Outcome between Diabetic versus Non-Diabetic Acute Stroke in a Black African Population: A Cohort Study

Background: Post-stroke outcomes are poorer in patients with diabetes mellitus (DM). The aim of this study was to determine the prevalence of DM in acute stroke and to compare the outcome in patients with or without diabetes in a tertiary care hospital in Douala, Cameroon. Methods: This was a hospital-based prospective cohort study included both diabetic and non-diabetic acute stroke patients (ASP). Demographic, clinical profile and outcome data was collected within 3 months of stroke onset. Descriptive statistics, t-test and chi square test used for comparisons while associations between DM and stroke outcomes, were analyzed using multiple logistic regression and survival analysis. Results: Of the 701 ASP included, the overall prevalence of diabetes in stroke was 34.2% (n = 240) while 9.4% (n = 66) had newly diagnosed diabetes. Mortality was significantly elevated amongst patients with diabetes during hospitalization (p = 0.034) and at 3 months post stroke onset (p = 0.004), but on multivariate analysis, diabetes was not an independent predictor of mortality [OR = 0.984;(95% CI: 0.506 - 1.913);p = 0.961)]. On Cox proportional hazards regression model, the risk of dying was about 1.5 times higher amongst the DM patients compared to non-diabetic patients [adjusted HR (95% CI) of 1.502 (1.128 - 2.000);p = 0.005]. Diabetes was not an independent predictor of poor functional outcome within 3 months post stroke. Conclusion: About 1 in 3 ASP had DM on admission. 近年来,利用生物特征的人体识别赢得了越来越多的关注。生物识别技术有着广泛的应用,以致于我们无法给它一个确切的定义。生物识别技术包括那些基于一个或多个固有的身体特征或行为特征唯一地识别人体的方法。生物特征可分为两大类：与身体的形状有关的生理特征以及与个人行为有关的动作特征。指纹识别、面部识别、DNA、手和手掌形状以及虹膜识别等属于基于前者的人体识别;打字节奏、步态识别、语音识别等属于基于后者的人体识别。但基于生物特征的识别不仅限于以上所说。一些研究人员将类似打字规律或鼠标手势等其分析可以不被用户动活动中断或干扰的作特征识别称为"behaviometrics"。作为一项生物特征识别新技术,步态识别近来越来越引起研究人员的兴趣。步态识别是一个过程,在这个过程里,人体运动特征被自动提取并且被应用到行人的身份识别中。它使得能够在没有交互和合作的情况下远距离被识别人成为一种可能,这也是它作为一种识别方法具有吸引力的特性。本文中,我们的步态识别方法基于规范视角合成(canonical view synthesis,CVS)和步态能量图(gait energy image, GEI)。由于对步态方向的依赖是步态识别中具有挑战性的问题之一。经典的规范视角合成技术就是被用来处理视角变化问题。特别的,通过使用平面单应性变换,CVS可以减少对步态特征对方向的依赖。GEI被用在步态的描述,是一种用轮廓构造的时空步态描述。最后,为了保存步态主要组成部分,我们使用主成分分析法(PCA)来降低维数特征空间使得GEI特征被完好的保存。

Water Quality Assessment in the Bamoun Plateau, Western-Cameroon: Hydrogeochemical Modelling and Multivariate Statistical Analysis Approach

This study focuses on the geochemical and bacteriological investigation of surface and ground water in the Bamoun plateau (Western-Cameroon). During the period from September 2013 to August 2014, 71 samples were collected from two springs, one borehole, four wells and the Nchi stream for analysis of major elements. In order to obtain the characteristics of the various species of bacteria, 7 samples were selected. The analytical method adopted for this study is the conventional hydrochemical technic and multivariate statistical analysis, coupled with the hydrogeochemical modelling. The results revealed that, water from the zone under study are acidic to basic, very weakly to weakly mineralized. Four types of water were identified: 1) CaMg-HCO3;2) CaMg-Cl-SO4;3) NaCl-SO4 and 4) NaK-HCO3. The major elements were all listed in the World Health Organization guidelines for drinking water quality, except for nitrates which was found at a concentration > 50 mg /l NO-3 in the borehole F401. As for the hydrobiological aspect, the entire sample contained all the bacteriological species except for spring S301 and well P401. According to the hydrogeochemical modelling, the Gibbs model and multivariate statistical tests, the quality of surface and ground water of the Foumban locality is influenced by two important factors: 1) the natural factors characterized by the water-rock interaction, evapotranspiration/crystallization, 2) the anthropogenic factors such as: uncontrolled discharges of liquid and solid effluents of all kinds and without any prior treatment within the ground and the strong urbanization accompanied by lack of sanitation and insufficient care.
