Transcutaneous auricular vague nerve stimulation improved brain connection activity on patients of disorders of consciousness: a pilot study

OBJECTIVE:To evaluate the clinical effect of transcutaneous auricular vagus nerve nerve stimulation(ta VNS)on disorders of consciousness(DOC)patients with Coma Recovery Scale-Revised(CRS-R)and cerebral cortex activity by electroencephalogram(EEG)detection.METHODS:Randomized controlled methods were used to evaluate the clinical effect of ta VNS on patients with DOC.Twelve patients with initial CRS-R of 6-10 were randomly divided into the treatment group of ta VNS and control group of transcutaneous non-auricular vague nerve stimulation(tn VNS).According to clinical diagnosis,the treatment group was divided into vegetative state(VS)group and minimally conscious state(MCS)group.RESULTS:The energy of delta and beta bands is positively correlated with the brain activity of patients.ta VNS has different regulatory effects on patients with different conscious States.In ta VNS group,the energy of delta band in local brain regions changed significantly.Significant changes in brain connection activity were limited to local brain regions.While in patients with MCS in the ta VNS group,delta and beta band energy significantly changed in multiple brain regions and crossbrain connection activity also changed significantly.CONCLUSION:These findings suggest that ta VNS may be a related extra method for arousing patients’awakening by improving brain connection activity.And the effect is remarkable in MCS patients.

Implantable cervical vagus nerve stimulation(i VNS) is a representative and promising neuromodulation.However,the invasive nature restricts its application.Traditional auricular acupuncture treatment has a long history.The auricular branch of the vagus nerve(ABVN) is a branch on the surface of the ear.Some studies demonstrates that transcutaneous auricular vagus nerve stimulation(ta VNS) would achieve similar effects as i VNS.Ta VNS and i VNS share a common anatomical basis and acting mechanism.In this article,we made a comparison between i VNS and ta VNS in indications and efficacy.The recent studies have revealed similar clinical efficacy of ta VNS,ta VNS would expand the indication of i VNS.Highquality clinical evidences are needed before ta VNS become be an alternative of i VNS.

Effect of acupuncture at Neiguan (PC 6) on cardiac function using echocardiography in myocardial ischemia rats induced by isoproterenol

OBJECTIVE: To investigate the effect of acupuncture at Neiguan(PC 6) on cardiac function using echocardiography in rat models of myocardial ischemia(MI) induced by isoproterenol(ISO).METHODS: Twenty-seven Sprague-Dawley rats were randomly assigned to normal, model, and acupuncture groups. The model and acupuncture groups were given injections of ISO(85 mg/kg) to establish the MI model. After model establishment,the acupuncture group was treated with acupuncture at Neiguan(PC 6) for 30 min. Echocardiography was used to monitor diastolic and systolic function for 30 min starting from the time after the acupuncture needles were removed. Changes in the length of left ventricular internal diameter at end-diastole(LVIDd), length of left ventricular internal diameter at end-systole(LVIDs), the ratio of mitral peak velocity at early diastole and atrial contraction(E/A), ejection fraction(EF), fractional shortening(FS), and stroke volume(SV) were measured.RESULTS: Compared with the model group at 0and 15 min after needles were removed, the means of LVIDd and LVIDs were significantly lower(P <0.01) and E/A, EF, FS, and SV significantly higher(P < 0.01) in the acupuncture group. In the acupuncture group, the means of LVIDd and LVIDs15 min after the needles were removed were significantly higher(P < 0.01) than those at 0 min. The means of E/A, EF, FS, and SV significantly decreased(P < 0.01) from 0 to 15 min in the acupuncture group.CONCLUSION: These findings indicate that acupuncture at Neiguan(PC 6) can affect cardiac function by increasing left ventricular diastolic and systolic function in MI rat models, but the effect only lasts for 15 min.

An alternative therapy for drug-resistant epilepsy： transcutaneous auricular vagus nerve stimulation

Background Previous studies demonstrated that vagus nerve stimulation （VNS） is an effective therapy for drugresistant epilepsy.Acupuncture is also used to treat epilepsy.This study was designed to examine the safety and effectiveness of transcutaneous auricular vagus nerve stimulation （ta-VNS） for patients with drug-resistant epilepsy.Methods A total of 50 volunteer patients with drug-resistant epilepsy were selected for a random clinical trial to observe the therapeutic effect of ta-VNS.The seizure frequency,quality of life,and severity were assessed in weeks 8,16,and 24 of the treatment according to the percentage of seizure frequency reduction.Results In the pilot study,47 of the 50 epilepsy patients completed the 24-week treatment; three dropped off.After 8-week treatment,six of the 47 patients （12％） were seizure free and 12 （24％） had a reduction in seizure frequency.In week 16 of the continuous treatment,six of the 47 patients （12％）were seizure free; 17 （34％） had a reduction in seizure frequency.After 24 weeks＇ treatment,eight patients （16％） were seizure free; 19 （38％） had reduced seizure frequency.Conclusion Similar to the therapeutic effect of VNS,ta-VNS can suppress epileptic seizures and is a safe,effective,economical,and widely applicable treatment option for drug-resistant epilepsy.（ChiCTR-TRC-10001023）

The correlation between the efficacy of escitalopram and the traditional Chinese medicine syndrome types of depressive disorders:a randomized controlled study

OBJECTIVE To clarify whether the therapeutic effect of escitalopram on depression patients is correlated with traditional Chinese medicine syndrome types,and to provide a basis for more accurate drug application. METHODS A total of 235 depression patients were recruited and classified according to traditional Chinese medicine syndrome differentiation into 5 types: liver and qi stagnation(45), liver stagnation and fire transformation(43), liver qi stagnation and spleen deficiency(79), heart and spleen deficiency(20), and liver and kidney deficiency(38). All patients were treated with escitalopram(10 mg·d-1)for eight weeks. The Montgomery Depression Rating Scale(MADRS) and the 17 Hamilton Depression Rating Scale(HAMD-17) were used to score before treatment and 2, 4, and 8 weeks after treatment, respectively.RESULTS Compared with before treatment, the MADRS and HAMD-17 scores and remission rates of each syndrome type significantly improved with increasing medication duration. The MADRS score relief rate of liver and kidney deficiency type was the most significant(69.3%),significantly higher than the other four syndrome types(47.5%-52.6%, P<0.05); The remission rate of HAMD-17score was significantly higher than that of heart and spleen deficiency(50.7% vs. 28.5%, P<0.05). The remission rate of HAMD-17 score in liver and qi stagnation type(52.2%) was significantly higher than that in liver stagnation and spleen deficiency type(37.0%, P<0.01)and heart spleen deficiency type(28.5%, P<0.05). CONCLUSION Escitalopram may have a more significant therapeutic effect on patients with liver and kidney deficiency and liver stagnation depression. This study provides a new approach for the more effective and accurate application of traditional Chinese medicine syndrome differentiation in the selection of clinical antidepressants.

抑郁症是严重威胁人类健康的全身性疾病,往往与疼痛、失眠、功能性胃肠病、糖尿病等疾病伴随发生,抑郁共病发病率高、机制复杂、疾病负担重、治疗手段有限。现阶段临床实践和科学研究多数是围绕单一疾病或系统展开,以单病为导向的临床诊疗模式亟须突破。本文原创性提出“调枢启神”治疗抑郁共病的中西医结合创新理论,形成耳穴电刺激经迷走神经异病同治创新治法,并获得其治疗疼痛/抑郁共病、失眠/抑郁共病、功能性胃肠病/抑郁共病、糖尿病/抑郁共病的神经生物学基础及循证医学证据,以期为临床治疗提供新思路、新方法。

Mechanisms underlying the structural and functional characteristics of acupoints

Acupoints are particular areas on the surface of the body through which the Qi and blood of the Zang-fu organs and meridians flow.Acupoints can respond to stimuli and reflect syndromes.Research on the essential morphology of acupoints has two key focuses:anatomical structure and physiological function.The structure is the material and anatomical basis for the function,while the function is the manifestation of the structure.The two factors not only have profound significance for our understanding of the essence of acupoints,but also have great value for clinical acupuncture and moxibustion practice.Based on the structural and functional characteristics of meridians under different physiological and pathological conditions,this paper illustrates the essence of acupoints and explores the rules and biological basis of the acupoint functions induced by acupuncture and moxibustion.

Activation of hypothalamic gono-like neurons in female rats during estrus

In mammals, gonadal function is controlled by the activity of hypothalamic gonadotropin-releasing hormone neurons, which control the secretion of adenohypophyseal and gonadal hormones. However, there are a number of unanswered questions in relation to gonadal function. It is currently unknown how erotogenic stimulation of the genitals influences the subpopulation of hypothalamic medial preoptic area neurons, antidromically identified as projecting to the median eminence at different periods of the estrous cycle. Additionally, the distinctiveness of hypothalamic medial preoptic area neurons, with respect to methods of feedback control by exogenous hormones, is also unknown. In this study, spontaneous discharges from individual neurons encountered within the medial preoptic area, gono-like neurons, were recorded extracellularly using glass microelectrodes. To confirm the cellular and histochemical properties of the recording units, antidromic stimulation was performed using a side-by-side bipolar stimulating electrode placed into the median eminence, alongside microiontophoretic injections of the conventional tracer, horseradish peroxidase. In addition, further immunohistochemical analyses were performed. Results showed that elevated gono-neuron activity was accompanied by increased background activity and greater responses to erotogenic stimuli during estrus. Application of clitoral traction stimulation resulted in increased activation of the gono-like neurons. This neuronal activity was noticeably inhibited by β-estradiol administration. Immunohistochemical analyses revealed the presence of gonadotropin-releasing hormone-reactive protein in hypothalamic cells in which electrophysiological recordings were taken. Thus, medial preoptic area neurons represent the subset of hypothalamic gonadotropin-releasing hormone neurons described from brain slices in vitro, and might serve as a useful physiological model to form the basis of future in vivo studies.

THE EFFECT OF ACUPUNCTURING ACUPOINTS ON THE CHANGE OF ELECTROENCEPHALOGRAM (EEG) IN ENDOTOXIC SHOCKED RATS

In present work,EEG and BP were used as the indexes to observe the relationbetween the change of EEG and the change of BP in the endotoxic shocked rats。At maintainingshock for 1 hr,dysrhythmia of EEG appeared in 38/46 cases.Simultaneously,there was a markeddrop in Bp,P<0.05.Following the shocked time prolonged,dysrhythmia was getting severe。AfterEA”Rengzhong"(n=14)or“Zusanli”(n=12),BP was significantly increased(P<0.05),anddysrhythmia of EEG showed clear improvement in most of the rats。There was a close relation be-tween the changes of EEG and BP,the change of EEG had a direct bearing on the change of BP.
