech is one of the biomarkers that enable the detection of Parkinson’s disease affection. Numerous researches are based on recordings from controlled environments;nonetheless fewer apply real circumstances. In the present study, three objectives were examined: recording fragmentation (paragraph, sentences, time-based), variable encodings (Pulse-Code Modulation [PCM], GSM-Full rate [FR], G.723.1) and majority voting on 8 kHz records using multiple classifiers. Support Vector Machine (SVM), Long Short-Term Memory (LSTM), i-vector and x-vector classifiers were evaluated in contrast with SVM as baseline. The highest results in accuracy and F1-score were achieved using i-vector models. Although variable encodings generally caused decrease in Parkinson-disease recognition, decline was within 2% - 3% at best. Moreover, fragmentation did not yield a clear outcome though some classifiers performed with the very similar efficiency along the differently fragmented sets. Majority voting did produce a slight increase in classification performance compared to as if no aggregation is used.

Abnormal Morphology: Mitral Dysplasia in a Cat

Mitral dysplasia is a pathology that describes abnormal mitral valve and valve-related structures. This congenital disease, which has been diagnosed increasingly with advancing technology, creates congestive heart failure when it progresses. Although echocardiography is accepted as the primary standard at the diagnosis stage, it may not be sufficient alone, and advanced tests may be required. The prognosis of the patients is poor. A two-month-old, mixed breed, female cat has respiratory problems and anorexia symptoms. The patient, who was taken to two different clinics, was diagnosed with bronchitis. Our hospital revealed that the diagnosis was not bronchitis but congestive heart failure due to mitral dysplasia. Medical treatment was applied to reduce the symptoms in the patient. Two days after the start of the medication, the patient began to eat and was significantly active. After the patient was stabilized, the patient was handed over to the owner in order to avoid nosocomial infections and to minimize stress. There was no further news from the patient. This is the first case reported for this disease in our country. It is also important because it is a case that reveals the importance of the multisystemic approach.

Preventing condensation of objective lens in noncontact wide-angle viewing systems during vitrectomy

AIM： To assess the optimal conditions for preventing condensation of objective lens during vitrectomy with noncontact wide-angle viewing systems（WAVSs）. METHODS： We explored the effectiveness of the coating with ophthalmic viscoelastic device（OVDs） on the corneal surface and the soaking the objective lens in warm-saline for preventing condensation of objective lens. First, to find the optimal soaking time to keep the objective lens warm, we measured the temperature of objective lens every minute after soaking in warm saline. Second, to find optimal distance between cornea and objective lens, which provide as wide a view as possible and less condensation at the same time, we measured the condensation time with different distances. With the obtained optimal soaking time and distance, we explored the effect of coating cornea with OVDs and soaking objective lens in warm saline on condensation ***： One and 5 min of soaking in warm saline was most effective for keeping the lens warm enough（45.1℃±2.1℃ for 1 min and 46.4℃±1.0℃ for 5 min, P=0.109）. The mean condensation times for the control group at 1, 3, and 5 mm from corneal surface to objective lens were 1±0.4, 4±1.4, 190±26.1 s, respectively, thus 5 mm was most optimal distance for vitrectomy with WAVSs. For the OVD coating group, the mean condensation times were 1.5±0.3, 13±1.4, and 200±23.9 s at 1, 3, and 5 mm distance and borderline significant compared with control group（P=0.068, 0.051, and 0.063, respectively）. With the 1-minute warm saline soaking group, the mean condensation time were extended to 188±34.4, 416±65.7, and 600±121.3 s at 1, 3, and 5 mm distance and statistically significant compared with control（P=0.043, 0.041 and 0.043, respectively）.CONCLUSION： OVD coating on corneal surface shows no difference on condensation time with control group. BACKGROUND Intrahepatic sarcomatoid chonalgiocarcinoma(s-CCC) is an extremely rare disease, accounting for less than 1% of hepatobiliary system malignancies, and its pathophysiology is not well known. On the hypothesis that its clinical, serologic,or radiologic diagnosis are not fully understood and its prognosis is poor, we investigated the distinguishing features of s-CCC compared with those of intrahepatic bile duct adenocarcinoma [cholangiocellular carcinoma(CCC)] in patients from a single *** To analyze the clinical, serologic, imaging, and histopathologic characteristics of intrahepatic s-CCC patients diagnosed in a single *** The clinical, serologic, imaging, and histopathologic features of 227 patients diagnosed with intrahepatic cholangiocarcinoma(IHCC) in a single medical center during the last 17 years were analyzed. The characteristics of 11 patients with s-CCC were compared with those of 216 patients with *** The number of patients with s-CCC who presented fever and abdominal pain and past history of chronic viral hepatitis or liver cirrhosis(LC) was higher than that of patients with CCC. In imaging studies, patients with s-CCC showed relatively aggressive features. However, no clear distinction was observed between s-CCC and CCC based on other clinical, serologic or radiologic examination results. An accurate diagnosis could be made only via a histopathologic examination through immunohistochemical staining.
