Caudal Anesthesia: Experience in the Post-Operative Analgesia in Pediatric Ambulatory Surgery at the University Hospital of Treichville

Introduction: the caudal anesthesia is used by many authors for postoperative analgesia. the purpose of this study was to report our experience in the practice of caudal block as post operative analgesia method in ambulatory surgery in a context of limited technical equipment. Patients and Method: Over a period of 5 months, a prospective study was conducted on 39 children aged 3 to 5 years weighing on average 15.12 kg. Children classified ASA I and II were selected. After premedication with midazolam (0.3 mg/kg) by intra rectal route, the inhalation induction was made with sevoflurane 8%, conveyed by fresh gas (50% O2 and 50% air). the caudal block was obtained with the levobupivacaine 0.25% at a dose of 1 ml/kg. the hemodynamic parameters (systolic and diastolic blood pressure, heart rates) and respiratory parameters (respiratory frequency) pre-, per- and post-operative were measured. Post-operative pain was assessed with the Objective Pain Scale (OPS). the date of first use of analgesia was noted. the adverse effects of caudal block (meningitis, respiratory disorders, acute urinary retention, cardiac disorders) have been assessed. Results: the average duration of the procedure was 55.2 minutes. the use of analgesia was made 4 hours after skin closure, when the OPS Broadmann score was greater than 3. An agitation was observed in 6 children. Haemodynamic parameters have not significantly varied from the pre- to the post-operative. no infectious complications or intolerance to local anesthetics were observed. Allthe children were able to drink 4 hours after the end of the intervention and issued their first urine later than 3 hours after surgery. Conclusion: this type of anesthesia has been found very suitable for ambulatory surgery of the child, and is always helpful. It assured a post operative analgesia of good quality, and a reduction in consumption of morphine intraoperatively.

Introduction: Maternal mortality, which is estimated at 614 deaths per 100,000 births in Côte d’Ivoire, is associated with a low national contraceptive prevalence rate of 13.9%. In this study, we are interested in the contraceptive experiences of women who attend health centers to estimate their rate of contraceptive use and to assess the factors involved in the use of modern contraceptive methods among women of childbearing age who are exposed to the risk of pregnancy. Method: A descriptive cross-sectional study was conducted from December 2018 to September 2019 among 423 women, who attended two health centers in Abidjan. A bivariate analysis identified factors associated with contraceptive use by these women. A descriptive analysis determined the means for the quantitative variables and the frequencies and percentages of qualitative variables. Interpretation of results was based on significance (α = 5%, 95% CI). Results: Contraceptive prevalence was 37%. there was a significant relationship between contraceptive use and the socio-demographic and reproductive characteristics of women (p Conclusion: Women who were over 35 years age, uneducated, primary school graduates, housewives, unemployed, in the informal sector, Muslims, nulliparous, and lacking information and decision-making power were less likely to use modern contraceptive methods. they are the target population for strategic interventions to improve contraceptive prevalence.

Anatomical Variations of Recurrent Laryngeal nerve: the Danger in thyroid Surgery

Objectives: We report two types of anatomical variations of the recurrent laryngeal nerve in two patients. through these two patients we wanted to highlight our surgical approach of the recurrent nerve in an unusual position and to describe the surgical implication of these almost rare variations. Case report: patient aged 28 and 58 admitted for goiter. they underwent a right lobisthmectomy. Both recurrences were approached retrograde. the anatomical variations of the nerve concerned the non-recurrent laryngeal nerve in the first patient. In the second patient there were three anatomical variations, namely an extra laryngeal bifurcation of the nerve, a pre-vascular position of the nerve and a branch connecting the vagus nerve and the recurrent nerve. no recurrence nerve injury was noted. Conclusion: the anatomical variations of the nerve are numerous. A careful dissection is a guarantee of a good prognosis.

Introduction: Chronic renal failure is a real public health problem because of its prevalence, the cost of its management and the high morbidity and mortality rate associated with it. the objective of this study was to determine the frequency, causes and main aggravating factors of chronic renal failure in the medical department of the hospital of Sikasso. Materials and Methods: this was a cross-sectional, retrospective, descriptive study conducted in the medical department of the Sikasso hospital from January 1, 2021 to november 30, 2022. It covered all the records of patients with chronic renal failure hospitalized in the department during this period. the MDRD formula was used to estimate the glomerular filtration rate. the KDIGO 2012 classification was used to stage CKD. Results: Of 820 patient records reviewed, we retained 197 records. the mean age of our patients was 42.25 ± 10 years. the male sex represented 114 (57.9%). the socio-economic level was considered low and precarious in 79.8% of cases. the frequency of chronic renal failure was 24%. the reason for hospitalization was mostly hyper creatinine > 185 μmol/l (92.4%). Herbal medicine was reported in 103 cases (52.3%). Medical history was hypertension 101 cases (51.3%), undocumented gastro duodenal ulcer (UGD) 14 cases (7.6%), hypertension and diabetes 11 cases (5.1%), diabetes 8 cases (4%), lower limb edema 9 cases (4.6%). Hypocalcemia was 147 cases (75.6%) with hyper phosphoremia was 153 cases (77.7%). Hemoglobin level was: n 6 - 8 g/dl, 77 cases (39.1%), from 8 - 10 g/dl, 54 cases (27.4%). the etiologies of CKD were vascular nephropathy 106 cases (53.8%), interstitial nephropathy, 44 cases (22.3%), glomerular nephropathy, 33 cases (16.8%), diabetic nephropathy, 12 cases (6.1%) and polycystic kidney disease 2 (1%). CKD was classified as stage 5, 171 cases (86.8%), stage 4, 11 cases (5.6%), stage 3, 13 cases (6.6%) and stage 2, 2 cases (1%.) Dialysis was performed in 1095

Audiometric Profile of Deafness at the University Hospital Center Gabriel Toure of Bamako

Objective: To determine the audiometric profile of deafness in our practice. Materials and Methods: Longitudinal prospective study was conducted out in the EnT department and cervicofacial surgery of Gabriel Toure University Hospital in Bamako, we made an exhaustive sampling of all the patients who consulted for hearing loss, tinnitus, vertigo, hiring report, medical expertise and whose deafness was confirmed to tonal audiometry with an age greater than or equal to 15 years. It was spread over 10 months (June 2016 to March 2017). A total of 200 patients were collected. Exclusion criteria were all patients under 15 years of age as well as deafness related to earwax or foreign bodies, and refusal to participate in the study Results: During our study period, 6055 outpatients were admitted out of which 734 patients underwent audiometric testing and 200 patients (3.30%) met our criteria. the male sex was the most represented with a rate of 60%. the sex ratio was 1.5 or 3 men for 2 women. the 15 to 25 age group was the most represented at 37.5%. the average age was 37.18 years old with extremes ranging from 15 to 83 years old. Pupils/students were the most represented with a rate of 29.5%, followed by housewives in 23%, farmers in 10.5% and military in 10%. As antecedent 26% of our patients had a chronic otitis media (OMC), against 23% who had no antecedent otological and 17% had a traumatic antecedent. Bilateral deafness was the most common with a rate of 64.5%. the mode of progressive appearance was the most frequent in 74.5%. As functional signs 46.22% of our patients had a hearing loss associated with tinnitus. Otoscopic examination was pathological in 34.5% of our patients. Mixed deafness was the most common in 43.35%, followed by perception deafness in 32.19% and transmission deafness 24.46%. Mean deafness was the most common with a rate of 48.91%, was severe in 20.22%, mild in 18.31%, deep in 11.47% and cophotic in 01.09%. Asymmetrical curves were the most found in

Primitive cerebral lymphoma (PCL) is a rare entity often poorly known. they are defined as extra nodal tumor interesting the CnS, brain and eye, and this in the absence of systemic lymphoma disease and brain metastases. the favoring factor is established immunosuppression which is explained by an increase in the incidence related to HIV. the basis of treatment is chemotherapy with consolidation radiotherapy. We report the experience of the national Institute of Oncology in rabat in management of this disease.

Application of the Dempster-Shafer theory to the Classification of Pixels from Aster Satellite Images and Spectral Indices

In this paper, it is proposed to apply the Dempster-Shafer theory (DST) or the theory of evidence to map vegetation, aquatic and mineral surfaces with a view to detecting potential areas of observation of outcrops of geological formations (rocks, breastplates, regolith, etc.). the proposed approach consists in aggregating information by using the DST. From pretreated Aster satellite images (geo-referencing, geometric correction and resampling at 15 m), new channels were produced by determining the spectral indices nDVI, MnDWI and nDBaI. then, the DST formalism was modeled and generated under the MATLAB software, an image segmented into six classes including three absolute classes (E,V,M) and three classes of confusion ({E,V}, {M,V}, {E,M}). the control on the land, based on geographic coordinates of pixels of different classes on said image, has made it possible to make a concordant interpretation thereof. Our contribution lies in taking into account imperfections (inaccuracies and uncertainties) related to source information by using mass functions based on a simple support model (two focal elements: the discernment framework and the potential set of belonging of the pixel to be classified) with a normal law for the good management of these.

A new enolate derivative of furostanol glycoside, named asparagusin A, was isolated from the roots of Asparagus filicinus and established as 3-O-beta-D-xylopyranosyl(1-->4)-beta-D-glucopyranosyl (25S)-furost-20(22)-ene-3 beta, 26-diol 26-O-beta-D-glucopyranoside (1) by spectroscopic and chemical methods. Asparagusin A (1) exhibited a cytotoxic activity effect on PC12 cells.

the Upper Guinea Forest is subject to heavy deforestation. In this context, many endemic and/or rare plant species are threatened with extinction. this is the case of Aubregrinia taïensis (Aubrév. & Pellegr.) Heine, a critically endangered Sapotaceae species, endemic to Côte d’Ivoire and Ghana. After 3 years of investigation in and around the Tai national Park (TnP) in the West of Côte d’Ivoire, only one individual of this species was located and no sexual reproductive organs (fruits, seeds) were observed. Woody plant species around this individual were inventoried in order to know the species that can co-occur with Aubregrinia taïensis. For that a plot of 30 m of diameter was established around the individual and all of the woody species of this circle were inventoried. then, the taxonomic diversity, the chorology of the species, the conservation status and their life-form were recorded. A total of 130 woody plant species belonging to 51 families were collected in the site. the most represented families are Euphorbiaceae (12 spp.), Rubiaceae (10 spp.), Annonaceae (7 spp.), Fabaceae, Moraceae, Malvaceae (6 spp. each) while the most represented genera are Diospyros L. (4 spp.), Cola Schott & Endl. and Vitex L. (3 spp.). Species from the Guineo-Congolese Region (GC) are the most abundant (72%). they are followed by West African endemic species (GCW, 19%). Microphanerophytes (mp) are the most abundant and represent more than 40% of the species. three vulnerable species were found in this plot: Campylospermum amplectens, Placodiscus boya and Trichoscypha cavalliensis. thus, the preservation of this forest is more than necessary.

这栋住宅位于南回归线上,是巴西降水最多的地区。住宅基地长55M,宽16M,位于海滩的最右端,一侧是海岸,另一侧是坡度50%的陡坡,与道路之间的高差是28M。山坡上绿树茵茵,必须尽可能地保护树木和地形,这一点启发了建筑师制定出整个项目的策略：如非迫不得已,不碰触基地的地面。三根钢筋混凝土柱用于支撑房屋主体,是整座建筑的着地点。由于成本限制,建筑师最终决定使用现浇混凝土,浇筑的过程变成由下至上。屋顶的平台跟道路在同一水平面。
